Aims/hypothesis
The aim of this work was to compare treatment intensification strategies based on orally administered vs injectable incretin-based antihyperglycaemic agents in patients with type 2 ...diabetes mellitus on metformin monotherapy.
Methods
In a 26 week, open-label study, 653 patients (baseline HbA
1c
= 8.2% 66 mmol/mol) were randomised at 111 sites in 21 countries in a 1:1 ratio to a strategy using oral agents (starting with sitagliptin 100 mg/day) or a strategy using the injectable drug liraglutide starting at a dose of 0.6 mg/day, up-titrated to 1.2 mg/day after 1 week. The following patients with type 2 diabetes mellitus were recruited for the study: those aged 18–79 years, on a stable dose of metformin monotherapy ≥1,500 mg/day for ≥12 weeks, with an HbA
1c
≥7.0% (53 mmol/mol) and ≤11.0% (97 mmol/mol) and a fasting fingerstick glucose (FFG) <15 mmol/l (<270 mg/dl) at the randomisation visit, deemed capable by the investigator of using a Victoza pen injection device (containing 6 mg/ml liraglutide; Novo Nordisk, Bagsværd, Denmark). Women taking part in the study agreed to remain abstinent or use an acceptable method of birth control during the study. Randomisation was performed via a computer-generated allocation schedule using an interactive voice response system. After 12 weeks, patients on sitagliptin with HbA
1c
≥ 7.0% (53 mmol/mol) and fasting glucose >6.1 mmol/l had their treatment intensified with glimepiride; patients on liraglutide with HbA
1c
≥ 7.0% (53 mmol/mol) had the dose up-titrated to 1.8 mg/day. The primary analysis assessed whether the strategy using oral drugs was non-inferior to that using an injectable drug regarding HbA
1c
change from baseline at week 26 using a per-protocol (PP) population and a non-inferiority margin of 0.4%.
Results
In the PP population (522 patients included: oral strategy,
n
= 269; injectable strategy,
n
= 253) antihyperglycaemic therapy was intensified at week 12 in 50.2% and 28.5%, respectively. HbA
1c
decreased over 26 weeks in both treatment strategy groups, with a larger initial reduction at week 12 in the injectable strategy group. The LS mean change in HbA
1c
at week 26 was −1.3% (95% CI −1.4, −1.2) in the oral strategy group and −1.4% (95% CI −1.5, −1.3) in the injectable strategy group; the study met the non-inferiority criterion. Both treatment regimens were generally well tolerated; hypoglycaemia was reported more often with the oral strategy, while nausea, vomiting, diarrhoea and abdominal pain were reported more often with the injectable strategy.
Conclusions/interpretation
An oral, incretin-based treatment strategy with sitagliptin and, if needed, glimepiride may be a good approach in many patients with type 2 diabetes mellitus for managing inadequate glycaemic control on metformin monotherapy, as compared with an injectable treatment strategy with liraglutide. The oral and injectable strategies had similar effects on HbA
1c
and had good overall tolerability.

Abstract Aim The study compared the duration of maintenance of treatment in patients with type 2 diabetes (T2D) using dual therapy with either metformin and sitagliptin (M-Sita) or metformin and a ...sulphonylurea (M-SU). Materials and methods This observational study included adult patients with T2D who had responded inadequately to metformin monotherapy and therefore had started de-novo treatment with Met-Sita or Met-SU within the previous eight weeks. Patient follow-up and changes to treatment were performed according to their general practitioner's usual clinical practice. The primary outcome was time to change in treatment for whatever cause. HbA1c and symptomatic hypoglycaemia were also documented. Results The median treatment duration for patients in the M-Sita group (43.2 months) was significantly longer ( P < 0.0001) than in the M-SU group (20.2 months). This difference persisted after adjusting for baseline differences and confounders. A similar reduction in HbA1c was noted in both arms (–0.6%), and the incidence of hypoglycaemia prior to treatment modification was lower with M-Sita (9.7%) than with M-SU (21.0%). Adverse events potentially related to treatment were reported in 2.8% ( n = 52) and 2.7% ( n = 20) of patients in the M-Sita and M-SU arms, respectively. Conclusion Under everyday conditions of primary diabetes care, dual therapy with M-Sita can be maintained for longer than M-SU. In addition, while efficacy, as measured by changes in HbA1c , was similar between treatments, the incidence of hypoglycaemia was lower in patients taking M-Sita.

Ninety-three individuals (78 families) with an undetermined leukodystrophy were selected on MRI-based criteria of childhood ataxia with central hypomyelination/vanishing white matter (CACH/VWM) for EIF2B genes analysis.
Eighty-nine percent of individuals with MRI criteria of CACH/VWM have a mutation in one of the eIF2B beta to epsilon subunits. For 83 individuals (68 families), 46 distinct mutations (90% missense) in four of the five eIF2B subunits (beta, gamma, delta, epsilon) were identified. Sixty-four percent were in the epsilon subunit, a R113H substitution was found in 71% of eIF2B epsilon-mutated families. A large clinical spectrum was observed from rapidly fatal infantile to asymptomatic adult forms. Disease severity was correlated with age at onset (p < 0.0001) but not with the type of the mutated subunit nor with the position of the mutation within the protein. Mutations R113H in the epsilon subunit and E213G in the beta subunit were significantly associated with milder forms.
The degree of eIF2B dysfunction, which is involved in the regulation of protein synthesis during cellular stress, may play a role in the clinical expression of eIF2B-related disorders.
Rationnel L'adhésion des patients diabétiques de type 2 (DT2) à des recommandations d'hygiène de vie et aux traitements est un objectif clé de la prise en charge. Ces recommandations sont données par ...le médecin durant les consultations de suivi du patient mais ne sont pas toujours mises en pratique par les patients qui n'osent pas faire part de leurs difficultés à leur médecin. Partager réellement la décision médicale avec le patient permettrait d'augmenter l'adhésion des patients aux objectifs fixés en commun. Matériels et Méthodes OPTIMA est un questionnaire développé afin d'optimiser la communication médecin-patient sur le diabète. Il comprend 5 modules (Activité physique (AP), Traitement, Alimentation, Connaissance du diabète (CD), Contrôle de la glycémie (CG)) ; les questions sont divisées en 3 parties (facilité, utilité, action) pour chaque tâche abordée. Cette étude transversale, multicentrique, observationnelle, conduite en France avec des patients DT2 se présentant à une consultation pour diabète auprès de généralistes et diabétologues, avait pour objectif principal d'évaluer le taux de consultations se terminant par la fixation d'un micro-objectif SMART. Les médecins demandaient à leurs patients de compléter un des modules du questionnaire OPTIMA. L'avis des patients et des médecins sur le questionnaire OPTIMA était évalué via un autre questionnaire. Résultats Au total, 807 patients ont été inclus par 186 médecins. Alors que 92,7 % des consultations se terminaient par la fixation d'un micro-objectif, seuls 22,3 % correspondaient à un micro-objectif SMART (34,3 % pour le module A P, 16,4 % pour « Traitement », 9,6 % pour « Alimentation », 25,2 % pour CD, 29,5 % pour CG). En moyenne, 79,0 % des patients rapportaient que l'AP était utile, 35,0 % qu'elle était facile et 25,8 % qu'ils la pratiquaient régulièrement. Le questionnaire OPTIMA promouvait la communication médecin-patient pour 92,8 % des patients et 69,4 % des médecins. Conclusions La fixation de micro-objectifs SMART n'est pas encore entrée dans la pratique médicale courante ; le questionnaire OPTIMA facilite la communication médecin-patient et son utilisation permettrait aux médecins de fixer plus systématiquement des micro-objectifs. L'AP est le domaine le plus susceptible d'induire la fixation d'un micro-objectif.
Reported are the clinical, neuroradiologic, and molecular findings in 18 patients with megalencephalic leukoencephalopathy and subcortical cysts (MLC) syndrome. Marked clinical intrafamilial and ...interfamilial variability in mutation-proven cases was found. A broad spectrum of pathogenetic mutations (missense, splice site, insertion, and deletions) were identified in the MLC1 gene, enlarging the spectrum of allelic variants without a straightforward genotype-phenotype correlation. Five patients did not harbor mutations in MLC1, supporting the existence of at least one other MLC locus.
Objectif Sulfamides et inhibiteurs de DPP4 sont deux classes thérapeutiques prescrites usuellement chez les patients diabétiques de type 2 (DT2) en association avec la metformine. L’objectif ...principal de l’étude Odyssée était de démontrer la supériorité de la durée de maintien sans modification de la bithérapie metformine+sitagliptine « MetSita » par rapport à la bithérapie metformine+sulfamide « MetSu ». Patients et méthodes Odyssée, étude observationnelle prospective menée en France, en médecine générale, compare la durée de traitement avant modification (ajout, retrait ou substitution de molécule) chez des patients DT2, préalablement traités ou pas, ayant initié sur la base de la décision du médecin une bithérapie MetSita ou MetSu (méthode de Kaplan Meier, test du log-rank, modèle de Cox). Les patients ont été suivis sur une période allant jusqu’à 3 ans. Résultats Les différences à l’inclusion entre les bras « MetSita » (n = 1864) et « MetSu » (n = 733) étaient modestes (respectivement, âge : 62,4 vs 64,2 ans ; IMC : 30,2 vs 29,6 kg/m2 ; ancienneté du diabète : 6,4 vs 7 ans). Les niveaux d’HbA1c étaient identiques (7,6 %). Une supériorité de 11,9 mois de la durée de maintien du traitement « MetSita » par rapport à « MetSu » a été observée (médiane : 31,2 mois vs 19,3 mois ; logrank p < 0,0001). Cette différence persistait après ajustement sur les caractéristiques des patients et des médecins à l’inclusion. Une diminution similaire de l’HbA1c était notée dans les deux bras (− 0,6 %) et l’incidence des hypoglycémies ressenties jusqu’à la modification de la bithérapie était plus élevée dans le bras « MetSu » (19,4 % vs 9,4 %). Conclusion Menée dans les conditions de vie réelle, l’étude Odyssée met en évidence qu’une bithérapie associant metformine et sitagliptine est maintenue sans modification plus longtemps qu’une bithérapie associant metformine et sulfamides, avec une moindre incidence d’hypoglycémies symptomatiques et une efficacité biologique similaire.
Objectif L’étude observationnelle prospective Odyssée réalisée chez des médecins généralistes (MG) avait pour objectif de décrire les schémas thérapeutiques et les caractéristiques des patients ...diabétiques de type 2 (DT2) traités par sitagliptine en pratique courante. Patients et méthodes Ont été inclus des patients DT2 ayant initié entre 2009 et 2010, sur la base de la décision du MG, un traitement par sitagliptine quel que soit le schéma thérapeutique. La représentativité des MG (n = 702) et des patients (n = 2709) a été vérifiée sur les données CNAM 2009 (sexe, âge, répartition géographique) et la base patients LPD 2009-2010 : les différences relevées étaient mineures et de faible pertinence clinique. Résultats Sur 2709 patients, 68,8 % étaient sous bithérapie avec metformine (« SitaMet »), 10,6 % en monothérapie (« SitaSeule »). Les autres schémas rassemblés en un groupe « SitaAutre » comprenaient essentiellement une trithérapie avec metformine+sulfamide (7,4 %) et l’association avec l’insuline (2,9 %). Les patients « SitaSeule », « SitaMet » et « SitaAutre » avaient respectivement en moyenne 64,4/62,4/66,0 ans, une ancienneté de diabète de 4,9/6,4/10,8 ans, une HbA1c à 7,1 %/7,5 %/7,8 %, un IMC de 29,6/30,2/30,3 kg/m2 . Un traitement antidiabétique antérieur était retrouvé chez 94,6 % des patients « SitaMet » (dont metformine : 90,6 % des cas) ; 74,5 % des patients « SitaSeule » (dont metformine : 54,2 %) ; 99,6 % des patients « SitaAutre » (majoritairement bithérapie metformine+sulfamide). La prévalence des facteurs de risque et complications allait croissant entre les groupes « SitaSeule », « SitaMet » et « SitaAutre » : dyslipidémie (respectivement 62,9 %/67,2 %/72,2 %), hypertension (63,6 %/68,4 %/75,0 %), rétinopathie (2,4 %/3,8 %/6,2 %), antécédents vasculaires (20,3 %/19,1 %/26,0 %), insuffisance rénale (4,5 %/1,9 %/5,7 %). Conclusion Les résultats sur cette large population de patients DT2 traités par sitagliptine suggèrent que les MG ont adapté leurs pratiques aux indications de l’AMM et utilisent la sitagliptine principalement en bithérapie avec metformine, les autres associations étant attribuées à des patients à un stade plus avancé de la maladie.
Objectif Comparer deux stratégies d’intensification du traitement anti-diabétique chez des diabétiques de type 2 (DT2) insuffisamment contrôlés par metformine seule. Patients et méthodes Les patients ...(N = 653 ; HbA1c = 8,2 %) ont été randomisés vers une stratégie orale (SO) en ajoutant à la metformine sitagliptine 100 mg/jour ou une stratégie injectable (SI) en ajoutant liraglutide d’abord 0,6 mg/jour, puis 1,2 mg/jour après une semaine. Après 12 semaines, chez les patients avec HbA1c ≥ 7,0 %, le traitement a été intensifié : SO en ajoutant glimépiride ; SI en augmentant le liraglutide à 1,8 mg/jour. Le critère principal (étude de non infériorité ; marge préspécifiée 0,4 %) était la variation de l’HbA1c entre inclusion et semaine 26 dans la population Per Protocole (PP) Résultats Dans la population PP (SO n = 269 ; SI n = 253), le traitement a été intensifié à S12 chez respectivement 50,2 % et 28,5 % des patients. À S26, la variation de l’HbA1c par rapport à l’inclusion était : SO, – 1,3 % (IC à 95 % : – 1,4, – 1,2) et SI, – 1,4 % (– 1,5, – 1,3) ; le critère de non-infériorité a été atteint. Les pourcentages de patients avec HbA1c < 7,0 % à S26 étaient : SO 62,8 %, SI 72,3 %. L’incidence d’événements indésirables gastro-intestinaux était significativement plus élevée dans SI (32,7 % vs 10,7 %) en particulier nausées, vomissements et diarrhées (p < 0,05). L’incidence d’hypoglycémies était significativement plus élevée dans SO (12 % vs 4 % : (p < 0,001). Le poids a diminué de - 2,8 kgs (SI), de - 0,4 kgs (SO). Conclusion Chez des patients DT2 insuffisamment contrôlés par metformine seule, une stratégie orale d’intensification thérapeutique en ajoutant sitagliptine, puis glimépiride si besoin, présente la même efficacité en termes d’HbA1c qu’une stratégie injectable d’intensification thérapeutique par le liraglutide, avec une tolérance globalement comparable.
To report clinical, neuroradiologic, neurophysiologic, and genetic findings on 16 patients from 11 unrelated families with a remarkable uniform phenotype characterized by infantile ascending ...hereditary spastic paralysis (IAHSP).
Sixteen patients from 11 families, originating from North Africa and Europe, who presented severe spastic paralysis and ascending progression were studied.
Spastic paraplegia started in the first 2 years of life in most patients and extended to the upper limbs by the end of the first decade. The disease progressed to tetraplegia, anarthria, dysphagia, and slow eye movements in the second decade. The clinical course showed a long survival and preservation of intellectual skills. Clinical, neuroradiologic, and neurophysiologic findings were consistent with a relatively selective early involvement of the corticospinal and corticobulbar pathways. No signs of lower motor neuron involvement were observed, whereas motor evoked potentials demonstrated predominant involvement of the upper motor neurons. MRI was normal in young patients but showed brain cortical atrophy in the oldest, predominant in the motor areas, and T2-weighted bilateral hyperintense signals in the posterior arm of the internal capsule. The ALS2 gene, recently found mutated in consanguineous Arabic families with either an ALS2 phenotype or a juvenile-onset primary lateral sclerosis, was analyzed. Alsin mutations were found in only 4 of the 10 families, whereas haplotype analysis excluded the ALS2 locus in one family.
The syndrome of IAHSP is genetically heterogeneous, and no clinical sign can help to distinguish patients with and without Alsin mutations.
To describe clinical and neuropathologic studies and linkage analysis on two sisters with a severe form of leukodystrophy.
A detailed study was performed on the second sister. Genotyping markers for ...chromosome 3, including eight additional markers surrounding the vanishing white matter (VWM) locus, were used.
During the first year of life, two sisters developed a severe neurologic condition after an intercurrent infection. It was accompanied by irritability and stupor with rapid loss of their motor abilities. Results of extensive metabolic studies were negative. Brain MRI showed severe and diffuse abnormalities of the encephalic white matter. Neuropathologic examination showed a severe lack of myelin with diffuse vacuolating white matter lesions in the brain, associated with an increased density of oligodendrocytes and a reduced number of astrocytes on morphometric analysis. In sharp contrast, the spinal cord white matter was preserved. The affected sibpairs shared a common haplotype for a broad region in chromosome 3. They were homozygous between markers D3S1565 and D3S3669, including the VWM locus.
This condition is an unusual variant of childhood ataxia with diffuse central hypomyelination (CACH)/VWM, with characteristic shrinking and perivascular clustering of astrocytes. Haplotype analysis suggests that this variant is allelic to the VWM locus located on chromosome 3q27.
