The monitoring of ecosystems and forests is an urgent requirement in the current framework of global change. It is particularly necessary on oceanic islands where their rich biodiversity is highly ...vulnerable, with many narrow-ranged endemic species. Quantifying and mapping forest health through key ecological variables are essential steps for management, but it will also be challenging and may require a lot of resources. Remote sensing has the potential to be a very useful tool to assess the development and conservation status of forests. We assessed the applicability of the light detection and ranging (LiDAR) on the laurel forests of La Gomera, making allometric equations for various measurements of the forest structure, linking field inventory from 2019 and 2017 LiDAR data through standard linear regressions. Decision trees and logistic regressions were also used to assess the performance of LiDAR in the recognition of young-growth and old-growth laurel forests. The obtained allometric models were a good fit in general and their predictions were in line with already known data. Likewise, decision tree and logistic regression to distinguish young-growth and old-growth forests had a similar performance in both cases, with a high to medium-high degree of accuracy. Therefore, LiDAR was revealed to be a useful tool for the monitoring of the laurel forest by the managers.
Post-fire salvage logging (SL) is a common management action that involves the harvesting of burnt trees. As a consequence, a large amount of biological legacies in the form of logs and other coarse ...woody debris are removed from the post-fire habitat, creating a more simplified landscape. Therefore, SL could act as an additional disturbance over that produced by fire. In this study, we seek to determine the effect of SL on the regeneration of the bryophyte community of a laurel forest from the Canary Islands (Spain). We hypothesized that SL will act as an additional disturbance and, consequently, salvaged areas will have a higher difference in community composition with respect to a reference ecosystem (RE). Mosses and liverworts were sampled 22 months after the salvage operations in salvaged plots, non-salvaged, and in an RE represented by areas of the original forest. Species richness did not differ between salvage and non-salvaged treatments. However, multivariate analysis and species-indicator analysis showed that non-salvaged plots had a composition closer to that of the RE, with a higher proportion of closed-canopy, perennial, and long-lived species, as well as some epiphytes. By contrast, salvaged plots were dominated by early-successional terrestrial species and species preferring open habitats. We conclude that post-fire SL represents an additional disturbance that further delays succession, a result that is consistent with previous studies using other taxonomic groups. SL should therefore be avoided or, if implemented, the possibility of leaving part of the post-fire biological legacies in situ should be considered.
Primary forests, defined here as forests where the signs of human impacts, if any, are strongly blurred due to decades without forest management, are scarce in Europe and continue to disappear. ...Despite these losses, we know little about where these forests occur. Here, we present a comprehensive geodatabase and map of Europe's known primary forests. Our geodatabase harmonizes 48 different, mostly field-based datasets of primary forests, and contains 18,411 individual patches (41.1 Mha) spread across 33 countries. When available, we provide information on each patch (name, location, naturalness, extent and dominant tree species) and the surrounding landscape (biogeographical regions, protection status, potential natural vegetation, current forest extent). Using Landsat satellite-image time series (1985-2018) we checked each patch for possible disturbance events since primary forests were identified, resulting in 94% of patches free of significant disturbances in the last 30 years. Although knowledge gaps remain, ours is the most comprehensive dataset on primary forests in Europe, and will be useful for ecological studies, and conservation planning to safeguard these unique forests.
Evidence-Based Practice (EBP) is a cost-effective approach for improving the quality of clinical care and implementing only well-tested evidence. Health professions, especially physiotherapy, must ...embrace EBP principles. This paper presents normative data from the Spanish physiotherapist population using the Health-Sciences Evidence-Based Practice questionnaire and explores EBP clusters/profiles of professionals in practice. An intentional sample of 419 practicing physiotherapists was recruited from the Spanish Professional Council of Physiotherapy. Participants completed a cross-sectional online survey with 60 Likert items (scale 1-10) measuring 5 dimensions: 1) Beliefs and attitudes, 2) Results from literature, 3) Professional practice, 4) Assessment of results, and 5) Barriers and Facilitators. The protocol also included sociodemographic, training, and practice-related contrast variables. Normative data were estimated and tabulated for each dimension and then a K-means clustering procedure was implemented using the contrast variables. Results for normative data showed, in descending order, the following 50th percentile values for the five EBP factors: Beliefs and attitudes (8.25), Professional practice (8.00), Assessment of results (7.42), Results from literature (6.71), and EBP Barriers and Facilitators (5.17); all expressed on a scale of 1 to 10. Academic degree, EBP training level, and work time shared in healthcare activity, research, or teaching activity were all statistically significant for discriminating EBP dimension scores. Finally, six different clusters showed that when EBP level is low, the scores in all dimensions are equally low, and vice-versa. The EBP dimensions "Beliefs and attitudes", "Professional practice", and "Evaluation of results" obtained better normative scores overall than "Search for bibliographic evidence and its inclusion in practice" and especially "Perception of EBP barriers", which had the worst score. Normative data are useful for comparing individual scores and the reference population, and information about clusters will enable appropriate global EBP intervention programs to be designed and implemented.

Obesity has increased in recent years with consequences on diabetes and other comorbidities. Thus, 1 out of 3 diabetic patients suffers cardiovascular disease (CVD). The network among glucose, immune ...system, endothelium and epicardial fat has an important role on pro-inflammatory and thrombotic mechanisms of atherogenesis. Since semaglutide, long-acting glucagon like peptide 1- receptor agonist (GLP-1-RA), a glucose-lowering drug, reduces body weight, we aimed to study its effects on human epicardial fat (EAT), aortic endothelial cells and neutrophils as atherogenesis involved-cardiovascular cells.
EAT and subcutaneous fat (SAT) were collected from patients undergoing cardiac surgery. Differential glucose consumption and protein cargo of fat-released exosomes, after semaglutide or/and insulin treatment were analyzed by enzymatic and TripleTOF, respectively. Human neutrophils phenotype and their adhesion to aortic endothelial cells (HAEC) or angiogenesis were analyzed by flow cytometry and functional fluorescence analysis. Immune cells and plasma protein markers were determined by flow cytometry and Luminex-multiplex on patients before and after 6 months treatment with semaglutide.
GLP-1 receptor was expressed on fat and neutrophils. Differential exosomes-protein cargo was identified on EAT explants after semaglutide treatment. This drug increased secretion of gelsolin, antithrombotic protein, by EAT, modulated CD11b on neutrophils, its migration and endothelial adhesion, induced by adiposity protein, FABP4, or a chemoattractant. Monocytes and neutrophils phenotype and plasma adiposity, stretch, mesothelial, fibrotic, and inflammatory markers on patients underwent semaglutide treatment for 6 months showed a 20% reduction with statistical significance on FABP4 levels and an 80% increase of neutrophils-CD88.
Semaglutide increases endocrine activity of epicardial fat with antithrombotic properties. Moreover, this drug modulates the pro-inflammatory and atherogenic profile induced by the adiposity marker, FABP4, which is also reduced in patients after semaglutide treatment.

Given advanced age, comorbidities, and immune dysfunction, chronic lymphocytic leukemia (CLL) patients may be at particularly high risk of infection and poor outcomes related to coronavirus disease ...2019 (COVID-19). Robust analysis of outcomes for CLL patients, particularly examining effects of baseline characteristics and CLL-directed therapy, is critical to optimally manage CLL patients through this evolving pandemic. CLL patients diagnosed with symptomatic COVID-19 across 43 international centers (n = 198) were included. Hospital admission occurred in 90%. Median age at COVID-19 diagnosis was 70.5 years. Median Cumulative Illness Rating Scale score was 8 (range, 4-32). Thirty-nine percent were treatment naive (“watch and wait”), while 61% had received ≥1 CLL-directed therapy (median, 2; range, 1-8). Ninety patients (45%) were receiving active CLL therapy at COVID-19 diagnosis, most commonly Bruton tyrosine kinase inhibitors (BTKi's; n = 68/90 76%). At a median follow-up of 16 days, the overall case fatality rate was 33%, though 25% remain admitted. Watch-and-wait and treated cohorts had similar rates of admission (89% vs 90%), intensive care unit admission (35% vs 36%), intubation (33% vs 25%), and mortality (37% vs 32%). CLL-directed treatment with BTKi's at COVID-19 diagnosis did not impact survival (case fatality rate, 34% vs 35%), though the BTKi was held during the COVID-19 course for most patients. These data suggest that the subgroup of CLL patients admitted with COVID-19, regardless of disease phase or treatment status, are at high risk of death. Future epidemiologic studies are needed to assess severe acute respiratory syndrome coronavirus 2 infection risk, these data should be validated independently, and randomized studies of BTKi's in COVID-19 are needed to provide definitive evidence of benefit.
•Both watch-and-wait and treated CLL patients have high mortality rates when admitted for COVID-19.•Receiving a BTKi for CLL at COVID-19 diagnosis severe enough to require hospitalization did not influence case fatality rate in this study.

Smoking and second‐hand smoke (SHS) exposure have been recently linked to a higher risk of breast cancer in women. The aim of this work is to estimate the number of deaths and disability‐adjusted ...life years (DALYs) from breast cancer attributable to these two risk factors in the European Union (EU‐28) in 2017. The comparative risk assessment method was used. Data on prevalence of smoking and SHS exposure were extracted from the Eurobarometer surveys, relative risks from a recent meta‐analysis, and data on mortality and DALYs from breast cancer were estimated from the Global Burden of Disease, Injuries and Risk Factors Study. In 2017, 82 239 DALYs and 3354 deaths from breast cancer in the EU‐28 could have been avoided by removing exposure to these two risk factors (smoking and SHS exposure). The proportion of DALYs from breast cancer lost respectively from smoking and SHS exposure was 2.6% and 1.0%, although geographically distributed with significant heterogeneity. These results represent the first estimates of breast cancer burden in women attributable to smoking and SHS exposure for the EU‐28. It is important to increase awareness among women, health professionals and wider society of the association between smoking, SHS exposure and breast cancer, a relationship that is not widely recognised or discussed.

Here, the authors publish the first analysis estimating how many deaths and years of disability can be attributed to smoking. Using the comparable risk assessment method, they determined that eliminating smoking could have saved 60 733 years of life with disability and 2719 deaths from breast cancer. Given the burden of disease from breast cancer attributable to smoking, it is important to increase awareness among health professionals and public.
Agricultural productivity in the Peruvian region of Amazonas is being jeopardized by conflicts and inadequate land use, that are ultimately contributing to environmental degradation. Therefore, our ...aim is to assess land suitability for potato (Solanum tuberosum L.) farming in the Jucusbamba and Tincas microwatersheds located in Amazonas, in order to improve land-use planning and enhance the crop productivity of small-scale farmers. The site selection methodology involved a pair-wise comparison matrix (PCM) and a weighted multicriteria analysis using the Analytical Hierarchy Process (AHP) on selected biophysical and socioeconomical drivers. Simultaneously, land cover mapping was conducted using field samples, remote sensing (RS), geostatistics and geographic information systems (GIS). The results indicated that for potato crop farming, the most important criteria are climatological (30.14%), edaphological (29.16%), topographical (25.72%) and socioeconomical (14.98%) in nature. The final output map indicated that 8.2% (22.91 km2) was highly suitable, 68.5% (190.37 km2) was moderately suitable, 21.6% (60.11 km2) was marginally suitable and 0.0% was not suitable for potato farming. Built-up areas (archaeological sites, urban and road networks) and bodies of water were discarded from this study (4.64 km2). This study intends to promote and guide sustainable agriculture through agricultural land planning.
AbstractOur aim was to provide a systematic review of studies on the burden of disease due to second-hand smoke (SHS) exposure, reviewing methods, exposure assessment, diseases causally linked to ...SHS, health outcomes, and estimates available to date. A literature review of studies on the burden of disease from SHS exposure, available in PubMed and SCOPUS, published 2007–2018 in English language, was carried out following the PRISMA recommendations. Overall, 588 studies were first identified, and 94 were eligible. Seventy-two studies were included in the systematic review. Most of them were based on the comparative risk assessment approach, assessing SHS exposure using mainly surveys on exposure at home/workplaces. Diseases more frequently studied were: lung cancer, ischemic heart disease, stroke, chronic obstructive pulmonary disease, asthma and breast cancer in adults; lower respiratory tract infection, otitis media, asthma, sudden infant death syndrome and low birth weight in children. The SHS exposure assessment and the reported population attributable fractions (PAF) were largely heterogeneous. As an example, the PAF from lung cancer varied between 0.6% and 20.5%. Moreover, PAF were estimated applying relative risks and SHS exposures with no consistent definitions or with different age classes. The research gap on the SHS exposure burden is shrinking. However, estimates are not yet available for a number of countries, particularly the Middle Eastern and African countries, and not all diseases with the strongest evidence of causation, such as sudden infant death syndrome, have been explored. Moreover, in some cases the applied methodology revealed relatively low quality of data.
