While interleukins (IL)-1alpha and beta are thought to play an important role in malignant disease, little is still known about their expression in breast cancer. We have used reverse ...transcription-polymerase chain reaction, enzyme-linked immunosorbent assay (ELISA), and immunohistochemistry (IHC) to analyze the expression of IL-1alpha and beta in breast cancer tissues, and compared their expression to estrogen receptor (ER) status and grading. In breast cancer cell lines, we found an inverse correlation between IL-1alpha and beta gene expression and differentiation, and only one highly invasive tumor cell line expressed IL-1alpha protein, while IL-beta was not detectable. Breast cancer tissue expressed variable amounts of IL-1alpha and beta messenger RNA, but consistently high levels of IL-1 type I receptor. IL-1alpha protein was detectable in malignant epithelium and adjacent stroma in 88% of cases. IL-1alpha expression was correlated with poor differentiation (P= 0.002; r= 0.469) and decreasing ERalpha expression (P= 0.004; r=-0.387). Stromal IL-1alpha was confined to areas with low or absent ERalpha protein expression in adjacent tumor epithelium (P= 0.001; r=-0.457). Taken together, we have demonstrated a functional IL-1 system in breast cancer and observed an inverse correlation between IL-1alpha and sex steroid receptor expression. We suggest that the expression of IL-1alpha in poorly differentiated, ERalpha-negative tumors contributes to their malignant phenotype.

Stromal fibroblasts deriving from malignant tissue of 10 women with invasive breast cancer, and from normal breast tissue of 10 women with benign breast disorders, were subjected to differential complementary DNA Microarray Analysis by using a 2400 gene cDNA array. Gene expression results were validated by immunohistochemistry.
3. Results:
In a cDNA Array that allows to analyze the differential gene expression of more than 2400 genes, the mRNA expression of 135 genes were increased more than 2 fold in fibroblasts from malignant breast tumors. The majority of these genes encode tumor-promoting cytokines, transcription factors and cell-matrix associated proteins. The mRNA expression of 110 genes decreased to less than 0.5 fold. The remaining 2155 genes were not significantly altered. Immunohistochemistry for selected proteins performed on biopsies from breast cancer and normal breast tissues confirmed the clinical relevance of our findings.
4. Conclusion:
Breast cancer-derived stromal fibroblasts show a distinctive gene expression pattern that differentiates them from normal breast stroma. Our observation of increased expression of tumor promotion-associated genes even in the absence of adjacent malignant epithelium suggests that tumor stroma is comprised of a fibroblastic subpopulation that provides for a microenvironment which supports tumor growth and invasion.

Purpose:
Neoadjuvant chemotherapy is the treatment of choice for locally-advanced breast cancer and leads to down staging and improved BCT rates. While its efficacy is well established, considerably ...less is known about the most effective regimen. Experimental Design: We have performed a retrospective analysis of 132 breast cancer patients who had undergone neoadjuvant chemotherapy at our institution. Patients had either received a) anthracyclines („A“, n=35), b) anthracyclines and taxanes („AT“, n=55), or c) neither of the two compounds („NoA/T“, n=42). Clinical response, pathological response and survival were evaluated in each arm.
Results:
While all three regimen resulted in significant tumor regression, AT was most effective with a mean tumor shrinkage of 39% (ultrasound) and 41% (mammography) (Kruskal-Wallis, p=0.004, and p=0.027). Breast conservation was achieved in 75% by AT, in 49% by A, and in 19% by NoA/T (Kruskal-Wallis, p<0.001). The treatment groups did not differ in respect to pCR (Χ
2
-test, p=0.068), although higher cumulative anthracycline doses were predictive of pCR in multivariate analyses (p=0.022). While the mammographic and not the ultrasound-determined tumor diameter determined whether a woman underwent BCT, only an ultrasound-determined size reduction was predictive for DFS and OS (log rank, p=0.0093, and p=0.044, respectively). Other parameters that affected BCT rates were age (p=0.003), year of diagnosis (p≤0.001), presence of multifocal disease (p=0.032) and the cumulative anthracycline dose (p≤0.001).
Conclusion:
While the combination of anthracyclines and taxanes is most effective in achieving clinical remission and BCT, the cumulative anthracycline dose appears most important for achieving pCR.
Invasiv growth requires degradation of extracellular matrix. A number of Zn 2+ -dependent matrix metalloproteinases (MMPs) are associated with this process. In most cases these enzymes are not ...produced by the malignant epithelium itself but by adjacent breast stroma, suggesting an important role for cell-cell interactions.
Materials and methods:
We determined the expression patterns of MMP1, MMP2, MMP9 and MMP11 by immunohistochemistry in tissue arrays, containing 50 paraffin-embedded sets of tissues obtained from the malignant tumor of the breast, and 50 paraffin-embedded sets of tissues from metastatic lymph nodes (purchased from Biomax, Biomax Inc, Rockville, MD).
Results:
MMP2S protein expression was significantly higher in lymph node tumor tissue when compared to matched breast tumor tissue (10% vs. 2%; p=0.005, Chi Square Test). Furthermore, MMP9S was also significantly up-regulated in lymph node tumor tissue when compared to breast tumor tissue (78.3% vs. 94.8%, p=0.01 Chi Square test) and MMP9E showed a borderline significance in lymph node tumor tissue (92.6% vs. 73.5%, p=0.05). While in the subgroup breast tissue significant correlations were observed between all MMPs except MMP2S this associations were less pronounced in the subgroup lymph node.
Conclusions:
The increased protein expression of MMP2S, MMP9S and MMP9E in lymph node tissue is in line with other studies which suggested MMP2 and MMP9 expression in primary breast cancer tissue samples was associated with breast cancer progression and invasion 20–23.
1. Objective:
Breast cysts are the most common cause of benign breast masses.
Simple breast cysts do not need further evaluation, but complex breast cysts require additional assessment due to the ...potential presence of malignancy. However these complex cysts have rarely been examined and quantified according the associated cancer detection rate. Our study is the first investigation that evaluates the malignancy rate of complex breast cysts identified by histopathological results.
2. Methods:
Imaging findings of complex cysts were correlated retrospectively with clinical and pathologic outcomes.
3. Results:
151 complex breast cysts were found in 131 women. We detected a high malignancy rate of 13.9%. Sonographic criteria of a complex cyst such as thick cyst wall (p=0.0006), lobulation (p=0.01) or hyperechogenicity (p=0.04) were predictive of neoplasm. Two or more criteria combined were associated with a 13.6– fold higher risk of malignancy (p<0.0001). Menopause status, family history or personal history of breast cancer did not influence the detection rate of neoplasm.
4. Conclusion:
Based on our results we raise the issue for the need of surgical intervention, compared to observation by ultrasound of complex breast cysts to prevent a missed or delayed diagnosis of breast cancer.

Aromatasehemmer sind der Standard der endokrinen Therapie des Hormonrezeptor positiven, postmenopausalen Mammakarzinoms. Vor kurzem konnte gezeigt werden, dass der BMI signifikant die ...Effektivität von Anastrozol bei Brustkrebspatientinnen beeinflusst. In dieser prospektiven Studie haben wir untersucht, ob unterschiedliche Veränderungen der Serumhormonspiegel von normalgewichtigen und übergewichtigen Patientinnen während der endokrinen Therapie mit einem Aromatasehemmer ursächlich für diesen Einfluss des BMI sind.
Methode:
54 postmenopausale Hormonrezeptor positive Brustkrebspatientinnen wurden in diese prospektive Studie eingeschlossen. BMI sowie Serumhormonspiegel (FSH, LH, Östradiol, Insulin, und SHBG) wurden unmittelbar vor Beginn der Therapie mit einem Aromatasehemmer sowie 3 Monate später gemessen. Ebenso wurde ein Fragebogen bezüglich spezifischer Nebenwirkungen vor und nach 3 Monaten Therapie ausgefüllt.
Ergebnisse:
10 normalgewichtige und 44 übergewichtige Brustkrebspatientinnen mit einem Durchschnittsalter von 67 Jahren wurden eingeschlossen. Die Serum-Östradiolspiegel waren überwiegend unter der Nachweisgrenze sowohl vor als auch nach 3 monatiger Therapie mit dem Aromatasehemmer. Somit wurden LH und FSH als hochsensitive Surrogatparameter für die Serum-Östradiol Spiegel herangezogen. Übergewichtige Patientinnen hatten signifikant niedrigere LH und FSH Spiegel als normalgewichtige Patientinnen sowohl vor Beginn der Therapie (p=0,029 und p=0,012) als auch nach 3 monatiger Therapie mit dem Aromatasehemmer (p=0,032 und p=0,007). LH und FSH Spiegel stiegen nach 3 monatiger Therapie mit dem Aromatasehemmer bei normalgewichtigen aber nicht bei übergewichtigen Patientinnen an.
Es konnte kein Unterschied im Hinblick auf Nebenwirkungen zwischen normalgewichtigen und übergewichtigen Patientinnen festgestellt werden.
Schlussfolgerung:
Unterschiedliche Veränderungen der Serumhormonspiegel während der endokrinen Therapie mit Aromatasehemmern scheinen für den klinischen Einfluss des BMI auf die Effektivität der Aromatasehemmer verantwortlich zu sein.
Fragestellung:
Der Einsatz von Aromatasehemmern ist Standard in der adjuvanten Therapie des postmenopausalen, Hormonrezeptor-positiven Brustkrebs. Aufgrund der langen Einnahmedauer von bis zu 5 ...Jahren haben Nebenwirkungen wie Dyspareunie, atrophe Kolpitis u.ä., welche die Lebensqualität stark beeinflussen können, eine große Bedeutung. Die Behandlung dieser vaginalen Beschwerden kann derzeit nur symptomatisch erfolgen, da die Gabe von Hormonen kontraindiziert ist. In der vorliegenden prospektiven Studie wurde untersucht, ob die vaginale Applikation von Östriol eine sichere und effektive Therapie urogenitaler Nebenwirkungen von Aromatasehemmern darstellt.
Methode:
Eine Nüchternblutabnahme zur Hormonspiegelbestimmung wurde bei 10 postmenopausalen Hormonrezeptor-positiven Brustkrebspatientinnen unter Aromatasehemmertherapie vor und nach 2 Wochen täglicher Therapie mit 0,5mg vaginalem Östriol (Ovestin®-Ovula) durchgeführt. Östradiol, luteinisierendes Hormon (LH), Follikel stimulierendes Hormon (FSH), Progesteron, Insulin, C-peptid und SHBG wurden mittels Electro-Chemiluminescence Immunoassay bestimmt. Die Messung des Serumöstriols und Serumöstradiols erfolgte mittels Gas-Chromatografie und Massenspektrometrie (GC/MS).
Ergebnisse:
Die 2 Wochen dauernde Behandlung mit täglich vaginalem Östriol führte zu keinem Anstieg der Serumöstradiol- bzw. Serumöstriolspiegel, unabhängig ob die Messung mit Electro-Chemiluminescence Immunoassay oder mit GC/MS erfolgte. Es zeigte sich aber eine signifikante Senkung der FSH (p=0,012) und LH (p=0,022) Spiegel durch die Hormontherapie. 5 von 6 Brustkrebspatientinnen berichteten über Verbesserung der vaginalen Beschwerden durch die Therapie.
Schlussfolgerung:
Eine vaginale Östrioltherapie zur Linderung urogentialer Nebenwirkungen von Aromatasehemmern führt nicht zu einer Erhöhung der Serumöstrogenspiegel.
Fragestellung
: Tuberin wird als funktionelle Komponente des Tuberous Sclerosis Complex bezeichnet und übernimmt in der Zellzyklus-Regulation eine entscheidende Rolle ein, indem es den ...Cyclin-dependent-Kinase-Inhibitor p27 aktiviert. Rezente Publikationen deuten auf eine Beteiligung von Tuberin und p27 in der Karzinogenese des Mammakarzinoms hin. Diese beiden Proteine wurden allerdings noch nicht in sporadischem und BRCA-assoziierten Brustkrebs evaluiert. Die vorliegende Studie untersucht die beiden Tumor-Suppressor-Gene in benignem Brustgewebe, im sporadischem, sowie im hereditären Mammakarzinom.
Methode
: Wir haben die Expression von tuberin und p27 immunhistochemisch in 26 BRCA 1/2 Mutationsträgern, 53 passenden sporadischen Brustkrebspatientinnen ohne eine familiäre Brust- oder Eierstockkrebsbelastung, sowie in benignem Brustgewebe von 74 Patientinnen in einer nested case control studie untersucht.
Ergebnisse
: Tuberin und p27 waren beide signifikant stärker in benignem, verglichen mit malignem Gewebe, exprimiert (p=0,01 and p=0,03). Sie zeigten jedoch keine signifikant unterschiedliche Expression in sporadischem und BRCA-assoziiertem Brustkrebs. Tuberin und p27 waren positviv miteinander korreliert (p=0,0017, r=0,2527) und negativ mit malignem Gewebe korreliert (p27: p=0,0227, r=–0,1841; tuberin: p=0,0020, r=–0,2495). Weiterhin zeigte sich eine postive Korrelation von p27 Expression mit dem Estrogenrezeptor und dem Progesteronrezeptor, sowie eine negative Korrelation mit der Tumorgröße. Die Expression von Tuberin und p27 im Mammakarzinom war in unserer Untersuchung weder mit dem krankheitsfreien- oder dem Gesamtüberleben, noch mit der Zeit bis zur Metastasierung assoziiert.
Schlussfolgerung
: Es konnte gezeigt werden, dass sich kein signifikanter Unterschied in der Tuberin und p27 Expression, zwischen dem BRCA assoziierten und dem sporadischen Mammakarzinom findet. Ebenso fand sich keine Assoziation der Proteinexpression mit klinischen Outcome-Parametern. Dies könnte auf die zu geringe Fallzahl zurückzuführen sein. Interessanterweise fand sich im sporadischen Mammakarzinom eine positive Korrelation zwischen p27 und Hormonrezeptorstatus sowie Tumorgröße. Weitere Untersuchungen mit größeren BRCA Fallzahlen sind erforderlich, um die Interaktionen von Tuberin und p27 besser zu verstehen und neue Strategien zur Hemmung des Brustkrebswachstums zu implementieren.
