To present cases of coexisting sarcoidosis and Sjogren's syndrome (SS).
We analyzed the clinical data of our 464 patients with SS with a clinical picture suggesting coexisting SS and sarcoidosis. We ...followed them for several years.
We found 5 patients with coexisting SS and sarcoidosis. In 3 patients, pure sarcoidosis could mimic SS.
In our experience, the frequency of sarcoidosis in SS is much higher than in the general population. One of the European criteria is that before the diagnosis of SS is made, the presence of sarcoidosis must be excluded. From our experience of 5 cases we determined that the 2 diseases can coexist, which indicates the value of these criteria.
This article seeks to study whether caseworkers and community social workers subscribe to similar social work values, such as promoting social change and social justice, by comparing the social and ...professional ideologies of 123 caseworkers and 113 community social workers in Israel. The findings reveal that the community social workers consistently showed a greater social orientation than the caseworkers. They were more inclined than the caseworkers to believe that the government is responsible for social provision and to support universal benefits. They were less inclined to believe that the welfare state has negative effects on its recipients and placed more emphasis on the social-structural causes of poverty. They attributed greater importance than the caseworkers to the goal of promoting social justice and were more inclined to believe that social workers should engage in community development and policy practice. However, alongside the differences, members of both groups attributed importance to social justice and government responsibility for welfare, and favoured social-structural explanations of poverty.
Readability and usability problems with patient information leaflets continue to be reported despite long-standing recognition of their existence and the availability of guidelines for developing ...health education materials. This exploratory study examined possible causes for such problems, based on interviews with professionals who developed leaflets in large health organizations. Findings suggest that readability is an important concern for developers, but that complex organizational processes are involved in creating leaflets, and that developers face a need to cope with organizational politics, goal conflicts and various other pressures. Six factors were identified, i.e. (1) initiators, (2) intended users and contexts of use, (3) goals, (4) work flow and content decisions, (5) readability considerations, and (6) evaluation practices, that can adversely affect the content, organization, and resulting comprehensibility and usability of leaflets. It is suggested to adopt a broad ecological view of the environments in which patient education materials are created and deployed. Implications for practice and for future related research are discussed.
To assess the safety of galantamine in subjects with mild cognitive impairment (MCI), the ability of galantamine to benefit cognition and global functioning in subjects with MCI, and the ability of ...galantamine to delay conversion to dementia.
In two studies, 2,048 subjects, 990 in Study 1 and 1,058 in Study 2, with a Clinical Dementia Rating (CDR) = 0.5, CDR memory score > or =0.5, without dementia were randomized to double-blind galantamine (16-24 mg/day) or placebo for 24 months. Primary efficacy endpoint at month 24 was number (%) of subjects who converted from MCI to dementia (CDR > or = 1.0).
There were no differences between galantamine and placebo in 24-month conversion rates (Study 1: 22.9% galantamine vs 22.6% placebo, p = 0.146; Study 2: 25.4% galantamine vs 31.2% placebo, p = 0.619). Mean CDR-sum of boxes declined less with galantamine than placebo at 12 and 24 months in Study 1 (p = 0.024 12 months and p = 0.028 24 months), but not in Study 2 (p = 0.662 12 months and p = 0.056 24 months). Digit Symbol Substitution Test scores improved with galantamine in Study 1 at 12 months and in Study 2 at 24 months (Study 1: p = 0.009 month 12 and p = 0.079 Month 24; Study 2: p = 0.154 month 12 and p = 0.020 month 24). The most frequently reported adverse event was nausea (galantamine, 29%; placebo, 10%). Serious AEs occurred in 19% of each group. Mortality of the cohort after retrospectively determining the status of subjects (98.3%) at 24 months was 1.4% (galantamine) and 0.3% (placebo); RR (95% CI), 1.70 (1.00, 2.90).
Galantamine failed to significantly influence conversion to dementia. Galantamine was generally well tolerated. Whereas recorded mortality was greater in the galantamine group than in the placebo group in the original per-protocol assessment, a post hoc analysis of the cohort was consistent with no increased risk.
Using aggregate time-series data from post-war Hungary, we investigated the effect of child-related benefits and pensions on overall fertility and fertility by birth order. The results indicate ...moderate effects that are robust across a wide range of specifications. According to our estimates, a 1-per-cent increase in child-related benefits would increase total fertility by 0.2 per cent, while the same increase in pensions would decrease fertility by 0.2 per cent. The magnitude of both effects increases by birth order; this is more robust for child-related benefits.
To correlate injury grading with computed tomography (CT) to treatment outcome.
Forty-eight patients with proved blunt hepatic injuries underwent dynamic CT with contrast material before treatment. ...The injuries were classified with a five-point CT-based grading system. The CT injury grades and the amount of hemoperitoneum seen on CT scans were compared with the clinical and surgical findings, radiologic follow-up, and outcome.
Hepatic injuries were classified as CT grade 1 in two patients, grade 2 in 15 patients, grade 3 in 21 patients, and grade 4 in 10 patients. Conservative treatment was performed in 38 patients, including 15 patients with grade 3 and 10 patients with grade 4 injuries; 10 of these patients also had major hemoperitoneum (> 500 mL). Conservative treatment was successful in 37 patients but was complicated in two patients by development of an arteriovenous fistula or a biloma and failed in one patient because of delayed hepatic rupture. Exploratory laparotomy was performed in 10 patients but revealed marked active hepatic bleeding in only one patient.
Injury grading with CT may reflect the degree of hepatic parenchymal damage but does not indicate patients in whom complications may develop or surgery is necessary. Monitoring of conservative treatment with CT allows detection of resultant complications.

The z -GAL survey observed 137 bright Herschel -selected galaxies with the IRAM Northern Extended Millimeter Array (NOEMA) to measure their spectroscopic redshift. It detected several multiple ...sources and secured the spectroscopic redshift of a total of 165 individual galaxies over the range 0.8 < z < 6.5. The wealth of information provided by the NOEMA spectra allowed us to study the physical properties of these galaxies. By modelling their spectral energy distributions, we determined their dust mass, M dust , temperature, T dust , and emissivity index, β dust and found a significant anti-correlation between T dust and β dust . Using the detected 12 CO lines, we derived the molecular gas mass of the z -GAL sources. Combining the molecular lines and the dust continuum emission, we computed their gas depletion timescales, τ dep : the z -GAL sample consists mainly of powerful starbursts with τ dep in the range between 0.1 and 1.0 Gyr, while only ~ 25% belongs to the ‘main sequence’ of star forming galaxies.
We argue that the institutional composition of funding consumption in the two dependent sections of the lifecycle, childhood and old age, are different. To put it sharply, children are raised by ...their parents, the elderly rely on society. Since the reallocation of resources within and between households are not registered in National Accounts, the majority of the resources transferred to children are not visible in contrast to resources flowing to the elderly, which are almost entirely observed in public statistics. For our analysis we apply a recent extension of National Accounts, called the National Transfer Accounts, which include private transfers; and a further, experimental extension, the National Time Transfer Accounts, which quantifies the value of time transferred among household members in the form of unpaid household labor. We show that less than one third of the full transfer package flowing to children is registered in the National Accounts and another roughly one third is made visible by the National Transfer Accounts. The remaining one third, which is the value of parents caring for their children, is made visible by the National Time Transfer Accounts. The corresponding shares in funding old age are quite different: 90 percent is observed in public statistics and the two accounting extensions unfold only a bit less than 10 percent.
