Chronic inflammation plays a major role in lung deterioration in cystic fibrosis (CF) patients and anti-inflammatory strategies have beneficial effects. To study the changes seen after a one-year ...course of low-dose dietary supplements with a mixture of fatty acids in adult patients with CF in chronic inflammation, pulmonary status (lung function, respiratory exacerbations and antibiotic consumption), quality of life and anthropometric parameters.
Seventeen adult subjects with CF received 324 mg of eicosapentaenoic, 216 mg of docosahexaenoic, 480 mg of linoleic and 258 mg of gammalinolenic acid daily. We assessed inflammation markers, spirometry parameters, number and severity of respiratory exacerbations, antibiotic consumption, quality of life (St George's QoL), anthropometric parameters and serum phospholipid fatty acid composition.
At the end of the treatment period TNF alpha levels fell significantly and its soluble receptors (60 and 80) rose significantly. Levels of IgG and IgM anti-oxidized LDL antibodies fell significantly. Spirometry improved significantly. Annual respiratory exacerbations and days of antibiotic treatment fell significantly. The improvement in QoL was not significant. Serum levels of docosahexaenoic, total omega-3 and linoleic acid rose significantly and more favourable profiles were seen in monoenoic acids, arachidonic acid and the arachidonic/docosahexaenoic ratio. The fat-free mass and hand grip dynamometry improved significantly.
Low-dose supplements of n-3 and gammalinolenic fatty acids over a long period (one year) appears to improve pulmonary status (lung function, respiratory exacerbations and antibiotic consumption), inflammatory and anthropometric parameters in adults with CF.
The relative importance of the usual diet in serum phospholipids in subjects with cystic fibrosis (CF) has been poorly studied. To compare the fatty acid profile in serum phospholipids from adult CF ...subjects with that of healthy subjects, and determine the role of the normal diet in this profile, we studied thirty-seven adult CF subjects with stable pulmonary disease and thirty-seven healthy controls matched for age, sex and nutritional status. A dietary questionnaire was obtained, anthropometric data were recorded, and the fatty acid profile measured by GLC. Compared with the controls, the percentages of myristic, palmitoleic and stearic acids and total MUFA were significantly higher in the CF group, and DHA, linoleic acid, total PUFA and n−6 fatty acids were significantly lower in the CF group. The CF subjects with worse pulmonary function and with pancreatic insufficiency had significantly lower levels of linoleic and n−6 fatty acids. The total energy intake was significantly higher in the CF subjects, although the energy distribution in the CF subjects and the controls was not different for the carbohydrates, lipids and proteins. No differences were detected in fat intake for MUFA (51 (sd 4) v. 52 (sd 4) %) or saturated fatty acids (33·5 (sd 5) v. 31·2 (sd 3·8) %), but the PUFA were slightly lower in the CF subjects (15·4 (sd 4·5) v. 17·4 (sd 4·2) %; P=0·02). The usual dietary intake of fatty acids by adult CF subjects does not appear to explain the difference in the fatty acid profile compared with controls. This suggests an abnormal fatty acid metabolism in CF subjects.
El libro colectivo que se publica y que tiene ahora tiene en sus manos es fruto del trabajo desarrollado en el marco del Plan de Apoyo a la Investigación, Desarrollo Tecnológico e Innovación del ...Vicerrectorado de Investigación de la Universidad de Jaén, bajo el patrocinio de la Fundación Caja Rural de Jaén. En efecto, en virtud de la convocatoria del bienio 2014-2015 se nos concedió la ayuda correspondiente para el desarrollo de la actividad investigadora denominada «La mediación como alternativa a la judicialización de asuntos mercantiles y administrativos. Aspectos sustantivos y procesales» (Código UJA2014/06/04). Han sido numerosas las actividades desarrolladas por los miembros de este proyecto durante estos dos últimos años.
Quizás, por la repercusión que tuvo, sea destacable la organización del Congreso Internacional que, sobre la mediación como alternativa a judicialización de asuntos civiles, mercantiles y administrativos, se desarrolló en la Universidad de Jaén en marzo de 2017. A raíz de este evento y como consecuencia del trabajo de investigación llevado a cabo anteriormente, han sido numerosas las personas con reconocida competencia en la materia que han querido colaborar para elaborar la obra que ahora ve la luz bajo el título genérico de La mediación como método para la resolución de conflictos. El libro se divide en cinco partes diferenciadas en donde se pretende profundizar en aspectos importantes de la vida diaria en donde podría aplicarse la mediación. En concreto, después de analizar la mediación desde una perspectiva general, se entra a estudiar diferentes aspectos de la mediación civil, mercantil y concursal.
El tercer bloque está dedicado al difícil encaje de la mediación en el Derecho administrativo y en el proceso contencioso-administrativo. Después se llevan a cabo estudios concretos sobre la mediación en materia de consumo, en el ámbito laboral y en el penal. Por último, se cierra con una serie de aportaciones de Derecho comparado, analizando y dando noticia de la regulación de la mediación en Italia, Reino Unido, Portugal y Colombia, además de estudiar la mediación internacional en el caso concreto del proceso de Oslo en el conflicto palestinoisraelí. Resumen Introducción La afectación pulmonar es la causa más importante de morbimortalidad en la fibrosis quística (FQ) y la inflamación-infección bronquial crónica condiciona el deterioro progresivo ...de la función pulmonar. Los tratamientos destinados a reducir la respuesta inflamatoria podrían ser beneficiosos. Objetivos Valorar el efecto de la suplementación oral durante un año en pacientes adultos con FQ, de una combinación de ácidos grasos sobre parámetros respiratorios, antropométricos, inflamatorios, de calidad de vida y sobre el perfil de AG de los fosfolípidos séricos (AGFS). Pacientes y método Diecisiete pacientes recibieron diariamente durante un año: 324 mg de ácido eicosapentaenoico, 216 mg de ácido docosahexaenoico, 480 mg de ácido linoleico y 258 mg de ácido gammalinolénico. Se valoraron marcadores inflamatorios, parámetros espirométricos, reagudizaciones respiratorias, consumo de antibióticos, calidad de vida (St. George), antropometría y los AGFS. Resultados Al final del tratamiento se observó, de forma significativa, una disminución de reagudizaciones y del consumo de antibióticos con mejoría de los parámetros espirométricos, de la masa magra y la dinamometría. Concomitantemente, se observó una reducción significativa de los anticuerpos anti-LDL oxidada (inmunoglobulina Ig G e IgM) y de los niveles séricos del factor de necrosis tumoral α, así como un incremento de sus receptores solubles. Los niveles de AGFS mejoraron con un aumento significativo de DHA, AG-omega-3 y ácido linoleico, y un descenso de monoinsaturados y del cociente araquidónico/DHA. Conclusiones La suplementación con una mezcla definida de AG durante un año parece mejorar los parámetros respiratorios (espirométricos, reagudizaciones y consumo de antibióticos), inflamatorios y antropométricos en pacientes adultos con FQ.

La afectación pulmonar es la causa más importante de morbimortalidad en la fibrosis quística (FQ) y la inflamación-infección bronquial crónica condiciona el deterioro progresivo de la función pulmonar. Los tratamientos destinados a reducir la respuesta inflamatoria podrían ser beneficiosos.
Valorar el efecto de la suplementación oral durante un año en pacientes adultos con FQ, de una combinación de ácidos grasos sobre parámetros respiratorios, antropométricos, inflamatorios, de calidad de vida y sobre el perfil de AG de los fosfolípidos séricos (AGFS).
Diecisiete pacientes recibieron diariamente durante un año: 324

Abstract Objective To identify different markers in order to validate the assessment of dietary intake in adults with cystic fibrosis (CF). Design Cross-sectional survey. Setting Tertiary care. ...Subjects We assessed 37 adults with CF whose nutritional and respiratory condition was stable and 37 healthy adults, matched for age, sex, and nutritional status. Interventions A consecutive, 7-day, prospective dietary survey was given to all the participants. Anthropometric variables were measured and a fasting blood sample was drawn to measure the composition of the serum phospholipid fatty acids by gas chromatography. We also measured fecal fat and nitrogen at 72 hours and 24-hour urine nitrogen. Results The ratio of energy intake to basal metabolic rate expenditure was significantly greater in the patients (2.1±0.4) than the controls (1.79±0.4) and the percentage of patients with the ratio of energy intake to basal metabolic rate lower than 1.55 was 24% in the controls (n=9) vs 8% in the patients (n=3). Fecal nitrogen correlated significantly with total energy and the intake of carbohydrates, lipids, and proteins. In the patients, total dietary energy and fat and protein intake correlated positively with fecal fat. The protein and fat intake and fecal nitrogen and fat correlated significantly with urine nitrogen. In the controls, significant correlations were seen between different parameters of intake and the percentage of certain serum phospholipid fatty acids. These correlations in the patients were either absent or less marked. Conclusions Use in persons with CF of the energy intake to basal metabolic rate ratio, measurement of fecal fat and nitrogen at 72 hours and of urine nitrogen may be useful to validate dietary surveys. The serum phospholipid fatty acid profile, however, may be less useful for this purpose in these patients.
