Pityriasis rubra pilaris (PRP) is a rare papulosquamous skin disorder, which is phenotypically related to psoriasis. Some familial PRP cases show autosomal dominant inheritance due to
mutations ...leading to increased nuclear factor κB (NFκB) activation. Moreover,
polymorphisms have also been implicated in sporadic PRP. A Hungarian PRP patient with childhood onset disease showing worsening of the symptoms in adulthood with poor therapeutic response underwent genetic screening for the CARD14 gene, revealing four genetic variants (rs117918077, rs2066964, rs28674001, and rs11652075). To confirm that the identified genetic variants would result in altered NFκB activity in the patient, functional studies were carried out. Immunofluorescent staining of the NFκB p65 subunit and NFκB-luciferase reporter assay demonstrated significantly increased NFκB activity in skin samples and keratinocytes from the PRP patient compared to healthy samples. Characterization of the cytokine profile of the keratinocytes and peripheral blood mononuclear cells demonstrated that the higher NFκB activation in PRP cells induces enhanced responses to inflammatory stimuli. These higher inflammatory reactions could not be explained solely by the observed
or other inflammation-related gene variants (determined by whole exome sequencing). Thus our study indicates the importance of investigations on other genetic factors related to PRP and their further functional characterization to bring us closer to the understanding of cellular and molecular background of disease pathogenesis.
During the last decades, the prevalence of allergy has dramatically increased. Allergen-specific immunotherapy is the only currently available medical intervention that has the potential to affect ...the natural course of the disease, but there are still many questions and unmet needs hindering its widespread use to fulfill its treatment potential and maximize its benefits for the society. To provide a comprehensive phenome-wide overview in sublingual immunotherapy, using ragweed allergy as a target, we planned and carried out a longitudinal, prospective, observational, open-label study (DesensIT). In this paper we present challenges of using deep and comprehensive phenotypes embracing biological, clinical and patient-reported outcomes in allergen-specific immunotherapy and show how we designed the DesensIT project to optimize data collection, processing and evaluation.
We describe a unique family where each of the 5 siblings in the second generation has rheumatoid arthritis (RA). Two other members of the family have RA and systemic lupus erythematosus (SLE), ...respectively. No members of previous generations in the family had documented inflammatory arthritis. Due to the suspected genetic predisposition, HLA-DR genotypes were determined in the affected siblings and their parents, children, and grandchildren. We investigated the possible role of various HLA-DR alleles in the evolution of RA in this multicase family.
HLA-DRB1* alleles were determined by polymerase chain reaction using the sequence-specific primer-Olerup method.
The most common alleles in the 6 persons with RA were HLA-DRB1*07 and DRB1*15, which are known to be protective and neutral in RA. No patient or family member carried any HLA-DR4 alleles.
HLA-DRB1*07 and DRB1*15 alleles are thought to be protective or neutral in RA. However, the majority of RA patients in the family and nearly half of all family members carried these alleles, suggesting a role of these genotypes in susceptibility to RA. No RA patient in this family carried HLA-DR4 alleles. Thus, in our rare family with 6 RA cases, an unexpected genetic background may be involved in the increased susceptibility to inflammatory arthritis.
Összefoglaló. Bevezetés: A Nemzeti Szívinfarktus Regiszterben 111 788 beteg 122 351 infarktusos eseményéhez kapcsolódó 145 292 kezelés adatai szerepelnek. Módszer: A rögzített adatokat az üzemeltetők ...folyamatosan kontrollálják, bemutatják azokat a minőségbiztosítási módszereket, amelyekkel az adatbázis teljességét és megfelelőségét biztosítják. Az online informatikai rendszerben az adatbevitel során 119 automatikus ellenőrzési algoritmust működtetnek. Az automatikus ellenőrzési algoritmussal nem kezelhető adatok ellenőrzését 5 részállású, egészségügyi képzettségű kontroller és 2 főállású munkatárs végzi. A regiszter működése során folyamatosan fejlesztették az ellenőrzés módszereit, ennek során 2018-tól a kontrollerek által ellenőrzött adatlapok utóellenőrzésére is sor kerül. Az utóellenőrzés során a már ellenőrzött adatlapok 2,4%-ában további javításra volt szükség. Eredmények: Az utóellenőrzés eredménye, hogy a kontrolleri munkát hatékonyabbá sikerült tenni, mivel egyre kevesebb az utóellenőrzés során hibásnak talált adatlapok száma. Megvizsgálták, hogy az adatlap kérdéseire milyen arányban kaptak értékelhető választ. Az értékelhető válaszok aránya a legtöbb esetben meghaladta a 90%-ot, azonban a panaszok kezdetének ideje az adatlapok 39%-ában volt megadva, míg a dohányzási szokásokkal kapcsolatos válasz az esetek 59%-ában volt megfelelő. Megbeszélés: A szerzők rámutatnak arra, hogy a Nemzeti Egészségbiztosítási Alapkezelő és a Nemzeti Szívinfarktus Regiszter adatbázisának folyamatos egyeztetése hozzájárul a regisztráció teljességének biztosításához, lehetővé teszi a betegek állapotának hosszú távú követését. Miután a program kötelező jellegűvé vált 2014. 01. 01-jén, az első évben a szívinfarktus-diagnózissal finanszírozott betegek kétharmada (67%) szerepelt a regiszter adatbázisában; ez az arány a 2017-2019-es években meghaladta a 90%-ot (91,7-93,6-91,3%). Következtetés: Vizsgálatukból a szerzők azt a következtetést vonják le, hogy a betegségregiszter működése során szükséges az adatok teljességének és megfelelőségének folyamatos ellenőrzése. A regiszter adatbázisának 90% feletti teljessége az ellátórendszer minőségi paramétereinek folyamatos követését teszi lehetővé. Orv Hetil. 2021; 162(2): 61-68.
The Hungarian Myocardial Infarction Registry contains data on 145 592 treatments related to the 111 788 patients and the 122 351 myocardial infarctions.
The recorded information is continuously monitored, and the quality assurance methods used to ensure the completeness and adequacy of the database are presented. In the online IT system, 119 automatic verification algorithms are operated during data entry. Data that cannot be handled by the automated verification algorithm is checked by five part-time health-qualified controllers and two full-time employees. During the operation of the register, the control methods were continuously developed, during which the data sheets checked by the controllers will be post-checked from 2018 onwards. During the post-checked process, 2.4% of the datasheets required further correction.
The number of data sheets found to be incorrect during the post-audit was decreasing. The authors examined the proportion of evaluable answers to the questionnaire. The rate of evaluable responses was over 90% in most cases; however, the time of the onset of symptoms was given in 39% of the datasheets, while the answer to smoking habits was adequate in 59% of cases.
The authors point out that the continuous consultation of the database of the National Health Fund Management Centre and the Hungarian Myocardial Infarction Registry contributes to ensuring the completeness of registration, enabling long-term monitoring of the condition of patients. In the first year of the mandatory period of the program, two-thirds (67%) of patients treated with a diagnosis of myocardial infarction were included in the registry database, and this proportion exceeded 90% in the years 2017-2019 (91.7-93.6-91.3%).
The study of the authors concludes that the completeness and adequacy of the data need to be constantly monitored during the operation of the patient registry. Az elmúlt években a cégek felismerték az imázs fontosságát, amelyet képviselnek és a termékük márkáinak építése mellett el kezdtek koncentrálni a vállalati márka formálására is. Jelen tanulmányban a ...vállalati márkák világát térképeztük fel két FMCG (gyorsan mozgó fogyasztási cikkek) vállalat példáján keresztül. A kutatás célja, hogy megvizsgáljuk a fogyasztók és a márkák, valamint a termékmárkák és a vállalati márkák közötti kapcsolatot, vagyis hogy hogyan befolyásolja a vállalati márka a fogyasztókat a vásárlási döntések meghozatalánál. A vásárlók céggel kapcsolatos ismeretét is megvizsgáltuk a vásárlási szokásokra. Feltételeztük, hogy a fogyasztók szempontjából a termékek közötti választást gyakran a termék és annak a márkájával kapcsolatos érzések befolyásolják és kevésbé a vállalati márkáé. Ennek ellenére, ha egy vállalat erős vállalati márkával bír, amely köré egy eszmét építettek, amelynek az üzenete hatékonyan van kommunikálva, akkor a vállalati márka létfontosságú lehet a döntéshozásnál.
Stable dexamethasone resistant and receptor-containing (R+) variants of L cells have been characterized by somatic cell hybridization. Neither of the variants had a clearly dominant phenotype in ...hybrids with dexamethasone-sensitive fibroblast lines, i.e. the resistance of the variants was not due to transdominant factors. Somatic cell hybrids formed between one of the R+-resistant clones and an independent resistant fibroblast cell line showed complementation--the hybrid clones were as sensitive to the steroid as the sensitive parental lines. Complementation, however, disappeared after continued culture of the clones. The return of the dexamethasone-sensitive phenotype was not always linked with similar changes in the responsiveness to another steroid, e.g. progesterone. Our clones can be considered to be resistant variants, designated death-less (d-), where the cells are defective in a non-receptor component involved in the hormone response. The fact that complementation can occur indicates the existence of at least two such steps in the pathway.
