Hypercalcemia is a rare metabolic disorder in course of B cell lymphoma. The mechanism of hypercalcemia in patients with malignancy may include the increased extrarenal production of vitamin D from ...tumoral cells or neighboring macrophages, i-PTH or PTHrP from tumoral cells. In this case we reported a 34 years old caucasian woman with acute renal failure and hypercalcemia as onset of splenic lymphoma in absence of abnormal levels of serum vitamin D and PTHrP. Because of dramatic recovery of renal function and hypercalcemia after splenectomy, we can speculate that main mechanism of hypercalcemia is related to vitamin D production from neighboring lymphoma macrophages.
Background:
The number of cesarean sections (CSs) is increasing in many countries, and there are concerns about their short- and long-term effects. A recent Brazilian study showed a 58% higher ...prevalence of obesity in young adults born by CS than in young adults born vaginally. Because CS-born individuals do not make contact at birth with maternal vaginal and intestinal bacteria, the authors proposed that this could lead to long-term changes in the gut microbiota that could contribute to obesity.
Objective:
We assessed whether CS births lead to increased obesity during childhood, adolescence, and early adulthood in 3 birth cohorts.
Design:
We analyzed data from 3 birth-cohort studies started in 1982, 1993, and 2004 in Southern Brazil. Subjects were assessed at different ages until 23 y of age. Poisson regression was used to estimate prevalence ratios with adjustment for ≤15 socioeconomic, demographic, maternal, anthropometric, and behavioral covariates.
Results:
In the crude analyses, subjects born by CS had ∼50% higher prevalence of obesity at 4, 11, and 15 y of age but not at 23 y of age. After adjustment for covariates, prevalence ratios were markedly reduced and no longer significant for men or women. The only exception was an association for 4-y-old boys in the 1993 cohort, which was not observed in the other 2 cohorts or for girls.
Conclusion:
In these 3 birth cohorts, CSs do not seem to lead to an important increased risk of obesity during childhood, adolescence, or early adulthood.
The CH- and OH-stretching vibrational circular dichroism (VCD) spectra of methyl lactate and related molecules in CCl4 solution have been investigated to identify solution conformations and establish ...a correlation between OH- and methine-stretching VCD intensity and molecular structure. Deuterium substitution was used when possible to remove overlapping absorption features in the CH-stretching region. Anisotropy ratios between +2.1 × 10-4 and +2.8 × 10-4 were measured for the methine-stretching VCD of the molecules with both α-oxy and α-CO substituents: (S)-methyl-d 3 lactate, (S)-methyl-d 3 2-(methoxy-d 3)-propionate, di(methyl-d 3) d-tartrate, (S)-methyl-d 3 mandelate, (S)-methyl-d 3 O-(acetyl-d 3)-mandelate, and (S)-benzoin. The methine-stretching VCD intensity serves as a marker for both absolute configuration and solution conformation in these molecules, as previously demonstrated for amino acids and peptides. In (S)-methyl 2-chloropropionate, (R)-methyl 3-hydroxy-2-methylpropionate, and (S)-methyl 3-hydroxybutyrate, the net CH-stretching VCD intensity is small relative to that in the other molecules studied. Ab initio calculations of geometries, vibrational frequencies, and unpolarized infrared absorption (IR) and VCD intensities were carried out to identify the most abundant solution conformers from the VCD spectra and to correlate OH- and methine-stretching VCD intensity with molecular conformations. Factors leading to large methine-stretching VCD anisotropy ratios are assessed.
Fibrillary glomerulonephritis (FibGN) is a rare cause of progressive renal dysfunction, often leading to dialysis within a few years. A 60-year-old woman presented with a 2 month history of ...right-sided retro-orbital pain and recent diplopia. Laboratory testing revealed an altered renal function with increased serum creatinine and mild proteinuria. MRI of the brain revealed the presence of a soft tissue mass on the right cavernous sinus compatible with the diagnosis of Tolosa-Hunt syndrome (THS). Renal biopsy showed a pattern compatible with fibrillary glomerulonephritis. For this reason steroid therapy was initiated at a dose of 1 mg/kg/day and adjusted according to the clinical course. Neurological symptoms regressed shortly after the beginning of therapy and renal function and proteinuria remained stable for the 3 years following the withdrawal of steroid therapy. Percutaneous renal biopsy was again performed and confirmed the previous diagnosis of FibGN in association with other glomerular-lesion-like mesangial widening, thickening of capillary walls and severe arterio-arteriolosclerosis. This case report describes what is believed to first report of the association of FibGN and THS, which both responded to steroid therapy.
The Epicurean teacher and poet Philodemus of Gadara (c. 110-c. 40/35 BC) exercised significant literary and philosophical influence on Roman writers of the Augustan Age, most notably the poets Vergil ...and Horace. Yet a modern appreciation for Philodemus' place in Roman intellectual history has had to wait on the decipherment of the charred remains of Philodemus' library, which was buried in Herculaneum by the eruption of Vesuvius in 79 AD. As improved texts and translations of Philodemus' writings have become available since the 1970s, scholars have taken a keen interest in his relations with leading Latin poets. The essays in this book, derived from papers presented at the First International Symposium on Philodemus, Vergil, and the Augustans held in 2000, offer a new baseline for understanding the effect of Philodemus and Epicureanism on both the thought and poetic practices of Vergil, Horace, and other Augustan writers. Sixteen leading scholars trace his influence on Vergil's early writings, the Eclogues and the Georgics, and on the Aeneid, as well as on the writings of Horace and others. The volume editors also provide a substantial introduction to Philodemus' philosophical ideas for all classicists seeking a fuller understanding of this pivotal figure.
