Introduction: The increase in life expectancy and the aging of the population are associated with an increase in the prevalence of chronic diseases. Comorbidities have an important impact on ...prognosis and functional capacity leading to a progressive deterioration of autonomy and quality of life and an increase in demand for medical care. Establishment of an accurate prognosis constitutes one of the primary objectives in healthcare. An accurate estimate of prognosis helps clinicians make diagnostic and therapeutic decisions, prevent iatrogenesis, and consider palliative care strategy as needed. It also allows the patient and family members to organize their preferences and priorities. Objective: To evaluate the PROFUND scale in patients with heart failure from a prognostic point of view. Methods: A multicenter cohort study including patients admitted for heart failure to internal medicine departments over a 6-month period will be carried out. Inclusion criteria are patients with a diagnosis of heart failure and at least two criteria of multipathological patients and NT-proBNP >1500 pg/ml upon admission. The PROFUND scale will be applied to all patients. Patients will be then stratified into four groups according to the PROFUND scale: low, moderate, moderate-high and high mortality risk. Conclusion: Our work is a prospective study that aims to apply the PROFUND scale to patients with heart failure in the hospital setting with the purpose of helping in decision-making with our patients, which could lead to improvements in the management of resources in our health system.
Resumen: En la presente revisión los autores abordan en forma pormenorizada la definición, los aspectos epidemiológicos, la fisiopatología, los pilares diagnósticos y el tratamiento de esta entidad ...que está adquiriendo cada vez mayor importancia en el campo de la cardiología. Los escasos ensayos clínicos realizados no han logrado aportar evidencias satisfactorias, por lo que especialmente su tratamiento constituye un reto intelectual y científico. Como muy interesante aporte se exponen las bases fundamentales del programa de las Unidades de Manejo Integral de Pacientes con Insuficiencia Cardíaca (UMIPIC) en España. Este programa tiene como principal objetivo ofrecer una asistencia sanitaria de calidad y sostenible por el sistema público y comprende la atención global de todas las enfermedades de los pacientes mayores, la educación, el uso eficiente de los recursos económicos y la motivación profesional. Se describen el plan general de trabajo y los resultados de los primeros 258 pacientes que completaron un año de seguimiento.
To assess whether a sustained optimal haemoglobin value in the 3 months after admission for heart failure (HF) decompensation reduces morbidity and mortality during the 12 months after admission for ...acute HF.
Retrospective study of the 1408 patients older than 65 years included in the RICA registry divided into 3 groups: no anaemia (group A), recovered anaemia (group B), and persistent anaemia (group C), according to haemoglobin levels on admission, and 3 months after discharge. Kaplan-Meier curves were constructed, comparing the groups using the log-rank test and a Cox regression model was performed to analyse survival.
578 (41.1%), 299 (21.2%) and 531 (37.7%) were included in groups A, B and C, respectively. We recorded a total of 768 deaths and readmissions. There were 23 (4%), 12 (4%) and 49 (9.2%), (p = 0.001) individuals who died due to HF and 154 (27%), 73 (24%) and 193 (36%) (P < 0.001) admissions for this pathology, respectively. Patients with persistent anaemia had a higher risk of death (RR 1.29, 95% CI 1.04−1.61, P = 0.024) or readmission (1.92, 95% CI 1.16–3, 19; P = 0.012) due to HF.
Persistent anaemia in the months after admission for HF increases morbidity and mortality in the subsequent year.
Evaluar si un valor óptimo de hemoglobina sostenido en los 3 meses posteriores al ingreso por descompensación de insuficiencia cardíaca (IC) reduce la morbimortalidad durante los 12 meses posteriores a un ingreso por IC aguda.
Estudio retrospectivo de los 1408 pacientes mayores de 65 años incluidos en el registro RICA divididos en 3 grupos: sin anemia (grupo A), anemia recuperada (grupo B) y anemia persistente (grupo C), según los niveles de hemoglobina en el ingreso y a los 3 meses tras el alta. Se construyeron curvas de Kaplan-Meier, comparando los grupos mediante la prueba de log-rank y se realizó un modelo de regresión de Cox para analizar la supervivencia.
Se incluyeron 578 (41,1%), 299 (21,2%) y 531 (37,7%) en los grupos A, B y C, respectivamente. Registramos un total de 768 muertes y reingresos. Hubo 23 (4%), 12 (4%) y 49 (9,2%) (p = 0,001) individuos que fallecieron debido a la IC, y 154 (27%), 73 (24%) y 193 (36%) (p < 0,001) reingresaron por esta patología, respectivamente. Los pacientes con anemia persistente tuvieron un riesgo superior de fallecimiento (RR: 1,29; IC95% de 1,04–1,61; p = 0,024) o reingreso (1,92; IC95% de 1,16–3,19; p = 0,012) por IC.
La anemia persistente en los meses posteriores a un ingreso por IC aumenta la morbimortalidad en el año posterior.
The general objective of the study was to reflect on the key factors to advance in optimal models of care for Heart Failure (HF) and specifically, on the macromanagement elements most necessary for ...the development of comprehensive HF management models.
An Advisory Committee, composed of 15 experts and a multidisciplinary group of 31 additional experts, was appointed, together forming a Delphi panel of 46 experts. Based on a systematic bibliographic review and the analysis of the care course of the patient with HF, an initial battery of key factors for the development of HF care models was identified by the Advisory Committee. This proposal was adjusted and prioritized by the Delphi panel applying Delphi Rand/UCLA methodology.
After two Delphi rounds, 75 key factors grouped into 7 challenges were defined. In the first of the challenges, related to the development of HF management models, 16 key factors were identified, 7 of which were valued as high priority and related to the establishment of common objectives, resources for the continuity of care and improving the measurement of health outcomes.
The definition of management elements at the macro level was considered a priority to advance in the development of optimal models of assistance to HF.
TARGET: To determine the impact of a monographic consultation, in which internists and specialized nursing play a main role, for patients with Chronic Heart Failure (CHF) and associated comorbidity ...on the quality of care
and hospital readmission of these patients, as well as its influence on hospital global admission due to CHF.
MATERIAL AND METHODS: Prospective and retrospective cohort study with a scope of 169 patients with CHF, belonging to two hospitals with a different care level (district and provincial level), included in two similar monographic consultations. The percentage
of patients receiving 1st-line drugs as well as their compliance are evaluated and compared to the admission rate due to CHF both during the previous year to the study and the year following the study. In order to find out the impact of these consultations
according to data provided by hospitals, the development of the total number of admissions and average stay length due to CHF (GRDs 127, 544) from 2001 to 2009 are analyzed. The beginning of consultations falls on the middle point of this period.
RESULTS: A high percentage of patients subject to monitoring received 1st-line drugs (97.8% IECAS/ARA-II and 88% BB) with a very high compliance (90%). The readmission rate has been reduced down to 69% as compared to previous year. According to the data provided by both hospitals,
the global stay length of Internal Medicine patients due to CHF has been reduced in 3.5 days, the number of patients handled in the Internal Medicine Area due to CHF has increased and the total admission rate due
to CHF during the year period 2001-2009 has been reduced.
CONCLUSION: On what CHF and comorbidity patients are concerned, following a health program in which internists and specialized nursing play a main role and which is based on a continuous and comprehensive tracking, patient information, staff training and availability, is a good way not only to reduce the readmission of monitored patients, but also to also to improven CHF hospital stay global rates of these patients.
OBJETIVO: Determinar la repercusión de una consulta monográfica para pacientes con insuficiencia cardíaca crónica (ICC) y comorbilidad asociada, dirigida por internistas y enfermería cualificada, sobre la calidad asistencial y los reingresos y la influencia en los ingresos de estos pacientes. Así como la influencia en los ingresos globales del hospital por IC.
MATERIAL Y MÉTODOS: Estudio de cohortes retro y prospectivo de 169 pacientes con ICC, incluidos en dos consultas monográficas similares, pertenecientes a dos hospitales de diferente ámbito asistencial (comarcal
y provincial). Se evalúan porcentaje de pacientes que recibe fármacos de 1º línea, la adherencia al seguimiento y se compara los ingresos por ICC realizados el año previo y posterior al seguimiento. Para conocer el impacto de estas consultas, en base a los datos de los centros, se analiza la evolución en el nº de ingresos totales y estancias medias por ICC (GRDs
127, 544) durante los años 2001-2009. Coincide el ecuador de este periodo con la puesta en marcha de estas consultas.
RESULTADOS: Los pacientes sometidos al seguimiento recibieron fármacos de 1º línea en un porcentaje elevado (97.8% para IECAS/ARA-II,y 88% para BB), con una adherencia al seguimiento muy alta (90%) y con una reducción de los reingresos de hasta un 69%, con respecto al año previo. Desde los datos facilitados por ambos hospitales, se observa una disminución de la estancia hospitalaria global del servicio de Medicina
interna por ICC de 3.5 días, un aumento de pacientes atendidos en Medicina interna y una disminución de los ingreso por ICC.
CONCLUSIONES: El seguimiento por internistas y enfermería de los pacientes con ICC y comorbilidad, basado en una atención continuada e integral,
en la información y en la disponibilidad, es un buen método no solo para disminuir reingresos hospitalarios , sino que además puede influir en la mejora de las cifras globales de la hospitalización por ICC de estos enfermos. Además es implantable y eficaz en diferentes ámbitos asistenciales
Introduction: Heart failure (HF) is a syndrome of epidemic proportions and one of the main reasons for hospitalization worldwide. Different scientific s ocieties have r isen interest in the c reation ...of their own registries to s tudy the characteristics and prognosis of these patients. Each registry covers a part of the spectrum of this heterogeneous syndrome and is useful to address questions that are difficult to answer in clinical trials. The RICA-2 is a national registry created by the Heart Failure Working Group of the Spanish Society of Internal Medicine and offers an Internal Medicine perspective of this pathology. Objectives: The main objective is to assess clinical and epidemiological characteristics and prognostic factors among patients with HF. Secondary objectives include: (1) To determine clinical/epidemiological/phenotypic characteristics and prognosis specifically i n p atients with HF a nd p reserved l eft-ventricle e jection f raction ( LVEF); ( 2) To examine h ow f unctional s tatus, cognition, frailty, and nutrition influence the prognosis of patients with HF; (3) To assess congestion and strategies to achieve decongestion during the acute decompensation phase. Methods: A multicentre, prospective, observational cohort study including patients with HF attended in Spanish Internal Medicine Departments. Patients will be recruited in the acute decompensation phase or in the stable phase in the outpatient setting, including de novo and chronically decompensated patients. Patients will be included regardless of HF aetiology, LVEF values and comorbidities. Conclusions: Our work is a prospective study that aims to improve knowledge regarding epidemiology and prognosis in patients with HF, with focus on functionality, cognition, frailty, nutrition, and congestion phase.
La insuficiencia cardiaca (IC) supone un reto para los sistemas sanitarios que se puede responder a través del desarrollo de modelos integrales de atención. Un grupo multidisciplinar de expertos ...reflexionó sobre los factores clave para avanzar en el desarrollo de este tipo de modelos, planteando una hoja de ruta dirigida a todos los agentes (administraciones, gestores y profesionales sanitarios).
Se conformó un panel Delphi multidisciplinar integrado por un comité asesor de 15 expertos y un panel adicional de 31 expertos. A través de una revisión bibliográfica sistemática y entrevistas individuales semiestructuradas se realizó un diagnóstico e identificación de retos y áreas de mejora a lo largo del proceso asistencial. El panel Delphi consensuó y priorizó los factores clave con la metodología Delphi Rand/UCLA, valorando su adecuación y necesidad.
Tras 2 rondas de valoración Delphi se consensuó una propuesta de 7 retos y 75 factores clave para el desarrollo de modelos integrados para la IC. Los 25 factores clave considerados altamente prioritarios se relacionan con la necesidad de una mayor coordinación y planificación a nivel de gestión sanitaria, el abordaje integral durante la hospitalización y la implantación de medidas de continuidad y coordinación asistencial, garantizando que se cubren las necesidades específicas de diferentes perfiles de pacientes.
La propuesta y priorización de acciones para avanzar en modelos de atención integral a la IC debe surgir de reflexiones multidisciplinares y multinivel que incluyan la visión de los pacientes y cuidadores.
Heart failure (HF) is a challenge for health systems that can be responded through the development of comprehensive care models. A multidisciplinary group of experts reflected on the key factors that could facilitate the development of this type of models, proposing a roadmap aimed at all agents (politicians, managers, administrators, and health professional).
A multidisciplinary Delphi panel was formed, made up of an advisory committee of 15 experts and an additional panel of 31 experts. After a systematic bibliographic review and semi-structured individual interviews, a diagnosis and identification of challenges and areas for improvement were made throughout the healthcare process. The Delphi panel agreed and prioritized the key factors applying Delphi Rand/UCLA methodology, assessing their appropriateness and need.
After 2 rounds of Delphi assessment, a proposal of 7 challenges and 75 key factors was agreed upon for the development of integrated models for HF. The 25 key factors considered high priority are related to the need for greater coordination and planning at the health management level, the comprehensive approach during hospitalization and the implementation of measures of continuity and care coordination, ensuring that the specific needs of different patient profiles.
The proposal and prioritization of actions to advance in models of comprehensive care for HF must arise from multidisciplinary and multilevel reflections that include the vision of patients and caregivers.
Los Objetivos de Desarrollo del Milenio (ODM) se encuentran actualmente
a mitad de su periodo de ejecución, siendo el año 2015 la fecha prevista por la
Organización de Naciones Unidas (ONU) para su ...alcance. El objetivo de este
artículo es revisar la situación actual en la que se encuentran los ODM a nivel
mundial y analizar las barreras que estarían impidiendo su consecución para cada
uno de los ámbitos de los ODM, así como valorar algunos de los indicadores evaluados.
Para ello, se ha revisado la literatura científica publicada sobre los ODM
en las principales bases de datos de ciencias de la salud y ciencias sociales, así
como los principales informes elaborados sobre el tema por Naciones Unidas.
Los estudios científicos en torno a los 8 ODM y sus 18 Metas permiten realizar
un análisis crítico sobre la situación en la que se encuentra en la actualidad cada
uno de ellos, identificando los determinantes que están impidiendo su consecución
y las acciones que se consideran necesarias para impulsar el avance. Aunque
a nivel global ha habido mejoras en algunas de las metas, la investigación realizada
hasta la fecha muestra barreras a la consecución de los ODM, como el insuficiente
peso de los estados de los países en desarrollo sobre las decisiones económicas
y políticas, así como la incoherencia entre las políticas económicas y las
políticas sociales y de salud. Por otra parte, África Subsahariana constituye la
región con mayor desventaja, lo que supone que no alcanzará la mayoría de los
ODM. España y los países desarrollados, además de aportar recursos, pueden
contribuir a los ODM mediante la identificación y erradicación de las barreras
que impiden su alcance. Esto significa promover unas relaciones económicas
internacionales en condiciones de justicia social, apoyando un mayor poder de
decisión para los países en desarrollo, y denunciando las actuaciones que incrementan
las desigualdades sociales y el empobrecimiento de la población.
