Hintergrund:
Die totale Pankreatektomie (TP) wird im Vergleich zur Pankreaslinks- oder -kopfresektion relativ selten durchgeführt. Die TP ist mit signifikanten metabolischen Abnormitäten assoziiert, ...die zu einer hohen Morbidität und Mortalität führen. Hauptindikationen sind ausgedehnte oder multilokulare Tumore, in Einzelfällen eine chronische Pankreatitis. Darüber hinaus ist die TP eine Option bei der konservativ nicht zu beherrschenden Anastomoseninsuffizienz nach Pankreaskopfresektion. Zur Evaluierung des Stellenwertes dieser Operation sowie des Outcomes sollten alle total pankreatektomierten Patienten untersucht werden.
Patienten und Methoden:
Die Patientendaten bezüglich des operativen Vorgehens und des postoperativen Outcomes wurden prospektiv gesammelt und in einer Datenbank festgehalten. Die Hospitalisierungsdauer bzw. der Aufenthalt auf einer Intensivstation die Gesamtüberlebenszeit wurden ermittelt. Es erfolgte eine retrospektive Analyse von 72 Patienten, die im Zeitraum von 1994 bis Dezember 2014 einer TP unterzogen wurden.
Ergebnisse:
Von 01/1994 bis 01/2015 wurden insgesamt 1474 Pankreasresektionen durchgeführt. Die häufigsten Operationen waren 1054 Pankreaskopfresektionen und 244 Pankreaslinksresektionen, sowie 72 TP. 35 Frauen und 37 Männer mit einem mittleren Alter von 62,6 ± 13,2 Jahren (Median 66,5 Jahren) wurden einer TP unterzogen. 22 Patienten (30,6%) erhielten eine sekundäre TP. 50 Patienten (69,4%) erhielten eine primäre TP. Im Mittel waren die Patienten 28,5 ± 19,4 Tage hospitalisiert, davon 10,6 ± 12,4 Tage auf einer Intensivstation. 9 Patienten (12,5%) verstarben im Rahmen des stationären Aufenthaltes. Das Gesamtüberleben betrug im Mittel 441,4 Tage, Patienten mit einer primären Pankreatektomie überlebten im Mittel 438,5 ± 763,1 Tage und Patienten mit einer sekundären Pankreatektomie im Mittel 447,7 ± 771,5 Tage.
Zusammenfassung:
Aus der TP resultierten signifikante metabolische Entgleisungen sowie exokrine Insuffizienz. Die Überlebensrate nach TP war stark reduziert. Bei der sekundären TP zeigte sich kein Nachteil bezüglich des Überlebens verglichen mit den Patienten nach primärer TP. Die interindividuelle Überlebenszeit variierte deutlich.

The advantages of minimally invasive liver resections for selected patients are evident. Robots provide new innovations that will influence minimally invasive liver surgery in the future. This ...article presents our initial experience with this technology in our patient population. Material und Methods: In 14 patients with benign or malignant liver tumours, robotic-assisted liver surgery was performed. Selection criteria were compensated liver function and resection of ≤ 3 liver segments. Chronic liver disease or previous abdominal surgery were no exclusion criteria.
Malignant liver tumours were removed in 10 patients (71%) and benign symptomatic liver tumors in 3 patients (21%), respectively, with histopathologically negative margins (R0). One patient suffering from HCC underwent intraoperative ablation. In one case (7%) conversion was necessary. Mean operation time was 296 min (120-458 min); mean estimated blood loss was 319 ± 298 ml. The mean hospital stay of the patients was 8 days (3-17 days). Three patients (21%) suffered from postoperative complications, which were manageable by conservative treatment (Clavien-Dindo I) in 2 cases (14%). One patient (7%) needed endoscopic treatment for postoperative bile leak (Clavien-Dindo III a). No patient died intra- or perioperatively.
Robotic-assisted liver surgery is a safe procedure, which provides patients with all benefits of minimally invasive surgery. This highly advanced technology requires surgeons to strive for an increasing level of specialisation, in addition to being well-trained in liver surgery. Hence, a clear definition of the procedures and standardised teaching programs are necessary.

Nach wie vor stellt die primäre Resektion die Standardtherapie des nichtmetastasierten Kolonkarzinoms dar. Analog zum weltweit anerkannten Konzept der totalen mesorektalen Exzision ...(„total mesorectal excision“, TME) für das Rektumkarzinom wurde für das Kolonkarzinom das Konzept der kompletten mesokolischen Exzision („complete mesocolic excision“, CME) vorgestellt, welches die Entfernung des jeweils onkologisch relevanten Mesokolons unter Respektierung der Integrität des Mesokolons sowie die zentrale vaskuläre Ligatur (CVL) mit radikaler Lymphknotendissektion beinhaltet. Die aktuelle Datenlage zur CME zeigt mit starker Evidenz, dass die Präparate nach CME-Resektionen bessere histopathologische Qualitätskriterien (inkl. einer höheren Anzahl an resezierten Lymphknoten und einer höheren Dissektionsrate in der korrekten mesokolischen Schicht) aufweisen als jene nach
konventioneller
, nicht den Prinzipien der CME folgenden Kolonresektion. Allerdings besteht bisher aufgrund einer teils heterogenen Studienlage und Schwächen der bisherigen Studien (fehlende Subgruppenanalysen nach Lokalisation des Karzinoms, Vergleich mit historischen Kollektiven und enorme Divergenz in der chirurgischen Ausführung der
konventionellen
Kolonresektionen) eine noch limitierte Evidenz für das onkologische Ergebnis und den Vergleich zwischen offener und laparoskopischer CME. Weitere Studien zum onkologischen Outcome sind notwendig, um der CME für Kolonkarzinome den gleichen Stellenwert wie der TME für Rektumkarzinome zuschreiben zu können.
Einleitung:
Resektionen an der Bauchspeicheldrüse sind komplexe Eingriffe mit einer hohen Morbidität und Mortalität. Durch die Analyse der prospektiv geführten Datenbank sollten die Ursachen der ...Mortalität nach resezierenden Pankreaseingriffe in unserem Pankreaszentrum analysiert werden.
Patienten und Methode:
Es wurden anamnestische (z.B. Alter, Geschlecht, Nebenerkrankungen, Symptome), operative (z.B. Weite des Pankreasganges, Konsistenz des Parenchyms, OP-Zeit, Erfahrung des Operateurs, Blutverlust) und postoperative Faktoren (z.B. Komplikationen: Pankreasfisteln (POF), postoperative Blutungen (PPH), Magenentleerungsstörung) analysiert. Als Signifikanztests wurden der t-Test für unabhängige Stichproben oder der exakte Test nach Fisher verwendet mit einem Signifikanzniveau von α < 0,05 gewählt.
Ergebnisse:
Von 01/1994 bis 12/2013 wurde an unserem Zentrum insgesamt 1335 Pankreasresektionen durchgeführt. Unter den Operationen waren u.a. 946 Pankreaskopfresektionen, 214 Linksresektionen und 33 Pankreatektomien. Die Hauptindikationen waren: bösartige Neubildungen in 57,6%, gutartige/unsichere Neubildungen in 32,4% und die chronische Pankreatitis in 10%. Es verstarben 49 Patienten (3,67%) der Patienten im Rahmen der postoperativen Behandlung im Krankenhaus. Es ergaben sich folgende Todesursachen: septisches Multiorganversagen nach POF oder Restpankreatitis: 20 (40,8% bzw. 1,46%), PPH: 7 (14,3% bzw. 0,52%), Darmischämie: 7 (14,3% bzw. 0,52%), Thombembolien: 3 (6,12% bzw. 0,22%), Herzinfarkt: 3 (6,12% bzw. 0,22%), Hirnblutung oder Hirninfarkte: 3 (6,12% bzw. 0,22%), Pneumonie: 3 (6,12% bzw. 0,22%) und andere Ursachen mit 3 (6,12% bzw. 0,22%).
Diskussion:
Unsere Datenanalyse über einen Zeitraum von 20 Jahren zeigt, dass sich eine niedrige Mortalitätsrate von 3,67% bei hohen Eingriffszahlen auch langfristig realisieren lässt. Ursache der meisten Todesfälle (34 von 49) war ein chirurgisch-technisches Problem (2,5% aller Patienten). Der Pankreaschirurg im spezialisierten Zentrum ist somit ein Risikofaktor für operationsassoziierte Mortalität ist. Daneben lassen sich patienten- und diagnosebezogene Risikofaktoren identifizieren, die in Hinblick auf eine Verbesserung der perioperativen Versorgungsqualität jedoch nicht beeinflussbar sind.
Hereditary pancreatitis in children is rare. The indications for surgery in these children are complications and severe pain that cannot be managed by conservative treatment. Surgical treatment ...options are duodenum preserving resections as well as drainage procedures. Recurrences are common following theses surgical procedures, because usually the whole pancreas is affected. The majority of the children with symptomatic hereditary pancreatitis are free of pain after total pancreatectomy. When total pancreatectomy is combined with islet autotransplantation, pancreoprivic diabetes can be attenuated or even prevented. The principle of spleen-preserving total pancreatectomy combined with subsequent islet autotransplantation is shown in a case of a 10-year-old child with chronic recurrent pancreatitis in this video.

If the surgeon has appropriate experience, spleen-preserving total pancreatectomy is a safe procedure. In combination with islet autotransplantation, it may attenuate or prevent diabetes mellitus associated with total pancreatectomy. In highly selected pediatric patients, this surgical procedure has a major benefit compared to a purely symptomatic therapy.
Pancreatic ductal adenocarcinoma (PDAC) is a fatal disease and one of the cancer entities with the lowest life expectancy. Beside surgical therapy, no effective therapeutic options are available yet. ...Here, we show that 4-phenylbutyrate (4-PB), a known and well-tolerable inhibitor of histone deacetylases (HDAC), induces up to 70% apoptosis in all cell lines tested (Panc 1, T4M-4, COLO 357, BxPc3). In contrast, it leads to cell cycle arrest in only half of the cell lines tested. This drug increases gap junction communication between adjacent T3M-4 cells in a concentration-dependent manner and efficiently inhibits cellular export mechanisms in Panc 1, T4M-4, COLO 357 and BxPc3 cells. Consequently, in combination with gemcitabine 4-PB shows an overadditive effect on induction of apoptosis in BxPc3 and T3M-4 cells (up to 4.5-fold compared to single drug treatment) with accompanied activation of Caspase 8, BH3 interacting domain death agonist (Bid) and poly (ADP-ribose) polymerase family, member 1 (PARP) cleavage. Although the inhibition of the mitogen-activated protein kinase-pathway has no influence on fulminant induction of apoptosis, the inhibition of the JNK-pathway by SP600125 completely abolishes the overadditive effect induced by the combined application of both drugs, firstly reported by this study.
Abstract
Background
Anti-tumour necrosis factor (TNF) antibodies are effectively used for treatment in many Crohn’s disease patients. Nevertheless, a relevant subgroup of patients does not respond to ...anti-TNF therapy. Here we characterised underlying molecular mechanisms that are associated with endoscopic resistance to anti-TNF therapy.
Methods
Mucosal and blood cells were isolated from 197 Crohn’s disease patients prior and during anti-TNF therapy. Cytokine profiles, cell surface markers, signalling proteins and cell apoptosis were assessed by microarray, immunohistochemistry, qPCR, ELISA, whole organ cultures and FACS.
Results
Crohn’s disease patients responding to anti-TNF therapy displayed a significantly higher expression of TNF receptor 2 (TNFR2) but not IL23R on mucosal T cells than non-responders prior to the initiation anti-TNF therapy. We performed an array analysis regarding the differentiated gene regulation profiles in intestinal biopsies of endoscopic non-responders compared with responders during ongoing anti-TNF therapy in CD patients. Within the cohort of CD susceptible genes, there was a significant upregulation of genes that are associated with IL23R-dependent signalling pathways in anti-TNF non-responders compared with responders. Apoptosis--resistant TNFR2+IL23R+ T cells were significantly expanded in anti-TNF non-responders compared with responders and expressed the gut tropic integrins α4β7. These cells exhibited increased expression of IFN-γ, T-bet, IL-17A and RORγt compared with TNFR2+IL23R- cells, indicating a mixed Th1/Th17-like phenotype. Intestinal TNFR2+IL23R+ T cells were activated by IL-23 derived from CD14+ macrophages, which were significantly more present in non-responders compared with responders prior to anti-TNF treatment. Administration of IL-23 to anti-TNF treated mucosal organ cultures led to the expansion of CD4+IL23R+TNFR2+ lymphocytes. There was no accumulation of CD4+TNFR2+ T cells which were negative for IL23R. Functional studies demonstrated that anti-TNF-induced apoptosis in mucosal T cells is abrogated by IL-23.
Conclusions
Expansion of apoptosis-resistant intestinal TNFR2+IL23R+ T cells is associated with resistance to anti-TNF therapy in Crohn’s disease. IL-23 is centrally involved in mediating resistance to anti-TNF therapy in Crohn’s disease patients and thereby represents a suitable molecular target in Crohn’s disease patients refractory to anti-TNF therapy.
