Reduced cardiac vagal control reflected in low heart rate variability (HRV) is associated with greater risks for cardiac morbidity and mortality. In two-stage meta-analyses of genome-wide association ...studies for three HRV traits in up to 53,174 individuals of European ancestry, we detect 17 genome-wide significant SNPs in eight loci. HRV SNPs tag non-synonymous SNPs (in NDUFA11 and KIAA1755), expression quantitative trait loci (eQTLs) (influencing GNG11, RGS6 and NEO1), or are located in genes preferentially expressed in the sinoatrial node (GNG11, RGS6 and HCN4). Genetic risk scores account for 0.9 to 2.6% of the HRV variance. Significant genetic correlation is found for HRV with heart rate (-0.74<r
<-0.55) and blood pressure (-0.35<r
<-0.20). These findings provide clinically relevant biological insight into heritable variation in vagal heart rhythm regulation, with a key role for genetic variants (GNG11, RGS6) that influence G-protein heterotrimer action in GIRK-channel induced pacemaker membrane hyperpolarization.
We examined acceptability, preference and feasibility of collecting nasal and oropharyngeal swabs, followed by microbiome analysis, in a population-based study with 524 participants. Anterior nasal ...and oropharyngeal swabs were collected by certified personnel. In addition, participants self-collected nasal swabs at home four weeks later. Four swab types were compared regarding (1) participants' satisfaction and acceptance and (2) detection of microbial community structures based on deep sequencing of the 16 S rRNA gene V1-V2 variable regions. All swabbing methods were highly accepted. Microbial community structure analysis revealed 846 phylotypes, 46 of which were unique to oropharynx and 164 unique to nares. The calcium alginate tipped swab was found unsuitable for microbiome determinations. Among the remaining three swab types, there were no differences in oropharyngeal microbiomes detected and only marginal differences in nasal microbiomes. Microbial community structures did not differ between staff-collected and self-collected nasal swabs. These results suggest (1) that nasal and oropharyngeal swabbing are highly feasible methods for human population-based studies that include the characterization of microbial community structures in these important ecological niches, and (2) that self-collection of nasal swabs at home can be used to reduce cost and resources needed, particularly when serial measurements are to be taken.

Bei der Nationalen Kohorte (NAKO) handelt es sich um die größte bevölkerungsbezogene Kohortenstudie in Deutschland. Ab dem Jahr 2014 werden insgesamt 200.000 Frauen und ...Männer im Alter von 20 bis 69 Jahren in 18 Studienzentren untersucht. Ziel der Studie ist die Erforschung der Ursachen chronischer Krankheiten im Zusammenhang mit Lebensgewohnheiten, genetischen, sozioökonomischen und umweltbedingten Faktoren und die Entwicklung von geeigneten Maßnahmen zur Früherkennung und Vorbeugung dieser Erkrankungen. Im Fokus stehen Erkrankungen des Herz-Kreislauf-Systems und der Atemwege, Krebs, Diabetes, neurodegenerative/-psychiatrische und muskuloskelettale Erkrankungen und Infektionskrankheiten. Im Rahmen von Pretest-Studien (Phasen 1 und 2) wurden Messmethoden und -instrumente ausgewählt, SOPs erstellt und ein Untersuchungsprogramm zusammengestellt, das hinsichtlich Akzeptanz, Zeitaufwand und Durchführbarkeit erprobt und geprüft wurde. Die Pretests umfassten Interviews und Fragebogenerhebungen, anthropometrische Messungen, eine Reihe medizinischer Untersuchungen und die Sammlung verschiedener Bioproben. Zudem konnte die für die Hauptstudie benötigte personelle, logistische und technische Infrastruktur aufgebaut werden, wie z. B. die Etablierung der Studienzentren, die zentrale Infrastruktur für das Datenmanagement und Prozesse zur Steuerung der Gesamtstudie, zur Sicherung des Datenschutzes und zur Qualitätssicherung. Auf Basis der Pretests wurde das Untersuchungsprogramm der Hauptphase der NAKO zusammengestellt und optimiert. Die NAKO ist eine bevölkerungsbezogene, hoch standardisierte und umfangreich phänotypisch charakterisierte Kohorte, die eine Basis für neue Strategien zur Risikoerfassung, Früherkennung und Prävention multifaktorieller Erkrankungen bietet.
CONTEXT Echocardiographic measures of left ventricular (LV) structure and function are heritable phenotypes of cardiovascular disease. OBJECTIVE To identify common genetic variants associated with ...cardiac structure and function by conducting a meta-analysis of genome-wide association data in 5 population-based cohort studies (stage 1) with replication (stage 2) in 2 other community-based samples. DESIGN, SETTING, AND PARTICIPANTS Within each of 5 community-based cohorts comprising the EchoGen consortium (stage 1; n = 12 612 individuals of European ancestry; 55% women, aged 26-95 years; examinations between 1978-2008), we estimated the association between approximately 2.5 million single-nucleotide polymorphisms (SNPs; imputed to the HapMap CEU panel) and echocardiographic traits. In stage 2, SNPs significantly associated with traits in stage 1 were tested for association in 2 other cohorts (n = 4094 people of European ancestry). Using a prespecified P value threshold of 5 × 10-7 to indicate genome-wide significance, we performed an inverse variance-weighted fixed-effects meta-analysis of genome-wide association data from each cohort. MAIN OUTCOME MEASURES Echocardiographic traits: LV mass, internal dimensions, wall thickness, systolic dysfunction, aortic root, and left atrial size. RESULTS In stage 1, 16 genetic loci were associated with 5 echocardiographic traits: 1 each with LV internal dimensions and systolic dysfunction, 3 each with LV mass and wall thickness, and 8 with aortic root size. In stage 2, 5 loci replicated (6q22 locus associated with LV diastolic dimensions, explaining <1% of trait variance; 5q23, 12p12, 12q14, and 17p13 associated with aortic root size, explaining 1%-3% of trait variance). CONCLUSIONS We identified 5 genetic loci harboring common variants that were associated with variation in LV diastolic dimensions and aortic root size, but such findings explained a very small proportion of variance. Further studies are required to replicate these findings, identify the causal variants at or near these loci, characterize their functional significance, and determine whether they are related to overt cardiovascular disease.
The German National Cohort (GNC) is the largest population-based cohort study in Germany. Beginning in 2014, a total of 200,000 women and men aged 20-69 years will be examined in 18 study centers. ...The aim of the study is to investigate the etiology of chronic diseases in relation to lifestyle, genetic, socioeconomic, and environmental factors and to develop appropriate methods for early diagnosis and prevention of diseases such as cardiovascular and respiratory diseases, cancer, diabetes, neurodegenerative/psychiatric diseases, as well as musculoskeletal and infectious diseases. Pretest studies (phase 1 and 2) were conducted to select methods, instruments, and procedures for the main study, to develop standard operating procedures, and to design and test the examination program according to acceptance, expected duration, and feasibility. The pretest studies included testing of interviews, questionnaires, anthropometric measurements, several medical examinations, and the collection of biosamples. In addition, the logistic, technical, and personnel infrastructure for the main study could be established including the study centers, the central infrastructure for data management, processes to coordinate the study, and data protection and quality management concepts. The examination program for the main phase of the GNC was designed and optimized based on the results of the pretest studies. The GNC is a population-based, highly standardized and excellently phenotyped cohort that will be the basis for new strategies for risk assessment and identification, early diagnosis, and prevention of multifactorial diseases.

In Deutschland gibt es bislang wenig populationsbezogene Daten zur regionalen Verteilung des Typ 2 Diabetes mellitus (T2DM). Ein Zusammenschluss von populationsbasierten Studien aus ...unterschiedlichen Regionen Deutschlands im DIAB-CORE Verbund des Kompetenznetz Diabetes liefert erstmals die Datengrundlage für regionale Analysen. Diese ermöglichen Schätzungen zur Prävalenz des T2DM und dessen Variabilität in unterschiedlichen Regionen Deutschlands.
Methoden:
Daten aus fünf populationsbasierten Kohortenstudien und einer überregionalen Studie mit Basiserhebungen zwischen 1997 und 2006 wurden analysiert: Im Nordosten Deutschlands die Study of Health in Pomerania SHIP, in Mitteldeutschland die Cardiovascular Disease, Living, and Ageing in Halle Study CARLA, im Westen die Heinz Nixdorf Recall Study RECALL und die Dortmunder Gesundheitsstudie DO-GS und im Süden die Kooperative Gesundheitsstudie im Raum Augsburg KORA. Als überregionale Referenzstudie diente der Bundesgesundheitssurvey 1998 BGS 98. Probanden im Alter zwischen 45 und 74 Jahren wurden in die vorliegende Datenauswertung einbezogen. Als Kriterium für die Diagnose eines T2DM diente die Selbstauskunft eines ärztlich diagnostizierten Diabetes oder einer antidiabetischen Medikation. Die regionale Prävalenz wurde alters- und geschlechtsspezifisch geschätzt mit 95% Konfidenzintervall (95% KI) und auf die deutsche Bevölkerung (31.12.2007) standardisiert.
Ergebnisse:
Von 11.688 Probanden der regionalen Studien hatten insgesamt 1.008 einen bekannten T2DM, entsprechend einer Prävalenz von 8,6% (8,1%-9,1%). Die Prävalenz der überregionalen Studie (BGS 98) wurde mit 8,2% (7,3%-9,2%) geschätzt. Männer wiesen mit 9,7% (8,9%-10,4%) eine höhere Prävalenz des T2DM im Vergleich zu Frauen mit 7,6% (6,9%-8,3%) auf. Die höchste regionale Prävalenz zeigte sich in der Region um Halle CARLA mit 12,0% (10,3%-13,7%) und die niedrigste Prävalenz im Süden Deutschlands KORA mit 5,8% (4,9%-6,7%). Bei Personen mit T2DM wurde eine Behandlung mit oralen Antidiabetika häufiger im Süden mit 56,9% (48,7%-65,9%) als im Nordosten SHIP mit 46,0% (39,8%-52,2%) berichtet. Eine Behandlung nur mit Insulin wurde häufiger im Nordosten SHIP mit 21,6% (16,5–26,7) als im Süden KORA mit 16,4% (10,4–22,5) berichtet.
Schlussfolgerungen:
Die Schätzungen der Prävalenz des T2DM weisen einen Süd-West und Nord-Ost Gradienten innerhalb Deutschlands auf mit der höchsten standardisierten Prävalenz in der Region Halle. Diese regionalen Unterschiede stimmen mit der Verteilung von Risikofaktoren für einen T2DM innerhalb Deutschlands überein.

Die Untersuchung der gesundheitsbezogenen Lebensqualität von Personen mit Typ 2 Diabetes ist Gegenstand eines Teilprojekts des DIAB-CORE Verbunds im Kompetenznetz Diabetes. Durch die gemeinsame ...Auswertung von fünf bevölkerungsbasierten epidemiologischen deutschen Studien sollen Referenzwerte für die Lebensqualität von Personen mit Diabetes im Vergleich zu Nicht-Erkrankten in Deutschland ermittelt werden und weitere Faktoren identifiziert werden, die die Lebensqualität von Personen mit Typ 2 Diabetes beeinträchtigen.
Als generische Instrumente stehen der SF-36 sowie die Kurzversion SF-12 zur Verfügung. Die Auswertung erfolgt durch Berechnung der körperlichen und der psychischen Summenskalen des SF-12. Die Berücksichtigung von Alter, Geschlecht und weiteren möglichen Einflussvariablen erfolgt mittels multivariater Regressionsmodelle. Für die hier beschriebenen Auswertungen stehen Daten von 9579 Personen im Alter zwischen 45–74 Jahren aus vier regionalen Surveys und dem bundesweiten Bundesgesundheitssurvey 1998 des Robert Koch Instituts (BGS98) zur Verfügung. Davon sind 8,8% nach eigenen Angaben an einem Diabetes erkrankt oder erhalten eine anti-diabetische Medikation.
Für Personen mit Typ 2 Diabetes lässt sich eine deutliche Verringerung der körperlichen gesundheitsbezogene Lebensqualität im Vergleich zu Personen ohne Typ 2 Diabetes feststellen (-4,1 Punkte auf der körperlichen Summenskala 95%KI: -4,8; -3,4). Nur bei Frauen zeigt sich ein entsprechender signifikanter Unterschied auch auf der psychischen Summenskala. Der Unterschied zwischen Frauen mit und ohne Typ 2 Diabetes beträgt -2,6 Punkte auf der psychischen Summenskala 95% KI: -3,5; -1,7). Auch unter Berücksichtigung von Komorbiditäten wie Herzinfarkt und Schlaganfall bleiben diese Unterschiede signifikant bestehen. Körperliche Lebensqualität ist unabhängig vom Diabetesstatus positiv mit gesunder Lebensführung, Normalgewicht und höherer Schulbildung assoziiert. Ebenso unabhängig vom Diabetesstatus geben Personen, die allein leben, eine deutlich geringere Lebensqualität im psychischen Bereich an (-1,6 95% KI: -2,1; -1,1).
Gesundheitsbezogene Lebensqualität ist eine wichtige patientenorientierte Zielgröße für die klinische Praxis. Die vorliegenden Ergebnisse zeigen, dass die Erkrankung an Typ 2 Diabetes mit einer beträchtlichen Verringerung der gesundheitsbezogenen Lebensqualität im körperlichen Bereich verbunden ist. Der zusätzliche Unterstützungsbedarf von Frauen mit Typ 2 Diabetes im Bereich der psychischen Lebensqualität sollte in Versorgungsmodellen verstärkt berücksichtigt werden. Die Kooperation im DIAB-CORE Verbund ermöglicht erstmals die Untersuchung von gesundheitsbezogener Lebensqualität an einer großen bevölkerungsbezogenen Stichprobe aus verschiedenen Regionen Deutschlands.

Background/objectives
The risk to die from an infectious disease in Germany has been continuously decreasing over the last century. Since infections are, however, not only causes of death but risk ...factors for diseases like cardiovascular diseases, it is essential to monitor and analyze their prevalence and frequency, especially in consideration of the increased life expectancy. To gain more knowledge about infectious diseases as risk factors and their implications on the condition and change of the immune status, the German National Cohort (GNC), a population-based prospective cohort study, will recruit 200,000 subjects between 2014 and 2017. In Pretest 1, a feasibility study for the GNC, we evaluated a self-administered and self-report questionnaire on infectious diseases and on the use of health care facilities (hereinafter called “ID Screen”) for feasibility and validity.
Methods
From August–November 2011, 435 participants between the ages of 20–69 completed the ID Screen. All subjects had been recruited via a random sample from the local residents’ registration offices by 4 of the 18 participating study centers. The questionnaire encompasses 77 variables in six sections assessing items such as 12-month prevalence of infections, cumulative prevalence of infectious diseases, visit of health care facilities and vaccination. The feasibility was amongst others evaluated by assessing the completeness and comprehensiveness of the questionnaire. To assess the questionnaires ability to measure “immune status” and “susceptibility to infections”, multivariate analysis was used.
Results
The overall practicability was good and most items were well understood, demonstrated by < 2/33 missing questions per questionnaire and only three variables: vaccination for influenza and pneumococci and infection with chickenpox had a frequency > 5 % of missing values. However, direct comparison of the items 12-month prevalence and lifetime prevalence of nephritis/pyelitis showed poor agreement and thereby poor understanding by 80 % of the participants, illustrating the necessity for a clear, lay person appropriate description of rare diseases to increase comprehensibility. The questionnaire will be used to support the assessment of immune dysfunction and frequency of infection. An analysis of these constructs in an exploratory factor analysis revealed limited applicability due to low interitem correlation (Cronbach’s α < 0.5). This is corroborated by the extraction of more than one factor with a Kaiser–Meyer–Olkin measure of 0.6 instead of a unidimensional latent construct for “immune status”.
Conclusion
All in all, the ID Screen is a good and reliable tool to measure infectious diseases as risk factors and outcome in general, but requires a better translation of infection specific terms into lay person terms. For the assessment of the overall immune status, the tool has strong limitations. Vaccinations status should also rather be assessed based on vaccination certificates than on participants’ recall.
The inclusion of biomarkers in epidemiological research provides new possibilities for exposure assessment and the study of early structural or functional changes and pre-clinical stages of diseases. ...At the same time issues of validity, reliability, and quality control as well as logistics require special attention. Usually epidemiological studies become more expensive with regard to time and cost. Interdisciplinary collaboration between epidemiology, basic research, and laboratory research is crucial. A prerequisite for this collaboration are agreements on definitions, methods and procedures. The definition of "biomarker" and a description of previous uses of biomarkers in epidemiological studies are presented in the first part of this paper. The second part addresses genetic markers and markers of individual sensitivity and susceptibility. We will end with a discussion about the possible future of biomarkers in epidemiology.
