Interferon (IFN)-γ release assays (IGRAs) are used to diagnose latent tuberculosis (TB) infection (LTBI). To improve the accuracy of these tests, different approaches, such as alternative cytokine ...detection and using different antigens, are considered. Following this purpose, this study aims to evaluate the addition of EspC, EspF and Rv2348-B to those present in the QuantiFERON-TB Gold In-Tube (QFN-G-IT). We included 115 subjects: 74 active TB patients, 17 LTBI individuals and 24 healthy controls. Whole blood samples were collected in QFN-G-IT and in-house tubes containing different combinations of EspC, EspF and Rv2348-B, together with ESAT-6, CFP-10, and TB7.7. After overnight incubation at 37 ºC, plasma was harvested and IFN-γ quantified. IFN-γ levels in the QFN-G-IT and in-house tubes correlated very good (Spearman Rho(r) > 0.86). In-house antigen combinations distinguished healthy individuals from those with active TB and LTBI (specificities and sensitivities higher than 87.5% and 96.3%, respectively AUC > 0.938). Adding EspC, EspF and Rv2348-B, increased the sensitivity of the test, being the addition of EspC and Rv2348-B the combination that yielded a higher sensitivity with no specificity loss. Addition of these antigens could improve diagnosis in patients with impaired or immature immune response who are at high risk of developing TB.
Highlights • A seroepidemiological study was conducted to determine the proportion of HCW susceptible to varicella. • The overall prevalence of immunity to VZV infection was 94.9% (95% CI: ...92.9–96.4). • HCWs aged 25–35 years had the highest susceptibility (8.1%, 95% CI: 4.6–13.0). • Neither a history of VZV infection nor VZV vaccination was significantly associated with serological immunity. • The high number of susceptible HCWs indicates the need to develop strategies for their screening and vaccination.
Healthcare workers (HCW) are exposed to preventable infectious diseases, notably hepatitis B (HB). The aim of this study was to determine the immunity of HCW against hepatitis B. We made a ...seroprevalence study using a self-administered survey and obtained blood samples. Antibodies against the HB surface antigen (anti-HBs) and against the HB core antigen (anti-HBc) were studied. The odds ratio (OR) and 95% confidence intervals (CI) were calculated. The adjusted OR were calculated using logistic regression.
Of the 644 HCW who participated (29.7% physicians, 38.7% nurses, 13.4% other clinical workers and 18.3% non-clinical workers), 46.4% were primary care workers and 53.6% hospital workers. The overall prevalence of anti-HBs was 64.4%. HCW aged <25 y had a significantly higher prevalence (86.7%) than those aged 35-44 y (adjusted OR 3.40; 95% CI 1.06-10.90). The prevalence of anti-HBc was 4.1%, and increased with age. Significant differences were found for HCW aged >44 y with respect to those aged 25-34 y. 75.6% of HCW stated they were vaccinated, but only 39.3% had a vaccination card. In HCW who stated they were unvaccinated, 10.8% had a vaccinated serological pattern (anti-HBc-negative and anti-HBs -positive). Written, updated vaccination records are essential to reliably determine the vaccination status.
The research program of the TCV tokamak ranges from conventional to advanced-tokamak scenarios and alternative divertor configurations, to exploratory plasmas driven by theoretical insight, ...exploiting the device's unique shaping capabilities. Disruption avoidance by real-time locked mode prevention or unlocking with electron-cyclotron resonance heating (ECRH) was thoroughly documented, using magnetic and radiation triggers. Runaway generation with high-Z noble-gas injection and runaway dissipation by subsequent Ne or Ar injection were studied for model validation. The new 1 MW neutral beam injector has expanded the parameter range, now encompassing ELMy H-modes in an ITER-like shape and nearly non-inductive H-mode discharges sustained by electron cyclotron and neutral beam current drive. In the H-mode, the pedestal pressure increases modestly with nitrogen seeding while fueling moves the density pedestal outwards, but the plasma stored energy is largely uncorrelated to either seeding or fueling. High fueling at high triangularity is key to accessing the attractive small edge-localized mode (type-II) regime. Turbulence is reduced in the core at negative triangularity, consistent with increased confinement and in accord with global gyrokinetic simulations. The geodesic acoustic mode, possibly coupled with avalanche events, has been linked with particle flow to the wall in diverted plasmas. Detachment, scrape-off layer transport, and turbulence were studied in L- and H-modes in both standard and alternative configurations (snowflake, super-X, and beyond). The detachment process is caused by power 'starvation' reducing the ionization source, with volume recombination playing only a minor role. Partial detachment in the H-mode is obtained with impurity seeding and has shown little dependence on flux expansion in standard single-null geometry. In the attached L-mode phase, increasing the outer connection length reduces the in-out heat-flow asymmetry. A doublet plasma, featuring an internal X-point, was achieved successfully, and a transport barrier was observed in the mantle just outside the internal separatrix. In the near future variable-configuration baffles and possibly divertor pumping will be introduced to investigate the effect of divertor closure on exhaust and performance, and 3.5 MW ECRH and 1 MW neutral beam injection heating will be added.
Childhood obesity and asthma are increasing worldwide. A possible link between the two conditions has been postulated.
Cross-sectional studies of stratified random samples of 8-12-year-old children ...(n = 10 652) (16 centres in affluent and 8 centres in non-affluent countries) used the standardized methodology of ISAAC Phase Two. Respiratory and allergic symptoms were ascertained by parental questionnaires. Tests for allergic disease were performed. Height and weight were measured, and overweight and obesity were defined according to international definitions. Prevalence rates and prevalence odds ratios were calculated.
Overweight (odds ratio = 1.14, 95%-confidence interval: 0.98; 1.33) and obesity (odds ratio = 1.67, 95%-confidence interval: 1.25; 2.21) were related to wheeze. The relationship was stronger in affluent than in non-affluent centres. Similar results were found for cough and phlegm, rhinitis and eczema but the associations were mostly driven by children with wheeze. There was a clear association of overweight and obesity with airways obstruction (change in FEV1/FVC, -0.90, 95%-confidence interval: -1.33%; -0.47%, for overweight and -2.46%, 95%-confidence interval: -3.84%; -1.07%, for obesity) whereas the results for the other objective markers, including atopy, were null.
Our data from a large international child population confirm that there is a strong relation of body mass index with wheeze especially in affluent countries. Moreover, body mass index is associated with an objective marker of airways obstruction (FEV1/FVC) but no other objective markers of respiratory and allergic disorders.

A forward model to resolve the fast-ion loss velocity-space on a fast-ion loss detector (FILD) probe head (FILDSIM) has been extended, making it possible to perform real-time analysis of the FILD ...data (“real-time FILDSIM”). Parametric pre-processing with FILDSIM enables real-time mapping of the raw FILD measurements to the velocity-space of the fast-ion distribution reaching the FILD probe, which depends on the local magnetic field at the probe head. Such parametric pre-processing facilitates the study of fast-ion losses in stages of the discharge other than the flat-top, such as the ramp-up phase when changes in the local magnetic field at the probe head cannot be neglected. Real-time FILDSIM has been applied to the existing and newly installed FILDs in ASDEX Upgrade and will be used for the forthcoming FILD in MAST-Upgrade. Due to the larger size of the MAST-U FILD probe, the approximation used in FILDSIM of a uniform magnetic field in the FILD region has been generalised to the case of a non-uniform field, with gyro-orbits calculated numerically in this field.
JT-60SA, the largest tokamak that will operate before ITER, has been designed and built jointly by Japan and Europe, and is due to start operation in 2020. Its main missions are to support ITER ...exploitation and to contribute to the demonstration fusion reactor machine and scenario design. Peculiar properties of JT-60SA are its capability to produce long-pulse, high-β, and highly shaped plasmas. The preparation of the JT-60SA Research Plan, plasma scenarios, and exploitation are producing physics results that are not only relevant to future JT-60SA experiments, but often constitute original contributions to plasma physics and fusion research. Results of this kind are presented in this paper, in particular in the areas of fast ion physics, high-beta plasma properties and control, and non-linear edge localised mode stability studies.
A magnetically driven fast-ion loss detector (FILD) is being designed for ITER. Due to the high heat load expected, fast movements and several measurement cycles will be required during its ...operation. The measurement cycle characteristics (measuring and parking periods, as well as the probe insertion and retraction times) will be defined as a compromise between the thermal load acting on the probe head and the measuring requirements. In this article, dynamic simulations of the detector movement are presented and the thermal behavior of the probe head is analyzed during its operation within a complete plasma discharge.
