Purpose In some patients, “surgery first” (SF) may represent a reasonable approach for the expedited correction of a maxillofacial deformity. Based on the prospective evaluation of a large sample, ...this article provides a specific orthodontic and surgical protocol, discusses the benefits and limitations of this approach, and updates its indications. Materials and Methods Forty-five patients were managed with an SF approach. Selected cases presented symmetrical skeletal malocclusions with no need for extractions or surgically assisted rapid palatal expansion. Periodontal or temporomandibular joint problems and management by an orthodontist without experience in orthognathic surgery were considered exclusion criteria. Virtual treatment planning included a 3-dimensional orthodontic setup. Standard orthognathic osteotomies were followed by buccal interdental corticotomies to amplify the regional acceleratory phenomenon. Miniscrews were placed for postoperative skeletal stabilization. Orthodontic treatment began 2 weeks after surgery. Archwires were changed every 2 to 3 weeks. At 12-month follow-up, patient satisfaction and orthodontist satisfaction were evaluated on a visual analog scale of 1 to 10. Descriptive statistics were computed for all study variables. Results The studied sample consisted of 27 women and 18 men (mean age, 23.5 yr). The main motivation for treatment was the wish to improve facial esthetics. Bimaxillary surgery was the most common procedure. Mean duration of orthodontic treatment was 37.8 weeks, with an average of 22 orthodontic appointments. Mean patient and orthodontist satisfaction scores were 9.4 (range, 8 to 10) and 9.7 (range, 8 to 10), respectively. Conclusions The SF approach significantly shortens total treatment time and is very favorably valued by patients and orthodontists. Nevertheless, careful patient selection, precise treatment planning, and fluent bidirectional feedback between the surgeon and the orthodontist are mandatory.
Introduction Compared to the conventional approach to orthognathic surgery, “surgery first” protocols could be advantageous in terms of shortened treatment times and immediate esthetic improvement. ...However, consensus regarding patient selection, technical protocol, and stability is still lacking. Methods A systematic review of the scientific literature on surgery-first treatment (January 2000 to January 2015) was performed. The PubMED and Cochrane Library databases were accessed. Patient selection criteria, specific surgical-orthodontic protocol, treatment duration, patient and orthodontist satisfaction, and stability of results were compared with a similar population treated conventionally. Results The search yielded 179 publications. The application of strict selection criteria gave the final group of 11 articles. In total, 295 patients were managed with a surgery-first approach. A Class III malocclusion was the most prevalent underlying malocclusion (84.7%). Total treatment duration was shorter in surgery-first patients than in those treated conventionally. There was substantial heterogeneity among articles and high reporting bias regarding the inclusion and exclusion criteria, the orthodontic and surgical protocols, and the stability of results. A meta-analysis of combined data was not possible. Conclusions The surgery-first approach is a new treatment paradigm for the management of dentomaxillofacial deformity. Studies have reported satisfactory outcomes and high acceptance. However, the results should be interpreted with caution because of the wide varieties of study designs and outcome variables, reporting biases, and lack of prospective long-term follow-ups.
Given the inconsistencies of wind gust trends under the widespread decline in near‐surface wind speed (stilling), our study aimed to assess trends of observed daily peak wind gusts (DPWG) across ...Spain and Portugal for 1961–2014 by analyzing trends of (i) the frequency (90th percentile) and (ii) the magnitude (wind speed maxima) of DPWG. Wind gust series were homogenized on a daily basis, using MM5‐simulated series as reference, resulting in 80 suitable station‐based data sets. The average DPWG 90th percentile frequency declined by −1.49 d decade−1 (p < 0.05) annually. This showed marked seasonal differences: decreasing in winter (−0.75 d decade−1; p < 0.05) and increasing in summer (+0.18 d decade−1; p > 0.10). A negligible trend was calculated for the annual magnitude of DPWG (−0.005 m s−1 decade−1; p > 0.10), with distinct seasonality: declining in winter (−0.168 m s−1 decade−1; p < 0.10) and increasing in summer (+0.130 m s−1 decade−1; p < 0.05). Combined, these results reveal less frequent and declining DPWG during the cold semester (November–April) and more frequent and increasing DPWG during the warm semester (May–October). Large‐scale atmospheric changes such as the North Atlantic Oscillation Index (negative correlations ~−0.4–−0.6; p < 0.05) and the Jenkinson and Collison scheme (positive correlations mainly with Westerly regime: ~+0.5–0.6; p < 0.05) partly account for the decadal fluctuations of both frequency and magnitude of DPWG, particularly in winter. However, the North Atlantic Oscillation index‐DPWG relationships are smaller in spring, summer, and autumn (~−0.1–−0.2; p > 0.10), especially for the frequency, suggesting the role of local‐to‐mesoscale drivers.

Daily and sub‐daily homogenization of climate variables have been intensively investigated in the last decades, but to the best of our knowledge, this is the first study on homogenization of hourly ...temperature in Romania. This paper describes the creation of a homogenized hourly air temperature data set at a country scale by combining data from four independent meteorological networks. The air temperature measurements for the period 2009 and 2017 were obtained from the following networks: Romanian National Meteorological Administration (ANM), National Network for Monitoring Air Quality (RNMCA), Regional Basic Synoptic Network (RBSN), and Meteorological Terminal Aviation Routine Weather Report network (METAR). The climatological limits, persistence, temporal variation (step test), and spatial consistency were the quality control tests used to isolate the errors due to malfunctioning of the temperature sensors, data coding or transmission. The Climatol homogenization method was successfully applied for identifying and correcting any suspicious values. The missing data were filled by considering the similarities between each station and the reference series. Comparing the output with the original data, it is apparent that the removal of the break points, correction and homogenization resulted in a new data set with statistical properties very similar to the raw data, but more reliable for climate research due to the increased homogeneity. Eventually, the procedure can be implemented in operational use for collecting more data from other networks.

The aim of time series homogenization is to remove nonclimatic effects, such as changes in station location, instrumentation, observation practices, and so on, from observed data. Statistical ...homogenization usually reduces the nonclimatic effects but does not remove them completely. In the Spanish “MULTITEST” project, the efficiencies of automatic homogenization methods were tested on large benchmark datasets of a wide range of statistical properties. In this study, test results for nine versions, based on five homogenization methods—the adapted Caussinus-Mestre algorithm for the homogenization of networks of climatic time series (ACMANT), “Climatol,” multiple analysis of series for homogenization (MASH), the pairwise homogenization algorithm (PHA), and “RHtests”—are presented and evaluated. The tests were executed with 12 synthetic/surrogate monthly temperature test datasets containing 100–500 networks with 5–40 time series in each. Residual centered root-mean-square errors and residual trend biases were calculated both for individual station series and for network mean series. The results show that a larger fraction of the nonclimatic biases can be removed from station series than from network-mean series. The largest error reduction is found for the long-term linear trends of individual time series in datasets with a high signal-to-noise ratio (SNR), where the mean residual error is only 14%–36% of the raw data error. When the SNR is low, most of the results still indicate error reductions, although with smaller ratios than for large SNR. In general, ACMANT gave the most accurate homogenization results. In the accuracy of individual time series ACMANT is closely followed by Climatol, and for the accurate calculation of mean climatic trends over large geographical regions both PHA and ACMANT are recommended.

Context. Probing the evaporation of exoplanet atmospheres is key to understanding the formation and evolution of exoplanetary systems. The main tracer of evaporation in the UV is the Lyman-α ...transition, which can reveal extended exospheres of neutral hydrogen. Recently, the near-infrared (NIR) metastable helium triplet (10 833 Å) revealed extended thermospheres in several exoplanets. This opens a new window into evaporation. Aims. We aim at spectrally resolving the first helium absorption signature detected in the warm Saturn WASP-107b with the Wide Filed Camera 3 on board the Hubble Space Telescope (HST/WFC3). Methods. We obtained one transit of WASP-107b with CARMENES installed on the 3.5 m telescope at the Calar Alto observatory. Results. We detect an excess helium absorption signature of 5.54 ± 0.27% (20σ) in the planet rest frame during the transit. The detection is in agreement with the previous detection achieved with HST/WFC3. The signature shows an excess absorption in the blue part of the lines, suggesting that He I atoms are escaping from the atmosphere of WASP-107b. We interpret the time-series absorption spectra using the 3D EVE code. Our observations can be explained by combining an extended thermosphere that fills half of the Roche lobe and a large exospheric tail sustained by an escape rate of metastable helium of about 106 g s−1. In this scenario, however, the upper atmosphere needs to be subjected to a reduced photoionisation and radiation pressure from the star for the model to match the observations. Conclusions. We confirm the presence of helium in the atmosphere of WASP-107b at high confidence. The helium feature is detected from space and from the ground. The ground-based high-resolution signal brings detailed information about the spatial and dynamical structure of the upper atmosphere, and simulations suggest that the He I signature of WASP-107b probes both its thermosphere and exosphere, establishing this signature as a robust probe of exoplanetary upper atmospheres. Surveys with NIR high-resolution spectrographs (e.g. CARMENES, the Spectromètre infrarouge (SPIRou), or the Near-Infrared Planet Searcher (NIRPS)) will deliver a statistical understanding of exoplanet thermospheres and exospheres through the helium triplet.
Using clustering analysis for the sea level pressure field of the ERA-Interim reanalysis between 1979 and 2016, five synoptic pressure patterns have been obtained for the Drake area and Antarctic ...Peninsula (AP) region (45°–75°S, 20°–120°W), and the resulting daily series has been made available to the scientific community. The five patterns have been named according to their most important features as follows: low over the Weddell Sea (LWS), low over the Amundsen and Bellingshausen Seas (LAB), low over the Drake Passage (LDP), zonal flow over the Drake Passage (ZDP), and ridge over the Antarctic Peninsula (RAP). Each atmospheric pattern is described after analyzing its development and evolution. A frequency analysis shows that the five atmospheric patterns present a similar annual frequency but a large seasonal variability. The transitions from one pattern to another tend to follow a cycle in which synoptic atmospheric waves are displaced eastward by a quarter wavelength. Four of the five atmospheric patterns (all except RAP) are very influenced by the southern annular mode (SAM); however, only LAB and LWS are influenced to some degree by ENSO. The occurrence of the LAB pattern presents a positive trend showing agreement with other studies that indicate an enhancement of the Amundsen–Bellingshausen Seas low. Finally, atmospheric circulation patterns have been related to the airmass advection and precipitation in Livingston Island, showing the potential application for studying the changes in the surface mass balance on the AP cryosphere.

Summary
Bacterial cold water disease (BCWD) is a globally distributed freshwater fish disease caused by the Gram‐negative bacterium Flavobacterium psychrophilum. It is a particularly devastating ...infection in fry salmonids and may lead to high levels of mortality. In spite of its economic impact on fish farms, neither the biology of the bacterium nor the bacterium–host interactions are well understood. This review provides a synopsis of the major problems related to critical remaining questions about research into the use of vaccines against F. psychrophilum and the development of a commercial vaccine against this disease. Studies using sera from convalescent rainbow trout have shown the antigenic properties of different proteins such as OmpH, OmpA and FspA, as well as low and high molecular mass lipopolysaccharide of F. psychrophilum, which are potential candidates for subunit vaccines. Inactivated F. psychrophilum bacterins have been successfully tested as vaccines under laboratory conditions by both immersion and intraperitoneal routes. However, the efficacy and the practical usefulness of these preparations still have to be proved. The use of attenuated and wild‐type strains to immunize fish showed that these systems offer high levels of protection. Nevertheless, their application clashes with the regulations for environmental protection in many countries. In conclusion, protective vaccines against BCWD are theoretically possible, but substantial efforts still have to be made in order to permit the development of a commercial vaccine.
A new high‐resolution (5 km) gridded daily precipitation dataset for Tunisia between 1979 and 2015 is introduced. This product combines 960 rain gauges with the SAFRAN analysis to produce the ...precipitation gridded data. A validation approach on two different datasets reveals that the SAFRAN analysis outperforms other standard interpolation methods such as Inverse Distance, Nearest Neighbors, Ordinary Kriging or Residual Kriging with altitude. When compared to EOBS, a widely used gridded dataset over Europe, a strong negative bias in EOBS precipitation is found. However due to the aridity and the low density of rain gauges in south Tunisia, results in this region must be analyzed with care. The SAFRAN product could be useful for various purposes such as climate model evaluation, climate studies, hydrological modelling to support the planning and management of surface water resources in Tunisia.

Assessing change in daily maximum wind speed and its likely causes is crucial for many applications such as wind power generation and wind disaster risk governance. Multidecadal variability of ...observed near-surface daily maximum wind speed (DMWS) from 778 stations over China is analyzed for 1975–2016. A robust homogenization protocol using the R package Climatol was applied to the DMWS observations. The homogenized dataset displayed a significant (p < 0.05) declining trend of −0.038 m s−1 decade−1 for all China annually, with decreases in winter (−0.355 m s−1 decade−1, p < 0.05) and autumn (−0.108 m s−1 decade−1; p < 0.05) and increases in summer (+0.272 m s−1 decade−1, p < 0.05) along with a weak recovery in spring (+0.032 m s−1 decade−1; p > 0.10); that is, DMWS declined during the cold semester (October–March) and increased during the warm semester (April–September). Correlation analysis of the Arctic Oscillation, the Southern Oscillation, and the west Pacific modes exhibited significant correlation with DMWS variability, unveiling their complementarity in modulating DMWS. Further, we explored potential physical processes relating to the atmospheric circulation changes and their impacts on DMWS and found that 1) overall weakened horizontal airflow large-scale mean horizontal pressure gradient (from −0.24 to +0.02 hPa decade−1) and geostrophic wind speed (from −0.6 to +0.6 m s−1 decade−1), 2) widely decreased atmospheric verticalmomentum transport atmospheric stratification thermal instability (from −3 to +1.5 decade−1) and vertical wind shear (from −0.4 to +0.2 m s−1 decade−1), and 3) decreased extratropical cyclones frequency (from −0.3 to 0 month decade−1) are likely causes of DMWS change.
