Cervicogenic headache (CEH) is a unilateral headache localised in the neck or occipital region, projecting to the frontal and temporal regions. Since the pathogenesis of this syndrome appears to have ...an anatomical basis in the cervical region, several surgical procedures aimed at reducing the nociceptive input on the cervical level, have been tested. We developed a sequence of various cervical radiofrequency neurotomies (facet joint denervations eventually followed by upper dorsal root ganglion neurotomies) that proved successful in a prospective pilot trial with 15 CEH patients. To further evaluate this sequential treatment program we conducted a randomised controlled trial
30 patients with cervicogenic headache according to the Sjaastad diagnostic criteria, were randomised. 15 patients received a sequence of radiofrequency treatments (cervical facet joint denervation, followed by cervical dorsal root ganglion lesions when necessary), and the other 15 patients underwent local injections with steroid and anaesthetic at the greater occipital nerve, followed by transcutaneous electrical nerve stimulation (TENS) when necessary. Visual analogue scores for pain, global perceived effects scores, quality of life scores were assessed at 8, 16, 24 and 48 weeks. Patients also kept a headache diary.
There were no statistically significant differences between the two treatment groups at any time point in the trial.
We did not find evidence that radiofrequency treatment of cervical facet joints and upper dorsal root ganglions is a better treatment than the infiltration of the greater occipital nerve, followed by TENS for patients fulfilling the clinical criteria of cervicogenic headache.

This paper reports on the Second International Nurse Rostering Competition (INRC-II). Its contributions are (1) a new problem formulation which, differently from INRC-I, is a multi-stage procedure, ...(2) a competition environment that, as in INRC-I, will continue to serve as a growing testbed for search approaches to the INRC-II problem, and (3) final results of the competition. We discuss also the competition environment, which is an infrastructure including problem and instance definitions, testbeds, validation/simulation tools and rules. The hardness of the competition instances has been evaluated through the behaviour of our own solvers, and confirmed by the solvers of the participants. Finally, we discuss general issues about both nurse rostering problems and optimisation competitions in general.
A single nucleotide polymorphism (SNP) is a difference in the DNA sequence of one nucleotide only. We recently proposed a lab-on-a-chip (LoC) system which has the potentiality of fast, sensitive and ...highly specific SNP detection. Most of the chip components are silicon based and fabricated within a single process. In this paper, the newly developed fabrication method for the silicon chip is presented. The robust and reliable process allows etching structures on the same chip with very different aspect ratios. The characterization of a crucial component to the LoC SNP detector, the microreactor where DNA amplification is performed, is also detailed. Thanks to innovative design and fabrication methodologies, the microreactor has an excellent thermal isolation from the surrounding silicon substrate. This allows for highly localized temperature control. Furthermore, the microreactor is demonstrated to have rapid heating and cooling rates, allowing for rapid amplification of the target DNA fragments. Successful DNA amplification in the microreactor is demonstrated.

First, we describe the problem formulation, which, differently from INRC-I, ...is a multi-stage procedure. Second, we illustrate all the necessary infrastructure do be used together with the participant's solver, including the testbed, the file formats, and the validation/simulation tools.
