Abstract
Laparoscopic adjustable gastric bands are a popular and effective surgical option to treat morbid obesity. The overall complication rate is 10–20% and the most common complication is of ...‘slippage’. Although other complications such as gastric band migration and erosion have been reported, the phenomenon of a migrated gastric band connecting tube eroding into the colon (after port removal) is seldom reported in the literature. In this article we describe such a case of an incidentally found colonic erosion on colonoscopy and describe the subsequent laparoscopic repair, as well as a review of the literature.

For clinicians, it is important to rely on accurate laboratory results for patient care and optimal use of health care resources. We sought to explore our observations that urine protein:creatinine ...ratios (PrCr) ≥30 mg/mmol are seen not infrequently associated with normal pregnancy outcome.
Urine samples were collected prospectively from 160 pregnant women attending high-risk maternity clinics at a tertiary care facility. Urinary protein was measured using a pyrocatechol violet assay and urinary creatinine by an enzymatic method on Vitros analysers. Maternal/perinatal outcomes were abstracted from hospital records.
91/233 (39.1%) samples had a PrCr ≥30 mg/mmol, especially when urinary creatinine concentration was <3 mM (94.1%) vs. ≥3 mM (16.4%) (p < 0.001). When using the last sample before delivery, 47/160 (29.4%) had a PrCr ≥30 mg/mmol in diluted urine vs. only 17/160 (15.4%) in more concentrated urine (p < 0.001); PrCr positive results were also more frequent among the 32 (20.0%) women with known normal pregnancy outcome (90.9% vs. 0) (p < 0.001). Using the same analyser, 0.12 g/L urinary protein was 'detected' in deionised water. Re-analysis of data from two cohorts revealed substantially less inflation of PrCr in dilute urine using a pyrogallol red assay.
Random urinary PrCr was overestimated in dilute urine when tested using a common pyrocatechol violet dye-based method. This effect was reduced in cohorts when pyrogallol red assays were used. False positive results can impact on diagnosis and patient care. This highlights the need for both clinical and laboratory quality improvement projects and standardization of laboratory protein measurement.
This study was done to define the seasonality of respiratory syncytial virus (RSV) epidemics in the southeastern United States.
We tested 5,092 fresh nasal aspirates or washings, using the Kallestad ...Pathfinder enzyme immunoassay (EIA) or the Directigen RSV membrane EIA according to manufacturers' directions.
A total of 1,419 (27.9%) respiratory specimens, collected from pediatric patients between May 1993 and December 1996 at two large southeastern medical centers, were positive for RSV antigen by enzyme immunoassay. A persistent RSV epidemic was documented between July 1993 and December 1996. We defined an epidemic as 2 consecutive months in each of which > or = 10% of the specimens were positive and > or = 2 positive specimens detected. The highest percentages of positive specimens were detected in December of 1993, 1994, and 1996, to date, and November 1995.
On the basis of our findings, we recommend health care workers consider RSV in the differential diagnosis of acute respiratory illness throughout the year in pediatric patients from the southeast United States.

The practice of Clinical Microbiology has many different forms and takes place in many different environments. From reference laboratories to rural community hospitals to public health laboratories, ...there are a diverse set of needs for microbiology expertise that extend far beyond the traditional model. In a larger clinical laboratory setting, such as an academic medical center or a reference laboratory, there are often personnel dedicated exclusively to the oversight and practice of microbiology. However, in many other environments and situations, there is a void of microbiology expertise which must be filled. These unmet needs can be addressed by a non-traditional microbiologist, the consultant. Like the practice of microbiology itself, serving as a microbiology consultant can take many forms and can be an appealing alternative for some who are looking to create a more flexible life-style yet continue to utilize their microbiology expertise. The following discussion will outline some key considerations for those looking to start their own microbiology consulting business. In addition, the manuscript will describe the various consulting opportunities that exist and what the “day in a life” of a consultant microbiologist might look like.
Acquired surgical disease of the pulmonary valve is rare. We report a 72-year-old man who presented with subacute endocarditic pulmonary regurgitation. This lesion was surgically corrected with a ...stentless bioprosthesis. Previously, homografts and various xenografts have been used for replacement of the pulmonary valve both in the pediatric population and in adult patients with congenital heart disease. Pulmonary regurgitation is a rare lesion, but if it is encountered our case demonstrates that it can be successfully and easily treated with pulmonary valve replacement by using a stentless bioprosthesis.
