IMPORTANCE: It remains uncertain whether nebulization of mucolytics with bronchodilators should be applied for clinical indication or preventively in intensive care unit (ICU) patients receiving ...invasive ventilation. OBJECTIVE: To determine if a strategy that uses nebulization for clinical indication (on-demand) is noninferior to one that uses preventive (routine) nebulization. DESIGN, SETTING, AND PARTICIPANTS: Randomized clinical trial enrolling adult patients expected to need invasive ventilation for more than 24 hours at 7 ICUs in the Netherlands. INTERVENTIONS: On-demand nebulization of acetylcysteine or salbutamol (based on strict clinical indications, n = 471) or routine nebulization of acetylcysteine with salbutamol (every 6 hours until end of invasive ventilation, n = 473). MAIN OUTCOMES AND MEASURES: The primary outcome was the number of ventilator-free days at day 28, with a noninferiority margin for a difference between groups of −0.5 days. Secondary outcomes included length of stay, mortality rates, occurrence of pulmonary complications, and adverse events. RESULTS: Nine hundred twenty-two patients (34% women; median age, 66 (interquartile range IQR, 54-75 years) were enrolled and completed follow-up. At 28 days, patients in the on-demand group had a median 21 (IQR, 0-26) ventilator-free days, and patients in the routine group had a median 20 (IQR, 0-26) ventilator-free days (1-sided 95% CI, −0.00003 to ∞). There was no significant difference in length of stay or mortality, or in the proportion of patients developing pulmonary complications, between the 2 groups. Adverse events (13.8% vs 29.3%; difference, −15.5% 95% CI, −20.7% to −10.3%; P < .001) were more frequent with routine nebulization and mainly related to tachyarrhythmia (12.5% vs 25.9%; difference, −13.4% 95% CI, −18.4% to −8.4%; P < .001) and agitation (0.2% vs 4.3%; difference, −4.1% 95% CI, −5.9% to −2.2%; P < .001). CONCLUSIONS AND RELEVANCE: Among ICU patients receiving invasive ventilation who were expected to not be extubated within 24 hours, on-demand compared with routine nebulization of acetylcysteine with salbutamol did not result in an inferior number of ventilator-free days. The century-old controversy over two contradicting theories on radiation pressure of light proposed by Abraham and Minkowski can come to an end if there is a direct method to measure the surface ...deformation of the target material due to momentum transfer of photons. Here we have investigated the effect of radiation pressure on the surface morphology of Graphene Oxide (GO) film, experienced due to low power focused laser irradiation. In-depth investigation has been carried out to probe the bending of the GO surface due to radiation pressure by Atomic Force Microscopy (AFM) and subsequently the uniaxial strain induced on the GO film has been probed by Raman Spectroscopy. Our results show GO film experience an inward pressure due to laser radiation resulting in inward bending of the surface, which is consistent with the Abraham theory. The bending diameter and depth of the irradiated spot show linear dependence with the laser power while an abrupt change in depth and diameter of the irradiated spot is observed at the breaking point. Such abrupt change in depth is attributed to the thinning of the GO film by laser irradiation.

Adhesion formation remains a major complication following hernia repair surgery. Physical barriers though effective for adhesion prevention in clinical settings are associated with major ...disadvantages, therefore, needs further investigation. This study evaluates silk fibroin hydrogel as a physical barrier on polypropylene mesh for the prevention of adhesion following ventral hernia repair.
Study design
Peritoneal explants were cultured on silk fibroin scaffold to evaluate its support for mesothelial cell growth. Full thickness uniform sized defects were created on the ventral abdominal wall of rabbits, and the defects were covered either with silk hydrogel coated polypropylene mesh or with plain polypropylene mesh as a control. The animals were killed after 1 month, and the adhesion formation was graded; healing response of peritoneum was evaluated by immunohistochemistry with calretinin, collagen staining of peritoneal sections, and expression of PCNA, collagen-I, TNFα, IL6 by real time PCR; and its adverse effect if any was determined.
Results
Silk fibroin scaffold showed excellent support for peritoneal cell growth in vitro and the cells expressed calretinin. A remarkable prevention of adhesion formation was observed in the animals implanted with silk hydrogel coated mesh compared to the control group; in these animals peritoneal healing was complete and predominantly by mesothelial cells with minimum fibrotic changes. Expression of inflammatory cytokines decreased compared to control animals, histology of abdominal organs, haematological and blood biochemical parameters remained normal.
Conclusion
Therefore, silk hydrogel coating of polypropylene mesh can improve peritoneal healing, minimize adhesion formation, is safe and can augment the outcome of hernia surgery.

In the vast physics program of the Belle II experiment, the vertex detector plays a crucial role for the determination of the B-meson decay vertices. It consists of two inner layers of pixelated silicon detectors and four outer layers of double-sided silicon strip detectors (SVD).
To achieve a design luminosity of 8×1035cm−2s−1, 40 times higher than the recorded luminosity of its predecessor, the SuperKEKB collider squeezes the beams to a vertical size of 50 nm (“nano-beam scheme”) and doubles the beam currents.
Therefore, the detectors are required to tolerate intense beam induced background due to the very high luminosity. During the 2019 spring run we measured the occupancy rate in the SVD to estimate the level of the beam induced background. With the low initial luminosity, the observed beam induced background mostly originated from Touschek processes and beam-gas scattering within individual beams. Since these different background contributions depend differently on accelerator conditions, such as the beam current, beam size and pressure, they can be disentangled. We estimate the background rate of each contribution and compare them with simulated ones. The results enable us to predict the background levels at increased beam currents and luminosity in the coming years. They also hint at background mitigation measures for running at higher luminosity. In this proceeding we present the results of our study of the beam induced background in the SVD and the prospects for future operation.
Since last few decades optics has already proved its strong potentiality for conducting parallel logic, arithmetic and algebraic operations due to its super-fast speed in communication and ...computation. So many different logical and sequential operations using all optical frequency encoding technique have been proposed by several authors. Here, we have keened out all optical dibit representation technique, which has the advantages of high speed operation as well as reducing the bit error problem. Exploiting this phenomenon, we have proposed all optical frequency encoded dibit based XOR and XNOR logic gates using the optical switches like add/drop multiplexer (ADM) and reflected semiconductor optical amplifier (RSOA). Also the operations of these gates have been verified through proper simulation using MATLAB (R2008a).
This paper presents the preliminary results of observation of total lightning characteristics of pre-monsoon thunderstorms of 2018 over Gangetic West Bengal around Kolkata. Mainly, the analysis ...results of total lightning associated with pre-monsoon moderate and severe thunderstorm events, locally known as Kalbaishakhi or Nor’wester, are presented. The total lightning data include both in-cloud and cloud-to-ground lightning flashes and are obtained from the Earth Networks Total Lightning Network. This study presents a rugged and cost-effective scheme for start-up and operation of a stand-alone squirrel cage induction machine (SCIG) for a wind energy conversion system (WECS). A voltage source ...converter (VSC) directly interfaces the SCIG with an equivalent dc load network, which could also be the DC link of an inverter. The VSC dc bus is supported by an electrolytic capacitor and the proposed scheme ensures reliable start up with these rudimentary components. The V/f control paradigm for motoring operation is suitably extended to the SCIG for controlling both voltage build-up and dynamic transients. A controller is specifically designed to maintain constant dc bus voltage under wind speed and electrical load variations. A steady-state machine flux is maintained constant up to the base speed, which maximises machine utilisation and power extraction especially at higher wind speeds. The overall system modelling and analytical control design is presented. The proposed control strategy has been validated through simulation and experimentally verified on a low power (2.2 kW) laboratory prototype.
