Background Although commonly used in combination with irinotecan or oxaliplatin (iri/oxal) for treatment of colorectal liver metastases before extirpation, the effects of preoperative bevacizumab on ...surgical outcomes are not established. The objective of this retrospective study was to determine if addition of bevacizumab to iri/oxal preoperative chemotherapy increases morbidity after hepatic resection. Study Design We compared demographics, clinicopathologic data, treatments, and postoperative outcomes between patients given preoperative iri/oxal with and without bevacizumab and patients who underwent hepatic resection within and after 8 weeks from the last dose of bevacizumab. Results From 1996 to 2006, 96 patients were treated with preoperative iri/oxal; 39 (40.6%) received concurrent bevacizumab. Preoperative bevacizumab treatment was associated with less blood loss (median 425 mL versus 600 mL, p = 0.01) and lower RBC transfusion rates (43.9% versus 23.1%, p = 0.06) after partial hepatectomy on univariable analysis. Only age ≥ 70 years (hazard ratio = 8.52, 95% CI 2.00 to 36.45) and concurrent extrahepatic procedures (hazard ratio = 4.12, 95% CI 1.49 to 11.39) independently predicted RBC transfusion and overall complications, respectively. There were no differences in overall (43.6% versus 38.6%), severe (28.2% versus 24.6%), hepatic (17.9% versus 26.3%), wound (10.3% versus 7%), or thromboembolic or bleeding (2.6% versus 5.3%) complications (all p > 0.05). For patients treated with iri/oxal and bevacizumab, overall complications were more common when resection was performed within 8 weeks after the last bevacizumab dose (62.5% versus 30.4%), but this difference was not statistically significant (p = 0.06). Conclusions If discontinued at least 8 weeks before hepatic resection, addition of bevacizumab to preoperative iri/oxal does not increase morbidity after hepatic resection.

We analytically study the stability of catenoide pinned at one contact line, with the other free to move on a substrate subject to axisymmetric and nonaxisymmetric perturbations. A variational ...formulation is applied to derive the corresponding stability criteria. The maximal stability region and the stability region are represented in the favorable and canonical phase diagrams, providing a complete description of catenoid equilibrium and stability. All catenoide are stable with respect to nonaxisymmetric perturbations. For a fixed contact angle, there exists a critical volume below which catenoide are unstable to axisymmetric perturbations. Equilibrium solution multiplicity is discussed in detail, and we elucidate how geometrical symmetry is reflected in the maximal stability and stability regions.
It has been reported that limited resection (segment or wedge) is equivalent to lobectomy in the management of early stage (T1–2 N0) non-small cell lung cancer.
A prospective, multiinstitutional ...randomized trial was instituted comparing limited resection with lobectomy for patients with peripheral T1 N0 non-small cell lung cancer documented at operation. Analysis included locoregional and distant recurrence rates, 5-year survival rates, perioperative morbidity and mortality, and late pulmonary function assessment.
There were 276 patients randomized, with 247 patients eligible for analysis. There were no significant differences for all stratification variables, selected prognostic factors, perioperative morbidity, mortality, or late pulmonary function. In patients undergoing limited resection, there was an observed 75% increase in recurrence rates (p = 0.02, one-sided) attributable to an observed tripling of the local recurrence rate (p = 0.008 two-sided), an observed 30% increase in overall death rate (p = 0.08, one-sided), and an observed 50% increase in death with cancer rate (p = 0.09, one-sided) compared to patients undergoing lobectomy (p = 0.10, one-sided was the predefined threshold for statistical significance for this equivalency study).
Compared with lobectomy, limited pulmonary resection does not confer improved perioperative morbidity, mortality, or late postoperative pulmonary function. Because of the higher death rate and locoregional recurrence rate associated with limited resection, lobectomy still must be considered the surgical procedure of choice for patients with peripheral T1 N0 non-small cell lung cancer.
The hadronic final states observed with the ALEPH detector at LEP in e+e- annihilation are analysed using 730 pb-1 of data collected between 91 and 209 GeV in the framework of QCD. In particular ...event-shape variables and inclusive charged particle spectra are measured. The energy evolution of quantities derived from these measurements is compared to analytic QCD predictions. The mean charged particle multiplicity, the charged particle momentum spectrum and its peak position are compared to predictions of the modified-leading-logarithmic approximation. The strong coupling constant alpha_s is determined from a fit of the QCD prediction to distributions of six event-shape variables at eight centre-of-mass energies. Mammalian Hedgehog (HH) signalling pathway plays an essential role in tissue homeostasis and its deregulation is linked to rheumatological disorders. UBR5 is the mammalian homologue of the E3 ...ubiquitin-protein ligase Hyd, a negative regulator of the Hh-pathway in Drosophila. To investigate a possible role of UBR5 in regulation of the musculoskeletal system through modulation of mammalian HH signaling, we created a mouse model for specific loss of Ubr5 function in limb bud mesenchyme. Our findings revealed a role for UBR5 in maintaining cartilage homeostasis and suppressing metaplasia. Ubr5 loss of function resulted in progressive and dramatic articular cartilage degradation, enlarged, abnormally shaped sesamoid bones and extensive heterotopic tissue metaplasia linked to calcification of tendons and ossification of synovium. Genetic suppression of smoothened (Smo), a key mediator of HH signalling, dramatically enhanced the Ubr5 mutant phenotype. Analysis of HH signalling in both mouse and cell model systems revealed that loss of Ubr5 stimulated canonical HH-signalling while also increasing PKA activity. In addition, human osteoarthritic samples revealed similar correlations between UBR5 expression, canonical HH signalling and PKA activity markers. Our studies identified a crucial function for the Ubr5 gene in the maintenance of skeletal tissue homeostasis and an unexpected mode of regulation of the HH signalling pathway.

Abstract Ischemic mitral regurgitation (MR) has been associated with worse outcome myocardial infarction. However, severity of MR and its impact on patients with STEMI undergoing primary PCI remains ...unknown. We sought to determine impact of increasing severity of ischemic MR on outcomes in patients with ST-elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PCI). All patients presenting with STEMI who underwent primary PCI within 12 hours of symptoms from 1994-2014 were included. Ischemic MR was graded from 0 to 4+ within 3 days of index MI by echocardiography. Overall, 4,005 STEMI patients were included. None, 1+, 2+, 3+ and 4+ MR was present in 3,200 (79.9%), 427 (10.7%), 260 (6.5%), 91 (2.3%) and 27 (0.7%) patients, respectively. On Multivariate logistic regression analysis, more severe MR was associated with older age, female gender, lower body mass index, anemia, inferior STEMI and longer door-to-balloon time (DBT). The 30-day mortality was 6.8, 7.3, 8.8, 19.8 and 26.1%, respectively, with increasing grade of MR. The 1-year mortality rate was 10.8, 12.4, 20.8, 37.4 and 37.1%, while 5-year mortality rates were 16.2, 23.1, 36.5, 53.8 and 63%, respectively (p<0.001 all) for none to 4+ MR. After adjusting for age, gender, co-morbidities, ejection fraction and shock by multivariate analysis, severity of ischemic MR was associated with incremental effect on long-term mortality (HR of 1.42, 1.83, 2.41 and 2.95 for 1+ to 4+ MR respectively, p <0.01 for all). In conclusion, higher grades of MR in patients with STEMI undergoing primary PCI are associated with worse short- and long-term outcomes.
A practical method is proposed for the fast shutdown of a large ignited tokamak. The method consists of injecting a rapid series of 30-45 (6 mm) deuterium pellets doped with a small (0.0005%) ...concentration of krypton impurity, and simultaneously ramping the plasma current and shaping fields down over a period of several seconds using the poloidal field system. Detailed modelling with the Tokamak Simulation Code using a newly developed pellet mass deposition model shows that this method should terminate the discharge in a controlled and stable way without producing significant numbers of runaway electrons. A partial prototyping of this technique was accomplished in TFTR.
The annual National Conference on Health Disparities (NCHD) was launched in 2000. It unites health professionals, researchers, community leaders, and government officials, and is a catalyzing force ...in developing policies, research interventions, and programs that address prevention, social determinants, health disparities, and health equity. The NCHD Student Research Forum (SRF) was established in 2011 at the Medical University of South Carolina to build high-quality biomedical research presentation capacity in primarily underrepresented undergraduate and graduate/professional students. This paper describes the unique research training and professional development aspects of the NCHD SRF. These include guidance in abstract development, a webinar on presentation techniques and methods, a vibrant student-centric conference, and professional development workshops on finding a mentor and locating scholarship/fellowship funding, networking, and strategies for handling ethical issues in research with mentors. Between 2011 and 2018, 400 undergraduate and graduate/professional students participated in the NCHD SRF. Most students were women (80.5%). Approximately half were African American or black (52.3%), 18.0% were white, and 21.3% were of Hispanic/Latinx ethnicity. The NCHD SRF is unique in several ways. First, it provides detailed instructions on developing a scientific abstract, including content area examples. Second, it establishes a mandatory pre-conference training webinar demonstrating how to prepare a scientific poster. Third, it works with the research mentors, faculty advisors, department chairs, and deans to help identify potential sources of travel funding for students with accepted abstracts. These features make the NCHD SRF different from many other conferences focused on students’ scientific presentations.
