Background & Objective: Pulse pressure is a marker of arterial stiffness. In old age the pressure wave reflection also contribute to disparity of pulse pressure between upper and lower limbs. This ...study was done to determine the relation between ageing and increased pulse pressure of upper and lower limbs. Materials & Methods: In this discriptive study blood pressure measured in 40 nonsmoker men, all free from medication and disease history, insubjects divided in two groups: 18-25 and 50-70 years old with mean age of 22±1.3 and 59±2 years respectively. Using a mercury-column sphygmomanometer (ERKA) with appropriate cuff size and after five minutes of quiet rest, the blood pressure was determined in supine posture at three separate times. The lower limb pressure was measured with placing the cuff on calf muscle and the stethoscope on posterior surface of internal malleolus. The mean of the second and third records rounded and were used for analysis. Results: The right and left brachial and ankles pulse pressure in two groups were: 44.50±2.03, 44.50±1.71, 37.90±2.24, 37.25±2.30 mmHg in 18-25 years old group and 47.10±2.74, 46.90±2.65, 56.05±3.18, 55.90±4.48 mmHg in 50-70 years old group, respectively. Significant differences were found between brachial and ankle pulse pressure in both sides in first group, (P<0.05). In the second group this difference was only significant at right, (P<0.05). The ratio of brachial pulse pressure to ankle pulse pressure in 18-25 years old group was greater than 1 and in 50-70 years old group it was less than .15 In both side the ankles pulse pressure was significantly greater in 50-70 years old group (P<0.05). Conclusion: This study showed that in subjects of more than 55 years of age, the increasing of pulse pressure was more prominent in lower limb. The ankle pulse pressure may be a proper index of central pulse pressure changes and atherosclerosis of elastic arteries with ageing.
AbstractObjectivesTo use data from the Global Burden of Diseases, Injuries, and Risk Factors Study 2019 (GBD 2019) to estimate mortality and disability trends for the population aged ≥70 and evaluate ...patterns in causes of death, disability, and risk factors.DesignSystematic analysis.SettingParticipants were aged ≥70 from 204 countries and territories, 1990-2019.Main outcomes measuresYears of life lost, years lived with disability, disability adjusted life years, life expectancy at age 70 (LE-70), healthy life expectancy at age 70 (HALE-70), proportion of years in ill health at age 70 (PYIH-70), risk factors, and data coverage index were estimated based on standardised GBD methods.ResultsGlobally the population of older adults has increased since 1990 and all cause death rates have decreased for men and women. However, mortality rates due to falls increased between 1990 and 2019. The probability of death among people aged 70-90 decreased, mainly because of reductions in non-communicable diseases. Globally disability burden was largely driven by functional decline, vision and hearing loss, and symptoms of pain. LE-70 and HALE-70 showed continuous increases since 1990 globally, with certain regional disparities. Globally higher LE-70 resulted in higher HALE-70 and slightly increased PYIH-70. Sociodemographic and healthcare access and quality indices were positively correlated with HALE-70 and LE-70. For high exposure risk factors, data coverage was moderate, while limited data were available for various dietary, environmental or occupational, and metabolic risks.ConclusionsLife expectancy at age 70 has continued to rise globally, mostly because of decreases in chronic diseases. Adults aged ≥70 living in high income countries and regions with better healthcare access and quality were found to experience the highest life expectancy and healthy life expectancy. Disability burden, however, remained constant, suggesting the need to enhance public health and intervention programmes to improve wellbeing among older adults.
Anemia is a globally widespread condition in women and is associated with reduced economic productivity and increased mortality worldwide. Here we map annual 2000-2018 geospatial estimates of anemia ...prevalence in women of reproductive age (15-49 years) across 82 low- and middle-income countries (LMICs), stratify anemia by severity and aggregate results to policy-relevant administrative and national levels. Additionally, we provide subnational disparity analyses to provide a comprehensive overview of anemia prevalence inequalities within these countries and predict progress toward the World Health Organization's Global Nutrition Target (WHO GNT) to reduce anemia by half by 2030. Our results demonstrate widespread moderate improvements in overall anemia prevalence but identify only three LMICs with a high probability of achieving the WHO GNT by 2030 at a national scale, and no LMIC is expected to achieve the target in all their subnational administrative units. Our maps show where large within-country disparities occur, as well as areas likely to fall short of the WHO GNT, offering precision public health tools so that adequate resource allocation and subsequent interventions can be targeted to the most vulnerable populations.
This paper presents a common grounded switched quasi-Z-source dc-dc converter by using one more power switch and diode in comparison with the conventional quasi-Z-source converter. The presented ...converter has used coupled inductor and switched capacitor cell in order to reach the following advantages: achieving high voltage gain with a small range of duty cycle, continuous input current, the low voltage stress on power switches, low voltage stress on output diode. These facilities make it reasonable to use in PV system applications to increase the level of the voltage. In this paper, the principle and analysis of operation modes for the presented converter are given and also, the comparison of the presented structure with conventional structures is evaluated. Finally, to certify the performance of the proposed converter and its theoretical relationships, the simulation results of 250 W of the proposed converter are presented.
