This article presents the features of the course of liver cirrhosis (LC) in a patient with a new coronavirus infection. The patient had no specific respiratory symptoms of COVID-19 (CoronaVirus ...Disease 2019), and the reason for outpatient examination for SARS-CoV-2 (severe acute respiratory syndrome coronavirus) RNA was the presence of these symptoms in relatives. Previously, patient E. had been undergoing in-patient examination and treatment for abdomen volume build-up against the background of prolonged alcoholization, and was diagnosed with alcoholic class B LC according to Child-Pugh classification. Conservative therapy was administered, and the patient was discharged with regression of ascites. Within a week after SARS-CoV-2 identification, patient E. showed signs of LC decompensation in the form of increasing abdominal volume, which required repeated inpatient treatment, during which portal vein thrombosis (PVT) and progression of chronic liver disease (CLD) in the post-coid period were revealed. Literature data on 30-day mortality in patients with LC against COVID-19 background are presented, as well as my own observations on the example of 580 case histories. Complications of new coronavirus infection in patients with CLD, methods of their correction are considered here. This observation demonstrates the social significance of the problem of COVID-19 incidence in patients with LC, the necessity for screening for COVID-19 in case of the presence of decompensation episodes, as well as active prevention of infection in these patients.
The amplitude and phase characteristics of metamaterial cells consisting of microstrip and dielectric resonators (DRs) were studied. Circuits containing resonators of fundamentally different quality ...factors in the microwave range and excited on different types of oscillations have two independent energy ways, which allows the simulation of Fano resonances and electromagnetically induced transparency (EIT). The effects associated with the anomalous dispersion of the metamaterial cell as well as the associated positive values of the group delay (GD) are demonstrated. The experimental and calculated characteristics of metamaterial cells were compared. As the model of metamaterial cell calculation, the lattice circuit of an all-passing filter based on lumped elements was used. It was shown that the numerical calculations of systems exhibiting resonant phenomena (including the Fano effect) can be simplified based on the four-pole network containing the following parameters: unloaded, external, and loaded quality factors of resonators and detuning between their resonant frequencies. The calculated and measured characteristics were found to be in good agreement, which indicates the adequacy of the proposed models.
The article discusses the modern principles of lipid-lowering therapy and approaches to its administration in terms of levels of cardiovascular risk. The characteristics of groups of patients with ...different levels of cardiovascular risk (from low to high) in primary prevention are given. The article defines the target levels of low- density lipoprotein cholesterol, which should be achieved with the modern lipid-lowering therapy. The clinical effects of statins in primary prevention are presented. The clinical results of two large studies of rosuvastatin that have changed the approach to statin prescribing in primary prevention are discussed. The article presents the results of the analysis of long-term follow-up of patients in the primary prevention after the completion of randomized clinical studies. Approaches to the use of statins for novel coronavirus infection are considered.
Rationale: The high prevalence of hepatitis D virus (HDV) infection in the Republic of Dagestan, significantly exceeding that in the European part of the Russian Federation, as well as the limited ...choice of therapeutic options, have led to the need to analyze the accumulated experience of interferon therapy in the real clinical practice to further select possible treatment regimens.
Aim: To evaluate the efficacy of pegylated interferon alpha (Peg-IFN-) in patients with chronic hepatitis D (CHD) in real world clinical practice.
Materials and methods: In a retrospective study, the efficacy, safety and tolerability of Peg-IFN- were analyzed in 34 patients with CHD living in the Republic of Dagestan. Main virological parameters (anti-HDV antibodies (anti-HDV), HDV RNA and HBV DNA), alanine aminotransferase levels, changes over time in liver density by elastometry, and clinical parameters (hematology and urine tests, clinical chemistry, thyroid function tests) were assessed at 24 and 48 weeks of therapy, as well as at 48 weeks after its completion.
Results: The treatment with Peg-IFN- for 48 weeks resulted in a virological response, defined as a HDV RNA decrease by more than 2 lg copies/mL, in 32% (11/34) of the patients. A sustained virological response, defined as undetectable HDV RNA at 48 weeks after the end of therapy, was not observed in any case. Treatment with interferon was associated with a decrease in liver density by average 4.1 kPa by week 48 on therapy. Normalization of alanine aminotransferase levels after 48 weeks of treatment was observed in 59% (20/34) of the patients; however, sustained biochemical response was achieved in none of them. Serious adverse events of high or moderate severity that resulted in the discontinuation of therapy were observed in 12% (4/34) of the cases.
Conclusion: The results obtained indicate low efficacy of Peg-IFN- in the treatment of CHD in the Dagestan patients. Further research is needed to refine optimal interferon therapy regimens, to search for predictors of treatment response, and to evaluate the impact of interferon treatment regimens on the risk of hepatic complications, clinical outcomes, and patient survival. However, the most obvious solution to the problem of CHD therapy is the development and implementation of interferon-free treatment regimens.
We study WW and WZ production with lvqq (l = e, mu) final states using data collected by the D0 detector at the Fermilab Tevatron Collider corresponding to 4.3 fb(-1) of integrated luminosity from p ...(p) over bar collisions at root s = 1.96 TeV. Assuming the ratio between the production cross sections sigma(WW) and sigma(WZ) as predicted by the standard model, we measure the total WV (V = W, Z) cross section to be sigma(WV) = 19.6(-3.0)(+3.2) pb and reject the background-only hypothesis at a level of 7.9 standard deviations. We also use b-jet discrimination to separate the WZ component from the dominant WW component. Simultaneously fitting WW and WZ contributions, we measure sigma(WW) = 15.9(-3.2)(+3.7) pb and sigma(WZ) = 3.3(-3.3)(+4.1) pb, which is consistent with the standard model predictions.
The start of collisions at the LHC brings a new era of particle physics and much improved potential to observe signatures of new physics. Some of these may be evident already from the very beginning ...of collisions. It's essential at this point in the experiment to be prepared to quickly and efficiently determine the quality of the incoming data. Easy visualization of data for the shift crew and experts is one of the key factors in the data quality assessment process. This paper describes the design and implementation of the Data Quality Monitoring Display and discusses experience from its usage and performance during ATLAS commissioning with cosmic ray and single beam data.

We report evidence for a narrow structure, X(5568), in the decay sequence X(5568) -> B-s(0)pi(+/-), B-s(0) -> J/psi phi, J/psi -> mu(+)mu(-), phi -> K+K-. This is evidence for the first ...instance of a hadronic state with valence quarks of four different flavors. The mass and natural width of this state are measured to be m = 5567.8 +/- 2.9(stat)(-1.9)(+0.9) (syst) MeV/c(2) and Gamma = 21.9 +/- 6.4(stat)(-2.5)(+5.0) (syst) MeV/c(2). If the decay is X(5568) -> B-s*pi(+/-). B-s(0)gamma pi(+/-) with an unseen gamma, m(X(5568)) will be shifted up by m(B-s*) - m(B-s(0)) similar to 49 MeV/c(2). This measurement is based on 10.4 fb(-1) of p (p) over bar collision data at root s = 1.96 TeV collected by the D0 experiment at the Fermilab Tevatron collider.
Drell-Yan lepton pairs produced in the process pp¯→ℓ+ℓ−+X through an intermediate γ*/Z boson have an asymmetry in their angular distribution related to the spontaneous symmetry breaking of the ...electroweak force and the associated mixing of its neutral gauge bosons. The CDF and D0 experiments have measured the effective-leptonic electroweak mixing parameter sin2θefflept using electron and muon pairs selected from the full Tevatron proton-antiproton data sets collected in 2001-2011, corresponding to 9–10 fb−1 of integrated luminosity. The combination of these measurements yields the most precise result from hadron colliders, sin2θefflept=0.23148±0.00033. This result is consistent with, and approaches in precision, the best measurements from electron-positron colliders. The standard model inference of the on-shell electroweak mixing parameter sin2θW, or equivalently the W-boson mass MW, using the zfitter software package yields sin2θW=0.22324±0.00033 or equivalently, MW=80.367±0.017 GeV/c2.
We present the observation of doubly-produced $J/\psi$ mesons with the D0 detector at Fermilab in $p\bar{p}$ collisions at $\sqrt{s}=1.96$ TeV. The production cross section for both singly and ...doubly-produced $J/\psi$ mesons is measured using a sample with an integrated luminosity of 8.1~fb$^{-1}$. For the first time, the double $J/\psi$ production cross section is separated into contributions due to single and double parton scatterings. Using these measurements, we determine the effective cross section \sigteff, a parameter characterizing an effective spatial area of the parton-parton interactions and related to the parton spatial density inside the nucleon.
