Abstract
An 81-year-old woman had undergone laparoscopic abdominoperineal resection for rectal cancer. A permanent colostomy was created through an intraperitoneal route. Three months after the ...surgery, the patient presented with lower abdominal pain and vomiting. Computed tomography showed gastric incarceration through the lateral space of the lifted sigmoid colostomy. Although the herniated stomach was reduced by nasogastric tube decompression, the patient experienced a recurrence of gastric hernia shortly thereafter. A laparoscopic operation was performed, and a new colostomy was constructed through an extraperitoneal route. The patient had no hernia recurrence during the 20 months of follow-up after the operation. Gastric internal hernia associated with colostomy can occur as a rare complication. Although reduction of the incarcerated stomach is possible by nasogastric tube decompression, surgical repair of the hernia may be the optimal management to prevent recurrence.

A right-sided round ligament (RSRL) is a rare, congenital anomaly of the intrahepatic portal vein, with a reported frequency of 0.2%-1.2%. For patients with perihilar cholangiocarcinoma associated ...with an RSRL, an accurate understanding of the vascular and biliary anatomy is indispensable.
We report a 70-year-old male with perihilar cholangiocarcinoma associated with an RSRL. After percutaneous transhepatic embolization of the left and anterior portal branches, we conducted a left trisectionectomy of the liver with extrahepatic bile duct resection and hepaticojejunostomy. The postoperative course was uneventful, and R0 resection was achieved. When the liver volume of each section was compared between 7 patients with an RSRL and 20 patients with normal portal vein anatomy, the posterior section in RSRL patients was significantly larger than that in patients with normal portal vein anatomy (median: 457 mL
306 mL,
= 0.031). In patients with perihilar cholangiocarcinoma associated with an RSRL, left trisectionectomy has several surgical advantages: (1) The posterior branch of the portal vein often ramifies independently, and the division of the portal vein is easily conducted; (2) A relatively large amount of remnant liver can be retained; and (3) The anatomy of the posterior branch of the Glissonian pedicle is similar to that in patients with normal anatomy.
In patients with an RSRL and perihilar cholangiocarcinoma that does not involve the posterior section, left trisectionectomy may be a favorable choice.
We implemented an evacuee support system in this study. This system consists of evacuee’s safety information registration, evacuation shelter management, and evacuation shelter congestion status ...visualization systems. By implementing these three systems, a disaster response headquarters can manage the evacuees at each evacuation shelter accurately and quickly. Additionally, residents who are considering evacuating to evacuation shelters can browse the visualized congestion status of each evacuation shelter and move to vacant evacuation shelters. We evaluated the operability, readability, functionality, necessity, effectiveness, the applicability of the entire system, effectiveness and operability of the safety information registration system, readability and effectiveness of the evacuation shelter congestion status visualization system, and functionality and effectiveness of the evacuation shelter management system. We conducted evaluation experiments on 30 subjects and obtained a high evaluation in each evaluation item.
In our previous work, we have proposed a new concept of ‘universal multimedia access' to narrow the digital divide by providing appropriate multimedia expressions according to users' (mental and ...physical) abilities, computer facilities, and network environments. In our approach, we consider some switching functions such as user interface switching, media switching and QoS switching. We evaluated some types of user interfaces for different age groups and devices. In this paper, we discuss the effectiveness of media switching and QoS Control functions and present the qualitative evaluation of multimedia contents for different media types and media quality.
In this study, we developed an “English learning AR application for infants” that
combines augmentedreality (AR) and deep learning by linking visually familiar objects
with English words. Infants,who ...are the target users of this study, can visually learn
the English words of an object as ARinformation by holding a smartphone over an
arbitrary object. We constructed 57 types of self-madedatasets, prepared 200 images for
each English word, and learned them 300 times. Additionally,we conducted a system
evaluation questionnaire survey for 44 subjects to evaluate this AR application.After
experiencing this application, the subjects evaluated the operability, relevance,
readability,functionality, applicability, and learning effect. Because of the system
evaluation questionnaire survey,although we could obtain high evaluations for many
items, the result was that issues remainedin functionality.
The photodegradation products of the anticancer drug, dacarbazine, cause adverse reactions including local venous pain when injected intravenously. In this study, we attempted to identify which of ...these products is responsible. We synthesized or purchased five photodegradation products of dacarbazine (dimethylamine, 5‐diazoimidazole‐4‐carboxamide (Diazo‐IC), 4‐carbamoylimidazolium‐5‐olate, 5‐carbamoyl‐2‐(4‐carbamoylimidazol‐5‐ylazo) imidazolium‐5‐olate and 2‐azahypoxanthine) and examined the pain reaction induced by their intraperitoneal administration in mice using an abdominal stretching or constriction assay. Only Diazo‐IC clearly induced pain reaction in mice in a dose‐dependent manner, the other products caused no pain reaction. The threshold concentration for pain reaction in mice was estimated to be about 0.1 mg mL−1. While diclofenac sodium significantly reduced acetic‐acid‐induced pain reaction in mice, it did not influence those induced by Diazo‐IC. This result suggests that the mechanism of Diazo‐IC‐induced pain is different from that of acetic‐acid‐induced inflammatory pain. Dacarbazine itself produced marked relaxation of rat thoracic aorta strips in a concentration‐dependent manner, but there was no difference between the activity of dacarbazine and its photo‐exposed solution, so constriction or relaxation of blood vessels is unlikely to be a factor in the pain reaction. In conclusion, Diazo‐IC generated by photodegradation of dacarbazine solution causes the side‐effect of venous pain. Dacarbazine solution that has turned pink should not be used, because Diazo‐IC is an intermediate in the formation of the reddish product, 5‐carbamoyl‐2‐(4‐carbamoylimidazol‐5‐ylazo) imidazolium‐5‐olate. Drip infusion preparations of dacarbazine should be shielded from light.
It is considered that the road condition in the winter is one of the significant issues for the safety driving by tourists or residents. However, there are many difficulties of the V2V networks such ...as the transmission range of wireless networks and the noises from the automobilefs bodies. Thus, this paper introduces the Adaptive Array Antenna (AAA) controls for the vehicle-to-vehicle (V2V) networks based the Delay Tolerant Networking (DTN) in the road surveillance system. In the proposed system, the vehicles equip the AAA control systems with IEEE802.11a/b/g based the DTN, and the wireless directions are controlled by the visual recognitions with Kalman filter algorithm to make the longer and stable wireless connections for the efficiency of the DTN. The porotype system is introduced in this paper, and the results are discussed for the future studies.
Paclitaxel, an anti-mitotic anti-cancer agent, is active against solid tumors. The inhibition of depolymerization and promotion of microtubular assembly are essential for the anti-tumor activity of ...paclitaxel. Microtubule-associated proteins (MAPs) co-polymerize with tubulin and play some roles in microtubular dynamics. We examined the effect of paclitaxel on the interaction between tubulin and MAPs. Human lung-cancer cells, PC-14, were synchronized to G1/S border by the thymidine-double-block technique. After release from exposure to thymidine, the cells were treated briefly with 2 nM paclitaxel and the levels of alpha and beta tubulins and MAPs were examined after various times. Immunoblot analysis of paclitaxel-treated cells showed no changes in the overall expression of alpha and beta tubulins, microtubule-associated protein 2 (MAP2) or MAPs in comparison with controls. The samples were immunoprecipitated with anti-alpha- and anti-beta-tubulin antibodies and reblotted with an anti-MAP2 antibody, which showed that the amount of co-immuno-precipitated MAP2 in the synchronized cells, were increased by the brief paclitaxel treatment. These results suggest that paclitaxel treatment enhances the interaction between alpha and beta tubulins and MAP2. Since the phosphorylation state of MAP2 regulates the affinity of MAP2 for tubulins, and mitogen-activated protein (MAP) kinase is considered to be one of the kinases responsible for MAP2 phosphorylation, the effect of paclitaxel treatment on the MAP-kinase activity of synchronized PC-14 cells was examined. Two bands with molecular masses of 42 and 44 kDa were detected by an "intra-gel" MAP-kinase assay using myelin basic protein as the substrate. Paclitaxel treatment inhibited the MAP-kinase activity of PC-14 cells and inhibition was maximal at the G2/M phase of the cell cycle. Similar, concentration-dependent inhibition by paclitaxel of cellular MAP kinase of human synchronized small-cell lung carcinoma, H69, was observed. No inhibition of the MAP kinase of the paclitaxel-resistant sub-line H69/Txl by paclitaxel was observed, suggesting that some change of the MAP-kinase cascade had occurred in these cells. No direct inhibition of MAP-kinase activity by paclitaxel was observed in the cell-free assay (in vitro), suggesting that paclitaxel did not inhibit MAP kinase directly. Since it has been speculated that p34cdc2 kinase is also a kinase that phosphorylates MAP2, the effect of paclitaxel treatment on the p34cdc2-kinase activity of synchronized PC-14 and PC-9 cells was examined. Paclitaxel inhibited p34cdc2-kinase activation at the G2/M phase. These results suggest that paclitaxel inhibited MAP kinase and p34cdc2 kinase in vivo indirectly. These actions of paclitaxel may be responsible for the increased affinity between MAP2 and tubulins that it induces.
In this research, we build the system to provide the digital contents according to the user's position and needs. This system provides necessary digital contents to users by combine with position ...information and sensor data. Digital contents are managed based on position information in this system, and we provide the service with which information providers and information receivers can exchange information smoothly. Therefore we have developed web application for information providers and smartphone application for information receivers. The digital contents information providers uploaded from web application are displayed as AR on information receiver's smartphone application. In this research, we evaluated targeted for the visitors in a zoo to evaluate the operability and usefulness of this system. As a result, we were able to confirm the great operability and usefulness of this system.
