El sistema tiene 3 componentes anatómicos complejos; 2 articulaciones temporomandibulares (ATM), el neuromuscular y la relación interdental con sus tejidos de soporte y que pueden dañarse en forma similar a las del resto del cuerpo humano. La intención de esta presentación de caso es la de describir una alternativa sin retratamiento de ortodoncia en Fase II de un paciente joven con osteoartritis de las ATM, ferulización muscular, hipertensión arterial y bruxismo.
To test the efficacy of cognitive-behavioural therapy (CBT) for lifestyle modification in patients with metabolic syndrome (MetS).
76 MetS patients completed this clinical trial, with 18 months ...follow-up. 45 participants from the experimental group (EG - CBT) and 31 to the control group (CG - usual care). The CBT programme was performed by a psychologist in a face-to-face group format, during 12 weekly sessions lasting 90 minutes. The intervention for the CG consisted of workshops with basic information about MetS and it's associated cardiovascular risk.

Results showed reduction in weight (mean difference - MD −2.633, 95%CI -4.322, −0.943; p<.003), waist circumference (MD −2.944, 95%CI -5.090, −0.798; p<.008), body mass index (MD −0.915, 95%CI -1.494, −0.335; p<.003), systolic (MD −0.046, 95%CI -0.685, −0.023; p<.0002) diastolic blood pressure (MD −4.777, 95%CI -7.750, −1.804; p<.002), and cardiovascular risk score after 18 months. An increase in adherence to the Mediterranean diet and assertiveness and a reduction in anger were observed in EG. The CG did not show any significant differences.
The CBT focused on changes in lifestyle seems to be effective in the reduction of MetS and cardiovascular risk factors.

Hyaline cartilage’s inability to self-repair can lead to osteoarthritis and joint replacement. Various treatments, including cell therapy, have been developed for cartilage damage. Autologous ...chondrocyte implantation (ACI) is considered the best option for focal chondral lesions. In this article, we aimed to create a narrative review that highlights the evolution and enhancement of our chondrocyte implantation technique: High-Density-ACI (HD-ACI) Membrane-assisted Autologous Chondrocyte Implantation (MACI) improved ACI using a collagen membrane as a carrier. However, low cell density in MACI resulted in softer regenerated tissue. HD-ACI was developed to improve MACI, implanting 5 million chondrocytes per cm2, providing higher cell density. In animal models, HD-ACI formed hyaline-like cartilage, while other treatments led to fibrocartilage. HD-ACI was further evaluated in patients with knee or ankle defects and expanded to treat hip lesions and bilateral defects. HD-ACI offers a potential solution for cartilage defects, improving outcomes in regenerative medicine and cell therapy. HD-ACI, with its higher cell density, shows promise for treating chondral defects and advancing cartilage repair in regenerative medicine and cell therapy.
El artículo tiene como objetivo centrar su atención en el análisis, para darlo a conocer a la comunidad archivística costarricense, de los principales antecedentes que dieron origen a la formación ...archivística universitaria en Costa Rica; es decir, se investigó sobre la antesala de lo que 1978 dio origen al Diplomado en Archivo Administrativo en la entonces Escuela de Historia y Geografía de Facultad de Ciencias Sociales de Universidad de Costa Rica. Por lo tanto, el trabajo corresponde a una investigación histórica y exploratoria; en el primer caso porque se analiza e interpreta un proceso en perspectiva histórica y el segundo porque es un tema poco estudiado por la historiografía archivística costarricense. La metodología para la realización del trabajo fue el análisis documental, a partir de fuentes de información bibliográficas y documentos de archivos, que quedan reflejados en apartado de referencias bibliográficas. Así las cosas, el artículo presenta un recorrido por el origen de la Archivística en Costa Rica y los diferentes acontecimientos que antecedieron a la creación del Diplomado en Archivo Administrativo. Se concluye, entre otras cosas, que el Proyecto Piloto de la Unesco sentó las bases para el desarrollo archivístico costarricense, la modernización de los archivos y el origen de la formación archivística reglada en el país.

Dyslipidemia is a frequent side effect associated with nilotinib treatment. Patients with chronic myeloid leukemia (CML) under treatment with nilotinib who develop dyslipidemia have been shown to ...have a higher risk of presenting atherosclerotic cardiovascular disease (ACVD). Therapeutic discontinuation in selected individuals could be a strategy in order to prevent the development of ACVD. Observational study of patients with CML under nilotinib treatment. The lipid values were gathered before starting with nilotinib and after 3 months. Such values were also measured before discontinuation in patients who suspended nilotinib treatment, as well as 3 and 12 months later. 32 patients were included, 19 of them treated in monotherapy with nilotinib. The concentrations of total cholesterol and low-density lipoproteins (LDL) increased significantly after 3 months of treatment (27.29 mg/dL ± 22.88, p < 0.01). Of the total number of patients treated, 12 discontinued the treatment. LDL concentration was significantly reduced after 3 months of the nilotinib discontinuation (- 27.58 mg/dL ± 38.30, p = 0.030), remaining substantially lower after 12 months, compared to the time previous to discontinuation (- 24.58 mg/dL ± 37.31, p = 0.043). Nilotinib suspension reduces significantly LDL concentrations. These data support the strategy of therapeutic discontinuation in order to prevent future cardiovascular complications, especially in patients with prior cardiovascular risk factors.

Objective: Renal denervation (RDN) is indicated in patients despite the use of three or more anti-hypertensive drugs including a diuretic, which, having ruled out secondary causes of hypertension ...(HT), are not controlled and are classified as resistant hypertension (RHT). The approximate prevalence is 10% -20% of the hypertensive population and represents a high incidence of adverse cardiovascular events in these patients. RND is a minimally invasive method, but the effectiveness of this technique has shown some contradictory data due to a heterogeneity in the design of these studies. Our objective is to evaluate the clinical experience in patients undergoing RDN in our unit. Design and method: Retrospective record of the 2009–2019 period. 16 patients with RHT treated by RDN with Symplicity-Flex or Spyral devices have been included. The statistical study was made with Spss program. Results: 56.3% were male. Mean age of 53 ± 8 years. The average preRDN clinical SBP 169 ± 19 mmHg; DBP clinical preRDN mean 101 ± 17 mmHg; Mean postRDN clinical SBP 149 ± 26 mmHg; DBP clinical postRDN mean 88 ± 11 mmHg; Average number of preRDN drugs 5.88 ± 0.8; Average number of postRDN drugs of 4.67 ± 1.4. 75% of the procedures were done with the spiral symplicity method. Average number of ablations 24 ± 9 per procedure. There was no deterioration of renal function or complications. Conclusions: Our data provide a decrease in clinical BP after RDN, with a decrease in the number of drugs used to control their BP, and there seems to be a better response to the spiral symplicity method with greater number of ablations. The RDN technique has been proven safe, with no relevant complications and preserved renal function. The data therefore guarantee the safety and efficacy of this technique, which coincide with the recent national registries published in real life and that support its use in selected patients and in experienced centers.

The incidence of hypertension is high in the elderly and is present in 2/3 of the patients older than 65 years. Prevalence can reach 90% in patients older than 80 years. The presence of isolated ...systolic hypertension (ISH) is characteristic of this population. However, the prevalence of hypertension by ambulatory blood pressure monitoring (ABPM) is not well known. In this study, we analyzed the special characteristics of hypertension in this population, giving special emphasis on ABPM readings.
