sont une cause majeure de décès des enfants de moins de 5 ans en Afrique Sub-saharienne. Un traitement rapide dépend de la disponibilité de ...médicaments appropriés au niveau des points de prestation de service. La fréquence des ruptures de stock des commodités antipaludiques, en particuliers celles utilisées pour le paludisme grave, avait nécessité une mise à jour des hypothèses de quantification. Les données issues de la collecte de routine du PNLP de 2007 à 2012 ont été comparées à celles rapportés par d'autres pays africains et utilisées pour orienter les discussions au cours d'un atelier organisé par le PNLP et ses partenaires techniques et financiers afin de dégager un consensus national. La proportion des cas de paludisme rapportés comme grave en RDC est resté autour d'une médiane de 7% avec un domaine de variation de 6 à 9%. Hormis la proportion rapportée au Kenya (2%), les pays africains ont rapporté une proportion de cas grave variant entre 5 et 7%. Il apparaît que la proportion de 1% précédemment utilisée pour la quantification en RDC a été sous-estimée dans le contexte de la gestion des cas graves sur terrain. Un consensus s'est dégagé autour de la proportion de 5% étant entendu que des efforts de renforcement des capacités seraient déployés afin d'améliorer le diagnostic au niveau des points de prestation des services.
RésuméL’objectif de cette étude était de relever les déterminants de l’adhésion des prestataires de soins, des vendeurs d’officines et des patients au traitement recommandé pour le paludisme ...(l’artesunate + amodiaquine (AS-AQ) en co-blister) au nord-est de la RDC. Une étude transversale descriptive a été réalisée de mars à juin 2009 dans 13 zones de santé sur un échantillon à choix raisonné portant sur les prestataires de soins, les vendeurs de points de vente de médicaments et les patients. Seuls 69 % de cas de paludisme simple étaient mis sous AS-AQ dont 62,3 % avec les posologies correctes. Les facteurs significativement liés à la prescription ou non de l’AS-AQ chez les prestataires étaient : la disponibilité de l’AS-AQ, l’intention d’en prescrire, une plus longue durée de consultation, le fait de fournir des explications aux patients, de travailler en milieu rural, d’avoir suivi une formation sur la nouvelle politique, de disposer d’un guide thérapeutique. En revanche, la faible disponibilité du traitement, les effets indésirables, la présence d’AS-AQ de mauvaise qualité et d’antipaludiques prohibés à moindre coût sur le marché étaient les principaux facteurs de non adhésion incriminés. Chez les patients, le principal facteur de non adhésion était les effets indésirables. L’amélioration de l’accessibilité de l’AS-AQ de qualité et la communication, la formation des prestataires, la mise à leur disposition de guides thérapeutiques et leur supervision, sont importantes pour la promotion de l’utilisation de l’AS-AQ.

We conducted a retrospective study of logistic management of vaccines in the Expanded Programme on Immunization (EPI) antenna in the Kisangani region over the period 2010-2014. A literature review completed by semi-structured interviews of immunization services managers and providers helped to evaluate vaccine management using the WHO GEV model in order to identify the gaps.
Provider's demonstrated low level of knowledge of the vaccines that cannot be frozen, of freezing tests and of other vaccine damages. Computerized data management was properly ensured in the antenna. No evaluation criteria reached the goal of 80%. Compliance with the storage temperature was 70% in the antenna; the criterion for vaccine management was 65% and 67% in the health area central office and in the health centre respectively. The maintenance criterion was zero at all levels.
Malfunctioning of the logistic system is remarkable at all levels of the health pyramid; this could interfere with the quality and the expected impact of vaccination. Particular attention should be paid to the maintenance of the equipment.
In Kisangani, the dropout rate of 17.5% among children aged 12 to 23 months is higher than the threshold of less than 10% tolerated by the WHO, despite the free availability of vaccines. This study ...aims to analyze vaccine-specific aspects in order to identify predictors associated with vaccine dropout among children aged 12 to 23 months. A cross-sectional observational study was conducted from October 25, 2022 to February 25, 2023 among mothers of 336 children aged 12 to 23 months in Kisangani. A pre-tested and administered questionnaire was used for data collection based on three-stage cluster sampling. Analyses were performed on STATA 13 using stepwise logistic regression with a threshold of 0.05. A total of 336 children aged 12 to 23 months from 5 health zones in the city of Kisangani were included in the analysis. We observed a 37.5% prevalence of early childhood vaccination drop-out in Kisangani. After adjustment by multivariate logistic regression, the following determinants were significantly associated with drop-out from early childhood vaccination : preference for vaccine administration mode, refusal of a vaccine during a mass campaign, delay in vaccinating when a new vaccine is introduced, lack of confidence in vaccinators who go door-to-door, preference of vaccination site, presence of adverse events following immunization and poor reception by health professionals. Thus, the results of this study can help planners, policy-makers and decision-makers to focus on both individuals and the communities in which they live.
Objective: To determine the seroprevalence of viral hepatitis B and C and HIV among blood donors in the Isangi Rural Health Zone. Methods: Cross-sectional descriptive study carried out in the rural ...health zone of Isangi from 1 January 2010 to 31 December 2017 involving 814 blood donors. The parameters of interest were sociodemographic characteristics and biological results (HIV, HBV, HCV, syphilis) using rapid diagnostic tests. Results: 814 blood donors (family and volunteers) were collected, including 725 males (89%) and 89 females (11%). The majority of donors belonged to the age groups of 17-25 years (44%) and 26-35 (32%) and was family donors (90%). The prevalence was 1.7%; 3.2%; 0.85%; 3.5% for syphilis, HBV, HIV and HCV, respectively. No epidemiological parameters studied were significantly associated with the seropositivity of the antigens sought (p <0.05). Conclusion: Seroprevalences of infectious markers were relatively low among blood donors in Isangi. These seroprevalences would be underestimated because of the use of rapid diagnostic tests. But they would reflect a difference in epidemiology of infectious agents between rural and urban areas. Improvement of transfusion safety in rural areas should be directed towards the abandonment of family donation, the promotion of voluntary donation, the organization of donors into "clubs" and their loyalty.
