Primary hyperparathyroidism typically presents with skeletal and renal manifestations. In most (80-85%) cases, the cause is a single parathyroid adenoma. We present a case of 36-year-old lady of ...primary hyperparathyroidism presented with multiple fractures, nephrolithiasis, and nephrocalcinosis. All of the imaging techniques to localize the cause were negative. She underwent surgical neck exploration with detection of left parathyroid adenoma. The introduction of a foreign body into the urethra is an exceptionally rare occurrence. It is often secondary, either to erotic curiosity or to psychiatric disorders. The symptomatology is multiple ...and the diagnosis is aided by radiologic imaging. The extraction is most often done by endoscopic. The management of these patients must be done systematically and will need psychiatric assessment. We report the case of two patients who inserted a foreign body into their urinary bladder. One was 22-year-old and the second was 20-year-old and all with no history of psychiatric disorders. The first accidentally inserted a pencil into the urethra while trying to flatten a vulvar papule. The second inserted a piece of broom through playful games. The foreign bodies were extracted endoscopically in both cases under rachianesthesia. The postoperative course was uneventful and both patients were referred to a psychiatric clinic for evaluation after discharge.

Background: The diagnosis of ischemic stroke remains a clinical one, with confirmatory evidence obtained through neuroimaging. ECG changes are common in patients with ischemic stroke. Objective: The ...objective of this study was to see ECG findings among ischemic stroke patients.
Materials and Methods: This study was carried out in the Department of Neurology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka from July 2006 to October 2008. A total of 36 patients with acute Ischemic stroke were selected by purposive sampling method and diagnosed by history, clinical findings ard was confirmed by CT scan of head. The clinical details, investigations of the respondents were reviewed. Data were recorded in a pre-designed data collection sheet.
Result: Majority of the subjects were in 7th decade 12(33.3%) and 6th decade 9(25%) with the male to female ratio was 1.25:1. Among the patients with abnormal electrocardiographic findings, 7(19.4%) patients each had myocardial ischemia, 4(11.1%) had conduction block and ventricular arrhythmias, 7(19.4%) had atrial fibrillation, 5(13.9%) had ventricular hypertrophy, 7(19.4%) had myocardial infarction, 6(16.7%) patients had non-specific ST changes.
Conclusion: Myocardial ischemia, atrial fibrillation and myocardial infarction are common electrocardiographic findings of ischemic stroke patients.

Background: Guillain-Barré syndrome (GBS) usually preceded by infections, in particular cytomegalovirus (CMV). It may occur by primary infection, reinfection or by reactivation of CMV. Objective: The ...aim of the present study was to evaluate the association of Guillain-Barré syndrome (GBS) with Cytomegalovirus (CMV) infection.
Methodology: This case control study was carried out in the indoor and outpatient Department of Neurology at Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka from 1st January 2010 to 31st December 2011 for the duration of two years. All patients with GBS, who attended in neurology OPD or inpatient department at BSMMU during the study period, fulfilling the inclusion and exclusion criterias were included in this study. Age & sex matched volunteers, patients attendants, patients other than GBS who were nondiabetic, had no renal or hepatic diseases or family history of polyneuropathy were included in control group.
Results: A total number of 78 respondents of which 39 patients were taken as cases and rest 39 were taken as controls who appeared in neurology OPD or inpatient department at BSMMU during the study period, fulfilling the inclusion and exclusion criterias were included in this study. The mean age ± SD of case and control groups were 30.82 ± 12.56 and 31.00 ± 12.77 years respectively (p=0.950). In case group the history of respiratory tract infection was present in 46.2% cases and absent in control group (p=0.001). In case group the history of gastroenteritis was present in 28.2% cases and absent in control group (p=0.001). In case group the history of fever was present in 30.8% cases and absent in control group (p=0.001). Anti-CMV IgM antibody was positive in 5.1% cases. Four fold rise of IgG in case group was present in 10.3% cases and absent in control group (p=0.040). Confirmed CMV infected GBS cases were 15.4% and absent in control group (p=0.011).
Conclusion: The findings of this study permit to conclude that there is a significant association of Guillain-Barré syndrome (GBS) with Cytomegalovirus (CMV) infection.
