Among the nosocomial pathogens, Pseudomonas aeruginosa is recognized as a major cause of morbidity and mortality. Data on 136 patients with P. aeruginosa bacteremia were retrospectively analyzed to ...evaluate risk factors for mortality. The median age of the patients was 55 years (range, 15-85 years), 78.7% of the cases were hospital-acquired, and the 30-day mortality rate was 39% (53 of 136 patients). Multivariate analysis demonstrated that risk factors for mortality included severe sepsis, pneumonia, delay in starting effective antimicrobial therapy, and an increasing APACHE II score (all P values <.05). In 123 of the 136 patients (excluding 13 patients treated with inadequate definitive antibiotics), 30-day mortality was 27.7% (13 of 47 patients) in the group of patients who received initially effective empirical antimicrobial therapy, and 43.4% (33 of 76) in the group of patients who received delayed effective antimicrobial therapy (P = .079). There was a trend toward higher mortality as the length of delay increased. Delay in starting effective antimicrobial therapy for P. aeruginosa bacteremia tended to be associated with higher mortality.
Introduction: Globally, non-alcoholic fatty liver disease (NAFLD) continues to rise and isoflavones exert antisteatotic effects by the regulation of hepatic lipogenesis/insulin resistance or ...adiposity/a variety of adipocytokines are related to hepatic steatosis. However, there is very little information regarding the potential effects of daidzein, the secondary abundant isoflavone, on NAFLD. Here, we have assessed the hepatic global transcription profiles, adipocytokines and adiposity in mice with high fat-induced NAFLD and their alteration by daidzein supplementation. Methods: C57BL/6J mice were fed with normal fat (16% fat of total energy), high fat (HF; 36% fat of total energy) and HF supplemented with daidzein (0.1, 0.5, 1 and 2 g per kg diet) for 12 weeks. Results: Daidzein supplementation (0.5 g per kg diet) reduced hepatic lipid concentrations and alleviated hepatic steatosis. The hepatic microarray showed that daidzein supplementation (1 g per kg diet) downregulated carbohydrate responsive element binding protein, a determinant of de novo lipogenesis, its upstream gene liver X receptor beta and its target genes encoding for lipogenic enzymes, thereby preventing hepatic steatosis and insulin resistance. These results were confirmed by lower insulin and blood glucose levels as well as homeostasis model assessment insulin resistance scores. In addition, daidzein supplementation inhibited adiposity by the upregulation of genes involved in fatty acid beta-oxidation and the antiadipogeneis, and moreover augmented antisteatohepatitic leptin and adiponectin mRNA levels, whereas it reduced the mRNA or concentration of steatotic tumor necrosis factor alpha and ghrelin. Conclusions: These findings show that daidzein might alleviate NAFLD through the direct regulation of hepatic de novo lipogenesis and insulin signaling, and the indirect control of adiposity and adipocytokines by the alteration of adipocyte metabolism.
Aim
We aimed to isolate and propagate internal and external anal sphincter progenitor cells from the human anal sphincter, with or without radiotherapy, for tailored cell therapy of faecal ...incontinence.
Methods
Sphincter progenitor cells were isolated from normal internal and external anal sphincters collected from 10 patients with rectal cancer who had undergone abdominoperineal resection with (n = 6) or without (n = 4) preoperative chemoradiotherapy. The isolated cells and differentiated muscle fibres were identified using immunofluorescence assay, western blotting and reverse transcription polymerase chain reaction (RT‐PCR). The proliferation of progenitor cells with and without radiotherapy was compared by quantitative 3‐(4,5‐dimethylthiazol‐2‐yl)‐2,5‐diphenyltetrazolium bromide (MTT) assay.
Results
The immunofluorescence assay before differentiation confirmed that the internal anal sphincter progenitor cells expressed CD34 and neural‐glial antigen 2 (NG2), whereas the external anal sphincter progenitor cells expressed CD34 and PAX7. After differentiation, the internal anal sphincter progenitor cells expressed desmin, calponin and α‐smooth muscle actin, whereas the external anal sphincter progenitor cells expressed desmin, myogenic factor 4 and myosin heavy chain. The differential expression profiles of both cell types were confirmed by western blotting and RT‐PCR. MTT assays showed that the viability of internal and external anal sphincter progenitor cells was significantly lower in the radiotherapy group than that in the nonradiotherapy group.
Conclusions
This study describes the differential harvest internal and external sphincter muscle progenitor cells from human anal sphincters. We confirm that radiotherapy decreases the viability of internal and external anal sphincter progenitor cells.
We conduct several sets of simulations with a version of NASA's Goddard Earth Observing System, version 5, (GEOS-5) Atmospheric Global Climate Model (AGCM) equipped with a two-moment cloud ...microphysical scheme to understand the role of biomass burning aerosol (BBA) emissions in Southeast Asia (SEA) in the pre-monsoon period of February–May. Our experiments are designed so that both direct and indirect aerosol effects can be evaluated. For climatologically prescribed monthly sea surface temperatures, we conduct sets of model integrations with and without biomass burning emissions in the area of peak burning activity, and with direct aerosol radiative effects either active or inactive. Taking appropriate differences between AGCM experiment sets, we find that BBA affects liquid clouds in statistically significantly ways, increasing cloud droplet number concentrations, decreasing droplet effective radii (i.e., a classic aerosol indirect effect), and locally suppressing precipitation due to a deceleration of the autoconversion process, with the latter effect apparently also leading to cloud condensate increases. Geographical re-arrangements of precipitation patterns, with precipitation increases downwind of aerosol sources are also seen, most likely because of advection of weakly precipitating cloud fields. Somewhat unexpectedly, the change in cloud radiative effect (cloud forcing) at surface is in the direction of lesser cooling because of decreases in cloud fraction. Overall, however, because of direct radiative effect contributions, aerosols exert a net negative forcing at both the top of the atmosphere and, perhaps most importantly, the surface, where decreased evaporation triggers feedbacks that further reduce precipitation. Invoking the approximation that direct and indirect aerosol effects are additive, we estimate that the overall precipitation reduction is about 40% due to the direct effects of absorbing aerosols, which stabilize the atmosphere and reduce surface latent heat fluxes via cooler land surface temperatures. Further refinements of our two-moment cloud microphysics scheme are needed for a more complete examination of the role of aerosol–convection interactions in the seasonal development of the SEA monsoon.

The nanoparticles were successfully used as catalysts for the Suzuki coupling reaction, and were readily oxidized to produce NiO nanoparticles.

Limits on the cross section for weakly interacting massive particles (WIMPs) elastic scattering on nuclei in NaI(Tl) detectors at the Yangyang Underground Laboratory are obtained from a ...2967.4 kg·day data exposure. The nuclei recoiling from the scattering process are identified by the pulse shape of the scintillation light signals that they produce. The data are consistent with a no nuclear-recoil hypothesis, and WIMP-mass-dependent 90% confidence-level upper-limits are set on WIMP-nuclei elastic scattering cross sections. These limits partially exclude the DAMA/LIBRA allowed region for WIMP-sodium interactions with the same NaI(Tl) target material. The 90% confidence level upper limit on the WIMP-nucleon spin-independent cross section is 3.26×10

A search for inelastic boosted dark matter (IBDM) using the COSINE-100 detector with 59.5 days of data is presented. This relativistic dark matter is theorized to interact with the target material ...through inelastic scattering with electrons, creating a heavier state that subsequently produces standard model particles, such as an electron-positron pair. In this study, we search for this electron-positron pair in coincidence with the initially scattered electron as a signature for an IBDM interaction. No excess over the predicted background event rate is observed. Therefore, we present limits on IBDM interactions under various hypotheses, one of which allows us to explore an area of the dark photon parameter space that has not yet been covered by other experiments. This is the first experimental search for IBDM using a terrestrial detector.

Normal anterior pituitary function is essential for fertility. Release from the gland of the reproductive hormones luteinising hormone and follicle‐stimulating hormone is regulated primarily by ...hypothalamically‐derived gonadotrophin‐releasing hormone (GnRH), although other releasing factors (RF) have been postulated to exist. Using a bioinformatic approach, we have identified a novel peptide, phoenixin, that regulates pituitary gonadotrophin secretion by modulating the expression of the GnRH receptor, an action with physiologically relevant consequences. Compromise of phoenixin in vivo using small interfering RNA resulted in the delayed appearance of oestrus and a reduction in GnRH receptor expression in the pituitary. Phoenixin may represent a new class of hypothalamically‐derived pituitary priming factors that sensitise the pituitary to the action of other RFs, rather than directly stimulating the fusion of secretary vesicles to pituitary membranes.
