Days out of role because of health problems are a major source of lost human capital. We examined the relative importance of commonly occurring physical and mental disorders in accounting for days ...out of role in 24 countries that participated in the World Health Organization (WHO) World Mental Health (WMH) surveys. Face-to-face interviews were carried out with 62 971 respondents (72.0% pooled response rate). Presence of ten chronic physical disorders and nine mental disorders was assessed for each respondent along with information about the number of days in the past month each respondent reported being totally unable to work or carry out their other normal daily activities because of problems with either physical or mental health. Multiple regression analysis was used to estimate associations of specific conditions and comorbidities with days out of role, controlling by basic socio-demographics (age, gender, employment status and country). Overall, 12.8% of respondents had some day totally out of role, with a median of 51.1 a year. The strongest individual-level effects (days out of role per year) were associated with neurological disorders (17.4), bipolar disorder (17.3) and post-traumatic stress disorder (15.2). The strongest population-level effect was associated with pain conditions, which accounted for 21.5% of all days out of role (population attributable risk proportion). The 19 conditions accounted for 62.2% of all days out of role. Common health conditions, including mental disorders, make up a large proportion of the number of days out of role across a wide range of countries and should be addressed to substantially increase overall productivity.

Background
Treatment of venous thromboembolism (VTE) in patients with cancer has a high rate of recurrence and bleeding complications. Guidelines recommend low‐molecular‐weight heparin (LMWH) ...for at least 3–6 months and possibly indefinitely for patients with active malignancy. There are, however, few data supporting treatment with LMWH beyond 6 months. The primary aim of the DALTECAN study (NCT00942968) was to determine the safety of dalteparin between 6 and 12 months in cancer‐associated VTE.
Methods
Patients with active cancer and newly diagnosed VTE were enrolled in a prospective, multicenter study and received subcutaneous dalteparin for 12 months. The rates of bleeding and recurrent VTE were evaluated at months 1, 2–6 and 7–12.
Findings
Of 334 patients enrolled, 185 and 109 completed 6 and 12 months of therapy; 49.1% had deep vein thrombosis (DVT); 38.9% had pulmonary embolism (PE); and 12.0% had both on presentation. The overall frequency of major bleeding was 10.2% (34/334). Major bleeding occurred in 3.6% (12/334) in the first month, and 1.1% (14/1237) and 0.7% (8/1086) per patient‐month during months 2–6 and 7–12, respectively. Recurrent VTE occurred in 11.1% (37/334); the incidence rate was 5.7% (19/334) for month 1, 3.4% (10/296) during months 2–6, and 4.1% (8/194) during months 7–12. One hundred and sixteen patients died, four due to recurrent VTE and two due to bleeding.
Conclusion
Major bleeding was less frequent during dalteparin therapy beyond 6 months. The risk of developing major bleeding complications or VTE recurrence was greatest in the first month of therapy and lower over the subsequent 11 months.
We present a measurement of the Hubble constant H0 using the gravitational wave (GW) event GW190814, which resulted from the coalescence of a 23 M black hole with a 2.6 M compact object, as a ...standard siren. No compelling electromagnetic counterpart has been identified for this event; thus our analysis accounts for thousands of potential host galaxies within a statistical framework. The redshift information is obtained from the photometric redshift (photo-z) catalog from the Dark Energy Survey. The luminosity distance is provided by the LIGO/Virgo gravitational wave sky map. Since this GW event has the second-smallest localization volume after GW170817, GW190814 is likely to provide the best constraint on cosmology from a single standard siren without identifying an electromagnetic counterpart. Our analysis uses photo-z probability distribution functions and corrects for photo-z biases. We also reanalyze the binary black hole GW170814 within this updated framework. We explore how our findings impact the H0 constraints from GW170817, the only GW merger associated with a unique host galaxy. From a combination of GW190814, GW170814, and GW170817, our analysis yields (68% highest-density interval, HDI) for a prior in H0 uniform between . The addition of GW190814 and GW170814 to GW170817 improves the 68% HDI from GW170817 alone by ∼18%, showing how well-localized mergers without counterparts can provide a significant contribution to standard siren measurements, provided that a complete galaxy catalog is available at the location of the event.
ABSTRACT
We present an improved measurement of the Hubble constant (H0) using the ‘inverse distance ladder’ method, which adds the information from 207 Type Ia supernovae (SNe Ia) from the Dark ...Energy Survey (DES) at redshift 0.018 < z < 0.85 to existing distance measurements of 122 low-redshift (z < 0.07) SNe Ia (Low-z) and measurements of Baryon Acoustic Oscillations (BAOs). Whereas traditional measurements of H0 with SNe Ia use a distance ladder of parallax and Cepheid variable stars, the inverse distance ladder relies on absolute distance measurements from the BAOs to calibrate the intrinsic magnitude of the SNe Ia. We find H0 = 67.8 ± 1.3 km s−1 Mpc−1 (statistical and systematic uncertainties, 68 per cent confidence). Our measurement makes minimal assumptions about the underlying cosmological model, and our analysis was blinded to reduce confirmation bias. We examine possible systematic uncertainties and all are below the statistical uncertainties. Our H0 value is consistent with estimates derived from the Cosmic Microwave Background assuming a ΛCDM universe.
We present accretion disk size measurements for 15 luminous quasars at 0.7 ≤ z ≤ 1.9 derived from griz light curves from the Dark Energy Survey. We measure the disk sizes with continuum reverberation ...mapping using two methods, both of which are derived from the expectation that accretion disks have a radial temperature gradient and the continuum emission at a given radius is well described by a single blackbody. In the first method we measure the relative lags between the multiband light curves, which provides the relative time lag between shorter and longer wavelength variations. From this, we are only able to constrain upper limits on disk sizes, as many are consistent with no lag the 2 level. The second method fits the model parameters for the canonical thin disk directly rather than solving for the individual time lags between the light curves. Our measurements demonstrate good agreement with the sizes predicted by this model for accretion rates between 0.3 and 1 times the Eddington rate. Given our large uncertainties, our measurements are also consistent with disk size measurements from gravitational microlensing studies of strongly lensed quasars, as well as other photometric reverberation mapping results, that find disk sizes that are a factor of a few (∼3) larger than predictions.
We present the results of a search for rapidly evolving transients in the Dark Energy Survey Supernova Programme. These events are characterized by fast light-curve evolution (rise to peak in ≲10 d ...and exponential decline in ≲30 d after peak). We discovered 72 events, including 37 transients with a spectroscopic redshift from host galaxy spectral features. The 37 events increase the total number of rapid optical transients by more than a factor of two. They are found at a wide range of redshifts (0.05 < z < 1.56) and peak brightnesses (-15.75 > Mg > -22.25). The multiband photometry is well fit by a blackbody up to few weeks after peak. The events appear to be hot (T ≈ 10 000–30 000 K) and large (R ≈ 1014 - 2 × 1015 cm) at peak, and generally expand and cool in time, though some events show evidence for a receding photosphere with roughly constant temperature. Spectra taken around peak are dominated by a blue featureless continuum consistent with hot, optically thick ejecta. We compare our events with a previously suggested physical scenario involving shock breakout in an optically thick wind surrounding a core-collapse supernova, we conclude that current models for such a scenario might need an additional power source to describe the exponential decline. We find that these transients tend to favour star-forming host galaxies, which could be consistent with a core-collapse origin. However, more detailed modelling of the light curves is necessary to determine their physical origin.
Although epilepsy is associated with substantial role impairment, it is also highly comorbid with other physical and mental disorders, making unclear the extent to which impairments associated with ...epilepsy are actually due to comorbidities. This issue was explored in the National Comorbidity Survey Replication (NCS-R), a nationally representative household survey of 5692 US adults. Medically recognized epilepsy was ascertained with self-report, comorbid physical disorders with a chronic conditions checklist, and comorbid DSM-IV mental disorders with the Composite International Diagnostic Interview. Lifetime epilepsy prevalence was estimated at 1.8%. Epilepsy was comorbid with numerous neurological and general medical conditions and with a sporadic cluster of mental comorbidities (panic, PTSD, conduct disorder and substance use disorders). Although comorbid disorders explain part of the significant gross associations of epilepsy with impairment, epilepsy remains significantly associated with work disability, cognitive impairment and days of role impairment after controlling comorbidities. The net association of epilepsy with days of role impairment after controlling for comorbidities is equivalent to an annualized 89.4 million excess role impairment days among US adults with epilepsy, arguing that role impairment is a major component of the societal costs of epilepsy per se rather than merely due to disorders comorbid with epilepsy. This estimated burden is likely conservative as some parts of the effects of epilepsy are presumably mediated by secondary comorbid disorders.
Abstract
We present results for the first three years of OzDES, a six year programme to obtain redshifts for objects in the Dark Energy Survey (DES) supernova fields using the 2dF fibre positioner ...and AAOmega spectrograph on the Anglo-Australian Telescope. OzDES is a multi-object spectroscopic survey targeting multiple types of targets at multiple epochs over a multiyear baseline and is one of the first multi-object spectroscopic surveys to dynamically include transients into the target list soon after their discovery. At the end of three years, OzDES has spectroscopically confirmed almost 100 supernovae, and has measured redshifts for 17 000 objects, including the redshifts of 2566 supernova hosts. We examine how our ability to measure redshifts for targets of various types depends on signal-to-noise ratio (S/N), magnitude and exposure time, finding that our redshift success rate increases significantly at a S/N of 2–3 per 1-Å bin. We also find that the change in S/N with exposure time closely matches the Poisson limit for stacked exposures as long as 10 h. We use these results to predict the redshift yield of the full OzDES survey, as well as the potential yields of future surveys on other facilities such as (i.e. the 4-m Multi-Object Spectroscopic Telescope, the Subaru Prime Focus Spectrograph and the Maunakea Spectroscopic Explorer). This work marks the first OzDES data release, comprising 14 693 redshifts. OzDES is on target to obtain over 30 000 redshifts over the 6-yr duration of the survey, including a yield of approximately 5700 supernova host-galaxy redshifts.
Rare earth elements have generally not been thought to have a biological role. However, recent work has demonstrated that the light REEs (LREEs: La, Ce, Pr, and Nd) are essential for at least some ...methanotrophs, being co-factors in the XoxF type of methanol dehydrogenase (MDH). We show here that dissolved LREEs were significantly removed in a submerged plume of methane-rich water during the Deepwater Horizon (DWH) well blowout. Furthermore, incubation experiments conducted with naturally methane-enriched waters from hydrocarbon seeps in the vicinity of the DWH wellhead also showed LREE removal concurrent with methane consumption. Metagenomic sequencing of incubation samples revealed that LREE-containing MDHs were present. Our field and laboratory observations provide further insight into the biochemical pathways of methanotrophy during the DWH blowout. Additionally, our results are the first observations of direct biological alteration of REE distributions in oceanic systems. In view of the ubiquity of LREE-containing MDHs in oceanic systems, our results suggest that biological uptake of LREEs is an overlooked aspect of the oceanic geochemistry of this group of elements previously thought to be biologically inactive and an unresolved factor in the flux of methane, a potent greenhouse gas, from the ocean.
Mammalian captive dietary specialists like folivores are prone to gastrointestinal distress and primate dietary specialists suffer the greatest gut microbiome diversity losses in captivity compared ...to the wild. Marmosets represent another group of dietary specialists, exudivores that eat plant exudates, but whose microbiome remains relatively less studied. The common occurrence of gastrointestinal distress in captive marmosets prompted us to study the Callithrix gut microbiome composition and predictive function through bacterial 16S ribosomal RNA V4 region sequencing. We sampled 59 wild and captive Callithrix across four species and their hybrids. Host environment had a stronger effect on the gut microbiome than host taxon. Wild Callithrix gut microbiomes were enriched for Bifidobacterium, which process host-indigestible carbohydrates. Captive marmoset guts were enriched for Enterobacteriaceae, a family containing pathogenic bacteria. While gut microbiome function was similar across marmosets, Enterobacteriaceae seem to carry out most functional activities in captive host guts. More diverse bacterial taxa seem to perform gut functions in wild marmosets, with Bifidobacterium being important for carbohydrate metabolism. Captive marmosets showed gut microbiome composition aspects seen in human gastrointestinal diseases. Thus, captivity may perturb the exudivore gut microbiome, which raises implications for captive exudivore welfare and calls for husbandry modifications.
