Background
Chronic exposure to stress has been linked to several negative physiological and psychological health outcomes. Among employees, stress and its associated effects can also result in ...productivity losses and higher healthcare costs. In‐person (face‐to‐face) and computer‐based (web‐ and mobile‐based) stress management interventions have been shown to be effective in reducing stress in employees compared to no intervention. However, it is unclear if one form of intervention delivery is more effective than the other. It is conceivable that computer‐based interventions are more accessible, convenient, and cost‐effective.
Objectives
To compare the effects of computer‐based interventions versus in‐person interventions for preventing and reducing stress in workers.
Search methods
We searched CENTRAL, MEDLINE, PubMed, Embase, PsycINFO, NIOSHTIC, NIOSHTIC‐2, HSELINE, CISDOC, and two trials registers up to February 2017.
Selection criteria
We included randomised controlled studies that compared the effectiveness of a computer‐based stress management intervention (using any technique) with a face‐to‐face intervention that had the same content. We included studies that measured stress or burnout as an outcome, and used workers from any occupation as participants.
Data collection and analysis
Three authors independently screened and selected 75 unique studies for full‐text review from 3431 unique reports identified from the search. We excluded 73 studies based on full‐text assessment. We included two studies. Two review authors independently extracted stress outcome data from the two included studies. We contacted study authors to gather additional data. We used standardised mean differences (SMDs) with 95% confidence intervals (CIs) to report study results. We did not perform meta‐analyses due to variability in the primary outcome and considerable statistical heterogeneity. We used the GRADE approach to rate the quality of the evidence.
Main results
Two studies met the inclusion criteria, including a total of 159 participants in the included arms of the studies (67 participants completed computer‐based interventions; 92 participants completed in‐person interventions). Workers were primarily white, Caucasian, middle‐aged, and college‐educated. Both studies delivered education about stress, its causes, and strategies to reduce stress (e.g. relaxation or mindfulness) via a computer in the computer‐based arm, and via small group sessions in the in‐person arm. Both studies measured stress using different scales at short‐term follow‐up only (less than one month). Due to considerable heterogeneity in the results, we could not pool the data, and we analysed the results of the studies separately. The SMD of stress levels in the computer‐based intervention group was 0.81 standard deviations higher (95% CI 0.21 to 1.41) than the in‐person group in one study, and 0.35 standard deviations lower (95% CI ‐0.76 to 0.05) than the in‐person group in another study. We judged both studies as having a high risk of bias.
Authors' conclusions
We found very low‐quality evidence with conflicting results, when comparing the effectiveness of computer‐based stress management interventions with in‐person stress management interventions in employees. We could include only two studies with small sample sizes. We have very little confidence in the effect estimates. It is very likely that future studies will change these conclusions.

For the most important and well-known infections spread by
Ixodes
ticks, Lyme borreliosis (LB) and tick-borne encephalitis (TBE), there are recommendations for diagnosis and management ...available from several health authorities and professional medical networks. However, other tick-borne microorganisms with potential to cause human disease are less known and clear recommendations on diagnosis and management are scarce. Therefore, we performed a systematic review of published studies and reviews focusing on evaluation of laboratory methods for clinical diagnosis of human tick-borne diseases (TBDs), other than acute LB and TBE. The specific aim was to evaluate the scientific support for laboratory diagnosis of human granulocytic anaplasmosis, rickettsiosis, neoehrlichiosis, babesiosis, hard tick relapsing fever, tularemia and bartonellosis, as well as tick-borne co-infections and persistent LB in spite of recommended standard antibiotic treatment.
Methods:
We performed a systematic literature search in 11 databases for research published from 2007 through 2017, and categorized potentially relevant references according to the predefined infections and study design. An expert group assessed the relevance and eligibility and reviewed the articles according to the QUADAS (diagnostic studies) or AMSTAR (systematic reviews) protocols, respectively. Clinical evaluations of one or several diagnostic tests and systematic reviews were included. Case reports, non-human studies and articles published in other languages than English were excluded.
Results:
A total of 48 studies fulfilled the inclusion criteria for evaluation. The majority of these studies were based on small sample sizes. There were no eligible studies for evaluation of tick-borne co-infections or for persistent LB after antibiotic treatment.
Conclusions:
Our findings highlight the need for larger evaluations of laboratory tests using clinical samples from well-defined cases taken at different time-points during the course of the diseases. Since the diseases occur at a relatively low frequency, single-center cross-sectional studies are practically not feasible, but multi-center case control studies could be a way forward.
Early trauma-focused cognitive-behavioural therapy (TFCBT) holds promise as a preventive intervention for people at risk of developing chronic post-traumatic stress disorder (PTSD). The aim of this ...review was to provide an updated evaluation of the effectiveness of early TFCBT on the prevention of PTSD in high risk populations.
We performed a systematic literature search in international electronic databases (MEDLINE, EMBASE, PsycINFO, CENTRAL, CINAHL, ISI and PILOTS) and included randomised controlled trials comparing TFCBT delivered within 3 months of trauma, to alternative interventions. All included studies were critically appraised using a standardised checklist. Two independent reviewers selected studies for inclusion and assessed study quality. Data extraction was performed by one reviewer and controlled by another. Where appropriate, we entered study results into meta-analyses.
Seven articles reporting the results of five RCTs were included. All compared TFCBT to supportive counselling (SC). The study population was patients with acute stress disorder (ASD) in four trials, and with a PTSD diagnosis disregarding the duration criterion in the fifth trial. The overall relative risk (RR) for a PTSD diagnosis was 0.56 (95% CI 0.42 to 0.76), 1.09 (95% CI 0.46 to 2.61) and 0.73 (95% CI 0.51 to 1.04) at 3-6 months, 9 months and 3-4 years post treatment, respectively. A subgroup analysis of the four ASD studies only resulted in RR = 0.36 (95% CI 0.17 to 0.78) for PTSD at 3-6 months. Anxiety and depression scores were generally lower in the TFCBT groups than in the SC groups.
There is evidence for the effectiveness of TFCBT compared to SC in preventing chronic PTSD in patients with an initial ASD diagnosis. As this evidence originates from one research team replications are necessary to assess generalisability. The evidence about the effectiveness of TFCBT in traumatised populations without an ASD diagnosis is insufficient.

The article is based on a search in MEDLINE, EMBASE, CINAHL, Cochrane CENTRAL, AMED, PEDro, and the Index to Chiropractic Literature in August 2017. Controlled studies were included to assess efficacy, and observational studies to reveal the risk of harm.
We reviewed 3 418 unique hits from the literature search. We included three randomised controlled studies on the efficacy of manual therapy or osteopathy and three observational studies on possible adverse effects. Two studies on manual therapy suggested that manipulation techniques do not have any greater effect on symmetry and motor outcomes than parental guidance, physiotherapy and home exercises. One study suggested that osteopathy may contribute to increased symmetry compared to placebo, but the clinical significance of this change is uncertain. Neither the three randomised studies nor a large patient series involving 695 infants found manipulation techniques to be associated with a risk of serious adverse events, but three patient histories demonstrated that manipulation techniques may cause harm if the observed asymmetry has serious underlying causes.
Manipulation techniques have not been shown to be efficacious in the treatment of infant torticollis, but the available evidence must be considered uncertain.
The objective was to find evidence to substantiate assertions that electronic applications for medication management in ambulatory care (electronic prescribing, clinical decision support (CDSS), ...electronic health record, and computer generated paper prescriptions), while intended to reduce prescribing errors, can themselves result in errors that might harm patients or increase risks to patient safety.
Because a scoping search for adverse events in randomized controlled trials (RCTs) yielded few relevant results, we systematically searched nine databases, including MEDLINE, EMBASE, and The Cochrane Database of Systematic Reviews for systematic reviews and studies of a wide variety of designs that reported on implementation of the interventions. Studies that had safety and adverse events as outcomes, monitored for them, reported anecdotally adverse events or other events that might indicate a threat to patient safety were included.
We found no systematic reviews that examined adverse events or patient harm caused by organizational interventions. Of the 4056 titles and abstracts screened, 176 full-text articles were assessed for inclusion. Sixty-one studies with appropriate interventions, settings and participants but without patient safety, adverse event outcomes or monitoring for risks were excluded, along with 77 other non-eligible studies. Eighteen randomized controlled trials (RCTs), 5 non-randomized controlled trials (non-R,CTs) and 15 observational studies were included. The most common electronic intervention studied was CDSS and the most frequent clinical area was cardio-vascular, including anti-coagulants. No RCTS or non-R,CTS reported adverse event. Adverse events reported in observational studies occurred less frequently after implementation of CDSS. One RCT and one observational study reported an increase in problematic prescriptions with electronic prescribing
The safety implications of electronic medication management in ambulatory care have not been established with results from studies included in this systematic review. Only a minority of studies that investigated these interventions included threats to patients' safety as outcomes or monitored for adverse events. It is therefore not surprising that we found little evidence to substantiate fears of new risks to patient safety with their implementation. More research is needed to focus on the draw-backs and negative outcomes that implementation of these interventions might introduce.

Objectives: This study aims to compare two different search methods for determining the scope of a requested systematic review or health technology assessment. The first method (called the Direct ...Search Method) included performing direct searches in the Cochrane Database of Systematic Reviews (CDSR), Database of Abstracts of Reviews of Effects (DARE) and the Health Technology Assessments (HTA). Using the comparison method (called the NHS Search Engine) we performed searches by means of the search engine of the British National Health Service, NHS Evidence. Methods: We used an adapted cross-over design with a random allocation of fifty-five requests for systematic reviews. The main analyses were based on repeated measurements adjusted for the order in which the searches were conducted. Results: The Direct Search Method generated on average fewer hits (48 percent 95 percent confidence interval {CI} 6 percent to 72 percent, had a higher precision (0.22 95 percent CI, 0.13 to 0.30) and more unique hits than when searching by means of the NHS Search Engine (50 percent 95 percent CI, 7 percent to 110 percent). On the other hand, the Direct Search Method took longer (14.58 minutes 95 percent CI, 7.20 to 21.97) and was perceived as somewhat less user-friendly than the NHS Search Engine (−0.60 95 percent CI, −1.11 to −0.09). Conclusions: Although the Direct Search Method had some drawbacks such as being more time-consuming and less user-friendly, it generated more unique hits than the NHS Search Engine, retrieved on average fewer references and fewer irrelevant results.

I denne artikkelen beskriver vi prinsipper for å utføre systematiske søk etter forskningslitteratur. I et godt søk bør det tas utgangspunkt i et klart formulert spørsmål som uttrykker hva slags ...forskning en ønsker å finne. For effektspørsmål kan dette formuleres ved hjelp av et PICO-skjema hvor det konkretiseres hvilke pasienter, tiltak og utfallsmål en er interessert i. I litteratursøket er det vanlig å bruke engelske søkeord som beskriver disse PICO-kategoriene. For å finne så mye relevant forskning som mulig, er det viktig å søke på alle relevante synonymer og staveformer. En kan også søke på standardiserte emneord som beskriver innholdet i publikasjonene. De boolske operatorene AND og OR brukes for å kombinere søkeordene i en samlet søkestrategi. For å få et håndterlig antall søketreff, kan søket avgrenses til publikasjonsår, spesielle aldersgrupper eller relevant studiedesign. Det kan være nyttig å dokumentere hvilke kilder og søkeord som er brukt. I de fleste kilder kan søket lagres og hentes frem senere og søkeresultatene kan eksporteres til referansehåndteringsprogrammer som Reference Manager eller EndNote. I noen tilfeller kan det være nyttig å supplere det systematiske søket med andre gjenfinningsmetoder som søk i studieregistre, gjennomgang av referanselister i relevante publikasjoner, siteringssøk og tips fra fageksperter. A well-conducted literature search is based on a clearly formulated question, expressing what one is looking for. Questions regarding effects of interventions can be structured with the aid of PICO, stating the population, intervention and outcomes of interest. The first step is to identify English search terms that describe the different PICO categories. It is important to search for all relevant synonyms and spellings in order to identify as many relevant studies as possible. One may also search for standardized subject headings that are used to describe the content of the publications. Search terms are then combined with the Boolean operators AND and OR. In order to end up with a reasonable amount of references, one can limit the search to specific publication years, age groups and relevant study designs. To ease the process of scanning through the search hits, it is advisable to export the search results into reference management software, such as Reference Manager or Endnote. Many databases also allow saving the searches for use at a later stage. Sometimes it might be necessary to use supplementary search methods, such as searching trials registers, scanning through reference lists of relevant publications, conducting citation searches, and contacting content experts to identify further relevant studies. The search process, including the selected sources and search terms, should be documented and reported. Even though advanced systematic searches often are performed by librarians, everyone can learn how to search for literature. It is a “learning by doing” process, and we recommend contacting a librarian for further guidance and help.
