Global concerns about climate change were once again expressed at the COP27 in Sharm El‐Sheikh. Seaweed is frequently presented as a solution for climate mitigation. For a proper appraisal of its ...contribution to mitigating climate change, it is necessary to distinguish between, and critically scrutinize, the various pathways seaweed‐based climate mitigations can take. This article identifies four different climate mitigation pathways and critically reflects on each. First, carbon sequestration, occurring when grown seaweed is left in the seas or, second, purposefully sunk. Third, carbon emission reduction, resulting when seaweed‐based products replace products with a higher carbon footprint, either fossil based products or other organic material. Fourth, carbon emission avoidance, taking place when seaweed products are used to avoid greenhouse gas emissions in other production processes. Each of these pathways requires specific methods to quantify their magnitude and comes with critical questions to ask. The sequestration pathway requires monitoring of net carbon production and the amount of carbon that is eventually exported to the deep sea. Pathways 3 and 4 require Life Cycle Assessment and/or Carbon Footprint with system boundaries set to include the production system itself and installation thereof. We propose an unequivocal categorization in a belief that confusion on the benefits of seaweed will eventually impede development of seaweed‐based solutions.

With a world population estimated at 10 billion people by 2050, the challenge to secure healthy and safe food is evident. Seaweed is a potential answer to this challenge. Expanding the use of seaweed ...in food systems requires an emphasis on safe practices to avoid adverse human health effects after consumption and irreversible damage to marine ecosystems. This study aims to evaluate relevant food safety and environmental safety hazards, monitoring measures, and mitigation strategies in the seaweed sector. For this study, a literature review, survey (
= 36), and interviews (
= 12) were conducted to identify hazards. The review and interviews aimed at pinpointing monitoring measures and mitigation strategies applied, while the survey revealed data gaps and further actions needed for the sector. Relevant food safety hazards include (inorganic) arsenic, iodine, and heavy metals, among others, such as pathogenic bacteria, while environmental hazards include environmental pathogens and parasites introduced into the ecosystem by domesticated seaweed, among others. Measures applied aim at preventing or mitigating hazards through good hygienic or manufacturing practices, food safety procedures or protocols, or pre-site farm selection. Although the future needs of the seaweed sector vary, for some, harmonized advice and protocols that align with a changing food system and hazard knowledge development as well as information on the benefits of seaweed and regulating climate and water quality may help.
Abstract Within the framework of Ecosystem Approach to Aquaculture (EAA), ecological carrying capacity (ECC) is a key concept that helps to determine the upper limit of production without ...compromising ecosystem functioning. The implementation of ECC is complex as ECC differs between type of farms and location and standardised methods should be developed for management. There is therefore a clear need for operational indicators. The objectives of this paper were: (1) to carry out a systematic literature review on shellfish and seaweed aquaculture‐environment interactions to list the most used environmental indicators, (2) to classify the indicators according to the effects they measure (i.e., benthic, water quality, food web interactions, cultured organism health, resource use) and the scale at which they are applied, and (3) to assess their potential based on four indicator criteria categories: sensitivity, accuracy and precision, feasibility and utility, and ecosystem‐level scalability. Overall, indicators describing benthic effects were the most highly cited and scored. Indicators identified for bivalve and seaweed culture were discussed and compared to previous work on salmon aquaculture indicators to highlight similarities and differences across trophic levels. In addition, questions related to the challenges of ECC indicators implementation were presented to a panel of experts. The scoring and consultation provided the source of discussion on environmental management consistent with EAA.
Marine kelp forests cover 1/3 of our world's coastlines, are heralded as a nature-based solution to address socio-environmental issues, connect hundreds of millions of people with the ocean, and ...support a rich web of biodiversity throughout our oceans. But they are increasingly threatened with some areas reporting over 90% declines in kelp forest cover in living memory. Despite their importance and the threats they face, kelp forests are entirely absent from the international conservation dialogue. No international laws, policies, or targets focus on kelp forests and very few countries consider them in their national policy. The Kelp Forest Challenge addresses that gap. Together with 252 kelp experts, professionals, and citizens from 25 countries, the Kelp Forest Challenge was developed as a grassroots vision of what the world can achieve for kelp forest conservation. It is a global call to restore 1 million and protect 3 million hectares of kelp forests by 2040. This is a monumental challenge, that will require coordination across multiple levels of society and the mobilization of immense resources. Pledges may therefore include area for protection or restoration, enabling pledges which assist in conservation (funding, equipment, professional expertise, capacity building), or awareness-based pledges which increase awareness or education about kelp forests. Correspondingly, participants may be from government, scientific institutions, private sector, NGOs, community groups, or individuals. This challenge is the beginning of a 17-year mission to save our kelp forests and anyone and any organisation is invited to participate.
Multiple sclerosis (MS) is the most common cause of neurological disability in young adults worldwide and approximately half of those affected are in Europe. The assessment of differential incidence ...and prevalence across populations can reveal spatial, temporal and demographic patterns which are important for identifying genetic and environmental factors contributing to MS. However, study methodologies vary and the quality of the methods can influence the estimates. This study aimed to systematically review European studies of incidence and prevalence of MS and to provide a quantitative assessment of their methodological quality.
A comprehensive literature search was performed to obtain all original population-based studies of MS incidence and prevalence in European populations conducted and published between January 1985 and January 2011. Only peer-reviewed full-text articles published in English or French were included. All abstracts were screened for eligibility and two trained reviewers abstracted the data and graded the quality of each study using a tool specifically designed for this study.
There were 123 studies that met the inclusion criteria. The study estimates were highly heterogeneous, even within regions or countries. Quality was generally higher in the more recent studies, which also tended to use current diagnostic criteria. Prevalence and incidence estimates tended to be higher in the more recent studies and were higher in the Nordic countries and in northern regions of the British Isles. With rare exceptions, prevalence and incidence estimates were higher in women with ratios as high as 3:1. Few studies examined ethnicity. Epidemiological data at the national level was uncommon and there were marked geographical disparities in available data, with large areas of Europe unrepresented and other regions well-represented in the literature. Only 37% of the studies provided standardized estimates.
Despite the breadth of the literature on the epidemiology of MS in Europe, inter-study comparisons are hampered by the lack of standardization. Further research should focus on regions not yet studied and the evaluation of ethnic differences in MS prevalence and incidence. National-level studies using current diagnostic criteria, validated case definitions and similar age- and sex-standardization would allow better geographical comparisons.

Individualization of vancomycin dosing based on therapeutic drug monitoring (TDM) data is known to improve patient outcomes compared with fixed or empirical dosing strategies. There is increasing ...evidence to support area-under-the-curve (AUC24)-guided TDM to inform vancomycin dosing decisions for patients receiving therapy for more than 48 hours. It is acknowledged that there may be institutional barriers to the implementation of AUC24-guided dosing, and additional effort is required to enable the transition from trough-based to AUC24-based strategies. Adequate documentation of sampling, correct storage and transport, accurate laboratory analysis, and pertinent data reporting are required to ensure appropriate interpretation of TDM data to guide vancomycin dosing recommendations. Ultimately, TDM data in the clinical context of the patient and their response to treatment should guide vancomycin therapy. Endorsed by the International Association of Therapeutic Drug Monitoring and Clinical Toxicology, the IATDMCT Anti-Infectives Committee, provides recommendations with respect to best clinical practice for vancomycin TDM.

Phosphatidylethanol can detect low levels of alcohol consumption in the previous two weeks. In this study we aimed to biochemically assess the prevalence of alcohol consumption during early pregnancy using phosphatidylethanol in blood and compare this with self-reported alcohol consumption.
To evaluate biochemically assessed prevalence of alcohol consumption during early pregnancy using phosphatidylethanol levels, we conducted a prospective, cross-sectional, single center study in the largest tertiary hospital of the Netherlands. All adult pregnant women who were under the care of the obstetric department of the Erasmus MC and who underwent routine blood testing at a gestational age of less than 15 weeks were eligible. No specified informed consent was needed.
The study was conducted between September 2016 and October 2017. In total, we received 1,002 residual samples of 992 women. After applying in- and exclusion criteria we analyzed 684 samples. Mean gestational age of all included women was 10.3 weeks (SD 1.9). Of these women, 36 (5.3 %) tested positive for phosphatidylethanol, indicating alcohol consumption in the previous two weeks. Of women with a positive phosphatidylethanol test, 89 % (n = 32) did not express alcohol consumption to their obstetric care provider.
One in nineteen women consumed alcohol during early pregnancy with a high percentage not reporting this use to their obstetric care provider. Questioning alcohol consumption by an obstetric care provider did not successfully identify (hazardous) alcohol consumption. Routine screening with phosphatidylethanol in maternal blood can be of added value to identify women who consume alcohol during pregnancy.
Bacterial DNA and immunostimulatory (i.s.) synthetic CpG‐oligodeoxynucleotides (ODN) act as adjuvants for Th1 responses and cytotoxic T cell responses to proteinaceous antigens. Dendritic cells (DC) ...can be referred to as
“nature's adjuvant” since they display the unique capacity to sensitize naive T cells. Here, we demonstrate that bacterial DNA or i.s. CpG‐ODN cause simultaneous maturation of immature DC and activation of mature DC to produce cytokines. These events are associated with the acquisition of professional antigen‐presenting cell (APC) function. Unfractionated murine bone marrow‐derived DC and FACS®‐fractionated MHC class IIlow (termed immature DC) or MHC class IIhigh populations (termed mature DC) were stimulated with bacterial DNA or i.s. CpG‐ODN. Similar to lipopolysaccharide, i.s. CpG‐ODN caused up‐regulation of MHC class II, CD40 and CD86, but not CD80 on immature and mature DC. In parallel both DC subsets were activated to produce large amounts of IL‐12, IL‐6 and TNF‐α. CpG‐ODN‐activated DC displayed professional APC function in allogeneic mixed lymphocyte reaction and in staphylococcal enterotoxin B‐driven naive T cell responses. We interpret these findings to mean that bacterial DNA and i.s. CpG‐ODN cause maturation (first step) and activation (second step) of DC to bring about conversion of immature DC into professional APC.
Recent studies have successfully demonstrated the use of deep-learning algorithms for dermatologist-level classification of suspicious lesions by the use of excessive proprietary image databases and ...limited numbers of dermatologists. For the first time, the performance of a deep-learning algorithm trained by open-source images exclusively is compared to a large number of dermatologists covering all levels within the clinical hierarchy.
We used methods from enhanced deep learning to train a convolutional neural network (CNN) with 12,378 open-source dermoscopic images. We used 100 images to compare the performance of the CNN to that of the 157 dermatologists from 12 university hospitals in Germany. Outperformance of dermatologists by the deep neural network was measured in terms of sensitivity, specificity and receiver operating characteristics.
The mean sensitivity and specificity achieved by the dermatologists with dermoscopic images was 74.1% (range 40.0%–100%) and 60% (range 21.3%–91.3%), respectively. At a mean sensitivity of 74.1%, the CNN exhibited a mean specificity of 86.5% (range 70.8%–91.3%). At a mean specificity of 60%, a mean sensitivity of 87.5% (range 80%–95%) was achieved by our algorithm. Among the dermatologists, the chief physicians showed the highest mean specificity of 69.2% at a mean sensitivity of 73.3%. With the same high specificity of 69.2%, the CNN had a mean sensitivity of 84.5%.
A CNN trained by open-source images exclusively outperformed 136 of the 157 dermatologists and all the different levels of experience (from junior to chief physicians) in terms of average specificity and sensitivity.
•A convolutional neural network (CNN) received enhanced training with 12,378 open-source dermoscopic images.•In a head-to-head comparison, the CNN outperformed 136 of 157 participating dermatologists.•The CNN was capable to outperform dermatologists of all hierarchical subgroups (from junior to chief physicians) in dermoscopic melanoma image classification.
