Specialists and their practice administrators have been asking what role they play in accountable care organizations, patient-centered medical homes. They are not seeking alternatives to ...fee-for-service but, at a minimum, want to be a player at the table for the new payment models to avoid losing future referrals. Each specialty physician or group in your market will ultimately decide what makes the most business and personal sense for its practice. Nonetheless, waiting until specialty guidelines are published to engage specialists in this dialogue is a missed opportunity. Recognize that there is a finite and often limited number of valuable players on the sidelines now who will be needed for the daunting task of population health management; do not lessen your or their options. Allow them to be a part of the solution. At a minimum, develop a relationship; learn from each other; and, if common interests align, create an affiliation that mutually works. Build strategies together now rather than separate, competing strategies later.

The concept of the rock-like oxide (ROX) fuel has been developed for the annihilation of excess plutonium in light water reactors. Irradiation tests and post-irradiation examinations were carried out ...on candidate ROX fuels. The ternary fuel of YSZ–spinel–corundum system, the single-phase fuels of YSZ, the particle-dispersed fuels of YSZ in spinel or corundum matrix, and the blended fuels of YSZ and spinel or corundum matrix were fabricated and submitted to irradiation testings. The fuels containing spinel showed chemical instabilities with the vaporization of MgO component, which caused fuel restructuring. The swelling behavior was improved with the particle-dispersed fuels. However, the particle-dispersed fuels showed higher fractional gas release (FGR) than blended type fuels. The FGR of YSZ single-phase fuels were comparable to what would be expected for UO
2 fuel at the similar fuel temperatures. The YSZ single-phase fuel showed the best irradiation performance among the ROX fuels investigated.
Bone cutting experiments were conducted on a cadaver using a newly developed machine tool. The effectiveness of the system was evaluated by comparing the planned shape for the mating surface between ...the femur and an artificial joint with the cutting result. It is desirable to know the machining phenomena and characteristics at the microscale because the material and the tissue structure are anisotropic in both cortical and cancellous bones. In the paper, the influence of the material characteristics on machining is discussed for cortical bone. Pig cortical bone was used in the experiment for visualizing 2-dimensional micro-cutting.
Stability of La substituted SrTiO3 anode in coal syngas fuel was investigated. La0.25Sr0.75TiO3 and (La0.25Sr0.75)0.9TiO3 pellets were annealed in simulated coal syngas with 1 ppm PH3 atmosphere at ...900 oC for 100 hrs. Sr3(PO4)2 and TiO2 were observed on the surface of each sample after the exposure test. XRD and SEM/EDS results indicate that most Sr3(PO4)2 was formed on the surface of stoichiometric LST. Chemical stability of LST for PH3 was effectively improved by changing the A-site deficient composition.
Summary Background We aimed to investigate whether the benefits of blood pressure-lowering drugs are proportional to baseline cardiovascular risk, to establish whether absolute risk could be used to ...inform treatment decisions for blood pressure-lowering therapy, as is recommended for lipid-lowering therapy. Methods This meta-analysis included individual participant data from trials that randomly assigned patients to either blood pressure-lowering drugs or placebo, or to more intensive or less intensive blood pressure-lowering regimens. The primary outcome was total major cardiovascular events, consisting of stroke, heart attack, heart failure, or cardiovascular death. Participants were separated into four categories of baseline 5-year major cardiovascular risk using a risk prediction equation developed from the placebo groups of the included trials (<11%, 11–15%, 15–21%, >21%). Findings 11 trials and 26 randomised groups met the inclusion criteria, and included 67 475 individuals, of whom 51 917 had available data for the calculation of the risk equations. 4167 (8%) had a cardiovascular event during a median of 4·0 years (IQR 3·4–4·4) of follow-up. The mean estimated baseline levels of 5-year cardiovascular risk for each of the four risk groups were 6·0% (SD 2·0), 12·1% (1·5), 17·7% (1·7), and 26·8% (5·4). In each consecutive higher risk group, blood pressure-lowering treatment reduced the risk of cardiovascular events relatively by 18% (95% CI 7–27), 15% (4–25), 13% (2–22), and 15% (5–24), respectively (p=0·30 for trend). However, in absolute terms, treating 1000 patients in each group with blood pressure-lowering treatment for 5 years would prevent 14 (95% CI 8–21), 20 (8–31), 24 (8–40), and 38 (16–61) cardiovascular events, respectively (p=0·04 for trend). Interpretation Lowering blood pressure provides similar relative protection at all levels of baseline cardiovascular risk, but progressively greater absolute risk reductions as baseline risk increases. These results support the use of predicted baseline cardiovascular disease risk equations to inform blood pressure-lowering treatment decisions. Funding None.
Microwave radiation from extensive air showers is expected to provide a new technique to observe UHECR. We developed three set of radio telescopes in Osaka, in Kobe and at Telescope Array site in ...Utah, USA. In Osaka, we are coincidentally operating two Ku band radio telescopes with an air shower array which consists of nine plastic scintillators with about 10 m separation. In Kobe, we have started the operation of twelve radio telescopes of 1.2 m diameter in March 2012. In Utah, we installed two telescopes just beside the Black Rock Mesa fluorescence detector (FD) station of the Telescope Array experiment, and we operated the radio telescopes coincidentally with FD event triggers. We report the experimental setups and the results of these measurements.

The Telescope Array (TA) experiment, located in the western desert of Utah, USA, is designed for the observation of extensive air showers from extremely high energy cosmic rays. The experiment has a ...surface detector array surrounded by three fluorescence detectors to enable simultaneous detection of shower particles at ground level and fluorescence photons along the shower track. The TA surface detectors and fluorescence detectors started full hybrid observation in March, 2008. In this article we describe the design and technical features of the TA surface detector.
The Telescope Array (TA) collaboration has measured the energy spectrum of ultra-high energy cosmic rays (UHECRs) with primary energies above 1.6 x 10 super(18) eV. This measurement is based upon ...four years of observation by the surface detector component of TA. The spectrum shows a dip at an energy of 4.6 x 10 super(18) eV and a steepening at 5.4 x 10 super(19) eV which is consistent with the expectation from the GZK cutoff. We present the results of a technique, new to the analysis of UHECR surface detector data, that involves generating a complete simulation of UHECRs striking the TA surface detector. The procedure starts with shower simulations using the CORSIKA Monte Carlo program where we have solved the problems caused by use of the "thinning" approximation. This simulation method allows us to make an accurate calculation of the acceptance of the detector for the energies concerned.
This paper proposes a human activity estimation system using a wearable multi-sensor with a built-in electrocardiograph and triaxial accelerometers. The multi-sensor unconstraintly measures ...biological information, and provides these data to personal computer by wireless communication. We estimate human activity in a series of activities by the biological information. In our experiment, the subjects have several activities such as "Walking", "Rest" and "Strength training". The system estimates these activities by a decision tree. Branch conditions of the decision tree are aided by fuzzy logic and state of activity transition from previous activity. Fuzzy membership functions are constructed from exercise intensity, distinction frequency and transitional probability. As the results, the proposed method estimated activities with good accuracy.
