This study investigated the sex differences, and the shared genetic and environmental factors underlying the associations of sleep disturbances (insomnia and sleep quality) with pain and somatic symptoms in both adolescents and middle-aged adults. We recruited 259 adolescents (69 with current insomnia) and their parents (256 middle-aged adults, 78 with current insomnia). Insomnia severity and sleep quality were measured by the Insomnia Severity Inventory (ISI) and Pittsburgh Sleep Quality Index (PSQI), respectively. Pain and somatic symptoms were measured by the Somatic Symptom Inventory and Visual Analogue Scale for overall pain. Subjects with insomnia scored higher on all measures of pain and somatic symptoms than non-insomnia patients, in both adolescents and adults (P<.001). Both pain and somatic measures were associated with ISI and PSQI scores after controlling for age, sex, depressive and anxiety symptoms. There was an interaction effect between insomnia and female sex on pain and somatic symptoms (P<.05), especially in adults. Pain and somatic symptoms ran in family with moderate heritability (range h2=0.15–0.42). The phenotypic associations of ISI and PSQI with pain and somatic measures were both contributed by genetic (range pG=0.41–0.96) and environmental (range pE=0.27–0.40) factors with a major genetic contribution. In summary, insomnia and poor sleep quality are closely associated with pain and somatic symptoms. Insomnia seems to modulate the sex differences in pain and somatic symptoms, especially in the adult population. Hong Kong’s construction industry has shown significant improvement in safety performance since the turn of the century. The number of industrial accidents in the construction industry has decreased ...from 11,925 in 2000 to 3,833 in 2004, which is an encouraging drop of almost 68%. However, the category “fall of person from height” has always represented a large proportion of the industrial accidents, particularly fatal accidents. In 2004, fall of person from height represented just over 47% of the total number of fatal accidents in the construction industry. The statistics show that although the overall number of accidents has dropped immensely, the same does not apply for fall from height accidents. According to statistics provided by the Labor Dept. of the Hong Kong Special Administrative Region, there were a total of 22 fatal industrial accidents associated with fall of persons from height in repair, maintenance, alteration, and addition works during 2000–2004. When analyzing these case studies, 12 common factors were identified for analyzing these case studies and strategies were suggested to prevent recurrence of similar accidents in each case. The top five strategies were: (1) provide and maintain a safe system of work; (2) provide a suitable working platform; (3) (tier) provide safety information/training/instruction/supervision; (4) (tier) provide suitable fall arresting system/anchorage; and (5) maintain safe workplace.

Window of opportunity (WoO) trials create the opportunity to demonstrate pharmacodynamic parameters of a drug in vivo and have increasing use in breast cancer research. Most breast cancer tumours are ...oestrogen receptor-positive (ER+), leading to the development of multiple treatment options tailored towards this particular tumour subtype. The aim of this literature review is to review WoO trials pertaining to the pharmacodynamic activity of drugs available for use in ER+ breast cancer in order to help guide treatment for patients receiving neoadjuvant and primary endocrine therapy. Five databases (EMBASE, Cochrane, MEDLINE, PubMed, Web of Science) were searched for eligible studies. Studies performed in treatment-naïve patients with histologically confirmed ER+ breast cancer were included if they acquired pre- and post-treatment biopsies, compared measurement of a proteomic biomarker between these two biopsies and delivered treatment for a maximum mean duration of 31 days. Fifteen studies were eligible for inclusion and covered six different drug classes: three endocrine therapies (ETs) including aromatase inhibitors (AIs), selective oestrogen receptor modulators (SERMs), selective oestrogen receptor degraders (SERDs) and three non-ETs including mTOR inhibitors, AKT inhibitors and synthetic oestrogens. Ki67 was the most frequently measured marker, appearing in all studies. Progesterone receptor (PR) and ER were the next most frequently measured markers, appearing five and four studies, respectively. All three of these markers were significantly downregulated in both AIs and SERDs; Ki67 alone was downregulated in SERMs. Less commonly assessed markers including pS6, pGSH3B, FSH and IGF1 were downregulated while CD34, pAKT and SHBG were significantly upregulated. There were no significant changes in the other biomarkers measured such as phosphate and tensin homolog (PTEN), Bax and Bcl-2.WoO studies have been widely utilised within the ER+ breast cancer subtype, demonstrating their worth in pharmacodynamic research. However, research remains focused upon routinely measured biomarkers such ER PR and Ki67, with an array of less common markers sporadically used.
A novel medium manganese steel with composition Fe–8.3Mn–3.8Al–1.8Si–0.5C–0.06V–0.05Sn was developed and thermomechanically processed through hot rolling and intercritical annealing. The steel ...possessed a yield strength of 1 GPa, tensile strength of 1.13 GPa and ductility of 41 pct. In order to study the effect of cold rolling after intercritical annealing on subsequent tensile properties, the steel was further cold rolled up to 20 pct reduction. After cold rolling, it was observed that the strain hardening rate increased continuously with increasing cold rolling reduction but without a significant drop in ductility during subsequent tensile tests. The microstructural evolution with cold rolling reduction was analysed to understand the mechanisms behind this phenomena. It was found that cold rolling activated additional twinning systems which provided a large number of potent nucleation sites for strain induced martensite to form during subsequent tensile tests in what can be described as an enhanced TRIP effect.
Predicting the impending failure of hard disk drives (HDDs) is crucial for preventing essential data from losing. In this paper, a two-step parametric method was developed to predict the impending ...failure of HDDs using the aggregate of statistical models. This method deals with the problem of failure prediction in two steps: anomaly detection and failure prediction. First, Mahalanobis distance was used for aggregating all the monitored variables into one index, which was then transformed into Gaussian variables by Box-Cox transformation. By defining an appropriate threshold, anomalies in HDDs were detected as a result. Second, a sliding-window-based generalized likelihood ratio test was proposed to track the anomaly progression in an HDD. When the occurrence of anomalies in a time interval is found to be statistically significant, indicating the HDD is approaching failure. In this work, we also derived a new cost function to adjust the prediction rate. This is important in a way to balance the failure detection rate and false alarm rate as well as to provide an advanced warning of HDD failures to the users, whereby the users can back up their data in time. Then the developed method was applied on a synthetic data set showing its effectiveness on predicting failures. To demonstrate the practical usefulness, this method was also applied on a real-life HDD data set. The result shows that our method could achieve 68% failure detection rate with 0% false alarm rate. This is much better than the results achieved by the state-of-the-art methods, such as support vector machine and hidden Markov models.
Constipation is a common problem among advanced cancer patients; however, many of them find limited effective from current therapies. Thus, we aimed to test the effect of a traditional Chinese herbal ...formula, modified MaZiRenWan (MZRW), by comparing with placebo among palliative cancer patients with constipation.
This is a randomized, double-blind, placebo-controlled trial. Participants aged over 18 were recruited and randomized to MZRW or placebo group in addition to current prescriptions (including ongoing laxatives treatment) for two weeks. Exclusion criteria included cognitive impairment, presence of a colostomy or gastrointestinal obstruction and estimated life expectancy of less than one month. Individualized modification of MZRW was allowed according to the traditional Chinese medicine (TCM) pattern of patient. The primary outcome was the global assessment of improvement, which reflected whether the constipation had improved, remained the same or worsened.
Sixty patients, with mean age 75.2 years (range 47-95 years), were randomized to MZRW or placebo group. Among the MZRW group, 59.3% (16/27) had improvement in the global assessment score, as compared with 28.6% (8/28) of the placebo group (p-value = 0.022). Besides, the MZRW group had significant increase in stool frequency, and reduction in constipation severity and straining of defecation (p-value < 0.05). No serious adverse event was reported due to the research medication.
This pilot trial suggests modified MZRW is well-tolerated and effective for relief of constipation in patients with advance cancer. It could be considered as a potential treatment option for constipation in palliative care.

Building cooling load prediction is one of the key factors in the success of energy-saving measures. Many computational models available in the industry have been developed from either forward or ...inverse modeling approaches. However, these models usually require extensive computer resources and lengthy computation. This paper discusses the use of the multi-layer perceptron (MLP) model, one of the artificial neural network (ANN) models widely adopted in engineering applications, to estimate the cooling load of a building. The training samples used include weather data obtained from the Hong Kong Observatory and building-related data acquired from an existing prestigious commercial building in Hong Kong that houses a mega complex and operates 24
h a day. The paper also discusses the practical difficulties encountered in acquiring building-related data. In contrast to other studies that use ANN models to predict building cooling load, this paper includes the building occupancy rate as one of the input parameters used to determine building cooling load. The results demonstrate that the building occupancy rate plays a critical role in building cooling load prediction and significantly improves predictive accuracy.

N/A MEASUREMENTS AND RESULTS: Four hundred twenty-one depressed outpatients were recruited at baseline, and 371 patients (mean age 44.6 ± 10.4 yr, female 81.8%; response rate 88.1%) completed the reassessments, in which 41% were classified as remitted cases. One year prevalence of frequent insomnia at baseline and follow-up in remitted patients was 38.0% and 19.3%, respectively. One year prevalence of frequent nightmares at baseline and follow-up was 24.0% and 9.3%, respectively. Remitted patients with residual insomnia were more likely to be divorced (P < 0.05) and scored higher on the anxiety subscale (P < 0.05). Remitted patients with residual nightmares were younger (P < 0.05) and scored higher on neuroticism (P < 0.05) and anxiety subscales (P < 0.01). Residual insomnia and nightmares were associated with various aspects of impaired quality of life. Residual nightmares was associated with suicidal ideation (odds ratio = 8.40; 95% confidence interval 1.79-39.33).
Residual sleep disturbances, including insomnia and nightmares, were commonly reported in remitted depressed patients with impaired quality of life and suicidal ideation. A constellation of psychosocial and personality factors, baseline sleep disturbances, and comorbid anxiety symptoms may account for the residual sleep disturbances. Routine assessment and management of sleep symptoms are indicated in the integrated management of depression.
A hard disk drive (HDD) failure may cause serious data loss and catastrophic consequences. Online health monitoring provides information about the degradation trend of the HDD, and hence the early ...warning of failures, which gives us a chance to save the data. This paper developed an approach for HDD anomaly detection using Mahalanobis distance (MD). Critical parameters were selected using failure modes, mechanisms, and effects analysis (FMMEA), and the minimum redundancy maximum relevance (mRMR) method. A self-monitoring, analysis, and reporting technology (SMART) data set is used to evaluate the performance of the developed approach. The result shows that about 67% of the anomalies of failed drives can be detected with zero false alarm rate, and most of them can provide users with at least 20 hours during which to backup the data.
BACKGROUND: There are limited neuroprotective treatment options for patients with aneurysmal subarachnoid hemorrhage (SAH). Cerebrolysin, a brain-specific proposed pleiotropic neuroprotective agent, ...has been suggested to improve global functional outcomes in ischemic stroke. We investigated the efficacy, safety and feasibility of administering Cerebrolysin for SAH patients.
This was a prospective, randomized, double-blind, placebo-controlled, single-center, parallel-group pilot study. Fifty patients received either daily Cerebrolysin (30 ml/day) or a placebo (saline) for 14 days (25 patients per study group). The primary endpoint was a favorable Extended Glasgow Outcome Scale (GOSE) of 5 to 8 (moderate disability to good recovery) at six-months. Secondary endpoints included the modified Ranking Scale (mRS), the Montreal Cognitive Assessment (MOCA) score, occurrence of adverse effects and the occurrence of delayed cerebral ischemia (DCI).
No severe adverse effects or mortality attributable to Cerebrolysin were observed. No significant difference was detected in the proportion of patients with favorable six-month GOSE in either study group (odds ratio (OR): 1.49; 95% confidence interval (CI): 0.43-5.17). Secondary functional outcome measures for favorable six-month recovery i.e. a mRS of 0 to 3 (OR: 3.45; 95% CI 0.79-15.01) were comparable for both groups. Similarly, there was no difference in MOCA neurocognitive performance (p-value: 0.75) and in the incidence of DCI (OR: 0.85 95% CI: 0.28-2.59).
Use of Cerebrolysin in addition to standard-of-care management of aneurysmal SAH is safe, well tolerated and feasible. However, the neutral results of this trial suggest that it does not improve the six-month global functional performance of patients.
