Abstract
This is the first paper of a series exploring the multifrequency properties of a sample of 11 nearby low-excitation radio galaxies (LERGs) in the southern sky. We are conducting an extensive ...study of different galaxy components (stars, warm and cold gas, radio jets) with the aim of improving our understanding of the active galactic nucleus (AGN) fuelling/feedback cycle in LERGs. We present ALMA band 6 12CO(2–1) and continuum observations of nine sources. Continuum emission from the radio cores was detected in all objects. Six sources also show mm emission from jets on kpc/sub-kpc scales. The jet structures are very similar at mm and cm wavelengths. We conclude that synchrotron emission associated with the radio jets dominates the continuum spectra up to 230 GHz. The 12CO(2–1) line was detected in emission in six out of nine objects, with molecular gas masses ranging from 2 × 107 to 2 × 1010 M⊙. The CO detections show disc-like structures on scales from ≈0.2 to ≈10 kpc. In one case (NGC 3100) the CO disc presents some asymmetries and is disrupted in the direction of the northern radio jet, indicating a possible jet/disc interaction. In IC 4296, CO is detected in absorption against the radio core as well as in emission. In four of the six galaxies with CO detections, the gas rotation axes are roughly parallel to the radio jets in projection; the remaining two cases show large misalignments. In those objects where optical imaging is available, dust and CO appear to be co-spatial.
Abstract
In this paper we present steady-state RMHD simulations that include a mass-load term to study the process of jet deceleration. The mass-load mimics the injection of a proton-electron plasma ...from stellar winds within the host galaxy into initially pair plasma jets, with mean stellar mass-losses ranging from 10−14 to 10−9 M⊙ yr−1. The spatial jet evolution covers ∼500 pc from jet injection in the grid at 10 pc from the jet nozzle. Our simulations use a relativistic gas equation of state and a pressure profile for the ambient medium. We compare these simulations with previous dynamical simulations of relativistic, non-magnetised jets. Our results show that toroidal magnetic fields can prevent fast jet expansion and the subsequent embedding of further stars via magnetic tension. In this sense, magnetic fields avoid a runaway deceleration process. Furthermore, when the mass-load is large enough to increase the jet density and produce fast, differential jet expansion, the conversion of magnetic energy flux into kinetic energy flux (i.e., magnetic acceleration), helps to delay the deceleration process with respect to non-magnetised jets. We conclude that the typical stellar population in elliptical galaxies cannot explain jet deceleration in classical FRI radio galaxies. However, we observe a significant change in the jet composition, thermodynamical parameters and energy dissipation along its evolution, even for moderate values of the mass-load.

Here, we present a study of the molecular gas kinematics of six sample galaxies detected in 12CO(2-1) with the Atacama Large Millimeter/submillimeter Array (ALMA). In all cases, our modelling suggests that the bulk of the gas in the observed (sub-)kpc CO discs is in ordered rotation. Nevertheless, low-level distortions are ubiquitous, indicating that the molecular gas is not fully relaxed into the host galaxy potential. The majority of the discs, however, are only marginally resolved, preventing us from drawing strong conclusions. NGC 3557 and NGC 3100 are special cases. The features observed in the CO velocity curve of NGC 3557 allow us to estimate a supermassive black hole (SMBH) mass of (7.10 ± 0.02) × 108 M⊙, in agreement with expectations from the MSMBH–σ* relation. The rotation pattern of NGC 3100 shows distortions that appear to be consistent with the presence of both a position angle and an inclination warp. Non-negligible radial motions are also found in the plane of the CO disc, likely consistent with streaming motions associated with the spiral pattern found in the inner regions of the disc. The dominant radial motions are likely to be inflows, supporting a scenario in which the cold gas is contributing to the fuelling of the AGN.
Late diagnosis is an important cause of HIV-related morbidity, mortality and healthcare costs in the UK and undiagnosed infection limits efforts to reduce transmission. National guidelines provide ...recommendations to increase HIV testing in all healthcare settings. We evaluated progress towards these recommendations by comparing missed opportunities for HIV testing and late diagnosis in two six year cohorts from North East Scotland.
We reviewed diagnostic pathways of all patients newly diagnosed with HIV referred to infectious diseases and genito-urinary medicine services between 1995 and 2000 (n=48) and 2004 to 2009 (n=117). Missed presentations (failure to diagnose≤1 month of a clinical or non-clinical indicator for testing), late diagnosis (CD4<350 cells/mm3), and time to diagnosis (months from first presentation to diagnosis) were compared between cohorts using χ2 and log-rank tests. Determinants of missed presentation were explored by multivariate logistic regression. Breslow-Day tests assessed change in diagnostic performance by patient subgroup.
There were significant decreases in missed presentations (33% to 17%; P=0.02) and time to diagnosis (mean 17 months to 4 months; P=0.005) but not in late diagnosis (56% vs. 60%; P=0.57) between earlier and later cohorts. In the later cohort patients were significantly more likely to have acquired HIV abroad and presented with early HIV disease, and testing was more likely to be indicated by transmission risk or contact with GUM services than by clinical presentation. Missed presentation remained significantly less likely in the later cohort (OR=0.28, 95% CI 0.11 to 0.72; P=0.008) after adjustment for age, transmission risks and number of clinical indicators. Reductions in missed presentation were greater in patients<40 years, of non-UK origin, living in least deprived neighbourhoods and with early disease at presentation (P<0.05). 27% of missed presentations occurred in primary care and 46% in general secondary care.
While early diagnosis has improved in epidemiological risk groups, clinical indications for HIV testing continue to be missed, particularly in patients who are older, of UK origin and from more deprived communities. Increasing testing in non-specialist services is a priority.

ABSTRACT
This is the fourth paper of a series investigating the AGN fuelling/feedback processes in a sample of 11 nearby low-excitation radio galaxies (LERGs). In this paper, we present follow-up ...Atacama Large Millimeter/submillimeter Array (ALMA) observations of one source, NGC 3100, targeting the 12CO(1-0), 12CO(3-2), HCO+(4-3), SiO(3-2), and HNCO(6-5) molecular transitions. 12CO(1-0) and 12CO(3-2) lines are nicely detected and complement our previous 12CO(2-1) data. By comparing the relative strength of these three CO transitions, we find extreme gas excitation conditions (i.e. Tex ≳ 50 K) in regions that are spatially correlated with the radio lobes, supporting the case for a jet–ISM interaction. An accurate study of the CO kinematics demonstrates that although the bulk of the gas is regularly rotating, two distinct non-rotational kinematic components can be identified in the inner gas regions: one can be associated to inflow/outflow streaming motions induced by a two-armed spiral perturbation; the second one is consistent with a jet-induced outflow with vmax ≈ 200 km s−1 and $\dot{M}\lesssim 0.12$ M⊙ yr−1. These values indicate that the jet-CO coupling ongoing in NGC 3100 is only mildly affecting the gas kinematics, as opposed to what expected from existing simulations and other observational studies of (sub-)kpc scale jet–cold gas interactions. HCO+(4-3) emission is tentatively detected in a small area adjacent to the base of the northern radio lobe, possibly tracing a region of jet-induced gas compression. The SiO(3-2) and HNCO(6-5) shock tracers are undetected: this – along with the tentative HCO+(4-3) detection – may be consistent with a deficiency of very dense (i.e. ncrit > 106 cm−3) cold gas in the central regions of NGC 3100.

We are conducting an ...extensive study of different galaxy components (stars, dust, warm and cold gas, radio jets) with the aim of better understanding the AGN fuelling/feedback cycle in LERGs. Here, we present new, deep, sub-kpc resolution Karl G. Jansky Very Large Array (JVLA) data for five sample sources at 10 GHz. Coupling these data with previously acquired Atacama Large Millimetre/submillimetre Array (ALMA) CO(2–1) observations and measurements of comparable quality from the literature, we carry out for the first time a full 3D analysis of the relative orientations of jet and disc rotation axes in six FR I LERGs. This analysis shows (albeit with significant uncertainties) that the relative orientation angles span a wide range (≈30○–60○). There is no case where both axes are accurately aligned and there is a marginally significant tendency for jets to avoid the disc plane. Our study also provides further evidence for the presence of a jet-CO disc interaction (already inferred from other observational indicators) in at least one source, NGC 3100. In this case, the limited extent of the radio jets, along with distortions in both the molecular gas and the jet components, suggest that the jets are young, interacting with the surrounding matter and rapidly decelerating.

Databases were searched in July 2021 (16th-21st), ...including Ovid's Medline, Embase, Emcare and PsycINFO; Scopus; Web of Science Core Collection, the Directory of Open Access Journals; Informit and; ProQuest Dissertations and Theses Global.
Eligible articles included peer-reviewed literature documenting journey mapping methodologies and studies conducted in healthcare services. Reviewers used Covidence to screen titles and abstracts of located sources, and to screen full-text articles. A table was used to extract data and synthesize results.
Eighty-one articles were included. An acceleration of patient journey mapping research was observed, with 76.5% (n = 62) of articles published since 2015. Diverse mapping approaches were identified. Reporting of studies was inconsistent and largely non-adherent with relevant, established reporting guidelines.
Patient journey mapping is a relatively novel approach for understanding patient experiences and is increasingly being adopted. There is variation in process details reported. Considerations for improving reporting standards are provided.
Patient journey mapping is a rapidly growing approach for better understanding how people enter, experience and exit health services. This type of methodology has significant potential to inform new, patient centred models of care and facilitate clinicians, patients and health professionals to better understand gaps and strategies in health services. The synthesised results of this review alert researchers to options available for journey mapping research and provide preliminary guidance for elevating reporting quality.
We use 2.0 Msec of Chandra observations to investigate the cocoon shocks of Cygnus A and some implications for its lobes and jet. Measured shock Mach numbers vary in the range 1.18-1.66 around the ...cocoon. We estimate a total outburst energy of , with an age of . The average postshock pressure is found to be , which agrees with the average pressure of the thin rim of compressed gas between the radio lobes and shocks, as determined from X-ray spectra. However, average rim pressures are found to be lower in the western lobe than in the eastern lobe by 20%. Pressure estimates for hotspots A and D from synchrotron self-Compton models imply that each jet exerts a ram pressure 3 times its static pressure, consistent with the positions of the hotspots moving about on the cocoon shock over time. A steady, one-dimensional flow model is used to estimate jet properties, finding mildly relativistic flow speeds within the allowed parameter range. Models in which the jet carries a negligible flux of rest mass are consistent with the observed properties of the jets and hotspots. This favors the jets being light, implying that the kinetic power and momentum flux are carried primarily by the internal energy of the jet plasma rather than by its rest mass.
Summary Background Adjuvant chemotherapy for early breast cancer has improved outcomes but causes toxicity. The UK TACT2 trial used a 2×2 factorial design to test two hypotheses: whether use of ...accelerated epirubicin would improve time to tumour recurrence (TTR); and whether use of oral capecitabine instead of cyclophosphamide would be non-inferior in terms of patients' outcomes and would improve toxicity, quality of life, or both. Methods In this multicentre, phase 3, randomised, controlled trial, we enrolled patients aged 18 years or older from 129 UK centres who had histologically confirmed node-positive or high-risk node-negative operable breast cancer, had undergone complete excision, and were due to receive adjuvant chemotherapy. Patients were randomly assigned to receive four cycles of 100 mg/m2 epirubicin either every 3 weeks (standard epirubicin) or every 2 weeks with 6 mg pegfilgrastim on day 2 of each cycle (accelerated epirubicin), followed by four 4-week cycles of either classic cyclophosphamide, methotrexate, and fluorouracil (CMF; 600 mg/m2 cyclophosphamide intravenously on days 1 and 8 or 100 mg/m2 orally on days 1–14; 40 mg/m2 methotrexate intravenously on days 1 and 8; and 600 mg/m2 fluorouracil intravenously on days 1 and 8 of each cycle) or four 3-week cycles of 2500 mg/m2 capecitabine (1250 mg/m2 given twice daily on days 1–14 of each cycle). The randomisation schedule was computer generated in random permuted blocks, stratified by centre, number of nodes involved (none vs one to three vs four or more), age (≤50 years vs >50 years), and planned endocrine treatment (yes vs no). The primary endpoint was TTR, defined as time from randomisation to first invasive relapse or breast cancer death, with intention-to-treat analysis of standard versus accelerated epirubicin and per-protocol analysis of CMF versus capecitabine. This trial is registered with ISRCTN, number 68068041, and with ClinicalTrials.gov , number NCT00301925. Findings From Dec 16, 2005, to Dec 5, 2008, 4391 patients (4371 women and 20 men) were recruited. At a median follow-up of 85·6 months (IQR 80·6–95·9) no significant difference was seen in the proportions of patients free from TTR events between the accelerated and standard epirubicin groups (overall hazard ratio HR 0·94, 95% CI 0·81–1·09; stratified p=0·42). At 5 years, 85·9% (95% CI 84·3–87·3) of patients receiving standard epirubicin and 87·1% (85·6–88·4) of those receiving accelerated epirubicin were free from TTR events. 4358 patients were included in the per-protocol analysis, and no difference was seen in the proportions of patients free from TTR events between the CMF and capecitabine groups (HR 0·98, 95% CI 0·85–1.14; stratified p=0·00092 for non-inferiority). Compared with baseline, significantly more patients taking CMF than those taking capecitabine had clinically relevant worsening of quality of life at end of treatment (255 58% of 441 vs 235 50% of 475; p=0·011) and at 12 months (114 34% of 334 vs 89 22% of 401; p<0·001 at 12 months) and had worse quality of life over time (p<0·0001). Detailed toxicity and quality-of-life data were collected from 2115 (48%) of treated patients. The most common grade 3 or higher adverse events in cycles 1–4 were neutropenia (175 16%) and fatigue (56 5%) of the 1070 patients treated with standard epirubicin, and fatigue (63 6%) and infection (34 3%) of the 1045 patients treated with accelerated epirubicin. In cycles 5–8, the most common grade 3 or higher adverse events were neutropenia (321 31%) and fatigue (109 11%) in the patients treated with CMF, and hand-foot syndrome (129 12%) and diarrhoea (67 6%) in the 1044 patients treated with capcitabine. Interpretation We found no benefit from increasing the dose density of the anthracycline component of chemotherapy. However, capecitabine could be used in place of CMF without significant loss of efficacy and with improved quality of life. Funding Cancer Research UK, Amgen, Pfizer, and Roche.
