Dental trauma is a significant public health problem because of its frequency, impact on economic productivity and quality of life. It is not a disease and no individual is ever at zero risk of ...sustaining these potentially life‐changing injuries. The aim of this article was to review the literature on the prevalence, incidence, aetiology, prognosis and outcomes of dental trauma. The importance of standardized reporting, oral health policy, adjunctive research methods, prevention and education will also be discussed. A search for relevant articles appearing in databases such as Medline, Cochrane and SSCI formed the basis of this review. Epidemiological studies indicate the annual incidence of dental trauma globally is at about 4.5%. Approximately one‐third of children and toddlers (primary teeth) and one‐fifth of adolescents and adults (permanent teeth) sustained a traumatic dental injury. The majority involved the maxillary central incisors, mainly from falls in toddlers at home and contact sport in adolescents. Despite these trends, there is considerable variation between studies within and across jurisdictions. There is a need to standardize research with a consistent approach to reporting, classification and methodology. This will improve research and form a greater basis for predicting prognosis. This research basis will assist in consent and clinical management.

Orally administered silver has been described to be absorbed in a range of 0.4–18% in mammals with a human value of 18%. Based on findings in animals, silver seems to be distributed to all of the organs investigated, with the highest levels being observed in the intestine and stomach. In the skin, silver induces a blue–grey discoloration termed argyria. Excretion occurs via the bile and urine. The following dose-dependent animal toxicity findings have been reported: death, weight loss, hypoactivity, altered neurotransmitter levels, altered liver enzymes, altered blood values, enlarged hearts and immunological effects. Substantial evidence exists suggesting that the effects induced by particulate silver are mediated via silver ions that are released from the particle surface. With the current data regarding toxicity and average human dietary exposure, a Margin of Safety calculation indicates at least a factor of five before a level of concern to the general population is reached.
Planktic foraminiferal biostratigraphic zonation schemes are critical for providing first-order relative age control in deep-sea sediments and provide the basis on which to interpret evolutionary ...dynamics through time. Over the previous decades, the majority of published biostratigraphic zonation schemes focused on the tropical regions of the world. The mid-latitude or temperate regions, especially of the northwest Pacific, have been understudied in terms of recording plankton occurrences. Lack of detailed biostratigraphic studies have largely left out this region from plankton evolutionary analyses, thus how this part of the world ocean, which is characterized by the Kuroshio Current Extension (KCE), may contribute to global plankton biodiversity is unknown. In this study, we present the first magnetostratigraphically-calibrated late Neogene to Quaternary (15.12-0 Ma) planktic foraminiferal zonation schemes from the northwest Pacific for three Ocean Drilling Program Leg 198 holes (1207A, 1208A, and 1209A) that span the KCE. We utilize previously published warm subtropical, cool subtropical, and temperate zonation schemes from the southwest Pacific, with modifications. We find examples of significant diachroneity among primary marker taxa used to construct biozones at the three northwest Pacific sites, which ranges from 0.075 to 2.29 million years. Comparison of our primary datum markers with those of the tropical planktic foraminiferal zonation scheme also reveal diachroneity on the scale of 0.022 to 4.8 million years. We have identified times of intense dissolution in the northwest Pacific, namely in the middle to late Miocene that likely contribute to the observed diachroneity of datums. This study highlights the need for regionally specific mid-latitude biostratigraphic zonation schemes, as diachronous datums and differing assemblages may be hallmarks of oceanic ecotones created by major boundary current systems. These data also provide a framework to characterize local plankton evolutionary dynamics and paleobiogeographic patterns in future studies.

There is growing interest in glutamatergic agents in depression, particularly ketamine, a glutamate N-methyl-d-aspartate (NMDA) receptor antagonist. We aimed to assess the efficacy of ketamine in ...major depressive episodes.
We searched EMBASE, PsycINFO, CENTRAL, and Medline from 1962 to January 2014 to identify double-blind, randomized controlled trials with allocation concealment evaluating ketamine in major depressive episodes. Clinical remission, response and depressive symptoms were extracted by two independent raters. The primary outcome measure was clinical remission at 24 h, 3 days and 7 days post-treatment. Analyses employed a random-effects model.
Data were synthesized from seven RCTs employing an intravenous infusion and one RCT employing intranasal ketamine, representing 73 subjects in parallel arms and 110 subjects in cross-over designs n = 34 with bipolar disorder (BD), n = 149 with major depressive disorder (MDD). Ketamine was associated with higher rates of clinical remission relative to comparator (saline or midazolam) at 24 h OR 7.06, number needed to treat (NNT) = 5, 3 days (OR 3.86, NNT = 6), and 7 days (OR 4.00, NNT = 6), as well as higher rates of clinical response at 24 h (OR 9.10, NNT = 3), 3 days (OR 6.77, NNT = 3), and 7 days (OR 4.87, NNT = 4). A standardized mean difference of 0.90 in favor of ketamine was observed at 24 h based on depression rating scale scores, with group comparisons revealing greater efficacy in unipolar depression compared to bipolar depression (1.07 v. 0.68). Ketamine was associated with transient psychotomimetic effects, but no persistent psychosis or affective switches.
Our meta-analysis suggests that single administrations ketamine are efficacious in the rapid treatment of unipolar and bipolar depression. Additional research is required to determine optimal dosing schedules, route, treatment schedules, and the potential efficacy of other glutamatergic agents.
The rescue effect describes the phenomenon where irradiated cells or organisms derive benefits from the feedback signals sent from the bystander unirradiated cells or organisms. An example of the ...benefit is the mitigation of radiation-induced DNA damages in the irradiated cells. The rescue effect can compromise the efficacy of radioimmunotherapy (RIT) (and actually all radiotherapy). In this paper, the discovery and subsequent confirmation studies on the rescue effect were reviewed. The mechanisms and the chemical messengers responsible for the rescue effect studied to date were summarized. The rescue effect between irradiated and bystander unirradiated zebrafish embryos in vivo sharing the same medium was also described. In the discussion section, the mechanism proposed for the rescue effect involving activation of the nuclear factor κB (NF-κB) pathway was scrutinized. This mechanism could explain the promotion of cellular survival and correct repair of DNA damage, dependence on cyclic adenosine monophosphate (cAMP) and modulation of intracellular reactive oxygen species (ROS) level in irradiated cells. Exploitation of the NF-κB pathway to improve the effectiveness of RIT was proposed. Finally, the possibility of using zebrafish embryos as the model to study the efficacy of RIT in treating solid tumors was also discussed.
The aim of this review is to describe the challenges and barriers to conducting research in long-term care facilities.
A literature search was conducted in Ovid MEDLINE, Embase, Cochrane Central, ...PsycINFO and CINAHL. Keywords used included "long term care", "nursing home", "research", "trial", "challenge" and "barrier", etc. Resulting references were screened in order to identify relevant studies that reported on challenges derived from first-hand experience of empirical research studies. Challenges were summarized and synthesized.
Of 1723 references, 39 articles were selected for inclusion. To facilitate understanding we proposed a classification framework of 8 main themes to categorize the research challenges presented in the 39 studies, relating to the characteristics of facility/owner/administrator, resident, staff caregiver, family caregiver, investigator, ethical or legal concerns, methodology, and budgetary considerations.
Conducting research in long-term care facilities is full of challenges which can be categorized into 8 main themes. Investigators should be aware of all these challenges and specifically address them when planning their studies. Stakeholders should be involved from an early stage and flexibility should be built into both the methodology and research budget.
The osteogenic effect of a composite electrospun core-shell nanofiber membrane encapsulated with Emdogain® (EMD) was evaluated. The membrane was developed through coaxial electrospinning using ...polycaprolactone as the shell and polyethylene glycol as the core. The effects of the membrane on the osteogenic differentiation of periodontal ligament stem cells (PDLSCs) were examined using Alizarin Red S staining and qRT-PCR. Characterization of the nanofiber membrane demonstrated core-shell morphology with a mean diameter of ~1 µm. Examination of the release of fluorescein isothiocyanate-conjugated bovine serum albumin (FITC-BSA) from core-shell nanofibers over a 22-day period showed improved release profile of encapsulated proteins as compared to solid nanofibers. When cultured on EMD-containing core-shell nanofibers, PDLSCs showed significantly improved osteogenic differentiation with increased Alizarin Red S staining and enhanced osteogenic gene expression, namely OCN, RUNX2, ALP, and OPN. Core-shell nanofiber membranes may improve outcomes in periodontal regenerative therapy through simultaneous mechanical barrier and controlled drug delivery function.
Transforming an initial quantum state into a target state through the fastest possible route—a quantum brachistochrone—is a fundamental challenge for many technologies based on quantum mechanics. In ...two-level systems, the quantum brachistochrone solutions are long known. These solutions, however, are not applicable to larger systems, especially when the target state cannot be reached through a local transformation. Here, we demonstrate fast coherent transport of an atomic wave packet over a distance of 15 times its size—a paradigmatic case of quantum processes going beyond the two-level system. Our measurements of the transport fidelity reveal the existence of a minimum duration—a quantum speed limit—for the coherent splitting and recombination of matter waves. We obtain physical insight into this limit by relying on a geometric interpretation of quantum state dynamics. These results shed light on a fundamental limit of quantum state dynamics and are expected to find relevant applications in quantum sensing and quantum computing.
