Any modern society should be concerned over the well-being of the visually impaired citizen. It is the responsibility of the society to lessen the inconvenience and anxiety experienced by the ...handicapped. In addition to helping one to avoid obstacles on the sidewalks, technology can further help in navigating to places. In this paper we attempt to create a supportive environment with timely and useful information to guide the visually impaired to comfortably roam in a city that cares.
Intraneuronal deposition containing alpha-synuclein is implicated in the pathogenesis of synuclein-opathies including Parkinsons disease (PD). Although it has been demonstrated that cytoplasmic ...inclusions of wild type alpha-synuclein are observed in the brain of PD patients and that alpha-synuclein mutations such as A30P and A53T accelerate aggregate formation, the exact mechanism by which alpha-synuclein forms insoluble aggregates is still controversial. In the present study, to understand the possible involvement of tissue transglutaminase (tTG) in aggregate formation of alpha-synuclein, SH-SY5Y cell lines stably expressing wild type or mutant (A30P or A53T) alpha-synuclein were created and aggregate formation of alpha-synuclein was observed upon activation of tTG. The data demonstrated that alpha-synuclein negligibly interacted with tTG and that activation of tTG did not result in the aggregate formation of alpha-synuclein in SH-SY5Y cells overexpressing either wild type or mutant alpha-synuclein. In addition, alpha-synuclein was not modified by activated tTG in situ. These data suggest that tTG is unlikely to be a contributing factor to the formation of aggregates of alpha-synuclein in a stable cell model.

BACKGROUND AND OBJECTIVES: The main causes of ...failure after allogeneic hematopoietic stem cell transplantation (HSCT) in patients with severe aplastic anemia (SAA) are graft-versus-host disease (GVHD), infection and graft failure, often exacerbated by large numbers of transfusions and prolonged disease duration before transplant. This study retrospectively analyzes the outcome and factors related to survival or graft failure in high-risk patients with SAA receiving HSCT in our institution. DESIGN AND METHODS: Between January 1995 and December 1999, 40 consecutive adult patients who were multi-transfused (more than 40 units of red blood cells +/- platelets) and/or had a 3 years or longer period prior to transplant were enrolled. Their median age was 27.5 years (range, 16 to 43) and 21 (52.5%) were women. All donors were human leukocyte antigen (HLA)-matched siblings. Before transplant, 29 patients (72.5%) received a course of antithymocyte globulin (ATG) and cyclosporin A (CsA). The median interval from diagnosis to transplant was 59 months (range, 2 to 216). The median number of transfusions was 115 units (range, 10 to 480). All patients received a conditioning regimen of cyclophosphamide, ATG, and procarbazine. Our patients received either bone marrow (BM) alone (n=20) or BM+peripheral blood stem cells (PBSC) (n=20) as a stem cell source. T-cells of PBSC were depleted using the CD34 enrichment method. GVHD prophylaxis consisted of CsA and short-term methotrexate. RESULTS: In the BM+PBSC group, neutrophil recovery to 0.5 x 10(9)/L and platelet recovery to 20 x 10(9)/L were achieved more rapidly than in the BM group (p=0.005 and 0.039, respectively). The incidences of graft failure, grade II to IV acute GVHD, and chronic GVHD were 22.5%, 12.8% and 23.1%, respectively. Graft failure occurred in 2 of 20 patients (10%) receiving BM+PBSC and in 7 of 20 (35%) receiving BM alone (p=0.069). Seven of 9 patients who had graft failure received a booster treatment and recovered normal marrow function. GVHD incidence was comparable between the BM+PBSC and BM groups. Six patients (15%) died from graft failure (n=2), interstitial pneumonia (n=2), cyclophosphamide-induced heart failure (n=1), and chronic GVHD followed by pneumonia (n=1). The Kaplan-Meier estimate of survival was 83.7% with a median follow-up duration of 40.5 months (range 8-67). In multivariate analysis only chronic GVHD adversely influenced survival (p=0.042). INTERPRETATION AND CONCLUSIONS: These results suggest that HSCT is an effective treatment for multi-transfused SAA patients with prolonged disease duration. It is highly possible that the infusion of a large number of stem cells leads to a reduction of graft failure and a faster speed of engraftment. Booster treatment is successful in achieving engraftment in patients with graft failure.
To investigate the types and quantities of beverages that increase urinary hippuric acid (HA) excretion, the authors recruited 137 healthy medical students and divided them into quintiles according ...to consumption of benzoic acid (BA) in beverages. Using chromatography, urinary HA before and 1.5 hours and 3 hours after consumption of various beverages and BA consumption in the beverages were measured. The mean age of the group was 24.2 years; 21 were female. The range of BAin 13 beverages was 0-1.02 mg/mL. The geometric means of urinary HA before consuming them in five groups were 0.276, 0.270, 0.207, 0.262, and 0.316 g/L, respectively(P = 0.567); 1.5 hours after consuming the beverages, they were 0.210,0.603, 1.026, 1.066, and 1.688 g/L, respectively, and significantly increased after adjustment for urinary HA before ingestion (P < 0.001). Three hours after ingestion, the geometric means were 0.160, 0.232, 0.306, 0.287, and 0.337 g/L, respectively (P < 0.001). The authors estimate that beverages that contain more than 100 mg BA could increase urinary HA excretion significantly, up to 1.12 g/L without toluene exposure. Checking dietary and beverage-intake histories is essential in the interpretation of urinary HA concentrations in population studies.
Though the surgical treatment for medial temporal lobe epilepsy yields a high success rate, more studies are needed in order to determine the most efficacious pre-operative algorithm. The authors ...studied the relationship between surgical outcome and the localization results of various pre-operative diagnostic tests to assess the predictive value. Seventy-one consecutive patients who had undergone anterior temporal lobectomy with amygdalohippocampectomy with the diagnosis of non-lesional medial temporal lobe epilepsy, who had been followed up more than 24 months, were analyzed retrospectively. Electroencephalogy (EEG), magnetic resonance imaging (MRI), proton emission tomography (PET), single photon emission computed tomography (SPECT), the Wada test, and neuropsychological testing were analyzed. There was no diagnostic test that was found to have a statistically significant relationship between Engel Class I outcome and localization results (P& 0.05). SPECT, neuropsychological testing, and the Wada test all had less predictive values (P< 0.01). EEG and PET had comparable predictive values for Engel Class I with MRI (P& 0.05). No single diagnostic test alone is sufficient to make a diagnosis of non-lesional medial temporal lobe epilepsy. MRI, EEG and PET had comparable predictive values for Engel Class I. SPECT, neuropsychological testing, and the Wada test had less predictive values.
Acute pancreatitis in cancer patients can be secondary to the malignant process itself or a complication of antineoplastic agent administration. However, acute pancreatitis caused by metastatic ...carcinoma of the pancreas is an uncommon condition with a poor prognosis. We report a case of a 63-year-old man with small cell carcinoma of the lung, who developed acute pancreatitis lately. Thirteen months earlier, he developed small cell carcinoma of the lung and received 6 cycles of chemotherapy. Abdominal CT scan showed swelling of the pancreas with multiple masses. The patient was managed conservatively and pancreatitis subsided. This case indicates that metastasis induced acute pancreatitis can be a manifestation of lung cancer, especially in small cell carcinoma.
Three members of a Taiwanese kindred developed severe, systemic, early onset (<age 25 years), biopsy‐proven amyloidosis. Clinical features included upper and lower extremity sensorimotor neuropathy, ...abdominal pain, vomiting, corneal ulcerations, cardiomyopathy, and syncope. Immunohistochemical analysis indicated that the deposits consisted of transthyretin. Molecular genetic studies revealed a heterozygous codon 55 point mutation, resulting in a proline for leucine substitution, a mutation previously associated with aggressive familial amyloidosis in a US kindred of Dutch and German descent. The clinical courses and echocardiographic findings are typical for many types of amyloidosis; the pathologic data and genetic studies were necessary to establish a precise diagnosis.
