The voltage measurement was recorded with various configurations of temporary grounds. One of configurations is considered lethal. The results indicate that two of the configurations are effective means of protecting distribution line workers.
Metabolic syndrome (MetS) is associated with increased risk for diabetes and coronary heart disease. Data suggest that MetS starts even in children. Thus, it is important to understand the role of ...MetS and the risks related to it. Furthermore, white blood cell count (WBCC) is available in routine examination and it has been proved to be related to risks of MetS. A total of 1657 subjects aged 14-19 years were enrolled, with normal WBCC (less than or equal to10 x 10superscript 9 cells/L). The subjects were divided into four quartiles according to WBCC (WBCC1-4, from the lowest to highest WBCC) in both genders. The female subjects had significantly lower systolic blood pressure, fasting plasma glucose (FPG), triglyceride (TG), body mass index (BMI) and higher high-density lipoprotein-cholesterol (HDL-C) than the male subjects. When comparing the risks of the quartiles of WBCC, WBCC4 had significant higher BMI than WBCC1 and WBCC2 in both genders; and lower HDL-C than WBCC3 in male subjects. On multiple regression WBCC was positively related with BMI and TG and negatively related to HDL-C in male subjects. In female subjects only BMI was positively correlated with WBCC. Subjects with MetS did not have significant higher WBCC than those without MetS. In adolescents with normal WBCC, BMI is significantly related to the levels of WBCC and is the earliest component of MetS to be noted in adolescents. Elevated TG and decreased HDL-C could also be important markers for future risk factors only in male subjects. PUBLICATION ABSTRACT
저자 등은 왼쪽 골반뼈통증 및 보행장애로 내원한 환자에서 자기공명영상상 형질세포종 혹은 골전이 의심되어 정형외과에서 다양한 검사를 시행하였으나 초기에 환자의 칼슘증가 수치는 악성 골전이에 의한 것으로 의심되었으나, 조직검사 결과 갈색종 의심된다는 보고와 함께 혈청 칼슘 재측정 및 혈청 부갑상선 호르몬을 측정하여 부갑상선 선종으로 인한 원발성 부갑상선 ...기능항진증으로 진단되었다. 부갑상선 절제술 후 치료 경과를 관찰한 결과 골밀도의 증가 및 보행장애가 해소된 증례를 경험하였기에 보고하는 바이다. 갈색종은 크기가 크다 하더라도 부갑상선 절제술만으로도 급격히 호전을 보이는 질환임을 염두에 두어 골 용해 병변으로 방문한 환자에게서 혈청 칼슘 증가 시 불필요한 수술이나 시술을 피할 수 있도록 부갑상선 기능항진증을 초기 감별 진단 시 고려해 보아야 할 것으로 사료된다.
Primary hyperparathyroidism is mainly caused by parathyroid adenoma (85%) and is characterized by hypercalcemia, osteoporosis, renal stones, and gastrointestinal and neurological disorders. Because of improvements in blood analysis over the last two decades, primary hyperparathyroidism is typically diagnosed early and asymptomatically. A rare clinical manifestations of primary hyperparathyroidism, brown tumors (osteitis fibrosa cystica), are osteolytic lesions resulting from long-term hyperparathyroidism. Radiologically, it is difficult to distinguish a brown tumor from plasmacytoma, multiple myeloma, or bone metastasis. We report a case of a 44-year-old man with primary hyperparathyroidism that caused a large brown tumor (11×5×8cm) that mimicked plasmacytoma or cancer metastasis on pelvic magnetic resonance imaging. After a bone biopsy report that was highly suggestive of a brown tumor, serum calcium and intact parathyroid hormone levels were determined. The lesion was ultimately diagnosed as a brown tumor and a parathyroidectomy was performed. After 1 year, the lesion has nearly regressed by follow up of the anteroposterior view of the pelvis and bone mineral density has improved. The present case highlights the importance of considering brown tumors in the evaluation of patients presenting with hypercalcemia and osteolytic lesions without definite primary neoplasm. The impact the metabolic syndrome (MetS) components on the severity of insulin
resistance (IR) has not been reported. We enrolled 564 subjects with MetS and
they were divided into quartiles according ...to the level of each component; and an
insulin suppression test was performed to measure IR. In males, steady state plasma
glucose (SSPG) levels in the highest quartiles, corresponding to body mass
index (BMI) and fasting plasma glucose (FPG), were higher than the other three
quartiles and the highest quartiles, corresponding to the diastolic blood pressure
and triglycerides, were higher than in the lowest two quartiles. In females, SSPG
levels in the highest quartiles, corresponding to the BMI and triglycerides, were higher
than in all other quartiles. No significant differences existed between genders, other
than the mean SSPG levels in males were greater in the highest quartile corresponding
to BMI than that in the highest quartile corresponding to HDL-cholesterol levels.
The factor analysis identified two underlying factors (IR and blood pressure factors)
among the MetS variables. The clustering of the SSPG, BMI, triglyceride and HDLcholesterol
was noted. Our data suggest that adiposity, higher FPG and triglyceride
levels have stronger correlation with IR and subjects with the highest BMI have the

CA 125의 상승은 난소의 악성종양에 나타날 수 있지만 비특이적이다. CA 125의 상승은 다양한 양성 질환 혹은 악성 종양에서 나타날 수 있다. 저자들은 최근 지속적으로 혈중 CA 125 수치가 증가되어 있지만 경질 초음파, 흉부 CT, PET-CT 등에서 악성 종양이 발견되지 않은 기관지확장증 1예를 경험하였다. 14개월의 추적 관찰 기간 중에도 ...난소암과 같은 악성 종양은 발견되지 않고 기관지확장증에 병발된 폐렴 이외에는 특이 소견이 없어, 혈중 CA 125의 상승은 기관지확장증에 의한 급성 폐 손상에 의한 것으로 추정된다. 혈중 CA 125 수치의 상승은 환자의 임상 양상과 연관하여 조심스럽게 판단하여야 될 것으로 생각된다.

There was no evidence of underlying malignant disease on positron emission tomography or on gynecologic examination, including transvaginal ultrasonography. During follow-up for 14 months, we could not find any clue of malignant disease that could have been the cause of the elevated levels of serum CA 125. Elevated serum CA 125 level should be interpreted carefully according to the patient`s clinical condition. In addition, our case suggests that CA 125 may be used as a surrogate marker for acute inflammatory status for chronic pulmonary diseases.

2 to receive ROS 4 mg daily for 12 weeks. Fifteen age-, sex-, and body mass index–matched healthy subjects were recruited as controls. A 75-g oral glucose tolerance test (OGTT), hemoglobin A
1c, fasting glucose, insulin, C-peptide, lipid profiles, adiponectin, and CRP levels were determined before initiation and at the end of the 12-week ROS treatment. Insulin resistance and beta-cell function were calculated using the homeostasis model assessment method (HOMA-IR and HOMA-
β, respectively). Compared with healthy controls, the ROS-treated subjects had significantly higher glycemic indices, HOMA-IR, CRP, and glucose and insulin concentrations in response to OGTT, and lower HOMA-
β level. After 12 weeks of ROS therapy, the results showed statistically significant changes from baseline in 2-hour plasma glucose during OGTT (9.4 ± 0.3 vs 8.3 ± 0.4 mmol/L,
P < .05), HOMA-IR (2.6 ± 0.2 vs 1.9 ± 0.3,
P < .05), HOMA-
β (63.4 ± 12.5 vs 90.1 ± 13.0,
P < .05), and glucose and insulin concentrations during OGTT in nonobese subjects with IGT. In addition, elevation of serum adiponectin and decrease in CRP levels were significantly found after ROS treatment. Of 21 patients treated with ROS, 5 subjects were converted to normal (converter), 1 progressed to diabetes, and 15 remained in IGT status (nonconverter). There was a significant amelioration in HOMA-IR (−2.10 ± 1.03 vs −0.07 ± 0.33,
P < .05) without significant changes in adiponectin and CRP levels in converter compared with nonconverter. We conclude that ROS effectively enhanced insulin sensitivity and beta-cell function to improve adiponectin and CRP levels in nonobese patients with IGT. The amelioration of insulin resistance may be a major determinant to predict the conversion of IGT independent of the changes in adiponectin and CRP.
