Abstract
Background
Acute kidney injury is a common complication in solid organ transplants, notably liver transplantation. The MELD is a score validated to predict mortality of cirrhotic patients, ...which is also used for organ allocation, however the influence of this allocation criteria on AKI incidence and mortality after liver transplantation is still uncertain.
Methods
This is a retrospective single center study of a cohort of patients submitted to liver transplant in a tertiary Brazilian hospital: Jan/2002 to Dec/2013, divided in two groups, before and after MELD implementation (pre-MELD and post MELD). We evaluate the differences in AKI based on KDIGO stages and mortality rates between the two groups.
Results
Eight hundred seventy-four patients were included, 408 in pre-MELD and 466 in the post MELD era. The proportion of patients that developed AKI was lower in the post MELD era (
p
0.04), although renal replacement therapy requirement was more frequent in this group (
p
< 0.01). Overall mortality rate at 28, 90 and 365 days was respectively 7%, 11% and 15%. The 1-year mortality rate was lower in the post MELD era (20% vs. 11%,
p
< 0.01). AKI incidence was 50% lower in the post MELD era even when adjusted for clinically relevant covariates (
p
< 0.01).
Conclusion
Liver transplants performed in the post MELD era had a lower incidence of AKI, although there were more cases requiring dialysis. 1-year mortality was lower in the post MELD era, suggesting that patient care was improved during this period.
Objetivo: analisar o conhecimento dos docentes do curso de Graduação Enfermagem acerca do tema doação de órgãos e tecidos para transplante. Metodologia: pesquisa descritiva, exploratória, com ...abordagem qualitativa realizada no período de abril a maio de 2016. Do total de vinte e sete docentes enfermeiros de uma Instituição de Ensino Superior pública de Belém, Estado do Pará, foram entrevistados dezesseis. Utilizamos como técnica de coleta de dados entrevistas semiestruturadas, tendo como instrumento para a coleta um roteiro de entrevista elaborado com perguntas abertas e para análise o referencial metodológico proposto por Bardin. Resultados: constatamos que a maioria dos docentes não possui conhecimentos sólidos sobre o tema em questão, pautando sua retórica em experiências pessoais e com baixo teor científico. Contudo, os mesmos percebem a importância e complexidade do tema. Conclusão: há insuficiência no conhecimento dos docentes no que concerne ao processo de doação como um todo. Tal realidade evidencia a necessidade e a importância de maiores investimentos em cursos de atualização voltados para docentes do ensino superior.
Objetivo: Analisar a percepção e o posicionamento dos acadêmicos de enfermagem sobre doação de órgãos e tecidos para transplante, antes e após ação educativa. Métodos: Pesquisa descritiva, ...exploratória com abordagem quantitativa. Participaram da pesquisa 100 acadêmicos de Enfermagem do 7o ao 10o semestre do curso de bacharelado em Enfermagem, em uma instituição privada de ensino superior de Belém/PA. Resultados: Do universo de 100 participantes da pesquisa, com faixa etária entre 19 e 54 anos, a maioria possuía idade entre 34 e 39 anos, representando 23,96% da amostra, sendo 82% do gênero feminino e 53% católicos. Quanto às questões de conhecimento, houve crescimento do número de acertos após a realização da ação educativa de 67,6% para 87%, assim como um aumento no percentual quanto ao posicionamento em relação à doação de órgãos e tecidos após a morte, de 82% para 96%. Obteve-se um resultado positivo, na medida em que, quanto analisadas as respostas antes e após a realização da ação educativa, houve crescimento dos conhecimentos dos discentes e sensibilização quanto à importância da doação de órgãos e tecidos para transplante. Conclusão: A realidade do ensino nas instituições de ensino superior (IES) encontra-se distanciada e descontextualizada das diretrizes estabelecidas pela legislação que trata de doação de órgãos e tecidos para transplante. Entende-se ser imprescindível que o profissional tenha contato com temas polêmicos, como o processo de doação e transplante de órgãos, desde a vivência acadêmica, e busque cada vez mais se aperfeiçoar e promover discussões em seu meio de convivência. As Diretrizes Curriculares Nacionais enfatizam a necessidade e o dever das IES em formar profissionais de saúde voltados ao SUS, com a finalidade de adequar a formação às necessidades de saúde da população brasileira. Dessa forma, a tem tica doação de órgãos e tecidos para transplante deve ser inserida nos desenhos curriculares dos cursos de graduação, para formar profissionais capacitados e preparados para lidar com as demandas sociais.
Background: The use of distinct drugs and techniques for establishing balanced anesthesia protocols has shown promising results in birds. The techniques of locoregional block can be incorporated to ...these protocols, thereby providing intra- and post-operative analgesia and reducing the requirement for general anesthesia. Additionally, the use of neurostimulators increases the chances of success and reduces the risk of toxicity; however, there are limited reports in the literature of its applicability in wild birds. Therefore, the aim of this study was to describe the brachial plexus block technique guided using a neurolocalizer in a striped owl (Asio clamator) submitted for right wing amputation.Case: A striped owl weighing 400 g with a history of exposed fracture of the right wing was supplied by the clinical sector at the Veterinarian Hospital of the Federal University of Bahia. Following hydration and stabilization of vital signs, the animal was referred to the surgical center for amputation of the limb. Dexmedetomidine (10 µg.kg−1 IM) was administered as premedication, and after 20 min, anesthetic induction was performed using sevoflurane (FiO2 = 100%) via a mask followed by maintenance using the same drug. The animal was positioned in a left lateral decubitus position with access to the brachial plexus determined by palpation and identification of the border of the following muscles: pectoral, cranial branch of the brachial biceps, and dorsal branch of the ventral serratus. The brachial plexus nerves are situated in the subcutaneous site craniodorsal to the axillary depression. For the block, a neurolocalizer was used, fixing the positive electrode to approximately 5 cm from the needle insertion site (21G × 2'”) in the axillary depression, which remained connected to the neurostimulator by the second electrode. At first, the needle was attached to the peripheral nerve stimulator using a pulse frequency of 1 Hz with an impulse duration of 0.1 ms and initial current of 1 mA. The needle was advanced in the direction of the nerve plexus until it was observed that muscular contractions and movement in the limb were blocked, gradually decreasing the contractions, which disappeared at a current of 0.3 mA. At this juncture, after verifying the absence of blood on aspiration, ropivacaine was injected (2 mg/kg) with a latency period of 20 min. Data were recorded via monitoring of vital signs (DigicareLifeWindowLW9xVet), recording an average hearth rate of 140 ± 9.84 bpm, respiratory rate of 30 ± 4.18 mpm, oxyhemoglobin saturation of 99%, end-tidal carbon dioxide of 26 ± 1.98 mmHg, and temperature of 37.4°C ± 0.11°C over the course of 20 min of the surgical procedure. No movements were observed in response to pain stimuli, and the animal exhibited normal recovery, free of excitation or signs of pain. Discussion: Considering the imprecise history regarding fracture time as well as the lack of specific tests to clearly elucidate the condition of this animal, selective and safe drugs were selected additionally: dexmedetomidine and sevoflurane, which provided superior quality of sedation during the handling of the animal and rapid and smooth anesthetic induction, respectively. Based on experiences with numerous species, the inclusion of the locoregional block has substantial value for lengthening the analgesic duration and quality in addition to reducing adverse effects inherent in general anesthesia. In the present study, the use of ropivacaine in the brachial plexus block with the use of a neurolocalizer in the striped owl was demonstrated to be easy to perform using the axillary approach, demonstrating efficacy confirmed by the stability of physiological variables and muscle relaxation as well as the absence of adverse events during recovery.

O objetivo deste estudo foi avaliar o comportamento à corrosão do aço 1010 processado por por Prensagem em Canais Equiangulares (ECAP), uma técnica que induz deformação plástica severa (DPS) em ...materiais. As amostras foram processadas em um e dois passes nas rotas A e C. O comportamento em relação à corrosão foi estudado por meio da polarização potenciodinâmica e espectroscopia de impedância eletroquímica em soluções de NaCl (pH = 6,5) e hidróxido de sódio/fosfato de sódio (pH = 10,5). Os testes de polarização apresentaram melhores desempenhos para as amostras processadas por ECAP do que nas amostras sem processamento, e isso ocorreu tanto na solução alcalina quanto na neutra. Os resultados mostram que os materiais no estado bruto de fusão e em apenas um passe de ECAP apresentaram maior taxa de corrosão em solução alcalina. Foi observado que as amostras processadas nas rotas A e C promoveram uma estabilização do filme em suas respectivas superfícies. Ademais, os valores de resistência de polarização obtidos por espectroscopia de impedância eletroquímica em ambiente de NaCl foram maiores para a amostra processada na rota C.
This study compared selective control versus strategic control against Rhipicephalus (Boophilus) microplus, under conditions of a real dairy farm. Strategic control requires that all animals be ...treated with acaricide at regular pre-determined intervals. Selective control only requires treatment of infested animals and only when they are at or above a pre-determined threshold. Tick counts on animals and in pasture were performed and the susceptibility of tick populations to the different treatment methods was evaluated at the beginning and end of the study using the Larval Packet Test, Larval Immersion Test, and Adult Immersion Test. Over the four years of the study strategic control was more advantageous than the selective control as the group experienced lower tick burden on animals and in pasture, absence of skin lesions and myiases, lower operating costs, treatments concentrated in fewer months of the year and lower resistance pressure.
