Distal biceps tendon rupture occurs more often in middle-aged male population, involving the dominant arm. In this retrospective study, it's been described the occurrence of the most frequent adverse ...events and the clinical outcomes of patients undergoing surgical repair of distal biceps tendon rupture with the modified Morrey's double-incision approach, to determine better indications for patients with acute tendon injury.
Sixty-three patients with acute distal biceps tendon rupture treated with a modified double-incision technique between 2003 and 2015 were retrospectively evaluated at a mean 24 months of follow-up. Clinical evaluation including range of motion (ROM) and isometric strength recovery compared to the healthy contralateral side assessment, together with documentation of nerve injury, was performed. Patients were asked to answer DASH, OES and MEPS scores.
The ROM recovery showed excellent results compared to the healthy contralateral side. The reported major complications included: one case of proximal radio-ulnar synostosis, 3 cases of posterior interosseous nerve (PIN) palsy and one case of a-traumatic tendon re-rupture. Concerning minor complications, intermittent pain, ROM deficiency < 30° in flexion/extension and pronation/supination, isometric flexion strength deficiency < 30% and isometric supination strength deficiency < 60%, lateral antebrachial cutaneous nerve (LACBN) injury, were observed. The average DASH score was 8.5; the average OES was 41.5 and the MEPS was 96.3.
The Morrey modified double-incision technique finds its indication in young and active patients if performed within 2 weeks from injury. If performed by experienced surgeons, the advantages can exceed the drawbacks of possible complications.

Methylation tests have been used for decades in regular DNA diagnostics focusing primarily on Imprinting disorders or specific loci annotated to specific disease associated gene promotors. With the ...introduction of DNA methylation (DNAm) arrays such as the Illumina Infinium HumanMethylation450 Beadchip array or the Illumina Infinium Methylation EPIC Beadchip array (850 k), it has become feasible to study the epigenome in a timely and cost-effective way. This has led to new insights regarding the complexity of well-studied imprinting disorders such as the Beckwith Wiedemann syndrome, but it has also led to the introduction of tests such as EpiSign, implemented as a diagnostic test in which a single array experiment can be compared to databases with known episignatures of multiple genetic disorders, especially neurodevelopmental disorders. The successful use of such DNAm tests is rapidly expanding. More and more disorders are found to be associated with discrete episignatures which enables fast and definite diagnoses, as we have shown. The first examples of environmentally induced clinical disorders characterized by discrete aberrant DNAm are discussed underlining the broad application of DNAm testing in regular diagnostics. Here we discuss exemplary findings in our laboratory covering this broad range of applications and we discuss further use of DNAm tests in the near future.
SUMMARY
The Gutenberg–Richter b-value is thought to be a proxy of stress conditions in the crust and therefore able to locate asperities as zones of stress concentration responsible for the ...nucleation of strong events. The scientific literature contains a broad range of case studies showing precursory drops of b, just before the occurrence of strong events, and subsequent rises, during the early part of aftershocks sequences. Translating these results into hazard assessment, the b-value has assumed the status of a candidate precursor to the occurrence of an imminent large event. This issue is analysed here for three major seismic sequences that occurred recently in Italy. In comparison to previous studies, this investigation indicates that the variability of b may not be a reliable indicator of stress or a significant precursor in these examples, and instead may be assigned to a combination of chance, inhomogeneities in the data and inefficiencies in estimation methodologies. Consequently, extreme caution is required when we interpret b-values both as a proxy of physical processes involved in a seismic sequence and as a precursor to the occurrence of imminent strong events.
We combine extinction maps from the Two Micron All Sky Survey (2MASS) with Hipparcos and Tycho parallaxes to obtain reliable and high-precision estimates of the distance to the Ophiuchus and Lupus ...dark complexes. Our analysis, based on a rigorous maximum-likelihood approach, shows that the ρ -Ophiuchi cloud is located at $(119 \pm 6) \mbox{ pc}$ and the Lupus complex is located at $(155 \pm 8) \mbox{ pc}$; in addition, we are able to put constraints on the thickness of the clouds and on their orientation on the sky (both these effects are not included in the error estimate quoted above). For Ophiuchus, we find some evidence that the streamers are closer to us than the core. The method applied in this paper is currently limited to nearby molecular clouds, but it will find many natural applications in the GAIA-era, when it will be possible to pin down the distance and three-dimensional structure of virtually every molecular cloud in the Galaxy.

z
≃ 0.201 is one of the most massive galaxy clusters known, very likely in a post-merging phase. Data from several observational windows suggest a complex mass structure with ...interacting subsystems, which makes the reconstruction of a realistic merging scenario very difficult. A missing key element in this sense is unveiling the cluster mass distribution at high resolution. We perform such a reconstruction of the cluster inner total mass through a strong lensing model based on new spectroscopic redshift measurements. We use data from the Multi Unit Spectroscopic Explorer on the Very Large Telescope to confirm 12 multiple images of four sources with redshift values from 1.16 to 2.72. We also discover four new multiple images and identify 29 cluster members and 35 foreground and background sources. The resulting galaxy member and image catalogs are used to build five cluster total mass models. The fiducial model consists of 111 small-scale subhalos, plus a diffuse component, which is centered ∼2″ away from the BCG belonging to the east Abell 2163 subcluster. We confirm that the latter is well represented by a single, large-scale mass component. Its strong elongation towards a second (west) subcluster confirms the existence of a preferential axis, corresponding to the merging direction. From the fiducial model, we extrapolate the cumulative projected total mass profile and measure a value of

, which has a significantly reduced statistical error compared with previous estimates, thanks to the inclusion of the spectroscopic redshifts. Our strong lensing results are very accurate: the model-predicted positions of the multiple images are, on average, only 0″.15 away from the observed ones.
We generalize the technique of Lada et al. (CITE) to map dust column density through a molecular cloud (Nice) to an optimized multi-band technique (Nicer) that can be applied to any multi-band survey ...of molecular clouds. We present a first application to a ~$625 { deg}^2$ subset of the Two Micron All Sky Survey (2MASS) data and show that when compared to Nice, the optimized Nicer technique (i) achieves the same extinction peak values, (ii) improves the noise variance of the map by a factor of 2 and (iii) is able to reach $3 \sigma$ dust extinction measurements as low as $A_V \simeq 0.5 { magnitudes}$, better than or equivalent to classical optical star count techniques and below the threshold column density for the formation of CO, the brightest ${H}_2$ tracer in radio-spectroscopy techniques. The application of the Nicer techniques to near-infrared data obtained with a 8 meter-class telescope with a state-of-the-art NIR camera, such as the VLT-ISAAC combination, will be able to achieve dynamic ranges from below $10^{21} { protons cm}^{-2}$ to over $10^{23} { protons cm}^{-2}$ (AV in the range $0.3, 60$) and spatial resolutions < $10''$, making use of a single and straightforward column density tracer, extinction by interstellar dust.
Study objectiveTraumatic eye injuries are common emergency department presentations worldwide, and diagnosis may be delayed because of concurrent injuries and lack of guidelines in regard to the ...utility of clinical examination, computed tomography (CT), and point-of-care ultrasonography. In this study, we compare point-of-care ultrasonography with ophthalmologist clinical examination and CT for 6 types of traumatic eye injury. MethodsWe conducted a prospective cohort study evaluating patients with suspected traumatic eye injury who were recruited at an academic medical center in Tabriz, Iran. Each patient was evaluated by an emergency physician with point-of-care ultrasonography using a 7- to 15-MHz linear transducer, by a radiologist with orbital CT imaging, and by an ophthalmologist with a complete bedside ocular examination. Obtained results were tabulated. Sensitivity, specificity, and likelihood ratios were subsequently calculated. Cohen’s κ was assessed to evaluate the agreement between ocular point-of-care ultrasonography with orbital CT and point-of-care ultrasonography with complete bedside ocular examination. ResultsTwo hundred thirty-two patients (351 eyes) with suspected traumatic eye injury were included. In all measures of accuracy, diagnosis by point-of-care ultrasonography compared favorably with CT and a complete bedside ocular examination by an ophthalmologist in the 6 ocular injury patterns included in this study. Compared with CT imaging, point-of-care ultrasonography provided a specificity of 99.4% (95% confidence interval CI 97.8% to 99.9%) and a sensitivity of 96.8% (95% CI 83.3% to 99.9%) in the diagnosis of lens dislocation, and a specificity of 99.7% (95% CI 98.3% to 100.0%) and sensitivity of 95.7% (95% CI 78.1% to 99.9%) in the diagnosis of retrobulbar hematoma. Compared with complete bedside ocular examination by an ophthalmologist, point-of-care ultrasonography provided a specificity of 98.7% (95% CI 96.7% to 99.6%) and sensitivity of 97.8% (95% CI 88.2% to 99.9%) in the diagnosis of vitreous hemorrhage. In all injury types, positive likelihood ratios were high and negative ones were low. ConclusionPoint-of-care ultrasonography demonstrates high sensitivity and specificity in the diagnosis of traumatic eye injury, and represents a valuable diagnostic tool in addition to orbital CT and complete beside ocular examination by an ophthalmologist in the diagnosis of traumatic eye injury.
We present a novel approach for a combined analysis of X-ray and gravitational lensing data and apply this technique to the merging galaxy cluster MACS J0416.1-2403. The method exploits the ...information on the intracluster gas distribution that comes from a fit of the X-ray surface brightness and then includes the hot gas as a fixed mass component in the strong-lensing analysis. With our new technique, we can separate the collisional from the collision-less diffuse mass components, thus obtaining a more accurate reconstruction of the dark matter distribution in the core of a cluster. We introduce an analytical description of the X-ray emission coming from a set of dual pseudo-isothermal elliptical mass distributions, which can be directly used in most lensing softwares. By combining Chandra observations with Hubble Frontier Fields imaging and Multi Unit Spectroscopic Explorer spectroscopy in MACS J0416.1-2403, we measure a projected gas-to-total mass fraction of approximately 10% at 350 kpc from the cluster center. Compared to the results of a more traditional cluster mass model (diffuse halos plus member galaxies), we find a significant difference in the cumulative projected mass profile of the dark matter component and that the dark matter over total mass fraction is almost constant, out to more than 350 kpc. In the coming era of large surveys, these results show the need of multiprobe analyses for detailed dark matter studies in galaxy clusters.
In a very brief period, the COVID-19 pandemic has swept across the planet leaving governments, societies, and healthcare systems unprepared and under-resourced. New York City now represents the ...global viral epicenter with roughly one-third of all mortalities in the United States. To date, our hospital has treated thousands of COVID-19 positive patients and sits at the forefront of the United States response to this pandemic. The goal of this paper is to share the lessons learned by our spine division during a crisis when hospital resources and personnel are stretched thin. Such experiences include management of elective and emergent cases, outpatient clinics, physician redeployment, and general health and wellness. As peak infections spread across the United States, we hope this article will serve as a resource for other spine departments on how to manage patient care and healthcare worker deployment during the COVID-19 crisis.
