Este aspecto y el continuo cambio de la ciencia enfermera ...hacían necesaria su revisión. El proyecto se llevó a cabo en dos fases. Se analizaron un total de 99 artículos. Los factores relacionados más identificados fueron presencia de secreciones, intubación/ventilación asistida e infección respiratoria. No hubo relaciones estadísticamente significativas entre estos factores y el tipo de paciente, el contexto ni el año de publicación de los artículos. Se propusieron dos nuevos factores relacionados: «tapón mucoso» y «exposición a sustancias nocivas».
Chronic obstructive pulmonary disease (COPD) patients present a high prevalence of cardiovascular disease. This excess of comorbidity could be related to a common pathogenic mechanism, but it could ...also be explained by the existence of common risk factors. The objective of this study was to determine whether COPD patients present greater cardiovascular comorbidity than control subjects and whether COPD can be considered a risk factor per se.
1200 COPD patients and 300 control subjects were recruited for this multicenter, cross-sectional, case-control study.
Compared with the control group, the COPD group showed a significantly higher prevalence of ischemic heart disease (12.5% versus 4.7%; P < 0.0001), cerebrovascular disease (10% versus 2%; P < 0.0001), and peripheral vascular disease (16.4% versus 4.1%; P < 0.001). In the univariate risk analysis, COPD, hypertension, diabetes, obesity, and dyslipidemia were risk factors for ischemic heart disease. In the multivariate analysis adjusted for the remaining factors, COPD was still an independent risk factor (odds ratio: 2.23; 95% confidence interval: 1.18-4.24; P = 0.014).
COPD patients show a high prevalence of cardiovascular disease, higher than expected given their age and the coexistence of classic cardiovascular risk factors.

Un buen manejo y seguimiento continuo de los pacientes es fundamental para un control ...adecuado de la enfermedad, la cual requiere tanto medidas farmacológicas como no farmacológicas para su tratamiento. El personal de enfermería especializada en asma puede contribuir al correcto manejo de los pacientes asmáticos. Este tiene un papel fundamental en las pruebas diagnósticas, en la administración de medicamentos, en el seguimiento de los casos y en su educación. Este consenso nació de la necesidad de abordar un aspecto del manejo de la enfermedad que no cuenta con recomendaciones específicas en las guías actuales. Este documento destaca y actualiza el papel del personal de enfermería especializada en el cuidado y manejo de las personas con asma, para ofrecer una serie de conclusiones y recomendaciones prácticas con el objetivo de mejorar su implicación en el abordaje de esta patología. Las recomendaciones que se proponen son el resultado de un consenso de tipo nominal desarrollado a lo largo del año 2019 y fueron validadas a comienzos del año 2020. To identify practices that do not add value, cause harm, or subject patients with chronic obstructive pulmonary disease (COPD) to a level of risk that outweighs possible benefits (overuse).
A ...qualitative approach was applied. First, a multidisciplinary group of healthcare professionals used the Metaplan technique to draft and rank a list of overused procedures as well as self-care practices in patients with stable and exacerbated COPD. Second, in successive consensus-building rounds, description files were created for each "do not do" (DND) recommendation, consisting of a definition, description, quality of supporting evidence for the recommendation, and the indicator used to measure the degree of overuse. The consensus group comprised 6 pulmonologists, 2 general practitioners, 1 nurse, and 1 physiotherapist.
In total, 16 DND recommendations were made for patients with COPD: 6 for stable COPD, 6 for exacerbated COPD, and 4 concerning self-care.
Overuse poses a risk for patients and jeopardizes care quality. These 16 DND recommendations for COPD will lower care risks and improve disease management, facilitate communication between physicians and patients, and bolster patient ability to provide self-care.
