AIM: To describe the clinical and histological characteristics of a group of adults with small-bowel nodular lymphoid hyperplasia (NLH).
METHODS: Patients were searched for five years in pathology ...records of our institution. The biopsy material was reassessed using strict histopathological criteria. Clinical data were obtained from medical records.
RESULTS: Small-bowel NLH was diagnosed in 18 cases. The female: male ratio was 2 : 1. The most frequent symptoms were diarrhea (72%), involuntary weight loss (72%) and abdominal pain (61%). Nine patients (50%) had immunodeficiency. Small-bowel bacterial overgrowth was found in three (17%) cases. At small-bowel NLH diagnosis, three (17%) had associated lymphoma: two intestinal and one extra-intestinal lymphomas. In two patients with villous atrophy and anti-endomysial antibodies the diagnosis of celiac disease was established. Giardia larnblia infection was found in only one patient with hypogammaglobulinemia (Herman's syndrome).
CONCLUSIONS: NLH is uncommon in adult patients. Associated diseases are immunodeficiency and lymphoid tissue malignancies.

Anabaptist theology is the expression and communication of theological convictions that have sustained the ordinary life of faith communities within the Anabaptist tradition of Christianity. ...Alongside this ordinary Anabaptist theology, a more formal, ‘academic’ version has developed since at least the eighteenth century. In this respect, this article will first address some methodological issues and then present a historical overview of Anabaptist theology, from the emergence of Anabaptism in the Reformation era to recent global developments. The article concludes with an examination of some key theological issues within the Anabaptist theological tradition, namely, the status of confessional documents, biblical interpretation, Christology, discipleship, salvation, and ecclesiology.
INTRODUCTION/AIMIn North America and Europe, the prevalence of celiac disease (CD) might be much greater than expected in previous estimates. Until recently, the prevalence of CD in Latin America ...remained largely unknown. So far, information regarding CD in Mexico is limited, and it is still considered a rare disease. Our aim was to determine the prevalence of tTGA in a large group of apparently healthy blood donors.
SUBJECTS AND METHODSSerum samples from 1009 consecutive blood donors, who attended a third level referral center in Mexico City, were collected between June 2004 and December 2004. Only Mexican Mestizo individuals were included. All sera were tested with a new generation human recombinant protein based tTGA-IgA ELISA commercial kit (Aeskulisa tTG-IgA, Wendelsheim, Germany). The cut-off value provided by the manufacturer was 15 U/mL.
RESULTSThe mean age of the blood donors was 34±10 years and 68% (n=683) were men. Six hundred fifty two subjects (65%) were born in Mexico City; and from the remaining 357 subjects, at least one was born in each of the 31 different states in our country. Twenty-seven (2.7%) blood donors were positive for tTGA-IgA; all of them with tTGA-IgA values above 30 U/mL (range 36 to 1639). Overall prevalence was 1:37 27/1009, 95% confidence interval (CI)=1.6-3.7. The prevalence among women was 1:33 (10/326, 95% CI=1.04-5.09) and for men 1:40 (17/683, 95% CI=1.24-3.73).
CONCLUSIONSOn the basis of a well-recognized serologic screening method performed to blood donor samples, we demonstrated an unexpectedly high prevalence of tTGA positivity (2.6%) in the adult Mexican Mestizo population. Thus, the prevalence of CD in Mexico could be higher or similar to that observed in other countries. This observation contributes to increase the awareness for this under diagnosed disease in clinical practice and to consider CD as a global health problem.
BACKGROUND AND AIMSRecent studies have shown that celiac disease (CD) could affect 0.5% to 3% of the general population, including Mexican Mestizos, which represents a complex mixture of genetics, ...and constitutes the core of Mexican and Latin American populations. However, the association between CD and other conditions, specifically type-1 diabetes mellitus, in this population remains unknown. Thus, our aim was to determine the prevalence of both serologic and biopsy proven CD in Mexican Mestizo adults with type-1 diabetes.
METHODSOver a 6-month period, serum samples obtained from consecutive Mexican Mestizo adult patients (age ≥18 y) with type-1 diabetes were tested with a new generation human recombinant protein based IgA tissue transglutaminase enzyme-linked immunosorbent assay commercial kit. All patients with positive serologic test results underwent upper gastrointestinal endoscopy and small intestinal biopsies to confirm CD.
RESULTSEighty-four type-1 diabetic patients were included (62 women, mean age 28.9±9 y). Overall, 9 patients (9/84) were positive for IgA tissue transglutaminase with a point prevalence of 10.7% (95% CI, 4%-17%). Seven patients agreed to undergo endoscopy. Five subjects had biopsy-proven CD (5.9%, 95% CI, 1.9%-13.3%). One patient had chronic diarrhea and other abdominal bloating; whereas the remaining 3 were asymptomatic. CD associated type-1 diabetic patients tended to have higher hemoglobin A1c levels (P=0.07), reflecting poor glycemic control.
CONCLUSIONSAs in other populations, we demonstrated a high prevalence of biopsy-proven CD (5.9%) among Mexican Mestizo patients with type-1 diabetes. Clinicians should be aware of this common association in this ethnic group.

Description of eight cases of Peutz-Jeghers syndrome and literature review.
A retrospective review of all the discharge diagnosis was doing between January 1987 to February 2004. The diagnosis of Peutz-Jeghers syndrome was made on clinical and anatomical grounds. The clinical features, follow-up, treatment and last visit clinical status were analyzed in all the patients.
Eight patients with the Peutz-Jeghers syndrome were investigated, five women and three men. The median of time at diagnosis was 31 years-old (range, 26-37). All the patients had mucocutaneous pigmentation and hamartomatous polyps. The ileum and large bowel were the most frequent sites of the polyps, and there were generally sessile and pedunculated. The major abdominal symptoms were abdominal pain, GI bleeding, intestinal obstruction, weight loss and intussusception. One case of small-bowel cancer and one of serous cystadenoma of the ovary were detected. Surgical interventions were doing in five patients; the most frequent indication was polyp-induced bowel obstruction. All the patients are alive to date.

Para ello se presenta la crítica de la religión del mero culto desde la noción de una;
religión racional. No obstante, para comprender dicha crítica, se expone en primer;
lugar la noción de fe racional, como idea que da origen al concepto de religión;
racional. Este es el contenido del segundo capítulo. Con la presentación de todos;
estos elementos se presenta en el tercer capítulo, a modo de conclusión, la;
incidencia que tiene la religión en el ámbito de lo político.
Background
Textbook outcome (TO) is a multidimensional measure used to assess the quality of care. It is the “ideal” surgical result, based on a series of established indicators. In the field of ...bariatric surgery (BS), only one publication on TO is available.
Objectives
To determine TO in our BS unit and identify the factors linked to TO.
Setting
University public hospital in Alicante (Spain).
Methods
Retrospective observational study of all primary BS was performed. TO for BS was defined in relation to the following features: no major postoperative complications (Clavien-Dindo >II), hospital stay <75th percentile, and no mortality or readmissions within 30 days of surgery. Comparative analysis of the characteristics of the TO and non-TO groups was performed, as well as univariate and multivariate logistic regressions, to identify the independent factors associated with obtaining TO.
Results
In 970 patients, TO was achieved in 71.5%. The hospital stay was the one that most affected achievement of TO. Analysis according to the type of procedure (sleeve gastrectomy and gastric bypass) did not reveal any differences between both procedures in terms of obtaining TO (71.5 vs 71.26%). Logistic regression identified smoking, heart disease, operative time, and upper gastrointestinal bleeding as independent factors associated with obtaining TO (
p
<0.05). Analysis of the annual evolution of TO reveals a progressive increase in its achievement (7.7–86.4%).
Conclusion
In our series, TO was obtained in 71.5% of patients. The standardization of the technique and the experience gained over the years has improved our TO results.
