Globalization of disease with rapidly increasing technologic sophistication in multiple sites; wide diversity of pharmacologic availability and use; varying government regulation and evolution of ...practice; increasing recognition of multicultural prerogatives in end-of-life care and organ donation policies; reserve about overly enthusiastic integration and adoption of inadequately validated protocols; general availability of advanced resuscitative techniques with unknown long-term survival impact; and a variety of exciting, innovative, and promising interventions and support strategies must make the specialty vibrant; global; and, of necessity, more scrutinized by and responsible to societal and cultural prerogatives.

The Congress theme--One Step Further; The Pursuit of Excellence in Critical Care--is an appropriate platform from which to highlight the important content in the August issue beginning with several manuscripts dealing with early mobilization and the important benefits such strategies accrue for inhospital patient well-being and ultimate post intensive care unit discharge health, both mental and physical.

The journal has published manuscripts focusing on education in the past; however, in this issue, manuscripts dealing with practice (Outcomes of an emergency department ICU in a tertiary medical ...center in Taiwan), prediction (Basal functional status predicts functional recovery in critically ill patients with multiple organ failure), engineering science (Engineering control into medicine), and education per se (Innovative strategies in critical care education) vie for pride of place among more conventional fare.
An interesting development has been an increased number of submissions dealing with communication and teamwork, post-intensive care unit (ICU) discharge stress disorders, the importance of real-time ...nutritional assessment and modification, and the importance of exercise regimens and physiotherapy support in improving ICU outcomes.
Coordinated by Dr David Crippen, Professor in the Departments of Critical Care Medicine and Neurological Surgery, University of Pittsburgh Medical Center, the series promises to provide a ...comprehensive review of the literature detailing the past and present controversies and current practices surrounding the diagnosis and utilization of the diagnosis of death with specific reference to the topic of organ donation. The invited editorial commentaries and the letters to the editor, which address issues related to neurally adjusted ventilatory assist, bear attention as does Turnbull, Parker and Needham's editorial "Supporting small steps toward big innovations: the importance of rigorous pilot studies in critical care"; it is difficult to know when pilot studies rise to the importance of accepted manuscripts, and this commentary provides important insight into the evaluation and selection process.

The April issue of the Journal of Critical Care provides interesting discussions across the panoply of critical care interests; administration; outcomes; renal failure and fluid resuscitation; ...sedation and delirium, including a discussion regarding sunlight exposure; a rare case report detailing hypotensive resuscitation; and a series of letters and comments to the editor. 2014 will continue the Journal's focus on integrating clinical research with administrative, cultural, and societal concerns/constraints in a manner that provides the international community a forum in which to express novel concepts geared to improving the patient and family experience in the most resource consumptive arena in hospital medicine.

The manner in which our profession is practiced and the instruction of new physicians remains challenging, not only because of the increased scope of necessary skills required for current and future ...practitioners, but also because of the recognition that best practices, although a critically important concept, require continuous validation and reaffirmation. ...the importance of outcome-related research cannot be overemphasized whether related to interventional techniques, the implementation of administrative or therapeutic protocols, or the engagement of patients and their families in the process and application of care. The importance of multidisciplinary, multicultural, and multiethnic opinions cannot be underestimated, especially when the decisions impact care at the end of life, organ donation, cessation of life support, and terminal weaning to name but a few critical decision points in the management of complex patients and their families.

The same comments could be applied equally to articles published in the medical literature; letters to the editor are often motivated similarly, and it is important for Journal of Critical Care to represent the constituencies of its parent organizations, the World Federation of Societies of Intensive Care Medicine and the Society for Complexity in Acute Illness, when selecting content.
