Recent findings have indicated that lithium treatment markedly reduces suicide risk in major affective disorders. To compare the effect of lithium with carbamazepine and amitriptyline, suicidal ...behavior was analyzed during the randomized prospective long-term MAP study (N = 378; duration 2.5 years). Of the nine suicides and five attempted suicides, none took place during lithium treatment. The findings support the view that lithium has a specific antisuicidal effect over and above its prophylactic benefit.
The human serotonin transporter gene (5-HTT) demonstrates two polymorphisms with possible functional impact: a 44-bp insertion/deletion polymorphism of the promoter region and a 17-bp variable number ...of tandem repeat polymorphism (VNTR) in intron 2 (STin2). Such genetic polymorphisms in the serotoninergic system may increase the susceptibility to schizophrenia or may serve as predictors of therapeutic response. We therefore analyzed these polymorphisms as susceptibility factors for schizophrenia by comparison of 684 schizophrenic inpatients with 587 healthy controls. We furthermore compared the therapeutic outcome of schizophrenic patients differentiated by the 5-HTT genotypes. Schizo-affective patients were more frequently homozygous for the 44-bp insertion allele (Odds ratio, OR: 1.6, 95% confidence interval, CI: 1.1--2.3, P < 0.03) than were all other schizophrenic patients and controls. The 17-bp VNTR alleles found were: STin2.7, 9, 10, and 12. Sequence analysis revealed seven different sequence motifs with an invariable arrangement. Patients with schizo-paranoid schizophrenia were more frequently homozygous for the STin2.12 allele than were controls (OR: 1.4, CI: 1.1--1.8, P < 0.007) and all other schizophrenic patients (OR: 1.6, CI: 1.2--2.3). The STin2.9 allele represented a risk factor for the residual subtype of schizophrenia (OR: 6.4, CI: 2.5--16.2, P < 0.001). On the basis of global clinical impressions, as well as measurements with the positive and negative syndrome scale we found no association of the polymorphisms with therapeutic response. In conclusion, the 44-bp polymorphism may be associated with the schizo-affective and the 17-bp VNTR with the residual and schizo-paranoid subtype of schizophrenia, findings which require further biochemical and epidemiological confirmation.
Research into serotonergic parameters associated with suicidal behaviour has resulted in many inconsistent and ambiguous findings. In this mini-review, we have tried to name some examples of ...methodological and conceptual vagueness and pitfalls in biological suicide research. The existing literature indicates various critical issues considered when interpreting existing data in this area: -- The 'suicidality' construct is not useful in biological suicide research. Autodestructive tendencies mostly occur in suicide completers and are probably not found in many suicide attempters and individuals with suicidal thoughts. No consistent association has been found between suicide intent and injury, suicide intent and suicide risk, choice of suicide method (violent vs. non-violent) and suicide risk, suicidal thoughts or attempted suicide. No close relationship between different degrees and different forms of suicidal behaviour has yet been demonstrated with any degree of consistency. According to many studies, serotonergic markers do not correlate with the various qualities and intensities of suicidal behaviour. -- The association between suicidality and impulsivity or aggression is weak. There probably is a disturbance in impulse and aggression control, but only in a subgroup of'suicidal' patients. -- It might be misleading to use nosological constructs of depression in order to characterize suicidal individuals in biological suicide research. Biological variables might be associated with specific depressive symptoms, but not with some nosological construct or the sum score of a depression rating scale. -- The alleged association between peripheral and central serotonergic parameters is based on assumptions for which there is insufficient proof. Several studies indicate that there is no correlation between changes in central and peripheral serotonergic parameters. The mechanism of changes of peripheral serotonergic parameters is not sufficiently understood.
Objective Low platelet MAO-B activity has been associated with various forms of impulsive behaviour and suicidality. The present study investigated the relationship between MAO-B activity in ...platelets and aspects of suicidality in depressed patients and controls. Method In 87 patients with affective spectrum disorders (58% suffering from a Major Depressive Episode - MDE) the potential association between platelet MAO-B activity and suicidality was examined. 59 of the patients had committed suicide attempt recently (SA - “suicide attempters”), 28 patients were acutely depressed without having shown suicidal thoughts or suicidal behaviour in the past (NA - “non suicide attempters”). Results SA and NA were comparable as to their diagnoses and general demographic and psychopathological parameters. MAO-B activity did not differ between SA and NA. No systematic correlations existed between MAO-B activity and any dimensions of suicidal behaviour or psychopathology. As a single finding only a weak positive association of higher MAO-B activity in SA with a fatal intention of the suicide attempt was observed. Conclusion Our findings do not support a consistent association of platelet MAO-B activity and suicidal behaviour in general, but specific facts of suicidality might be associated.

Die Behandlung suizidaler Patienten stellt ein häufiges, zumeist akutpsychiatrisches Problem dar. Vor dem Hintergrund der Häufigkeit dieses Phänomens imponiert die aktuelle Datenlage ...zu suizidverhindernden Wirkeffekten von Psychopharmaka insgesamt unzureichend und enttäuschend. Mit Ausnahme von Lithium und Clozapin ist eine suizidverhindernde Wirkung von Psychopharmaka nicht belegt, ja nach bislang vorliegenden Studien unwahrscheinlich. Nach wie vor fehlen zum einen kontrollierte, randomisierte Studien, zum anderen limitieren ethische und methodische Probleme die klinische Forschung auf diesem Gebiet, was insbesondere an der aktuellen Datenlage zu den immer wieder behaupteten suizidpräventiven Wirkeffekten von Antidepressiva deutlich wird. Im Rahmen der Akutbehandlung ist unter Antidepressiva eine Reduktion der depressiven Störung zugeordneten Suizidgedanken möglich, jedoch muss die ebenfalls mögliche suizidprovozierende Wirkung insbesondere der neueren Antidepressiva gleichzeitig berücksichtigt werden. Der zeitnahen symptomatischen Behandlung psychomotorischer Erregung und Ängstlichkeit muss in Form einer sedierenden Pharmakotherapie mit Benzodiazepinen oder niedrig-potenten Neuroleptika höchste Bedeutung beigemessen werden. Im Rahmen der Erhaltungstherapie sollten Patienten vor dem Hintergrund der aktuellen Datenlage nach stattgehabten Suizidversuchen oder chronischer Suizidalität im Rahmen bipolarer oder unipolarer affektiver Erkrankungen von den positiven Effekten einer Behandlung mit Lithium und im Rahmen psychotischer Erkrankungen einer Behandlung mit Clozapin möglichst überzeugt werden.

Hintergrund
Die Therapie der Depression bei Palliativpatienten erfordert die Abwägung des erwarteten Nutzens und Schadens von Antidepressiva (AD) bei ggf. kurzer Lebenserwartung, ...reduziertem Allgemeinzustand, fortgeschrittenem Alter und erhöhtem Risiko für Nebenwirkungen und Medikamenteninteraktionen. Die vorliegende systematische Übersicht stellt die Evidenz zur Wirksamkeit und Verträglichkeit unterschiedlicher Wirkstoffklassen in Abhängigkeit von der Art und Schwere der Grunderkrankung dar.
Methode
Es wurde eine systematische Datenbanksuche (Medline und EMBASE) nach klinischen Studien durchgeführt und die Literaturverzeichnisse der identifizierten relevanten Literatur gesichtet. Für den Einschluss mussten Patienten eine im Suchstring definierte Erkrankung (wie multiple Sklerose, Morbus Parkinson, Alzheimer-Demenz, HIV/AIDS, Krebs, COPD, Herzinsuffizienz) aufweisen. Berücksichtigt wurden Studien, welche die Wirksamkeit von AD im Vergleich mit Placebo, einem anderen AD, einem Benzodiazepin, Psychostimulans oder einer Psychotherapie geprüft hatten. Im 1. Schritt wurden nur Studien an Patienten eingeschlossen, deren Depression mittels etablierter Diagnosekriterien diagnostiziert wurden und bei denen die Veränderung der Depression als primärer Endpunkt erhoben wurde. In einem 2. Schritt wurden auch Studien eingeschlossen, die die beiden letztgenannten Kriterien nicht erfüllten, aber Patienten am Lebensende untersuchten.
Ergebnisse
Vierzig Studien (die vorwiegend von SSRI oder NSMRI einsetzten) wurden eingeschlossen. Sechzehn Studien untersuchten Antidepressiva bei neurologischen, 24 bei internistischen Erkrankungen. Neun Studien untersuchten Patienten am Lebensende bzw. in weit fortgeschrittenen Erkrankungsstadien. Aus den heterogenen Studiendesigns lässt sich nicht ableiten, inwieweit Wirksamkeit und Verträglichkeit der AD von der Krankheitsschwere abhängig sind. Die Studien waren für die Erhebung begrenzter Behandlungseffekte vermutlich häufig zu klein. Dass AD gerade in den größeren Studien nicht wirksamer als Placebo waren, weist auf einen möglichen Publikationsbias hin. Bei den internistischen Erkrankungen stehen sich zumeist heterogene Ergebnisse gegenüber. Entgegen der gern geübten Verordnungspraxis verweisen die Studienergebnisse bei Alzheimer-Demenz auf fehlende Wirksamkeit der SSRI. Bei Morbus Parkinson liegen nur Studien an kleinen Populationen vor, sodass aus dem fehlenden Nachweis der Überlegenheit von SSRI in der Mehrzahl der Studien nicht sicher auf das Fehlen eines Effektes geschlossen werden kann.
Schlussfolgerung
Die Übersicht über die Datenlage erlaubt nur begrenzte Rückschlüsse für den Einsatz von AD bei unterschiedlichen körperlichen Erkrankungen am Ende des Lebens. Eindeutige Konsequenzen für die Behandlung mit AD in Abhängigkeit von der Schwere der Grunderkrankung oder im Vergleich mit anderen Behandlungsoptionen (Psychotherapie, Benzodiazepine etc.) können aus den eingeschlossenen Studien nicht direkt abgeleitet werden.
68 patients with affective disorders, and receiving lithium prophylaxis in a specialized lithium clinic were followed up for 8 years on average. Patients were selected for this study according to 2 ...criteria: They had been given lithium for at least 12 months, and they had attempted suicide at least once before onset of lithium prophylaxis. Outcome was analysed in terms of suicidal and parasuicidal behaviour. 55 patients took their lithium regularly, 13 discontinued or dropped the medication. One third of those patients having discontinued the medication died from suicide. Only one suicide occurred in patients with regular lithium intake and proven compliance during the last check before death. An impressive drop of parasuicides was observed in responders as well as in apparent non-responders. In total, 11 of 13 patients showed suicidal or parasuicidal behaviour 2 weeks-44 months after lithium discontinuation, which in about half of these cases took place on advice or with consent of the treating psychiatrist. It is concluded that lithium may have specific anti-suicidal properties, possibly related to its anti-aggressive effect, and that patients apparently not responding satisfactorily in terms of reduced number of episodes may still be protected against suicide or parasuicide.
Recovered depressive or manic-depressive patients on long-term lithium medication may show oscillations in their state of well-being, neuropsychological impairment, and subclinical mood disturbances. ...It remains an open question as to whether such changes should be labeled as “subthreshold” symptoms of depression or mania, as a particular syndromal entity not directly related to the “mood disorder,” or as subtle side effects of lithium. Within a psychological theory of the action of lithium, mental effects of lithium must be postulated and should not be simply considered as side effects. Depending on medical common sense and economic restrictions, the optimization and fine-tuning of lithium prophylaxis may be advisable in selected cases, including counseling, family therapy, cognitive therapy, well-being therapy, or interpersonal psychotherapy. Also, a change of medication may be considered in patients with subjective or objective cognitive impairment.
