Agriculture plays a leading role in the economic growth and development process. Ensuring the continuity of production depends on the purchasing power of the producers. The sustainability of ...production and the development of the rural welfare index in favor of the producers will increase the driving role of the agricultural sector, which is also a resource for other sectors. This study determined the variables affecting the Domestic Terms of Trade of the greenhouse tomato producers in Antalya, Türkiye. The product price has been determined as the major factor affecting the Domestic Terms of Trade. In addition, it was concluded that a one-unit increase in the product price increases the Domestic Terms of Trade by 34.99 units. Interviews revealed that the continuity of consumption needs is more important than the continuity of production. It has been determined that the high and variable input costs cause erosion in the incomes of the producers. Agricultural policies will be more effective both in the short and long term when the aim is to improve the welfare and purchasing power of producers, rather than intervening in agricultural prices or reducing prices.
RESUMO: A agricultura tem um papel preponderante no processo de crescimento e desenvolvimento econômico. Garantir a continuidade da produção depende do poder de compra dos produtores. A sustentabilidade da produção e o desenvolvimento do índice de bem-estar rural em favor dos produtores aumentarão o protagonismo do setor agropecuário, que também é um recurso para outros setores. O objetivo principal deste estudo é determinar as variáveis que afetam os Termos de Comércio Domésticos dos produtores de tomate de estufa em Antalya, Turquia. O preço do produto foi determinado como o principal fator que afeta os Termos de Comércio Domésticos. Além disso, concluiu-se que um aumento de uma unidade no preço do produto aumenta os Termos de Troca Domésticos em 34,99 unidades. As entrevistas revelaram que a continuidade das necessidades de consumo é mais importante do que a continuidade da produção. Constatou-se que os altos e variáveis custos dos insumos causam erosão na renda dos produtores. As políticas agrícolas serão mais eficazes, tanto no curto quanto no longo prazo, quando o objetivo for melhorar o bem-estar e o poder de compra dos produtores, em vez de intervir nos preços agrícolas ou reduzir os preços.

There is ongoing controversy related to the relationship between bisphosphonates and atrial fibrillation (AF). Our aim in this study was to evaluate the potential acute effect of zoledronic acid (ZA) ...infusion on AF development by using 24-hour Holter recordings.
The study was designed to be a self-controlled case series study, and 33 consecutive patients with osteoporosis (29 females, age: 62.3±9.0 years) who were scheduled to receive ZA infusion constituted the study population. Patients underwent 24-hour Holter rhythm recordings at two different times; the first one was 48 hours before the planned ZA infusion, and the second one was on the morning of the infusion day. Heart rate, frequency and type of arrhythmias, as well as heart rate variability (HRV) from the two recordings were compared.
There were no episodes of AF greater than 30 sec in any of the 24-hour Holter recordings obtained before and on the day of drug infusion. Holter recordings before drug infusion showed that only 1 patient had an atrial run of 3 beats long. Holter recordings obtained on the day of drug infusion revealed that 5 patients (15.2%) had atrial runs with lengths ranging between 3 and 12 beats (p=0.046). Regarding HRV variables, SDANN values were found to be significantly depressed on the day of ZA infusion (113.6±26.9 vs. 98.2±29.9, p=0.007).
None of the patients developed AF during or early after ZA infusion. However, there was an increase in atrial ectopy in some patients, which might be due to alterations in cardiac autonomic activity.

Abstract Coronary artery fistulas are abnormal and rare communications between a coronary artery and a cardiac chamber or a major vessel. Some devices have been designed to treat coronary fistulas ...percutaneously like coils and vascular occlusion devices. But most of these have been developed for fistulas unaccompanied by coronary artery disease. It is rational to treat a coronary fistula adjacent to a stenosis with one covered stent graft. This report describes a case of a coronary-to-pulmonary artery fistula with an adjacent right coronary artery stenosis that were both successfully treated by a single expanded polytetrafluoroethylene covered stent graft.

In this paper we want to demonstrate whether higher than normal levels of RDW, and lower than normal levels of MPV can be used as indicators of subclinical inflammation and tools for treatment ...decision in FMF or not. The participants in this study included 89 patients with FMF during attack-free periods and 30 healthy controls. The RDW and platelet counts were significantly higher, while the MPV was significantly lower in the patients with FMF group than healthy control group ( P < 0.001 ; P = 0.005 ; P < 0.001 , resp.). In the attack-free FMF group, a negative correlation was found between the MPV and RDW values ( P < 0.001 , r = - 0.40 ). The positive correlation was found between the RDW and ESR ( r = 0.23 , P = 0.028 ). And the negative correlation was found between the MPV and CRP ( r = - 0.216 , P = 0.042 ). Consequently, our results suggest that low MPV and high RDW levels may provide additional information about subclinical inflammation in FMF patients. But other strong predisposing factors affecting subclinical inflammation in FMF should be considered. Further studies with large numbers of patients are needed. Treatment of FMF should include not only prevention of acute attacks but also decreasing of the subclinical inflammation.
Abstract Background Although QRS-complex changes during ischemia have been described previously, their relation with no-reflow is not clear. Purpose To evaluate relation of admission QRS duration ...with angiographic no-reflow, we studied 162 patients who underwent primary angioplasty. Methods Twelve-lead electrocardiogram with a paper speed of 50 mm/s was recorded on admission and repeated after angioplasty. Patients were divided into reflow and no-reflow groups based on postangioplasty coronary thrombolysis in myocardial infarction flow grade. Results Patients in the no-reflow group (26 patients) were older ( P = .001) and had significantly longer pain-to-balloon interval ( P = .007). The patients in the no-reflow group had significantly longer QRS duration on admission electrocardiogram compared with patients in the reflow group (interquartile range, 80-93 median, 84 milliseconds vs 60-80 median, 76 milliseconds, respectively; P < .001). After adjusting all variables, QRS duration on admission was found to be independently related to angiographic no-reflow (odds ratio, 1.07; 95% confidence interval, 1.02-1.12; P = .003). Conclusion QRS duration on admission may be valuable in predicting no-reflow.
T-segment yükselmesi, akut miyokard infarktüsünde (AMİ) prekordiyal ya da inferior derivasyonlarda en sık görülen elektrokardiyografi (EKG) bulgusudur. Bu hastalarda derivasyonların her ikisinde de ...görülen ST-segment yükselmesi ise nadir bir durumdur. Acil serviste çekilen EKG'de DI, aVL, V1-6 derivasyonlarında ST-segment yükselmesi ve inferior derivasyonlarda resiprokal değişiklikler görülen 48 yaşında erkek hasta anterior AMİ tanısıyla yatırıldı. Kateter laboratuvarına alınmadan önce çekilen ikinci EKG'de anterior derivasyonlardaki ST-segment yüksekliğinin izoelektrik hata yaklaştığı, ancak D2, D3 ve aVF derivasyonlarında devam ettiği görüldü. Koroner anjiyografide, sol ön inen koroner arterde %95 darlık ve distalde tam tıkanıklık, sağ koroner arterde ise kritik olmayan darlıklar görüldü. Sol ön inen koroner artere çıplak stent yerleştirildi; distaldeki lezyon ise balonla genişletildi. Hasta yatışının yedinci gününde taburcu edildi.
In general, ST-segment elevation in either the precordial
or inferior leads is the most common electrocardiography
(ECG) finding in patients with acute myocardial infarction
(AMI). However, simultaneous ST-segment elevation in
the precordial and inferior leads is a rare finding. A 48-
year-old male patient was admitted with a diagnosis of
AMI upon detection of ST-segment elevation in the DI,
aVL, and V1-6 leads and reciprocal changes in inferior
leads. A subsequent ECG taken before submission to the
cardiac catheterization laboratory showed that the STsegment
approached the isoelectric line in the anterior
leads, while remaining elevated in the D2, D3, and aVF
leads. Coronary angiography revealed 95% stenosis in
the left anterior descending (LAD) coronary artery with
total occlusion distally, and some insignificant stenoses in
the right coronary artery. A bare metal stent was placed
in the LAD, with balloon dilatation of the distal lesion. He
was discharged on the seventh day of admission.
