L’encéphalite auto-immune à anticorps antirécepteur N-méthyl-D-aspartate du glutamate (NMDA-R) est une pathologie grave, de description récente et touchant principalement de jeunes patients. Le ...diagnostic repose sur une triade clinique associant troubles psychiatriques d’installation brutale, épilepsie et troubles mnésiques antérogrades, sur l’imagerie cérébrale par résonance magnétique ainsi que sur la présence d’anticorps anti-NMDA dans le sang et le liquide céphalorachidien. Le traitement consiste en la résection d’une tumeur parfois associée et la prescription d’immunosuppresseurs sur une longue durée. Nous rapportons l’observation d’une jeune patiente de 17ans, admise en urgence pour malaise, avec bilan étiologique retrouvant une méningite virale bénigne rapidement résolutive biologiquement. Cliniquement, la patiente présente des troubles du comportement avec agitation, hypomanie alternant avec des phases de catatonie et angoisse massive nécessitant la prescription de psychotropes ainsi que l’orientation en unité psychiatrique. Le diagnostic d’encéphalite limbique n’est posé que trois semaines plus tard, suite à l’apparition de crises d’épilepsie graves nécessitant un passage en réanimation. Un traitement par antiépileptiques et immunosuppresseurs est alors mis en place. Puis, après amélioration clinique, une prise en charge en centre de rééducation est débutée. Après un rappel neurobiologique, nous décrivons la clinique de la catatonie et son évolution dans le DSM-V puis nous évoquons la complexité diagnostique de cette pathologie.
Anti-N-methyl-D-aspartate of the glutamate receptor encephalitis (NMDA-R) is a severe disorder, recently described, especially in young patients. The diagnosis is based on a clinical set of three associating seizure psychiatric disorders, epilepsy and anterograde amnesia, on irregularities on cerebral MRI and on a presence of antibody anti-NMDA in the blood and the cerebrospinal fluid. The treatment consists of the resection of the tumor in paraneoplasic cases and the prescription of immunosuppresseurs. We report the observation of a young 17-year-old patient admitted in urgency for faintness initially attribute to a viral meningitis self-limited biologically. Clinically, the patient presents behavior disorders with agitation, hypomania alternating with periods of catatonia and massive anxiety requiring the prescription of psychotropic drugs and orientation in psychiatry unity. The diagnosis of limbic encephalitis is made only three weeks later, further to the appearance of severe epileptic seizure requiring a passage in recovery unit. A treatment by antiepileptics and immunosuppressive drugs is setting up. Then, after clinical improvement, a medical care in rehabilitation center is initiated. After a neurobiological reminder, we describe the clinical catatonia and its evolution in DSM-V, then we approach the diagnostic complexity.

Les conduites de scarifications à l’adolescence sont devenues extrêmement fréquentes. Leur définition est d’autant plus complexe qu’elles sont transnosographiques. Nous proposons de nous intéresser ...ici aux scarifications débutant à l’adolescence, c’est-à-dire inhérente au travail psychique spécifique à cet âge, infligées par l’adolescent lui-même, répétitives, avec une conscience de soi préservée, même fragile et fluctuante. Dès lors, selon notre hypothèse, les scarifications témoigneraient d’une effraction de l’enveloppe psychique du jeune sous le poids des remaniements identitaires et pulsionnels. Cette effraction mettrait à jour la défaillance du « Moi-peau », mais tenterait également selon nous d’y remédier, en restaurant une enveloppe psychique par la sensation de l’enveloppe cutanée. Les conduites de scarifications témoigneraient alors d’une fragilité de l’enveloppe psychique en lien avec un avatar des primorelations du jeune.
Scarring behaviours during adolescence are now extremely frequent. It is quite complex to definite them since you can find them in different pathologies. Our purpose is to concentrate our attention on the scarring starting at the adolescence (period in life with a specific mental work out), inflicted to oneself, in a repetitive way, with a preserved self-awareness, although fragile and fluctuating. Our assumption is that scarring could point out a break-through the psychic enveloppe of the youngster under the pressure of various modifications of identity and drive. The “breaking-through” would point out the weakness (or failure) of the ego skin but would try to cope with it, trying to restore a psychic enveloppe with the sensations from the skin. Then, scarring behaviours would testify a weakness of the psychic enveloppe, related to troubles in the first relationships of the youngster.

Notre expérience de pédopsychiatre de liaison en pédiatrie nous permet un repérage de l’évolution des différents regards portés sur l’enfant : ceux de la science, de la génétique, ceux de la ...néonatalogie, de la pédiatrie, de la psychanalyse. Si la tendance a longtemps été d’opposer l’enfant de la science à celui de la psychanalyse, il nous semble que cette opposition est en voie d’être caduque ; la clinique impose d’elle-même un tel rapprochement. Il ne s’agit plus d’opposer mais de composer. D’ailleurs, ce n’est sans doute pas un hasard si la psychanalyse est plus que jamais d’actualité. Plus la science avance, plus la question du singulier se pose. Les neurosciences et la génétique sont amenées à repérer des mécanismes universaux qui aboutissent à produire de l’unique, remettant dans une actualité nouvelle ce qui fait le propre de la psychanalyse. « Pourquoi la psychanalyse ? » Roudinesco E. Pourquoi la psychanalyse ? Paris: Fayard; 1999. Sans doute parce qu’elle permet d’éviter le piège des causalités et des déterminismes (qu’ils soient génétiques, épidémiologiques, pédiatriques, sociologiques et même pédopsychiatriques) et parce qu’elle offre un espace de liberté et d’imprévisible, signant là que l’histoire n’est pas le destin. Mais aussi parce que le transfert est la voie essentielle par laquelle une démarche de soin peut trouver tout son sens. Nous développons ces différents points dans le champ de la pédopsychiatrie de liaison. Dans un premier temps, nous situons les enjeux de la pédopsychiatrie de liaison. Entre l’enfant de la science et l’enfant de la psychanalyse, nous abordons la question des déterminismes en précisant que « l’avenir n’est pas écrit » Jacquard A, Kahn A. L’avenir n’est pas écrit. Paris: Bayard; 2001 pp. 105–126. Puis, nous évoquons la place de la psychanalyse et celle du pédopsychiatre, en précisant ce en quoi il est un praticien de l’inattendu, un artisan de la rencontre. Enfin, dans ce contexte, nous abordons la question de la prévention.
Our experience as consultation-liaison child psychiatrist in paediatrics allows us to observe the different viewpoints applied to children: those of science, of genetics, those of neonatalogy, of paediatrics, of psychoanalysis. If tendency has long been to pit the science's child against the psychoanalysis one, it appears to us that this opposition is becoming obsolete; clinical medicine itself imposes such a correction. The matter is no more to pit but to make up. Besides, it's probably no accident that psychoanalysis is more than ever topical. The more science progresses, the more the question of singularity is obvious. Neurosciences and genetics seek universal mechanisms that finally produce something singular, making still relevant what is peculiar to psychoanalysis. “Why psychoanalysis?”. Probably because it enables us to avoid the trap of causalities and determinisms (genetic, epidemiological, paediatric, sociologic and even child psychiatric determinisms) and because it provides freedom and unexpected, highlighting that history isn’t destiny. But also because a care reasoning finds its meaning through transference. We develop these items in the consultation-liaison child psychiatry's domain. First, we point out the consultation-liaison child psychiatry's issues. Between the child viewed by science and the one viewed by psychoanalysis, we tackle the question of determinisms, specifying that “futur isn’t written”. Subsequently, we recall the psychoanalysis’ and the child psychiatrist's stead, specifying why he is a practitioner of the unexpected, an artisan of the meeting. Finally, in this context, we tackle the question of prevention.
Summary
Over the last two decades, the prevalence of peanut and tree nut allergy has increased throughout the western world. Adverse reactions to these foods account for over 50% of all deaths ...resulting from food‐related anaphylaxis. Until recently, evidence suggested that all peanut and tree nut allergy were permanent. It is now known that about 20% and 10%, respectively, of young patients outgrow peanut and tree nut allergies. Achieving tolerance is associated with increasing circulating T regulatory cells and reduced production of allergen‐specific IgE. Reliable predictors of resolution are not yet available. A direct correlation between skin test weal size and allergen‐specific IgE, at the time of diagnosis and likelihood of resolution, has been reported. Resolution of peanut or tree nut allergy cannot be determined conclusively by either allergen‐specific IgE analysis or by skin prick testing. Oral food challenge is the gold standard for determining resolution of food allergy. Food challenges should only be undertaken in a clinical setting fully equipped to deal with a potential severe adverse reaction. Approximately 8% of patients who outgrow peanut allergy may suffer a recurrence, but recurrent tree nut allergy has not been reported to date. Infrequent ingestion of peanut may be related to the re‐emergence of allergy. Induction of tolerance through oral immunotherapy or sublingual immunotherapy is now being actively studied, but remains experimental. Studies have reported short‐term desensitization to peanut, but ongoing follow‐up will determine whether tolerance is achieved long term.

Puberty and chronic disease have in common deep physical and psychic modifications. Far from evolving in a coherent movement, those two processes seem to compete with each other, the risk being that the latter (chronic disease) may hide the former (sexualisation process).

The results of medical treatment of severe obesity in the adolescent population (balanced diet and physical activity) are often unsatisfactory, and bariatric surgery is questioned. The psychological ...determinants for requesting bariatric surgery in these adolescents are unclear. The objective of this study was to report the psychiatric and psychological aspects as well as the determinants of the medical decision for surgery in a cohort of obese adolescents requesting bariatric surgery by laparoscopic adjustable gastric banding.
Thirty-five adolescents (12.3–17.7 years of age), were recruited from January 2007 to December 2012. Semistructured interviews were conducted.
Fifty-four percent of the adolescents had a psychiatric history and 85% had psychiatric comorbidities. In adolescents undergoing surgery, excess weight loss was 46% after 1 year and 51% after 2years. For patients not receiving surgery, excess weight loss was 0.43% after 1 year (P=0.001). Compliance with medical treatment was the only significant element contributing to the decision to perform surgery. Results in terms of satisfaction and perception 1 and 2years after surgery were encouraging.
Bariatric surgery is feasible in young patients and produces good results in terms of excess weight loss. We argue that compliance with medical treatment is probably one of the most important elements for making the decision to perform bariatric surgery and in excess weight loss after surgery. We probably need to focus on the compliance of young patients and evaluate how this can be improved.

Notre propos souligne l'importance de l'évaluation psychiatrique au cours des troubles du comportement alimentaire chez l'enfant et met l'accent sur une forme particulière de troubles du comportement ...alimentaire des enfants âgés de trois à dix ans, alors que les principaux travaux au sujet de l'anorexie mentale prépubère se focalisent soit sur la période précédant immédiatement l'adolescence (souvent entre dix et treize ans), soit sur le deuxième semestre de vie chez le bébé. Dans notre expérience, la plupart des enfants de trois à dix ans ayant des troubles du comportement alimentaire présentent un tableau mixte, proche de ce qui est décrit chez l'adolescent sous le nom d'évitement alimentaire avec trouble affectif (
food avoidance emotional disorder). Ces troubles du comportement alimentaire (restrictions alimentaires diverses avec ou sans vomissements provoqués) sont longtemps ignorés chez ces jeunes enfants du fait d'un développement staturopondéral globalement satisfaisant, mais il est n'est pas rare que ces enfants rencontrent les soins dans un contexte d'urgence pédiatrique vraie du fait d'un refus alimentaire brutal et complet. Une prise en charge en consultation thérapeutique permet à ces enfants d'exprimer autrement que par leur corps leurs très fortes angoisses concernant la maladie, l'empoisonnement et la mort, pour eux-mêmes ou pour leurs proches, en particulier la mère. La reconnaissance précoce et la prise en charge de ces difficultés de conciliation entre le corps et la pensée imposent une étroite collaboration entre les équipes de pédiatrie et de pédopsychiatrie.
We underline the clinical importance of a specific eating disorder in 3 to 10 years old children, when the majority of the works about the prepubertal eating disorders focus either on the period just preceding adolescence (often between 10 and 13 years), or on the second half of the first year of the baby. Within the eating disorders described in the literature, we compare the clinical presentation of most of these 3 to 10 years old children with the food avoidance emotional disorder described during adolescence. These problems of eating behaviour (various selective eating with or without provoked vomiting) are ignored for a long time in these young children because of quite a satisfactory growth, but these children are often seen in emergency rooms because of a brutally complete eating refusal. Therapeutic consultations allow these children to express their fears about diseases, poisoning and death, for themselves or for their close relations, in particular the mother, without endangering their body. The early recognition and care of these difficulties of conciliation between the body and the thoughts impose a narrow collaboration between paediatric and psychiatric staffs.
