Objectives: To assess the impact of early childhood caries (ECC) on the quality of life in a population of preschool children and their families in Hong Kong. Methods: A random sample of 1,296 ...Chinese preschool children participated in the survey and were subjected to an oral examination for their ECC status by 2 trained examiners. The parents were asked to respond to the Early Childhood Oral Health Impact Scale (ECOHIS) and an extra set of questions concerning their sociodemographic background. Results:The final data analysis included 1,261 children with a mean (±SD) age of 3.9 ± 0.66 years. The prevalence figures for ECC and severe ECC were 19.9 and 15.2%, respectively. Decayed teeth were found in 34.3% of the children while only 5.2% had filled teeth. The mean (±SD) dmft in this group of children was 1.5 ± 2.98. Higher ECOHIS scores were found in parents with lower education or income level, or with children who were born in mainland China (p < 0.05), or with children who had decayed, missing, or filled teeth (p < 0.001). In multiple regression analyses, decayed teeth and filled teeth in primary dentition were the better predictors (p < 0.001) of the ECOHIS score among the various parent and child characteristics collected in this survey. Conclusions: The findings of this study showed that parents of young children with dental caries experience perceived that both the children and other family members had poorer quality of life.
Background/Aims: Root caries among elderly communities is of growing public health concern globally. This controlled clinical trial investigated the effectiveness of silver diamine fluoride and oral ...health education in preventing and arresting root caries. Methods: Two hundred sixty-six elderly subjects who had at least 5 teeth with exposed root surfaces and did not have serious life-threatening medical diseases were allocated to 3 groups according to a computer-generated random list: group 1 (the control group) received oral hygiene instructions (OHI) annually; group 2 received OHI and silver diamine fluoride (SDF) application annually, and group 3 was given OHI and SDF application annually, plus an oral health education (OHE) programme every 6 months. Results: Two hundred twenty-seven elderly subjects were followed for 24 months. The mean numbers of new root caries surfaces in groups 1, 2 and 3 were 1.33, 1.00 and 0.70, respectively (ANOVA, p < 0.05). Group 3 had fewer root surfaces with new caries than group 1 (Scheffé multiple-comparison test, p < 0.05). The mean numbers of arrested root caries surfaces in groups 1, 2 and 3 were 0.04, 0.28 and 0.33, respectively (ANOVA, p < 0.01). Group 3 and group 2 had a greater number of active root caries surfaces which became arrested than group 1 (Scheffé multiple-comparison test, p < 0.05). Conclusion: Annual application of SDF together with biannual OHE was effective in preventing new root caries and arresting root caries among community-dwelling elderly subjects.

HESS J0632+057 belongs to a rare subclass of binary systems that emit gamma rays above 100 GeV. It stands out for its distinctive high-energy light curve, which features a sharp “primary” ...peak and broader “secondary” peak. We present the results of contemporaneous observations by NuSTAR and VERITAS during the secondary peak between 2019 December and 2020 February, when the orbital phase (
ϕ
) is between 0.55 and 0.75. NuSTAR detected X-ray spectral evolution, while VERITAS detected TeV emission. We fit a leptonic wind-collision model to the multiwavelength spectra data obtained over the four NuSTAR and VERITAS observations, constraining the pulsar spin-down luminosity and the magnetization parameter at the shock. Despite long-term monitoring of the source from 2019 October to 2020 March, the MDM observatory did not detect significant variation in H
α
and H
β
line equivalent widths, an expected signature of Be-disk interaction with the pulsar. Furthermore, fitting folded Swift-XRT light-curve data with an intrabinary shock model constrained the orbital parameters, suggesting two orbital phases (at

= 0.13 and 0.37), where the pulsar crosses the Be-disk, as well as phases for the periastron (

= 0.75). The broadband X-ray spectra with Swift-XRT and NuSTAR allowed us to measure a higher neutral hydrogen column density at one of the predicted disk-passing phases.
Abstract
Flat-spectrum radio quasars (FSRQs) are the most luminous blazars at GeV energies but only rarely emit detectable fluxes of TeV gamma rays, typically during bright GeV flares. We explore the ...gamma-ray variability and spectral characteristics of three FSRQs that have been observed at GeV and TeV energies by Fermi-LAT and VERITAS, making use of almost 100 hr of VERITAS observations spread over 10 yr: 3C 279, PKS 1222+216, and Ton 599. We explain the GeV flux distributions of the sources in terms of a model derived from a stochastic differential equation describing fluctuations in the magnetic field in the accretion disk and estimate the timescales of magnetic flux accumulation and stochastic instabilities in their accretion disks. We identify distinct flares using a procedure based on Bayesian blocks and analyze their daily and subdaily variability and gamma-ray energy spectra. Using observations from VERITAS, as well as Fermi, Swift, and the Steward Observatory, we model the broadband spectral energy distributions of PKS 1222+216 and Ton 599 during very high energy (VHE)–detected flares in 2014 and 2017, respectively, strongly constraining the jet Doppler factors and gamma-ray emission region locations during these events. Finally, we place theoretical constraints on the potential production of PeV-scale neutrinos during these VHE flares.
Abstract
We report on the search for very-high-energy gamma-ray emission from the regions around three nearby supersonic pulsars (PSR B0355+54, PSR J0357+3205, and PSR J1740+1000) that exhibit long ...X-ray tails. To date there is no clear detection of TeV emission from any pulsar tail that is prominent in X-ray or radio. We provide upper limits on the TeV flux, and luminosity, and also compare these limits with other pulsar wind nebulae detected in X-rays and the tail emission model predictions. We find that at least one of the three tails is likely to be detected in observations that are a factor of 2–3 more sensitive. The analysis presented here also has implications for deriving the properties of pulsar tails, for those pulsars whose tails could be detected in TeV.
Accurate prediction of premorbid functioning is important in neuropsychological assessment. We aimed to investigate the predictive accuracy of the TOPF and examine this word list at an item level ...against WASI-II scores, using Australian pronunciations. The sample of 219 healthy Australians were aged 18-82 years. Multiple regression analyses were used to replicate the TOPF and simple demographic models based on the US TOPF standardization. Rasch analyses provided a comparison of Australian, US and UK word order from the proportion of words pronounced correctly. The variance explained in WASI-II index scores ranged from R

=.12 (PRI) to .33 (FSIQ-2), which was approximately half that reported in the US standardization study. The accuracy of predicted WASI-II scores was also slightly less in our sample. Thirty-two words were out of place by five places or more compared with the US word order and 30 compared with the UK. These results add to concerns about the application the TOPF with norms developed in the US and UK in the Australian context. Clinicians are advised not to apply the five error discontinue rule when using the TOPF in the local context. Development of a more accurate word reading task for use in Australia is warranted.

OBJECTIVE—The purpose of this study was to investigate the ability of high-density lipoproteins (HDLs) to upregulate genes with the potential to protect against inflammation in endothelial cells.
...METHODS AND RESULTS—Human coronary artery endothelial cells (HCAECs) were exposed to reconstituted HDLs (rHDLs) for 16 hours before being activated with tumor necrosis factor-α (TNF-α) for 5 hours. rHDLs decreased vascular cell adhesion molecule-1 (VCAM-1) promoter activity by 75% (P<0.05), via the nuclear factor-kappa B (NF-κB) binding site. rHDLs suppressed the canonical NF-κB pathway and decreased many NF-κB target genes. Suppression of NF-κB and VCAM-1 expression by rHDLs or native HDLs was dependent on an increase in 3β-hydroxysteroid-Δ24 reductase (DHCR24) levels (P<0.05). The effect of HDLs on DHCR24 is dependent on SR-BI but not ABCAI or ABCGI. Silencing DHCR24 expression increased NF-κB (1.2-fold, P<0.05), VCAM-1 (30-fold, P<0.05), and NF-κB p50 (4-fold, P<0.05) and p65 subunits (150-fold, P<0.05). TNF-α activation of siDHCR24-treated cells increased expression of VCAM-1 (550-fold, P<0.001) and NF-κB (9-fold, P<0.001) that could no longer be suppressed by rHDLs.
CONCLUSIONS—Results suggest that antiinflammatory effects of rHDLs are mediated partly through an upregulation of DHCR24. These findings raise the possibility of considering DHCR24 as a target for therapeutic modulation.
Background
Pelvic exenteration for locally recurrent rectal cancer (LRRC) is associated with variable outcomes, with the majority of data from single‐centre series. This study analysed data from an ...international collaboration to determine robust parameters that could inform clinical decision‐making.
Methods
Anonymized data on patients who had pelvic exenteration for LRRC between 2004 and 2014 were accrued from 27 specialist centres. The primary endpoint was survival. The impact of resection margin, bone resection, node status and use of neoadjuvant therapy (before exenteration) was assessed.
Results
Of 1184 patients, 614 (51·9 per cent) had neoadjuvant therapy. A clear resection margin (R0 resection) was achieved in 55·4 per cent of operations. Twenty‐one patients (1·8 per cent) died within 30 days and 380 (32·1 per cent) experienced a major complication. Median overall survival was 36 months following R0 resection, 27 months after R1 resection and 16 months following R2 resection (P < 0·001). Patients who received neoadjuvant therapy had more postoperative complications (unadjusted odds ratio (OR) 1·53), readmissions (unadjusted OR 2·33) and radiological reinterventions (unadjusted OR 2·12). Three‐year survival rates were 48·1 per cent, 33·9 per cent and 15 per cent respectively. Bone resection (when required) was associated with a longer median survival (36 versus 29 months; P < 0·001). Node‐positive patients had a shorter median overall survival than those with node‐negative disease (22 versus 29 months respectively). Multivariable analysis identified margin status and bone resection as significant determinants of long‐term survival.
Conclusion
Negative margins and bone resection (where needed) were identified as the most important factors influencing overall survival. Neoadjuvant therapy before pelvic exenteration did not affect survival, but was associated with higher rates of readmission, complications and radiological reintervention.

Abstract We use the Very Energetic Radiation Imaging telescope Array System (VERITAS) imaging air Cherenkov telescope array to obtain the first measured angular diameter of β UMa at visual ...wavelengths using stellar intensity interferometry (SII) and independently constrain the limb-darkened angular diameter. The age of the Ursa Major moving group has been assessed from the ages of its members, including nuclear member Merak ( β UMa), an A1-type subgiant, by comparing effective temperature and luminosity constraints to model stellar evolution tracks. Previous interferometric limb-darkened angular-diameter measurements of β UMa in the near-infrared (Center for High Angular Resolution Astronomy (CHARA) Array, 1.149 ± 0.014 mas) and mid-infrared (Keck Nuller, 1.08 ± 0.07 mas), together with the measured parallax and bolometric flux, have constrained the effective temperature. This paper presents current VERITAS-SII observation and analysis procedures to derive squared visibilities from correlation functions. We fit the resulting squared visibilities to find a limb-darkened angular diameter of 1.07 ± 0.04 (stat) ± 0.05 (sys) mas, using synthetic visibilities from a stellar atmosphere model that provides a good match to the spectrum of β UMa in the optical wave band. The VERITAS-SII limb-darkened angular diameter yields an effective temperature of 9700 ± 200 ± 200 K, consistent with ultraviolet spectrophotometry, and an age of 390 ± 29 ± 32 Myr, using MESA Isochrones and Stellar Tracks. This age is consistent with 408 ± 6 Myr from the CHARA Array angular diameter.
Objectives
The objective of this study was to review orofacial functional impairments among patients following stroke, including objective and subjective assessment.
Methods
A structured search ...strategy was applied to three electronic databases (Pubmed, Embase, and Web of Science) to identify effective papers. Relevant data regarding subjects, method, outcomes, and key findings were extracted from the effective papers and the results were summarized.
Results
The initial search yielded 5227 papers, and 18 effective papers (Kappa: 0.971) were in accordance with the inclusion criteria. The patients with stroke consistently showed a decreased lip force, salivary flow rate, and chewing performance compared with the healthy controls. Due to equivocal results gained from the effective papers, the qualitative assessments regarding whether there was any change in masticatory force on the affected side and oral health‐related quality of life were inconclusive.
Conclusions
Existing evidence highlights a number of compromised orofacial functions experienced by patients following stroke. These impairments appear to be sustained, with spontaneous recovery unlikely to occur. While rehabilitative approaches may have the potential to improve orofacial function and quality of life following stroke, there is currently a lack of evidence‐based interventions available to inform the development of comprehensive rehabilitation protocols.
