Background
Observational studies on osteoporotic fractures in patients with type 2 diabetes indicate their increased incidence compared to those without diabetes, but results are inconsistent. ...Currently, type 2 diabetes is not considered as an independent risk factor for low-energy fractures in elder subjects. The aim of the study was to assess the association between type 2 diabetes and risk for hip and vertebral fractures in postmenopausal women.
Materials and methods
We searched Medline, Web of Science and Cochrane databases for articles published before September 2013. Studies assessing fractures in women aged >50 diagnosed with type 2 diabetes, regardless of the diabetes treatment, were deemed eligible. To estimate fracture risk meta-analysis in a random effect model was performed. The results were shown by the odds ratio (OR) and 95 % confidence interval (CI). Heterogeneity was tested using a Q-Cochrane test (significance was analyzed with
p
< 0.10) and
I

measure.
Results
A total of 15 observational studies (11 cohort and 4 cross-sectional, 263.006 diabetics and 502.115 controls) were included. Thirteen papers provided information on the incidence of hip fractures, and seven on vertebral ones. The meta-analysis revealed type 2 diabetes was associated with higher risk for hip fracture (OR 1.296, 95 % CI (1.069–1.571), but not vertebral fracture (OR = 1.134, 95 % CI (0.936–1.374). There was significant heterogeneity between hip fracture studies. American origin was identified as a potential source of such heterogeneity.
Conclusions
The results of our meta-analysis indicate there is an increased risk for hip fracture in postmenopausal women with type 2 diabetes.
Covering: early studies through to March 2016Cardenolides and bufadienolides constitute an attractive class of biologically active steroid derivatives which have been used for the treatment of heart ...disease in traditional remedies as well as in modern medicinal therapy. Due to their application as therapeutic agents and their unique molecular structures, bearing unsaturated 5- or 6-membered lactones (or other heterocycles) attached to the steroid core, cardio-active steroids have received great attention, which has intensified during the last decade, in the synthetic organic community. Advances in the field of cross-coupling reactions have provided a powerful tool for the attachment of lactone subunits to the steroid core. This current review covers a methodological analysis of synthetic efforts to cardenolide and bufadienolide aglycones. Special emphasis is given to cross-coupling reactions applied for the attachment of lactone subunits at sterically very hindered positions of the steroid core. The carefully selected partial and total syntheses of representative cardio-active steroids will also be presented to exemplify recent achievements (improvements) in the field.
Coronary artery bypass revascularization is still the optimal treatment for complex coronary artery disease with good long-term results. The relation between inflammatory activation in the ...post-operative period and the long-term prognosis was already postulated. The possible predictive role of preoperative inflammatory indexes after the off-pump coronary artery bypass grafting technique on long term survival was the aim of the study. Study population included 171 patients with a median age of 64 years (59-64) operated on using off-pump technique between January and December 2014. Patients enrolled in the current study were followed-up for 8 years. We conducted a multivariable analysis of pre-operative and post-operative inflammatory markers based on analysis of the whole blood count. The overall survival rate was 80% for the total follow-up period, while 34 deaths were reported (30-days mortality rate of 1%). In the multivariable analysis, a pre-operative value of systemic inflammatory response index (SIRI) >1.27 (HR = 6.16, 95% CI 2.17-17.48, p = 0.012) revealed a prognostic value for long-term mortality assessment after off-pump surgery. Preoperative inflammatory activation evaluated by systemic inflammatory reaction index (SIRI) possess a prognostic value for patients with complex coronary artery disease. The SIRI value above 1.27 indicates a worse late prognosis after off-pump coronary artery bypass (AUC = 0.682, p<0.001).

The presented method reconstructs a network (a graph) from AIS data, which reflects vessel traffic and can be used for route planning. The approach consists of three main steps: maneuvering points ...detection, waypoints discovery, and edge construction. The maneuvering points detection uses the CUSUM method and reduces the amount of data for further processing. The genetic algorithm with spatial partitioning is used for waypoints discovery. Finally, edges connecting these waypoints form the final maritime traffic network. The approach aims at advancing the practice of maritime voyage planning, which is typically done manually by a ship’s navigation officer. The authors demonstrate the results of the implementation using Apache Spark, a popular distributed and parallel computing framework. The method is evaluated by comparing the results with an on-line voyage planning application. The evaluation shows that the approach has the capacity to generate a graph which resembles the real-world maritime traffic network.
An efficient, easily scalable synthesis of 4-trifluoromethylquinolines and naphthydrines (as well as their difluoro- and perfluoro-analogues) as a result of tandem direct catalytic ...alkynylation/dehydrative condensation of o-aminofluoromethylketones (o-FMKs), for the first time catalyzed by NHC-copper(I) complexes on water, is reported. A wide range of terminal alkynes is tolerated under the reaction conditions, including β-lactam-, steroid-, and sugar-derived ones, leading to desired quinolines and naphthydrines with good yields. Further investigations proved that o-FMKs could be efficiently transformed into a rare class of heterocyclic compounds-dibenzob,f1,5diazocines-by a base-catalyzed condensation, also on water. The developed method was applied for gram-scale synthesis of a fluorinated analogue of G protein-coupled receptor antagonist (GPR91).
The mid-nineteenth century is not regarded as the time when the theory of extraterrestrial catastrophism developed. However, two German scholars independently introduced original concepts of ...terrestrial impacts of large celestial bodies at that time. Ludwig Pfeil (1803–1896), a self-educated wealthy landowner, and Karl Reichenbach (1788–1869), an eminent scientist and industrialist, independently proposed in the 1850s that the Earth is an aggregate of meteoritic masses and has experienced many impact-induced cataclysms throughout its geological history. Until 1891, Pfeil analyzed the effects of the collision of a comet's gaseous body with Earth. He tried to simulate the effects of tsunami waves generated by impacts into the ocean and inferred the route of “cometary currents” from the morphology and orientation of coastlines and associated mountain ranges. Reichenbach speculated about fertilization of the terrestrial surface by extraterrestrial dust in the context of an accretionary origin for Earth that also manifested in meteoritic sources of volcanic extrusions. He linked the Mesozoic succession of “buried living worlds” to geological catastrophes, caused by successive meteorite impacts. These cosmic bombardment concepts were comprehensively criticized by contemporary researchers, but soon found many conceptual successors in the German-speaking science community. Therefore, Pfeil and Reichenbach should be regarded as pioneers of the impact theory.
A novel method for the synthesis of epoxydibenzob,f1,5diazocines exhibiting a V-shaped molecular architecture is reported. The unique approach is based on unprecedented base-catalyzed, ...solvent-free autocondensation and cross-condensation of fluorinated o-aminophenones. The structure of the newly synthesized diazocines was confirmed independently by X-ray analysis and chiroptical methods. The rigidity of the diazocine scaffold allowed for the separation of the racemate into single enantiomers that proved to be thermally stable up to 140 °C. Furthermore, the inertness of the diazocine scaffold was demonstrated by performing a series of typical transformations, including transition metal-catalyzed reactions, proceeding without affecting the bis-hemiaminal subunit.
N-Heterocyclic carbenes have found many applications in modern metal catalysis, due to the formation of stable metal complexes, and organocatalysis. Among a myriad of N-heterocyclic carbene metal ...complexes, gold complexes have gained a lot of attention due to their unique propensity for the activation of carbon-carbon multiple bonds, allowing many useful transformations of alkynes, allenes, and alkenes, inaccessible by other metal complexes. The present review summarizes synthetic efforts towards the preparation of chiral N-heterocyclic gold(I) complexes exhibiting C2 and C1 symmetry, as well as their applications in enantioselective catalysis. Finally, the emerging area of rare gold(III) complexes and their preliminary usage in asymmetric catalysis is also presented.
