A computational modeling study of streamer propagation in a cold, atmospheric-pressure, helium jet in ambient air is presented. A self-consistent, multi-species, multi-temperature plasma model with ...detailed finite-rate chemistry and photoionization effects is used to provide fundamental insights into the structure and dynamics of the streamers. A parametric study of the streamer properties as a function of important discharge geometric and operating conditions is performed. The fluid mechanical mixing layer between the helium jet core and the ambient air is instrumental in guiding the propagation direction of the streamer and gives the plasma jet a visibly collimated appearance. The key chemical reactions which drive the streamer propagation are electron-impact ionization of helium neutral and nitrogen molecules. Photoionization plays a role in enhancing the propagation speed of the streamer, but is not necessary to sustain the streamer. The streamer yields a large radical concentration through chemical reactions in the streamer head and the body. The streamer propagation speed increases with reduced helium jet radius and increased helium-air mixing layer width. Impurities in the helium jet result in a significant increase in the discharge propagation speed within the tube through photoionization, but not after the streamer propagates into the open ambient region. It is also observed that thinner electrodes produce stronger electric-field concentrations that increase discharge propagation speeds within the tube but have a smaller influence on the discharge after it emerges out of the tube as a streamer.
Atrophic gastritis, intestinal metaplasia, and epithelial dysplasia of the stomach are common and are associated with an increased risk for gastric cancer. In the absence of guidelines, there is wide ...disparity in the management of patients with these premalignant conditions. The European Society of Gastrointestinal Endoscopy (ESGE), the European Helicobacter Study Group (EHSG), the European Society of Pathology (ESP) and the Sociedade Portuguesa de Endoscopia Digestiva (SPED) have therefore combined efforts to develop evidence-based guidelines on the management of patients with precancerous conditions and lesions of the stomach (termed MAPS). A multidisciplinary group of 63 experts from 24 countries developed these recommendations by means of repeat online voting and a meeting in June 2011 in Porto, Portugal. The recommendations emphasize the increased cancer risk in patients with gastric atrophy and metaplasia, and the need for adequate staging in the case of high grade dysplasia, and they focus on treatment and surveillance indications and methods.

turbulence model for a quantitative mutual comparison between the simulation and the experiment. The detailed variation of the density field associated with the Mach disk, the slip streams and the outer shear layers near the jet boundaries are successfully captured. Furthermore, the density profile along the jet centreline obtained by the present experiment is quantitatively compared with those from prior quantitative visualization studies, such as rainbow schlieren deflectometry, background oriented schlieren, moiré schlieren and Mach–Zehnder interferometer. The three-dimensional contour map coloured by the magnitude of the density gradient vector inside the microjet reveals the flow topology of the near-field shock systems and elucidates the spatial variation of the shock strength.
The gamma strength function and level density of 1^{-} states in ^{96}Mo have been extracted from a high-resolution study of the (pover →, pover →^{'}) reaction at 295 MeV and extreme forward angles. ...By comparison with compound nucleus γ decay experiments, this allows a test of the generalized Brink-Axel hypothesis in the energy region of the pygmy dipole resonance. The Brink-Axel hypothesis is commonly assumed in astrophysical reaction network calculations and states that the gamma strength function in nuclei is independent of the structure of the initial and final state. The present results validate the Brink-Axel hypothesis for ^{96}Mo and provide independent confirmation of the methods used to separate gamma strength function and level density in γ decay experiments.
Pharmacological therapy and pulmonary rehabilitation are useful for exertional dyspnoea in patients with chronic obstructive pulmonary disease (COPD); however, several patients have no meaningful ...improvements. Although acupuncture significantly improves exercise intolerance and dyspnoea, the pathophysiological mechanism of these effects is unknown; therefore, we evaluated this mechanism using cardiopulmonary exercise testing in a single-arm, open experimental study.
Sixteen patients with COPD underwent acupuncture once a week for 12 weeks. Primary outcomes were exercise performance parameters, including peak oxygen uptake in the incremental exercise tests (IETs) and the time to the limit of tolerance measured in constant work rate exercise tests (CWRETs) at 70% peak work-rate of the IET. IETs and CWRETs were performed at baseline and at weeks 12, 16, and 24.
During the time course, there were significant increases in peak oxygen uptake (p = 0.018) and minute ventilation (V̇
, p = 0.04) in the IETs. At 12 weeks, the endurance time significantly increased (810 ± 470 vs 1125 ± 657 s, p < 0.001) and oxygen uptakes at terminated exercise were significantly lower (771 ± 149 mL/min, p < 0.05) than those at baseline (822 ± 176 mL/min) in CWRETs. The significant decreases in oxygen uptake and minute ventilation and improvements in Borg scale scores were also observed during CWRETs after acupuncture. The decreases at isotime in the Borg scale (r = -0.789, p = 0.0007) and V̇
(r = -0.6736, p = 0.0042) were significantly correlated with the improvement of endurance time.
The positive effects of acupuncture on exertional dyspnoea were associated with increased endurance time influenced by improved oxygen utilisation and reduced ventilation during exercise. Acupuncture may be a new intervention for COPD in addition to conventional maintenance therapies.

MAC-PD who underwent regular follow-up for >5 years were retrospectively reviewed. Changes in plain chest radiograph (CXR) and baseline characteristics were compared between the stable and deteriorated groups.RESULTS: Seventy-two patients were investigated, including 30 patients who
were examined 10 years after the initial visit. One patient (1.4%) showed progressive or remarkably progressive disease on CXR at 1 year; this rate increased to 22.2% at 5 years and to 53.3% at 10 years. Body mass index (BMI) at the initial visit was lower in the deteriorated group than in
the stable group. Cavitary disease and resistance to a macrolide were seen more frequently at the initial visit in the deteriorated group than in the stable group.CONCLUSIONS: NBE MAC-PD is a slowly but substantially progressive long-term infection (5-10 years). Our data suggest
that patients with lower BMI, cavitary disease and resistance to a macrolide at initial visit are more likely to progress to deteriorating disease.
The nuclear structure of 66Se, nucleus beyond the N=Z line on the proton-rich side of the valley of stability, was investigated by the neutron knock-out reaction 67Se(12C,X)66Se using a 12C target. ...The analysis of the singles spectrum of the γ-rays emitted during the de-excitation of the populated low-lying excited states revealed two previously detected (927(4) keV, 1460(32) keV) and three new (744(6) keV, 1210(17) keV, 1661(23) keV) transitions. The 744-keV, the 1210-keV, and the 1460-keV transitions were found to be in coincidence with the one at 927 keV. The spectrum coincident with the 927-keV transition showed a further possible transition at 299(35) keV, which was obscured by significant atomic background in the singles spectrum. This transition might correspond to a peak previously reported at 273(5) keV that could not be assigned to 66Se unambiguously. Based on a comparison of the experimental data to theoretical calculations, four new excited states are proposed which suggest that 66Se exhibits shape coexistence.

single-centre study, data on serum levels of anti-glycopeptidolipid (GPL) core IgA antibody (U/ml) measured using the kit were analysed in patients diagnosed with MAC-PD according to American Thoracic Society criteria, in those with pulmonary tuberculosis (PTB) or pulmonary M. kansasii
disease and in healthy volunteers.RESULTS: The anti-GPL-core IgA antibody levels of serum were significantly higher (P < 0.0001) in patients with MAC-PD (n = 485) than in those with PTB (n = 133) or pulmonary M. kansasii disease (n = 23) or in healthy
subjects (n = 265). When the cut-off level was set at 0.7 U/ml, the sensitivity and specificity were respectively 78.6% and 96.9%. Higher antibody levels were observed in patients with greater extent of disease on chest computed tomography (P < 0.0001).CONCLUSIONS: The
serodiagnostic kit revealed good sensitivity and specificity. The antibody levels may reflect disease activity. Additional work is needed to determine whether the diagnostic assay could be used in conjunction with current diagnostic criteria to improve the diagnosis of MAC-PD.
This study examined the effect of learning progression-aligned formal embedded formative assessment on conceptual change and achievement in middle-school science. Fifty-two sixth graders were ...randomly assigned to either an experimental group or a control group. Both groups were taught about sinking and floating by the same teacher with identical curriculum materials and activities. The experimental group received, in addition, three sets of formal embedded formative assessments with qualitative feedback as to how to improve their understandings aligned with an expected learning progression during instruction. The control group spent the corresponding time between new curriculum activities conducting curriculum-specific extension activities. Overall, the experimental group experienced on average greater conceptual change than the control group. The experimental group also scored higher on average than the control group on general achievement tests, especially the performance assessment. This study supported the contention, then, that embedding formal formative assessments within a curricular sequence built around an expected learning progression is a useful way to promote conceptual change along that learning progression in science classrooms.
