Peritonitis is the most common cause of dialysis failure in children on chronic peritoneal dialysis. We performed a prospective study of 501 peritonitis episodes in 44 pediatric dialysis centers ...located in 14 countries that examined peritonitis etiology, efficiency of opinion-based management guidelines, and final outcomes. Culture-negative incidence varied significantly from 11% in North America to 67% in Mexico. Argentina and North America had the highest rate of Gram-negative episodes. Pseudomonas-based peritonitis was eightfold more common in the United States than in Europe, and correlated with the frequency of exit site cleansing and topical mupirocin administration. Significant regional variation in antibiotic susceptibility was noted for the first generation cephalosporins and aminoglycosides. Initial response rates to standardized empiric antibiotic treatment did not differ between regions; however, final outcomes were significantly less favorable in Eastern Europe. The wide regional variation in culture-negative peritonitis, and the distribution and antibiotic susceptibilities of causative bacteria needs to be taken into consideration when the guidelines for empiric therapy of pediatric dialysis-associated peritonitis are revised.
A study on the socio-economic, phenotypic, and technical features of Purgi goats in Kargil was undertaken. A total of 215 Purgi goat breeders from 8 villages were interviewed and 215 kids born ...between 2016 and 2018 were studied. The study revealed that Purgi goat farming under the study area is mainly practiced by poor farmers having an education level of middle to matric standard and belonging to the middle age group and scheduled tribe (Muslims) community. The average annual income (Lakhs) from goat farming, livestock rearing, agriculture, other sources and overall were 0.20±0.00, 0.32±0.02, 0.59±0.01, 0.59±0.01 and 1.40±0.02, respectively. Animals in small flocks are maintained semi-intensively. The predominant coat colour was black. The overall body weight (kg) was 1.08±0.04, 3.60±0.02, 5.83±0.03, 8.70±0.04, 10.58±0.06 and 13.46±0.14 at birth, weaning, 6, 9, 12 and 24 months. The least-squares means (cm) for body measurements, viz. height at withers (BH), body length (BL), heart girth (HG), and paunch girth (PG) at birth to yearling age were 58.87±0.03, 76.45±0.03, 71.21±0.05 and 74.22±0.12 cm, respectively. The age at first estrus, weight at first estrus, age at first service (AFS), weight at first service (WFS), age at first lambing (AFL), gestation period (GP), and inter kidding interval (KI) were 310.39±3.82 days, 74.22±0.12 (kg), 332.56±3.00 (days), 12.71±0.05 (kg), 513.06±2.36 (days), 149.93±0.11 (days) and 288.16±2.98 (days), respectively. It can be concluded that the Purgi goat is a unique, dwarf, and very important genetic resource of the country and it should be characterized and conserved for further improvements.
Alzheimer's disease (AD) is the most common dementia worldwide. According to the amyloid hypothesis, the early accumulation of the Aβ-peptide triggers tau phosphorylation, synaptic dysfunction, and ...eventually neuronal death leading to cognitive impairment, as well as behavioral and psychological symptoms of dementia. ScFv-h3D6 is a single-chain variable fragment that has already shown its ability to diminish the amyloid burden in 5-month-old 3xTg-AD mice. However, tau pathology is not evident at this early stage of the disease in this mouse model. In this study, the effects of scFv-h3D6 on Aβ and tau pathologies have been assessed in 22-month-old 3xTg-AD mice. Briefly, 3xTg-AD female mice were treated for 2 weeks with scFv-h3D6 and compared with 3xTg-AD and non-transgenic (NTg) mice treated with PBS. The treatment with scFv-h3D6 was unequivocally effective in reducing the area of Aβ staining. Furthermore, a tendency for a reduction in tau levels was also observed after treatment that points to the interplay between Aβ and tau pathologies. The pro-inflammatory state observed in the 3xTg-AD mice did not progress after scFv-h3D6 treatment. In addition, the treatment did not alter the levels of apolipoprotein E or apolipoprotein J. Thus, a 2-week treatment with scFv-h3D6 was able to reduce AD-like pathology in elderly 3xTg-AD female mice.
Purpose: the purpose of this article is to develop and build a financial model in the outline of the ecosystem of the creative spatial development of the region based on the resource financial ...potential.Methods: in preparing the article a wide range of research methods were used, primarily the method of analysis and synthesis, graphic method, method of statistical analysis, as well as the method of grouping in the formation of an integrated approach.Results: in the process of research, the sources of additional financial resources for formation of a profitable part of the budget, namely at the expense of leasing of property and receipt of additional taxes from registration of regional lands, used by legal and physical persons were revealed. The significance of the application of an integrated approach in the process of formation of the consolidated budget: tax and non-tax revenues; investments in fixed capital; financial profits of enterprises and organizations after taxation; capital and reserves of enterprises, organizations. It was revealed that in municipalities one ruble of total financial resources brings profit in relation to the gross regional product in each municipality differently, because not everywhere are effectively used financial resources.Сonclusions and Relevance: this study reveals information about the main trends and opportunities for the development of the region, identifies its strengths and weaknesses. The analysis and conclusions are important for specialists of municipalities and investors when they make decisions on investing capital in the facilities of the region. Nogo‐66 receptor (NgR) has recently been identified as the neuronal receptor of the myelin‐associated proteins Nogo‐A, oligodendrocyte protein (OMgp) and myelin‐associated glycoprotein (MAG), and ...mediates inhibition of axonal regeneration both in vitro and in vivo. Through database searches, we have identified two novel proteins (NgRH1 and NgRH2) that turned out to be homologous in their primary structures, biochemical properties and expression patterns to NgR. Like NgR, the homologues contain eight leucine‐rich repeats (LRR) flanked by a leucine‐rich repeat C‐terminus (LRRCT) and a leucine‐rich repeat N‐terminus (LRRNT), and also have a C‐terminal GPI signal sequence. Northern blot analysis showed predominant expression of NgRH1 and NgRH2 mRNA in the brain. In situ hybridization and immunohistochemistry on rat brain slices revealed neuronal expression of the genes. NgRH1 and NgRH2 were detected on the cell surface of recombinant cell lines as N‐glycosylated GPI anchored proteins and, consistent with other GPI anchored proteins, were localized within the lipid rafts of cellular membranes. In addition, an N‐terminal proteolytic fragment of NgR comprising the majority of the ectodomain was found to be constitutively secreted from cells. Our data indicate that NgR, NgRH1 and NgRH2 constitute a novel receptor protein family, which may play related roles within the CNS.
There is limited evidence regarding the prognostic effects of pathologic lymph node (LN) regression after neoadjuvant chemotherapy for esophageal adenocarcinoma, and a definition of LN response is ...lacking. This study aimed to evaluate how LN regression influences survival after surgery for esophageal adenocarcinoma.
Multicenter cohort study of patients with esophageal adenocarcinoma treated with neoadjuvant chemotherapy followed by surgical resection at five high-volume centers in the United Kingdom. LNs retrieved at esophagectomy were examined for chemotherapy response and given a LN regression score (LNRS)-LNRS 1, complete response; 2, <10% residual tumor; 3, 10%-50% residual tumor; 4, >50% residual tumor; and 5, no response. Survival analysis was performed using Cox regression adjusting for confounders including primary tumor regression. The discriminatory ability of different LN response classifications to predict survival was evaluated using Akaike information criterion and Harrell C-index.
In total, 17,930 LNs from 763 patients were examined. LN response classified as
(LNRS 1 ≥1 LN, no residual tumor in any LN; n = 62, 8.1%),
(LNRS 1-3 ≥1 LN, residual tumor ≥1 LN; n = 155, 20.3%),
(LNRS 4-5; n = 303, 39.7%), or
(no tumor/regression; n = 243, 31.8%) demonstrated superior discriminatory ability. Mortality was reduced in patients with complete LN response (hazard ratio HR, 0.35; 95% CI, 0.22 to 0.56), partial LN response (HR, 0.72; 95% CI, 0.57 to 0.93) or negative LNs (HR, 0.32; 95% CI, 0.25 to 0.42) compared with those with poor/no LN response. Primary tumor regression and LN regression were discordant in 165 patients (21.9%).
Pathologic LN regression after neoadjuvant chemotherapy was a strong prognostic factor and provides important information beyond pathologic TNM staging and primary tumor regression grading. LN regression should be included as standard in the pathologic reporting of esophagectomy specimens.
This study aimed to investigate the vulnerability of Sistan plain to fluctuations and Water Scarcity in Hirmand River using the vulnerability framework, by applying the resilience approach. The ...socioeconomic and biophysical components presented in this framework were embedded in a set of subsystems of the System Dynamics (SD) model. According to this, four levels of reference resilience were defined based on the annual flow from the Hirmand River, and the system attributes of concern were identified under the existing structure until 2050. Then, the proposed strategies to the socio-economic structure of the model were applied under two critical conditions of water scarcity and fluctuations of the river flow. The values associated to the system attributes of concern of the two mentioned conditions were compared with the reference resilience levels. The results showed the efficiency of the policy option in reducing water scarcity and the importance of the environmental impacts of the biophysical component. For example, the two modes of water scarcity and water inflow fluctuations had the revenues of 9490 and 5100 billion IRR (annual income according to the base price of 2011), but they had the same population and resident's utility, which was related to receiving 117 and 600 MCM of the environmental demand, respectively. Management, development and continuous support of the industrial sector can provide a "Success to the Successful" archetype for the socio-economic section of Sistan Plain.
