Maxillofacial defects often affect various type of tissues and require reconstruction using composite flaps. The radial forearm osteofasciocutaneous free flap is one of the least used. We present the ...preliminary results of maxillofacial reconstruction using this free flap.
We reviewed the records of patients having undergone a radial forearm osteofasciocutaneous flap procedure between 2009 and 2011. Mandibular defect were staged according to HCL classification. Maxillary defects were staged according to Cordeiro and Santamaria's classification. Functional results (swallowing and speech) were assessed at the sixth month.
The mean age of the 10 patients was 60.2 years. The average length of bone defect was 8 cm. The average cutaneous defect area was 36.8 cm(2). The mandibular defect was LCL in six cases, L in one case, and C in one case. The two cases of maxillary bone loss were classified IIIa. Six of the 10 microsurgical anastomoses were contralateral to the lesion. There was no reconstruction failure. Three patients had mild speech disorders, five had moderate speech disorders, and two major speech disorders. Four patients had a normal food intake, three patients needed mixed food, and three patients liquid food.
The indications of radial forearm osteofasciocutaneous free flap for complex maxillofacial reconstructions should be extended. Its vascularization is less compromised by arteritis. Its pedicle may be long and adequate for a contralateral anastomosis. It is reliable and easy to harvest. But the bone quality is not as good as fibula.

Nous rapportons un cas de kystes trichilemmaux multiples du cuir chevelu, découverts chez femme de 61ans ayant des antécédents familiaux. Le diagnostic a été confirmé par l’histologie de la pièce opératoire. Les kystes trichilemmaux se caractérisaient par remaniements intrakystiques, sous forme d’ossification. L’étendue des lésions a nécessité une exérèse chirurgicale élargie du cuir chevelu. La reconstruction a été faite par expansion cutanée puis par lambeau de couverture.
Le kyste trichilemmal est une lésion bénigne des annexes cutanées, d’évolution lente. Le caractère multiple de cette lésion nécessite parfois une exérèse chirurgicale élargie, posant le problème de la reconstruction. Le suivi des cas familiaux pourrait limiter ces inconvénients.
The trichilemmal cyst, also called pilar cyst, is a dermal epithelial cyst originating from the pilar follicle. It usually occurs in the scalp. Hereditary and ossifying cases are rare.
We report the case of a 61-year-old woman presenting with familial multiple trichilemmal cysts, located in the scalp. The histological examination after surgical exeresis confirmed the diagnosis. The trichilemmal cysts featured intra-cystic ossification. The wide dimension of the lesions dimension imposed an extensive surgical scalp exeresis. Reconstruction was performed with a flap after skin expansion.
The trichilemmal cyst is a benign tumor of skin appendages with a slow outcome. Multiple locations may require an extensive surgical scalp exeresis and lead to a problematic reconstruction. Hereditary cases require familial follow-up to prevent such problems.

The relative thermodynamic stabilities of keto-enol tautomers and the ease of interconversion of some ions are discussed. It is emphasised that CA spectra alone (i.e. in the absence of other information, such as heat of formation, etc.) may not easily be correlated with the appropriate ion structures.

Cette incision élargie peut provoquer des complications qui ont incité ...plusieurs auteurs à proposer des prises en charge endoscopiques des fractures de la paroi antérieure du sinus frontal. Nous présentons la technique de la reconstruction différée de la paroi antérieure du sinus frontal par comblement d’hydroxyapatite par voie endoscopique.
Deux incisions centimétriques ont été réalisées en arrière de la ligne d’implantation capillaire. Un décollement sous-périosté à l’aide d’une rugine a été effectué. Un endoscope de 30° à été utilisé pour visualiser la dépression. Cette dernière était comblée par Hydroset® (Stryker, États-Unis) comme substitut osseux de comblement.
En l’absence de critères d’exclusion décrits dans l’article, la reconstruction de la paroi antérieure du sinus frontal par comblement d’hydroxyapatite par voie endoscopique est une technique offrant de nombreux avantages dont des suites opératoires simples, une réduction de la durée du séjour hospitalier et une courbe d’apprentissage rapide.
Reconstruction of the anterior wall of the frontal sinus usually requires a coronal incision. This extended approach may lead to paresthesia, unsightly scars, bruises and cicatricial alopecia. These complications encouraged several authors to endoscopic management of this kind of fractures. We present a delayed technique of reconstruction of the anterior wall of the frontal sinus by means of endoscopic hydroxyapatite filling.
Two incisions were performed behind the hair line. Subperiosteal dissection using a periosteal elevator was performed. A 30° angled endoscope was used to visualize the depression. The latter was filled by Hydroset® (Stryker, USA) as a bone substitute.
In the absence of contra-indication, the reconstruction of the anterior wall of the frontal sinus by means of endoscopic hydroxyapatite filling has many advantages including uneventful outcome, reduction of the hospital stay and a fast learning curve.
Prosthetic replacement of TMJ is gradually becoming a common procedure because of good functional and aesthetic results and low morbidity. Prosthetic models available can be standard or custom-made. ...Custom-made prosthesis are usually reserved for complex cases, but we think that computer assistance for custom-made prosthesis should be indicated for each case because it gives a greater implant stability and fewer complications. Computer assistance will further enlarge TMJ prosthesis replacement indications.
Le remplacement prothétique de l’ATM devient progressivement une intervention courante grâce, d’une part, aux résultats fonctionnels et esthétiques et, d’autre part, à la faible incidence des complications. Les modèles utilisés sont soit standard, soit sur mesure. Ces derniers étant réservés aux cas complexes. Nous pensons que l’assistance par ordinateur devrait permettre une reconstruction sur mesure de chaque cas avec une meilleure pérennité implantaire par la stabilité prothétique avec moins de complications. Elle permettra en outre l’élargissement des indications.
The temporomandibular joint (TMJ) may be the location for tumoral, post-traumatic, degenerative, inflammatory, or congenital diseases requiring a prosthetic replacement. We present a technical note ...for total TMJ replacement with a Biomet® Microfixation prosthesis (Jacksonville, FL, USA).
The preoperative data includes dental panoramic X-ray, MRI of the TMJ, and CT-scan (or cone beam) of the facial skeleton. The last two mentioned examinations allow three-dimensional reconstructions and stereo-lithographic models required for surgical planning. The prosthetic device includes a temporal ultra-high density polyethylene component and a mandibular chrome-cobalt-molybdenum-titanium alloy component; in case of allergy the prosthesis is made in titanium. These components may be standard size or custom made. The surgery requires through two common surgical approaches: pre-auricular and sub-mandibular. When a custom-made prosthesis is used, osteotomies are performed using surgical guides provided by the manufacturer. After intermaxillary fixation, the various prosthetic components are positioned and fixed with specific titanium screws, 2mm (temporal component) and 2.7mm (mandibular component) in diameter. The occlusion and prosthesis functionality are tested after releasing the intermaxillary fixation. Soft food intake is recommended for 21days and physiotherapy is initiated on the second postoperative week for at least 1year.
The total replacement technique with a Biomet Microfixation® prosthesis is a simple procedure. It allows recovering a good articular function and relieves the patient.
L’articulation temporo-mandibulaire (ATM) peut être le siège de pathologies tumorales, dégénératives, inflammatoires, congénitales, ou post-traumatiques, pouvant nécessiter un remplacement prothétique. Nous présentons notre technique de remplacement de l’ATM par les prothèses Biomet Microfixation® (Jacksonville, FL, États-Unis).
Le bilan radiologique préopératoire comporte un panoramique dentaire, une IRM de l’ATM et un scanner (ou un cone beam) de l’ensemble du massif facial. Ces deux derniers examens permettent d’obtenir les reconstructions tridimensionnelles et les modèles stéréo-lithographiques, nécessaires pour la planification chirurgicale. Le matériel prothétique se compose d’un composant temporal en polyéthylène ultra-haute densité et d’un composant mandibulaire en alliage chrome-cobalt-molybdène-titane ; en cas d’allergie, la prothèse est en titane. Ces composants peuvent être de taille standard ou sur mesure. L’intervention nécessite deux voies d’abord conventionnelles, pré-auriculaire et sous-angulo-mandibulaire. En cas de prothèse sur mesure, les ostéotomies sont effectuées grâce à des guides chirurgicaux fournis par le fabricant. Après pose d’un blocage maxillo-mandibulaire, les composants prothétiques sont positionnés et fixés à l’aide de vis spécifiques en titane de différents diamètres, 2mm (composant temporal) et 2,7mm (composant mandibulaire). Le fonctionnement de la prothèse et l’occlusion sont testés après libération du blocage. Une alimentation molle est préconisée pendant trois semaines et la kinésithérapie est débutée à partir de la seconde semaine postopératoire pour une durée d’au moins un an.
Le remplacement de l’ATM par une prothèse Biomet Microfixation® est une technique simple. Il permet de restaurer une bonne fonction articulaire et de soulager le patient.
We demonstrate the use of a ring-shaped Bose-Einstein condensate as a rotation sensor by measuring the interference between two counter-propagating phonon modes imprinted azimuthally around the ring. ...We observe rapid decay of the excitations, quantified by quality factors of at most Q ≈ 27. We numerically model our experiment using the c-field methodology, allowing us to estimate the parameters that maximise the performance of our sensor. We explore the damping mechanisms underlying the observed phonon decay, and identify two distinct Landau scattering processes that each dominate at different driving amplitudes and temperatures. Our simulations reveal that Q is limited by strong damping of phonons even in the zero temperature limit.
