Purpose of Review
Idiopathic intracranial hypertension (IIH) is a disorder characterized by long-standing elevated intracranial pressure (ICP). As the name applies, no uniform cause has been ...identified. IIH is typically characterized by headaches, pulsatile tinnitus, and visual deterioration.
Recent Findings
Anomalies in cerebrospinal fluid (CSF) absorption are implicated in the pathophysiology of IIH. Non-invasive imaging of the brain parenchyma and the cerebral venous sinus has improved, and research has gained a better understanding of the role of cerebral venous sinus stenosis. Both have led to a better delineation of the role of arachnoid granulations (AG) and the glymphatic system in the development of IIH.
Summary
IIH may occur as a result of restrictions of CSF absorption from the venous system, and or the congestion and overflow of the glymphatic system. Elucidating these mechanisms will lead to greater understanding of its underlying pathophysiologic mechanisms.
Social problem solving (SPS) represents a social cognitive reasoning process that gives way to behavior when individuals are navigating challenging social situations. Autistic individuals have been ...shown to struggle with specific aspects of SPS, which, in turn, has been related to social difficulties in children. However, no previous work has measured how SPS components not only relate to one another but also discretely and conjointly predict autism-related symptoms and social difficulties in autistic children, specifically. =1.31) completed a self-administered, computerized assessment of SPS. To elucidate how SPS components discretely, and combined, contribute to autism-related symptoms and social difficulties, commonality analyses were conducted for each measure assessing autism-related symptoms and social difficulties. Socially normative problem identification, goal preference, and solution preference were related to fewer parent-reported autism-related social difficulties. Measures related to autism symptomatology, social perspective taking, and emotion recognition were not significantly associated with discrete SPS components in this sample. The problem identification aspect of SPS contributed the most unique variance to parent-reported autism-related social difficulties, while shared variance across all SPS components accounted for substantial variance in both parent-reported autism-related social difficulties models. Results suggest that SPS components are interrelated, but distinct, constructs in the autistic population. These findings not only further clarify the impact of SPS components on autism-related symptoms and social difficulties, but also have implications for refining SPS-focused interventions in the autistic population.
Recent studies have found an association between increased volume and increased intensive care unit (ICU) survival; however, this association might not hold true in ICUs with permanent intensivist ...coverage. Our objective was to determine whether ICU volume correlates with survival in the Spanish healthcare system.
Post hoc analysis of a prospective study of all patients admitted to 29 ICUs during 3 months. At ICU discharge, the authors recorded demographic variables, severity score, and specific ICU treatments. Follow-up variables included ICU readmission and hospital mortality. Statistics include logistic multivariate analyses for hospital mortality according to quartiles of volume of patients.
The authors studied 4,001 patients with a mean predicted risk of death of 23% (range at hospital level: 14-46%). Observed hospital mortality was 19% (range at hospital level: 11-35%), resulting in a standardized mortality ratio of 0.81 (range: 0.5-1.3). Among the 1,923 patients needing mechanical ventilation, the predicted risk of death was 32% (14-60%) and observed hospital mortality was 30% (12-61%), resulting in a standardized mortality ratio of 0.96 (0.5-1.7). The authors found no correlation between standardized mortality ratio and ICU volume in the entire population or in mechanically ventilated patients. Only mechanically ventilated patients in very low-volume ICUs had slightly worse outcome.
In the currently studied healthcare system characterized by 24/7 intensivist coverage, the authors found wide variability in outcome among ICUs even after adjusting for severity of illness but no relationship between ICU volume and outcome. Only mechanically ventilated patients in very low-volume centers had slightly worse outcomes.

Se autodiseñó una encuesta con los factores relacionados con la obesidad infantil, y se administró a los padres de adolescentes que cursaban primero de secundaria en 4 centros del Área V de Salud de Murcia. Se objetivó el peso, talla, perímetro abdominal e índice cintura-talla (ICT) de los alumnos, y se clasificaron en sobrepeso-obesidad. Se aplicó la técnica de reducción de dimensiones, generando factores que agruparon los ítems según materia, y se realizó una técnica multivalente para valorar la relación de dependencia entre las variables y el EP.
Fueron incluidos 421 alumnos, el 28 y el 35% con EP y obesidad abdominal, respectivamente. El análisis factorial agrupó los ítems en 4 factores: alimentación, actividad física, tecnologías y entorno, existiendo un subapartado de percepción corporal. El modelo de ecuaciones estructurales presentó un R2 de 0,440. Se obtuvo la mayor relación con el factor entorno (t 2,89) y percepción (t 14,61), seguido del uso de tecnologías. Además, reveló una relación directa respecto a la alimentación y la actividad física, aunque no significativa.
La percepción familiar y el ambiente social-escolar influyen de forma importante en el desarrollo del EP. Las intervenciones de educación para la salud con inclusión de padres y profesores probablemente sean las estrategias más inteligentes y rentables.

A self-designed questionnaire with factors related to childhood obesity was produced, and was administered to the parents of adolescents who were attending first year of high school in four centres in Health Area V in Murcia. Weight, height, abdominal circumference and Waist-Height Index (WHI) of the students were measured, and classified as overweight-obesity. A reduction technique was applied, generating factors that grouped the items according to subject, as well as a multivalent technique to assess the dependency relationship between the variables, and the SB-OI.
Of the 421 students included, 28 and 35% had excess weight and abdominal obesity, respectively. The factor analysis grouped the items into 4 factors: diet, physical activity, technologies, and environment, with a subsection about body perception. The structural equation model presented an R2 of 0.440. The highest relationship was obtained with the environment factor (t 2.89), and perception (t 14.61), followed by the use of technologies. A direct relationship was also revealed regarding diet and physical activity, although not significant.
Family perception and the social-school environment have an important influence on the development of the O-W. Health education interventions involving parents and teachers are probably the smartest and most cost-effective strategies.

Although there has been an improved management of Invasive Candidiasis in the last decade, controversial issues still remain, especially in the diagnostic and therapeutic approaches.
We sought to ...identify the core clinical knowledge and to achieve high level agreement recommendations required to care for critically ill adult patients with Invasive Candidiasis.
Prospective Spanish survey reaching consensus by the Delphi technique, anonymously conducted by electronic e-mail in a first term to 25 national multidisciplinary experts in invasive fungal infections from five national scientific societies, including Intensivists, Anesthesiologists, Microbiologists, Pharmacologists and Infectious Disease Specialists, responding to 47 questions prepared by a coordination group after a strict review of the literature in the last five years. The educational objectives spanned five categories, including epidemiology, diagnostic tools, prediction rules, and treatment and de-escalation approaches. The level of agreement achieved among the panel experts in each item should exceed 75% to be selected. In a second term, after extracting recommendations from the selected items, a face to face meeting was performed where more than 80 specialists in a second round were invited to validate the preselected recommendations.
In the first term, 20 recommendations were preselected (Epidemiology 4, Scores 3, Diagnostic tools 4, TREATMENT 6 and De-escalation approaches 3). After the second round, the following 12 were validated:
Think about Candidiasis in your ICU and do not forget that non-albicans species also exist. DIAGNOSTIC TOOLS: Blood cultures should be performed under suspicion every 2-3 days and, if positive, every 3 days until obtaining the first negative result. Obtain sterile fluid and tissue, if possible (direct examination of the sample is important). Use nonculture based methods of microbiological tools, whenever possible. Determination of antifungal susceptibility is mandatory. SCORES: As screening tool, use the Candida Score and determine multicolonization in high risk patients.
Start early. Choose Echinocandins. Withdraw the catheter. Fundoscopy is needed. DE-ESCALATION: Only applied when knowing susceptibility determinations and after 3 days of clinical stability. The higher rate of agreement was achieved in the optimization of microbiological tools and the withdrawal of the catheter, whereas the lower rate corresponded to de-escalation therapy and the use of scores.
The management of invasive candidiasis in ICU patients requires the application of a broad range of knowledge and skills that our summarized in our recommendations. These recommendations may help to identify the potential patients, standardize their global management and improve their outcomes, based on the DELPHI methodology.

The Consensus Document on Alternatives to Allogenic Blood Transfusion (AABT) has been drawn up by a panel of experts from 5 scientific societies. The Spanish Societies of Anesthesiology (SEDAR), ...Critical Care Medicine and Coronary Units (SEMICYUC), Hematology and Hemotherapy (AEHH), Blood Transfusion (SETS) and Thrombosis and Hemostasis (SETH) have sponsored and participated in this Consensus Document. Alternatives to blood transfusion have been divided into pharmacological and non-pharmacological, with 4 modules and 12 topics. The main objective variable was the reduction of allogenic blood transfusions and/or the number of transfused patients. The extent to which this objective was achieved by each AABT was evaluated using the Delphi method, which classifies the grade of recommendation from A (supported by controlled studies) to E (non-controlled studies and expert opinion). The experts concluded that most of the indications for AABT were based on middle or low grades of recommendation, "C", "D", or "E", thus indicating the need for further controlled studies.
