= 382; 23 + 0 - 36 + 6 weeks of gestational age) with a focus on the first 28 days of life and the clinical endpoint BPD (supplemental oxygen for longer than 28 days of age). In a subgroup of preterm infants, we characterized the immunological phenotype of Tregs (
= 23). The suppressive function of Tregs on CD4+CD25- T cells was compared in preterm, term and adult blood. We observed that extreme prematurity was associated with increased Treg frequencies which peaked in the second week of life. Independent of gestational age, increased Treg frequencies were noted to precede the development of BPD. The phenotype of preterm infant Tregs largely differed from adult Tregs and displayed an overall naïve Treg population (CD45RA+/HLA-DR-/Helios+), especially in the first days of life. On day 7 of life, a more activated Treg phenotype pattern (CCR6+, HLA-DR+, and Ki-67+) was observed. Tregs of preterm neonates had a higher immunosuppressive capacity against CD4+CD25- T cells compared to the Treg compartment of term neonates and adults. In conclusion, our data suggest increased frequencies and functions of Tregs in preterm neonates which display a distinct phenotype with dynamic changes in the first weeks of life. Hence, the continued abundance of Tregs may contribute to sustained inflammation preceding the development of BPD. Functional analyses are needed in order to elucidate whether Tregs have potential as future target for diagnostics and therapeutics.
Promover los estudiantes al inicio de sus estudios universitarios es un aspecto importante tanto en Chile como en el contexto internacional. Sin embargo, altos índices de abandono muestran que la ...transición educación media-educación superior es difícil para muchos alumnos, especialmente en matemáticas. La argumentación matemática es una actividad fundamental y compleja, la cual podría ser una de las principales razones de las dificultades que presentan los estudiantes en esta materia durante el proceso de transición. Este estudio analiza la calidad de las argumentaciones, los esquemas de prueba y el formalismo utilizado en las respuestas de N = 86 estudiantes de primer año, con el fin de entender mejor donde se ubica el problema. Los resultados muestran que la calidad de la argumentación, el uso de argumentos apropiados y los formalismos matemáticos tienen alta variación en las dos tareas reportadas: 10/% de los estudiantes probaron el primer teorema geométrico asignado y 65% probaron correctamente el segundo. Esto indica que los estudiantes están en una fase de desarrollo de sus habilidades argumentativas. Consecuentemente, el proceso de probar y construir argumentos analíticos debe ser explícitamente discutido en la Universidad, así los estudiantes podrán mejorar sus habilidades para el razonamiento matemático.

Trotz der Einführung der Bildungsstandards für Schulen in Deutschland zeigen Studien, dass auch Studentinnen und Studenten noch erhebliche Schwierigkeiten beim mathematischen Begründen haben. ...Mathematisches Argumentieren ist jedoch eine Kernkompetenz, die vor allem in mathematisch-naturwissenschaftlichen Studiengängen eine bedeutende Rolle spielt. Zur systematischen Förderung von Kompetenzen sind Modelle notwendig, die empirisch belegt sind. Da mathematisches Begründen eine komplexe Tätigkeit ist, die insbesondere in der Sekundarstufe II und im tertiären Bildungsbereich - also genau am Übergang Schule-Hochschule - viele verschiedene Fähigkeiten voraussetzt, müssen kleine Schritte unternommen werden, um zunächst einzelne Aspekte mathematischer Argumentationskompetenz zu identifizieren. Diese Vorgehensweise kann eine erste Annäherung an künftige Kompetenzmodelle für das mathematische Begründen in der Studieneingangsphase sein. Als Grundlage zur Identifizierung von solchen Aspekten dienen Ergebnisse einer querschnittlichen empirischen Studie, bei der 439 Studienanfängerinnen und -anfänger der Ingenieurwissenschaften im Rahmen eines mathematischen Vorkurses teilnahmen. Dabei wurden die folgenden vier Fragen untersucht: (1) Wie ist die Qualität von Argumentationen, die Studierende verwenden? (2) Welche Art von Argumenten wird beim Begründen herangezogen? (3) Welche Hilfsmittel werden von Studierenden verwendet, die korrekt argumentieren und so zu vollständigen Beweisen kommen? (4) Welche Rolle spielen die Inhalte der Argumentationsaufgabe? Die Ergebnisse zeigen einen erheblichen Einfluss des Inhalts der Argumentationsaufgaben, insbesondere einen differentiellen Effekt vor allem in Abhängigkeit davon, ob der Inhalt anschaulich oder abstrakt ist. Weiter wird deutlich, dass Routineprozeduren immer noch häufig eingesetzt werden; möglicherweise ein Hinweis darauf, dass die Einübung solcher Routinen im Schulunterricht immer noch dominiert. Diese Ergebnisse deuten darauf hin, dass die Qualität mathematischer Argumentationen vom Abstraktionsgrad der Inhalte sowie der Möglichkeit, prozedurale Verfahrensweisen anzuwenden, abhängig ist. Several studies show that university students in Germany still have problems in reasoning mathematically although this already should be fostered at high school since the implementation of standards for school mathematics. Mathematical argumentation is a core competence and highly important, especially in academic mathematics. To foster mathematical argumentation at the beginning of university studies, competence models are needed which give more detailed insights in the skills that are necessary for reasoning. As mathematical argumentation is a complex process, especially at the higher secondary level or at university, many little steps are needed to complete a competence model for argumentation at the secondary-tertiary transition gradually. A possible step can be to initially identify several aspects of mathematical argumentation competence that influence the reasoning quality. The empirical basis for identifying those aspects is a cross-sectional study with 439 engineering students who participate in a transition course in mathematics. We address the following questions: (1) how is the quality of student's reasoning? (2) Which kind of arguments do students use? (3) What resources do students who reasoned correctly use for solving the problems? (4) Does the content of the tasks play an important role? The results show a great influence of the content on the reasoning quality, especially if the content is abstract or concrete. Argumentation quality of students decreases with an increasing level of abstraction of the content. Furthermore, the results reveal that students often use routines for solving the problems. That indicates that procedural approaches still play an important role in school mathematics. If procedures could be used for solving the tasks, students are more successful. Competence models for mathematical argumentation at the beginning of the tertiary level should, therefore, include these factors. (DIPF/Orig.).

Background
Postsurgical hypoparathyroidism (PH) is the most frequent complication after thyroid surgery. The aim of this systematic review and meta-analysis is to summarize a unifying ...definition of PH and to elucidate the best possible approach for early detection of PH.
Methods
A systematic review of the literature according to the PICO framework using Embase, PUBMED and the Cochrane library was carried out on 1 December 2021 followed by analysis for risk of bias, data extraction and meta-analysis. All studies addressing the definition of postoperative hypoparathyroidism and/or diagnostic approaches for early detection and diagnosis were included. Case reports, commentaries, non-English articles, book chapters and pilot studies and reviews were excluded.
Results
From 13 704 articles, 188 articles were eligible for inclusion and further analysis. These articles provided heterogeneous definitions of PH. Meta-analysis revealed that postoperative measurements of parathormone (PTH) levels have a higher sensitivity and specificity than intraoperative PTH measurements to predict PH after thyroid surgery. None of the timeframes analysed after surgery within the first postoperative day (POD1) was superior to predict the onset of PH. PTH levels of less than 15 pg/ml and less than 10 pg/ml are both reliable threshold levels to predict the postoperative onset of PH. A relative reduction of mean(s.d.) PTH levels from pre- to postoperative values of 73 (standard deviation 11) per cent may also be predictive for the development of PH. The estimation of calcium levels on POD1 are recommended.
Conclusion
PH is best defined as an undetectable or inappropriately low postoperative PTH level in the context of hypocalcaemia with or without hypocalcaemic symptoms. PTH levels should be measured after surgery within 24 h. Both threshold levels below 10 and 15 pg/ml or relative loss of PTH before/after thyroid surgery are reliable to predict the onset of PH.

Der postoperative Hypoparathyreoidismus (PH) stellt eine der häufigsten Komplikationen nach Schilddrüsenoperationen dar. Ziel dieses systematischen Reviews und Metaanalyse ist die Erarbeitung einer ...einheitlichen Definition sowie die Ermittlung des bestmöglichen Ansatzes für eine frühzeitige Detektion des PH.
Nach Durchführung einer systematischen Literaturrecherche gemäß der PICo-Systematik unter Verwendung der Datenbanken Embase, Pubmed und der Cochrane Library, erfolgte die themenbezogene Aufarbeitung der eingeschlossenen Studien, sowie eine Bias-Bewertung und Metaanalyse geeigneter Arbeiten.
Von 13.704 Artikeln konnten 188 in die weitere Analyse eingeschlossen werden. In diesen fanden sich sehr heterogene Definitionen des PH. Sowohl in der systematischen Analyse als auch in der Metaanalyse zeigte sich eine genauere Vorhersagekraft des PH durch eine postoperative im Vergleich zu einer intraoperativen PTH-Messung. Keiner der analysierten Zeiträume innerhalb des ersten postoperativen Tages (POD1) zeigte eine signifikante Überlegenheit in der Vorhersage eines PH. Die PTH- Schwellenwerte 10 bzw. 15 pg/ml können einen PH zuverlässig detektieren. Als Entscheidungsgrundlage zwischen den beiden Werten kann die untere Normwertgrenze des angewendeten Testverfahrens herangezogen werden. Bei präoperativer PTH-Abnahme nach Anästhesieeinleitung ist ein relativer PTH-Abfall von prä- nach postoperativ von 73 ± 11% prädiktiv für die Entwicklung eines PH. Die Bestimmung des Calciumspiegels an POD1 ist obligat und optimiert insbesondere die Erkennung einer biochemischen Hypokalzämie.
Ein nicht nachweisbarer oder inadäquat niedriger postoperativer PTH-Spiegel im Zusammenhang mit einer biochemischen oder symptomatischen Hypokalzämie kann als einheitliche Definition des postoperativen Hypoparathyreoidismus vorgeschlagen werden. Die Messung des Parathormons sollte zwischen einer und sechs Stunden postoperativ, spätestens aber innerhalb von 24 Stunden erfolgen. Sowohl der Schwellenwert ≤ 15 pg/ml als auch ein relativer PTH-Abfall von prä- nach postoperativ sind zuverlässig in der Detektion gefährdeter Patienten.
