Despite the increased use and acceptance of complementary and alternative medicine (CAM) practices and practitioners by patients and health care providers, there is relatively little information ...available concerning the reasons for use or its effect on patient health status and well-being.
We conducted a survey of 542 patients attending 16 family practice clinics that belong to a community-based research network in San Diego, California, to determine patients' reasons for using CAM therapies in conjunction with a visit to a family physician and the impact of these therapies on their health and well-being.
Approximately 21% of the patients reported using one or more forms of CAM therapy in conjunction with the most important health problem underlying their visit to the physician. The most common forms of therapy were visiting chiropractors (34.5%) of CAM users), herbal remedies and supplements (26.7%), and massage therapy (17.2%). Recommendations from friends or coworkers, a desire to avoid the side effects of conventional treatments, or failure of conventional treatments to cure a problem were the most frequently cited reasons for using these therapies. Use of practitioner-based therapies was significantly and independently associated with poor perceived health status, poor emotional functioning, and a musculoskeletal disorder, usually low back pain. Use of self-care-based therapies was associated with high education and poor perceived general health compared with a year ago. Use of traditional folk remedies was associated with Hispanic ethnicity.
Sociodemographic characteristics and clinical conditions that predict use of CAM therapies by primary care patients in conjunction with a current health problem vary with the type of therapy used.

Registries are important in rare disease research. The Anorectal Malformation Network (ARM-Net) registry is a well-established European patient registry collecting demographic, clinical, and ...functional outcome data. We assessed the quality of this registry through review of the structure, data elements, collected data, and user experience.
Design and data elements were assessed for completeness, consistency, usefulness, accuracy, validity, and comparability. An intra- and inter-user variability study was conducted through monitoring and re-registration of patients. User experience was assessed via a questionnaire on registration, design of registry, and satisfaction.
We evaluated 119 data elements, of which 107 were utilized and comprised 42 string and 65 numeric elements. A minority (37.0%) of the 2278 included records had complete data, though this improved to 83.5% when follow-up elements were excluded. Intra-observer variability demonstrated 11.7% incongruence, while inter-observer variability was 14.7%. Users were predominantly pediatric surgeons and typically registered patients within 11–30 min. Users did not experience any significant difficulties with data entry and were generally satisfied with the registry, but preferred more longitudinal data and patient-reported outcomes.
The ARM-Net registry presents one of the largest ARM cohorts. Although its collected data are valuable, they are susceptible to error and user variability. Continuous evaluations are required to maintain relevant and high-quality data and to achieve long-term sustainability. With the recommendations resulting from this study, we call for rare disease patient registries to take example and aim to continuously improve their data quality to enhance the small, but impactful, field of rare disease research.

Earthquakes, floods, drought, and other natural hazards cause tens of thousands of deaths, hundreds of thousands of injuries, and billions of dollars in economic losses each year around the world. ...Many billions of dollars in humanitarian assistance, emergency loans, and development aid are expended annually. Yet efforts to reduce the risks of natural hazards remain largely uncoordinated across different hazard types and do not necessarily focus on areas at highest risk of disaster. Natural Disaster Hotspots presents a global view of major natural disaster risk hotspots – areas at relatively high risk of loss from one or more natural hazards. Unpolarized and beam-polarized fourfold cross sections (d^{4}σ/dQ^{2}dx_{B}dtdϕ) for the ep→e^{'}p^{'}γ reaction were measured using the CLAS detector and the 5.75-GeV polarized electron beam of the ...Jefferson Lab accelerator, for 110 (Q^{2},x_{B},t) bins over the widest phase space ever explored in the valence-quark region. Several models of generalized parton distributions (GPDs) describe the data well at most of our kinematics. This increases our confidence that we understand the GPD H, expected to be the dominant contributor to these observables. Through a leading-twist extraction of Compton form factors, these results support the model predictions of a larger nucleon size at lower quark-momentum fraction x_{B}.

The $f_1(1285)$ meson with mass $1281.0 \pm 0.8$ MeV/$c^2$ and width $18.4 \pm 1.4$ MeV (FWHM) was measured for the first time in photoproduction from a proton target using CLAS at Jefferson Lab. ...Differential cross sections were obtained via the $\eta\pi^{+}\pi^{-}$, $K^+\bar{K}^0\pi^-$, and $K^-K^0\pi^+$ decay channels from threshold up to a center-of-mass energy of 2.8 GeV. The mass, width, and an amplitude analysis of the $\eta\pi^{+}\pi^{-}$ final-state Dalitz distribution are consistent with the axial-vector $J^P=1^+$ $f_1(1285)$ identity, rather than the pseudoscalar $0^-$ $\eta(1295)$. The production mechanism is more consistent with $s$-channel decay of a high-mass $N^*$ state, and not with $t$-channel meson exchange. Decays to $\eta\pi\pi$ go dominantly via the intermediate $a_0^\pm(980)\pi^\mp$ states, with the branching ratio $\Gamma(a_0\pi \text{ (no} \bar{K} K\text{)}) / \Gamma(\eta\pi\pi \text{(all)}) = 0.74\pm0.09$. The branching ratios $\Gamma(K \bar{K} \pi)/\Gamma(\eta\pi\pi) = 0.216\pm0.033$ and $\Gamma(\gamma\rho^0)/\Gamma(\eta\pi\pi) = 0.047\pm0.018$ were also obtained. The first is in agreement with previous data for the $f_1(1285)$, while the latter is lower than the world average.
OBJECTIVE: The study’s aim was to compare outcomes of very low birth weight twins with those of matched singletons.
STUDY DESIGN: With data from the Neonatal Research Network registry (May 1991 to ...December 1994), univariable and multivariable comparisons of very low birth weight twin pairs and singletons were performed in 2 subgroups: (1) all paired twins and singletons with birth weights between 401 and 1500 g and (2) all paired twins and singletons born at <28 weeks’ gestation.
RESULTS: Twins constituted 19% of infants admitted with very low birth weight. Mothers of twins were more likely to receive prenatal care, have labor, have cesarean delivery, and receive antenatal glucocorticoids. Twins were more likely to have respiratory disease and to receive surfactant. Second-born twins had more early respiratory disease but similar longer-term outcomes. The risks of death, chronic lung disease, and grade III or IV intracranial hemorrhage were similar in twins and singletons.
CONCLUSIONS: Although very low birth weight twins compose a sizable proportion of admissions, in National Institute of Child Health and Human Development Neonatal Research Network intensive care units, twins and singletons have similar outcomes. (Am J Obstet Gynecol 1998;179:742-9.)
In this study, we present the final results for the deuteron spin structure functions obtained from the full data set collected with Jefferson Lab's CLAS in 2000-2001. Polarized electrons with ...energies of 1.6, 2.5, 4.2 and 5.8 GeV were scattered from deuteron (15ND3) targets, dynamically polarized along the beam direction, and detected with CLAS. From the measured double spin asymmetry, the virtual photon absorption asymmetry Ad1 and the polarized structure function gd1 were extracted over a wide kinematic range (0.05 GeV2 < Q2 < 5 GeV2 and 0.9 GeV < W < 3 GeV). We use an unfolding procedure and a parametrization of the corresponding proton results to extract from these data the polarized structure functions An1 and g1n of the (bound) neutron, which are so far unknown in the resonance region, W < 2 GeV. We compare our final results, including several moments of the deuteron and neutron spin structure functions, with various theoretical models and expectations as well as parametrizations of the world data. The unprecedented precision and dense kinematic coverage of these data can aid in future extractions of polarized parton distributions, tests of perturbative QCD predictions for the quark polarization at large x, a better understanding of quark-hadron duality, and more precise values for higher-twist matrix elements in the framework of the Operator Product Expansion.
