This paper introduces a knowledge-driven approach to real-time, continuous activity recognition based on multisensor data streams in smart homes. The approach goes beyond the traditional data-centric ...methods for activity recognition in three ways. First, it makes extensive use of domain knowledge in the life cycle of activity recognition. Second, it uses ontologies for explicit context and activity modeling and representation. Third and finally, it exploits semantic reasoning and classification for activity inferencing, thus enabling both coarse-grained and fine-grained activity recognition. In this paper, we analyze the characteristics of smart homes and Activities of Daily Living (ADL) upon which we built both context and ADL ontologies. We present a generic system architecture for the proposed knowledge-driven approach and describe the underlying ontology-based recognition process. Special emphasis is placed on semantic subsumption reasoning algorithms for activity recognition. The proposed approach has been implemented in a function-rich software system, which was deployed in a smart home research laboratory. We evaluated the proposed approach and the developed system through extensive experiments involving a number of various ADL use scenarios. An average activity recognition rate of 94.44 percent was achieved and the average recognition runtime per recognition operation was measured as 2.5 seconds.
The global population is aging; projections show that by 2050, more than 20% of the population will be aged over 64. This will lead to an increase in aging related illness, a decrease in informal ...support, and ultimately issues with providing care for these individuals. Assistive smart homes provide a promising solution to some of these issues. Nevertheless, they currently have issues hindering their adoption. To help address some of these issues, this study introduces a novel approach to implementing assistive smart homes. The devised approach is based upon an intention recognition mechanism incorporated into an intelligent agent architecture. This approach is detailed and evaluated. Evaluation was performed across three scenarios. Scenario 1 involved a web interface, focusing on testing the intention recognition mechanism. Scenarios 2 and 3 involved retrofitting a home with sensors and providing assistance with activities over a period of 3 months. The average accuracy for these three scenarios was 100%, 64.4%, and 83.3%, respectively. Future will extend and further evaluate this approach by implementing advanced sensor-filtering rules and evaluating more complex activities.
High levels of sedentary behaviour (SB) are associated with negative health consequences. Technology enhanced solutions such as mobile applications, activity monitors, prompting software, texts, ...emails and websites are being harnessed to reduce SB. The aim of this paper is to evaluate the effectiveness of such technology enhanced interventions aimed at reducing SB in healthy adults and to examine the behaviour change techniques (BCTs) used.
Five electronic databases were searched to identify randomised-controlled trials (RCTs), published up to June 2016. Interventions using computer, mobile or wearable technologies to facilitate a reduction in SB, using a measure of sedentary time as an outcome, were eligible for inclusion. Risk of bias was assessed using the Cochrane Collaboration's tool and interventions were coded using the BCT Taxonomy (v1).
Meta-analysis of 15/17 RCTs suggested that computer, mobile and wearable technology tools resulted in a mean reduction of -41.28 min per day (min/day) of sitting time (95% CI -60.99, -21.58, I2 = 77%, n = 1402), in favour of the intervention group at end point follow-up. The pooled effects showed mean reductions at short (≤ 3 months), medium (>3 to 6 months), and long-term follow-up (>6 months) of -42.42 min/day, -37.23 min/day and -1.65 min/day, respectively. Overall, 16/17 studies were deemed as having a high or unclear risk of bias, and 1/17 was judged to be at a low risk of bias. A total of 46 BCTs (14 unique) were coded for the computer, mobile and wearable components of the interventions. The most frequently coded were "prompts and cues", "self-monitoring of behaviour", "social support (unspecified)" and "goal setting (behaviour)".
Interventions using computer, mobile and wearable technologies can be effective in reducing SB. Effectiveness appeared most prominent in the short-term and lessened over time. A range of BCTs have been implemented in these interventions. Future studies need to improve reporting of BCTs within interventions and address the methodological flaws identified within the review through the use of more rigorously controlled study designs with longer-term follow-ups, objective measures of SB and the incorporation of strategies to reduce attrition.

Light transmission aggregometry (LTA) is the most common method used to assess platelet function. However, there is no universal standard for its performance. The Platelet Physiology Subcommittee of ...the Scientific and Standardization Committee (SSC) of the International Society on Thrombosis and Haemostasis formed a working party of experts with the aim of producing a series of consensus recommendations for standardizing LTA. Due to a lack of investigations that directly compared different methodologies to perform LTA studies, there were insufficient data to develop evidence-based guidelines. Therefore, the RAND method was used, which obtains a formal consensus among experts about the appropriateness of health care interventions, particularly when scientific evidence is absent, scarce and/or heterogeneous. Using this approach, each expert scored as "appropriate", "uncertain" or "inappropriate" a series of statements about the practice of LTA, which included pre-analytical variables, blood collection, blood processing, methodological details, choice of agonists and the evaluation and reporting of results. After presentation and public discussion at SSC meetings, the assessments were further refined to produce final consensus recommendations. Before delivering the recommendations, a formal literature review was performed using a series of defined search terms about LTA. Of the 1830 potentially relevant studies identified, only 14 publications were considered to be actually relevant for review. Based upon the additional information, 6 consensus statements were slightly modified. The final statements were presented and discussed at the SSC Meeting in Cairo (2010) and formed the basis of a consensus document, which is the subject of the present report. Sea salt aerosol (SSA) is a naturally occurring phenomenon that arises from the breaking of waves and consequent bubble bursting on the ocean's surface. The resulting particles exhibit a bimodal ...distribution spanning orders of magnitude in size that introduces significant uncertainties when estimating the total annual mass of SSA on a global scale. Although estimates of mass and volume are significantly influenced by the presence of giant particles (dry radius 1 µm), effectively observing and quantifying these particles proves to be challenging. Additionally, uncertainties persist regarding the contribution of SSA production along coastlines, but preliminary studies suggest that coastal interactions may increase SSA particle concentrations by orders of magnitude. Moreover, our knowledge regarding the vertical distribution of SSA particles in the marine boundary layer remains limited, resulting in significant gaps in understanding the vertical mixing of giant aerosol particles and specific environmental conditions facilitating their dispersion. By addressing these uncertainties, particularly in regions where SSA particles constitute a substantial percentage of total aerosol loading, we can enhance our comprehension of the complex relationships between the air, sea, aerosols, and clouds.
The most basic aspect of cloud formation is condensational growth onto cloud condensation nuclei (CCN). As such, condensational growth of cloud drops is often assumed to be a well-understood process ...described by the drop growth equation. When this process is represented in models, CCN activate into cloud drops at cloud base, and it is often assumed that drops consist of pure water or that the hygroscopic contribution after drop activation is small because of the inclusion of only small CCN. Drop growth rate in adiabatic ascent in such models is proportional to supersaturation and assumed to be inversely proportional to the drop radius, thereby making the drop spectrum narrow with altitude. However, the present study demonstrates that drop growth on giant sea-salt aerosol particles (GCCN; dry radius 0.5 m) behaves differently. For typical marine stratocumulus updrafts and for drops grown on GCCN with sizes m, these drops typically remain concentrated salt solutions. Because of this, their condensational growth is accelerated, and they rapidly attain precipitation drop sizes through condensation only. Additionally, drops formed on GCCN may also grow by condensation in cloudy downdrafts. The strong effect of condensation on GCCN is important when carried through to calculating rain-rate contribution as a function of aerosol size. GCCN larger than 2 m account for most of the rainfall rate in the modeled precipitating marine stratocumulus.

Inherited factor XIII (FXIII) deficiency is a rare bleeding disorder that can present with umbilical bleeding during the neonatal period, delayed soft tissue bruising, mucosal bleeding and ...life‐threatening intracranial haemorrhage. FXIII deficiency has also been associated with poor wound healing and recurrent miscarriages. FXIII plays an integral role in haemostasis by catalysing the cross‐linking of fibrin, platelet membrane and matrix proteins throughout thrombus formation, thus stabilizing the blood clot. The molecular basis of FXIII deficiency is characterized by a high degree of heterogeneity, which contributes to the different clinical manifestations of the disease. There have been more than 60 FXIII mutations identified in the current literature. In addition, single nucleotide polymorphisms have been described, some of which have been shown to affect FXIII activity, contributing further to the heterogeneity in patient presentation and severity of clinical symptoms. Although there is a lifelong risk of bleeding, the prognosis is excellent when current prophylactic treatment is available using cryoprecipitate or plasma‐derived FXIII concentrate.
High dose chemotherapy and radiotherapy have radically increased long-term survival of young cancer patients, but major side effects of these treatments are ovarian failure and infertility. Knowledge ...of the risks and probabilities of ovarian failure caused by treatment is crucial for patients and physicians in order to make informed choices that will best serve patients’ interests. This review presents data on ovarian damage and failure following exposure to radiotherapy, chemotherapy and ablative therapy. The risk is evaluated from the published literature according to patient’s age, treatment protocol and also according to the diagnosis of some common malignancies. Many of these patients will not be sterilized immediately following treatment, but will suffer from premature menopause. In order to prevent sterilization, ovarian transposition before pelvic irradiation is mandatory. Besides cryopreservation of ovarian tissue and embryos before administration of chemotherapy, the possible protective effect of pituitary–ovarian down-regulation is discussed. The mechanism of primordial follicles damage induced by radio/chemotherapy is presented as well as the role of apoptosis signalling pathways underlying destruction. Increased knowledge of these mechanisms could help to identify potential effective inhibitors that can block the path of primordial follicles destruction and reduce ovarian failure rate.
This guideline is an update of the 2011 European Guideline for the Management of Anogenital Warts. It is intended to support best practice in the care of patients with anogenital warts by including ...evidence‐based recommendations on diagnosis, treatment, follow‐up and advice to patients. It is intended for use by healthcare professionals in sexual healthcare or dermato‐venereology clinics in Europe but may be adapted for use in other settings where the management of anogenital warts is undertaken. As a European guideline, recommendations should be adapted according to national circumstances and healthcare systems. Despite the availability of vaccine to prevent HPV types 6 and 11, the cause of >95% anogenital warts, they remain an important and frequent health problem. The previous systematic review of randomized controlled trials for anogenital warts was updated. The changes in the present guideline include the following: Updated background information on the prevalence, natural history and transmission of human papillomavirus (HPV) infection and anogenital warts. Key recommendations for diagnosis and treatment have been graded according to the strength of the recommendation and the quality of supporting evidence. 5‐fluorouracil, local interferon and photodynamic therapy have been evaluated and included as potential second‐line treatment options. Evidence of the impact of HPV vaccination on the incidence of anogenital warts has been updated.
