Background
In humans, vestibular epilepsy (VE) is described as focal seizures with transient signs of vestibular disease. In dogs, 2 cases of vestibular episodes, called vestibular paroxysmia, are ...reported.
Hypothesis/objectives
The objective of this study was to define the clinical features, phenotypical manifestation, and outcome of suspected VE in dogs.
Animals
Ten dogs with recurrent vestibular episodes.
Methods
Retrospective study. Medical records between 2009 and 2023 were reviewed, and dogs with a normal neurological examination, a history of transient signs of vestibular disease, absence of abnormalities detected on blood exams and brain magnetic resonance imaging (MRI) or computed tomography (CT), besides a minimum 10‐month follow‐up were included. Clinical improvement was defined as a ≥50% reduction in frequency or the cessation of clinical signs after the onset of antiseizure medications (ASMs).
Results
Pugs were the most prevalent breed (5/10; 50%). In 2 cases, additional generalized tonic‐clonic (GTC) seizures were reported. MRI exam was performed in most cases (9/10; 90%), whereas 1 dog underwent a CT scan (1/10; 10%). Electroencephalography (EEG) was carried out in 3 dogs that showed interictal spikes in the fronto‐temporal and fronto‐parietal areas. All cases received ASMs, with clinical improvement in 10/10 dogs (100%).
Conclusion and clinical importance
The presence of GTC seizures, EEG interictal spikes, and responsiveness to ASMs supported the hypothesis of an epileptic origin of vestibular episodes and thus the existence of VE in these dogs, with a presumed idiopathic cause and apparent favorable outcome.
Background
Reports describing sciatic nerve injuries (SNI) and their outcome are scarce in veterinary medicine.
Hypothesis
Describe the causes of traumatic and iatrogenic SNI and evaluate which ...clinical and electrodiagnostic findings predict outcome.
Animals
Thirty‐eight dogs and 10 cats with confirmed SNI referred for neurologic and electrodiagnostic evaluation.
Methods
Clinical and electrodiagnostic examination results, including electromyography (EMG), motor nerve conduction studies, muscle‐evoked potential (MEP), F‐waves, sensory nerve conduction studies, and cord dorsum potential (CDP), were retrospectively evaluated. Quality of life (QoL) was assessed based on owner interviews.
Results
Surgery (42%) and trauma (33%) were the most common causes of SNI; in dogs, 24% were caused by bites from wild boars. Ability to flex and extend the tarsus was significantly associated with positive outcome in dogs. Mean time from onset of clinical signs until electrodiagnostic evaluation was 67 ± 65 (range, 7‐300) days and 65 ± 108 (range, 7‐365) days for dogs and cats, respectively. A cut‐off amplitude of 1.45 mV for compound motor action potentials (CMAP) was predictive of positive outcome in dogs (P = .01), with sensitivity of 58% and specificity of 100%.
Conclusions and Clinical Importance
Clinical motor function predicts recovery better than sensory function. Electrodiagnostic findings also may play a role in predicting the outcome of SNI. Application of the proposed CMAP cut‐off amplitude may assist clinicians in shortening the time to reassessment or for earlier suggestion of salvage procedures. Owners perceived a good quality of life (QoL), even in cases of hindlimb amputation.
Case summary
A 1-year-old male castrated domestic shorthair cat was presented in a condition of status epilepticus following incidental permethrin spot-on administration by its owner. General ...anaesthesia and mechanical positive pressure control ventilation were necessary to control the epileptic seizures and a progressive condition of hypoventilation. The cat was managed with an intravenous constant rate infusion of midazolam, propofol and ketamine associated with a low-dose intravenous lipid emulsion. A condition of non-convulsive status epilepticus was detected by serial continuous electroencephalogram (cEEG) monitoring. Initial cEEG showed paroxysmal epileptiform discharges; thus, antiseizure treatment with phenobarbital was added and a bolus of hypertonic saline solution was administered to treat suspected intracranial hypertension. A second cEEG performed 24 h later showed the presence of rare spikes and a burst-suppression pattern, so the decision was made to discontinue propofol. A third cEEG, 72 h post-hospitalisation, showed a normal encephalographic pattern; therefore, anaesthetic drugs were progressively tapered, and the patient was extubated. Five days after admission the cat was discharged on phenobarbital treatment, which was gradually tapered during the following months.
Relevance and novel information
This is the first reported case to describe cEEG monitoring during hospitalisation for feline permethrin intoxication. cEEG should be encouraged in cats with altered mental status that have previously suffered cluster seizures or status epilepticus, which could guide clinicians in the choice of antiseizure drugs.
Intervertebral disc herniation, degenerative myelopathy, fibrocartilaginous embolism and polyradiculoneuritis often affect dogs; and physiotherapy may improve the patient’s quality of life and/or ...reduce recovery times. The aim of this review was to evaluate the current scientific outcomes on these four neurological diseases and on their physiotherapy approaches. From the analysis of the published articles, it emerged that intervertebral disc herniation can be treated, with different rates of success, through a conservative or a surgical approach followed by physiotherapy. The literature is generally oriented toward the efficacy of the rehabilitation approach in this specific canine disease, often proposing intensive post-surgery physiotherapy for the most severe conditions with the absence of deep pain perception. When degenerative myelopathy, fibrocartilaginous embolism or polyradiculoneuritis occur, the existing literature supports the use of a physiotherapeutic approach: allowing a delay in the onset and worsening of the clinical signs in degenerative myelopathy, physical improvement, and, sometimes, complete remission during fibrocartilaginous embolism or acute idiopathic polyradiculoneuritis. However, papers on rehabilitation in dogs affected by polyradiculoneuritis are currently limited to single clinical cases and further blinded, controlled, prospective studies are still advisable for all four neurological diseases.
An 8-month-old female Lagotto Romagnolo dog was presented for a 1-month history of an initial severe reluctance to move, rapidly progressing to a marked stiff gait and progressive muscular weakness ...and evolving to tetraparesis, which persuaded the owner to request euthanasia. A primary muscle pathology was supported by necropsy and histopathological findings. Macroscopically, the muscles were moderately atrophic, except for the diaphragm and the neck muscles, which were markedly thickened. Histologically, all the skeletal muscles examined showed atrophy, hypertrophy, necrosis with calcification of the fibers, and mild fibrosis and inflammation. On immunohistochemistry, all three dystrophin domains and sarcoglycan proteins were absent. On Western blot analysis, no band was present for delta sarcoglycan. We sequenced the genome of the affected dog and compared the data to more than 900 control genomes of different dog breeds. Genetic analysis revealed a homozygous private protein-changing variant in the SGCD gene encoding delta- sarcoglycan in the affected dog. The variant was predicted to induce a SGCD:p.(Leu242Pro) change in the protein. In silico tools predicted the change to be deleterious. Other 770 Lagotto Romagnolo dogs were genotyped for the variant and all found to be homozygous wild type. Based on current knowledge of gene function in other mammalian species, including humans, hamsters, and dogs, we propose the SGCD missense variant as the causative variant of the observed form of muscular dystrophy in the index case. The absence of the variant allele in the Lagotto Romagnolo breeding population indicates a rare allele that has appeared recently.
A 10‐year‐old mixed‐breed male dog was referred for a subcutaneous mass on the ventral neck. Based on total‐body computed tomography (TBCT), the mass was located in the left thyroid lobe. Further ...alterations included enlargement of the ipsilateral mandibular and prescapular lymph nodes (LNs). Surgical excision of the mass and enlarged LNs was performed. Histopathology and immunohistochemistry were consistent with a medullary (C‐cell) thyroid carcinoma, with no evidence of nodal metastases. Surgery was considered curative, and no medical treatment was provided. Periodic follow‐up rechecks were unremarkable. After 18 months, the dog exhibited lethargy, vomiting, anorexia, and hind leg stiffness. TBCT revealed polyostotic osteopathy, and cytology suggested a metastatic endocrine carcinoma. Due to the dog's poor clinical condition and prognosis, the owner elected euthanasia, and a necropsy was performed. Based on gross pathology, histopathology, and immunohistochemistry, multiple metastases of the previous thyroid carcinoma were diagnosed, involving the occipital bone, multiple vertebrae, left sacral wing, fourth right rib, left scapula, left humerus, intrathoracic LNs, lung, spleen, and adrenal glands. This report describes a case of medullary thyroid carcinoma with distant multiorgan metastases and massive bone involvement after a disease‐free interval of 18 months.
Timely delivery of adjuvant chemotherapy has been shown to be advantageous in many human cancers and canine osteosarcoma. Adjuvant chemotherapy has been shown to improve outcome for canine splenic ...hemangiosarcoma. The aim of this retrospective study was to investigate whether timely adjuvant chemotherapy administration resulted in better outcome in dogs with non‐metastatic splenic hemangiosarcoma undergoing splenectomy. Medical records were searched for dogs with non‐metastatic, splenic hemangiosarcoma that received splenectomy and adjuvant chemotherapy. The number of days from surgery to the first chemotherapy dose (StoC) was evaluated to identify the cut‐off value associated with the best survival advantage. StoC and other possible prognostic factors were tested for influence on time to metastasis (TTM) and overall survival (OS). Seventy dogs were included. Median StoC was 20 days (range: 4–70). The time interval associated with the greatest survival benefit was 21 days. Median TTM and OS of dogs with StoC ≤ 21 days were significantly longer than those with StoC >21 days (TTM: 163 vs. 118 days, p = .001; OS: 238 vs. 146 days, p < .001). On multivariable analysis, StoC >21 days was the only variable significantly associated with increased risk of tumour progression (HR 2.1, p = .010) and death (HR 2.3; p = .008). Starting adjuvant chemotherapy within 21 days of surgery may be associated with a survival benefit in dogs with non‐metastatic splenic hemangiosarcoma, possibly due to the early targeting of newly recruited metastatic cells after surgery.
